Inflammatory bowel disease, a chronic relapsing-remitting disorder, affects three million adults in the United States. One of the main therapeutic goals in patients with inflammatory bowel disease is mucosal healing, measured by endoscopy, an invasive procedure that does not always reflect patient symptomatology. To address these limitations, clinicians have developed patient-reported outcome measures, questionnaires evaluating bowel symptoms and quality of life. However, the clinical utility of these patient-reported outcome measures and their correlation to objective markers of inflammation is unclear. Marijuana use during pregnancy has dramatically increased in recent years, driven in part by the increasing legalization of marijuana for medicinal and recreational use. However, the impact of prenatal exposure to marijuana on a developing fetus is not clear. Previous studies examining this effect have been limited by polysubstance use during pregnancy and confounding factors that make data interpretation challenging. In this study, we propose a prospective cohort study that will address the limitations of prior studies and measure the association between prenatal marijuana exposure and adverse neonatal outcomes. Inadequate postoperative pain management following spinal surgery contributes to delayed mobilization and chronic pain. The current standard of care following spinal surgery consists of opiates and anesthetics to target multiple pain pathways. However, opiates are limited by their significant adverse effects and anesthetics are limited by their short duration of delivery. One approach that may overcome these limitations is liposomal or lipid-encapsulated drug formulations, which have been shown to extend the duration of drug delivery to target tissues. Gestational diabetes is defined as impaired glucose tolerance with onset during pregnancy and affects approximately 8.2% of pregnancies. Recently, it has been shown that insulin resistance is a predictor of poor milk supply in women attempting to breastfeed. Some mothers with gestational diabetes who used metformin for optimal glycemic control during pregnancy have found that metformin use has improved their ability to produce milk after delivery; however, the effects of continuing metformin postpartum have not yet been studied. It is theorized that continued metformin use in the postpartum period in women with gestational diabetes may improve postpartum milk production. Using …

Chemsex, defined as the use of drugs before or during sex to enhance and facilitate the experience, is increasingly prevalent in urban populations of men who have sex with men. Chemsex is associated with higher risks of sexually transmitted infections including HIV, and it has negative effects on mental health and productivity. However, chemsex is under- or un-addressed in most primary care practices, and chemsex users rarely present to specialists in addiction medicine. Cardiac arrest, characterized by the loss of mechanical activity of the heart, is a life-threatening medical condition that is almost always fatal if not treated immediately. The protocols for cardiopulmonary resuscitation used by emergency medical personnel during out-of-hospital cardiac arrests do not account for cardiac and thoracic anatomic variation when positioning chest compressions. This study will examine whether evaluating chest compression efficacy using transthoracic echocardiography in the prehospital setting will increase the rates of survival to hospital admission in adults suffering atraumatic out-of-hospital cardiac arrest. Information Frictions And Access To The Paycheck Protection Program, John Eric Humphries, Christopher Neilson, Gabriel Ulyssea

The Paycheck Protection Program (PPP) extended 669 billion dollars of forgivable loans in an unprecedented effort to support small businesses affected by the COVID-19 crisis. This paper provides evidence that information frictions and the “first-come, first-served” design of the PPP program skewed its resources towards larger firms and may have permanently reduced its effectiveness. Using new daily survey data on small businesses in the U.S., we show that the smallest businesses were less aware of the PPP and less likely to apply. If they did apply, the smallest businesses applied later, faced longer processing times, and were less likely to …

Economic and financial time series data can exhibit nonstationary and nonlinear patterns simultaneously. This paper studies copula-based time series models that capture both patterns. We introduce a procedure where nonstationarity is removed via a filtration, and then the nonlinear temporal dependence in the filtered data is captured via a flexible Markov copula. We propose two estimators of the copula dependence parameters: the parametric (two-step) copula estimator where the marginal distribution of the filtered series is estimated parametrically; and the semiparametric (two-step) copula estimator where the marginal distribution is estimated via a rescaled empirical distribution of the filtered series. You Can Lead A Horse To Water: Spatial Learning And Path Dependence In Consumer Search, Charles Hodgson, Gregory Lewis

We develop a model of consumer search with spatial learning in which sampling the payoff of one product causes consumers to update their beliefs about the payoffs of other products that are nearby in attribute space. Spatial learning gives rise to path dependence, as each new search decision depends on past experiences through the updating process. We present evidence of spatial learning in data that records online search for digital cameras. Consumers’ search paths tend to converge to the chosen product in attribute space, and consumers take larger steps away from rarely purchased products. We estimate the structural parameters of …

This paper is focused not on the Internet architecture – as defined by layering, the narrow waist of IP, and other core design principles – but on the Internet infrastructure, as embodied in the technologies and organizations that provide Internet service. In this paper we discuss both the challenges and the opportunities that make this an auspicious time to revisit how we might best structure the Internet’s infrastructure. Currently, the tasks of transit-between-domains and last-mile-delivery are jointly handled by a set of ISPs who interconnect through BGP. In this paper we propose cleanly separating these two tasks.
