At the beginning of the COVID-19 pandemic, contact tracing was implemented as a public health measure to prevent the further spread of disease. In this thesis, I analyze contact tracing as a case study of public health surveillance. I argue that public health does not sufficiently study the social components and consequences of its surveillant activities, and as a result is hyperopic; it sees health-related phenomena at a distance with clarity, but has not brought its own logics under view. To remedy this, I utilize perspectives from the field of surveillance studies, which studies surveillance as social and cultural phenomena. …

BackgroundIn 2020, approximately 26 million refugees resettled in a new country—fleeing their homes due to conflict, persecution, violence, and human rights violations. Refugees often migrate from developing countries with limited access to adequate health care, routine immunizations, and public health resources. Because of low immunization coverage and potentially underlying health conditions, including stress, trauma, overcrowding during transit, poor hygiene and sanitation services, and malnutrition, refugees face a greater risk of contracting vaccine-preventable diseases (VPDs). This presents a key public health challenge and ethical imperative to serve the health needs of refugees resettling each year.

Introduction: Salmonellosis is the second leading bacterial foodborne illness in the United States, and is mainly characterized by symptoms of gastroenteritis and their consequences in afflicted individuals. Salmonella accounts for 1.35 million infections, 26,500 hospitalizations and 420 deaths in the United States every year. Live poultry act as a reservoir of Salmonella and causes an annual national outbreak. A growing literature suggests that mass incarceration impacts not only the individual who is incarcerated, but also their partners, family members, and children. However, few studies have examined race differences in network exposure to mass incarceration. This study explores associations between race and various measures of network exposure to incarceration: relationships, interactions with the criminal legal system, and housing. We draw on data from the 2017-2018 baseline survey from the Justice, Housing, and Health Study. Rare disease definition varies among countries/regions. China has published The First National List of Rare Diseases, including 121 kinds of rare diseases in 2018. It was the first time that China has defined rare diseases officially. China has also started releasing a range of encouraging regulations regarding orphan drug development and approval in recent years. Nevertheless, the FDA has introduced Orphan Drug Act in the 1980s, Japan has launched a government-led support initiative in the 1990s, and the European Parliament has adopted the Orphan Regulation in 2000. Background: Adolescent vaccination coverage varies considerably between Tdap, meningococcal, and HPV vaccines. While evidence suggests that health care access affects vaccination coverage, evidence does not explain whether access drives delayed or no vaccination. This study evaluates whether measures of health care access are associated with delayed vaccination or not being vaccinated by age 17 years for Tdap, meningococcal, and HPV vaccines when controlling for sociodemographic factors as proxies for vaccine hesitancy. The secondary objective assesses whether health care access measures had consistent associations across the different vaccines. Objective: To investigate the impact of COVID-19 on the participants of a multi-city fruit and vegetable incentive program.Design: A six-month cohort study was conducted. Pre-program and post-program data were collected using an online survey platform. Setting: Eight cities across the United States. Participants: A total of 822 individuals were recruited through local site partners to participate in a fruit and vegetable incentive program (n=822; 79.4% women, 20.6% men; median age: 57 years old). Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is the causative agent of acute respiratory coronavirus disease 2019 (COVID-19), a disease that erupted into a global pandemic in March of 2020.2 Recently, a genome wide CRISPR screen was performed on SARS-lineage coronaviruses and two additional recombinant viruses; the screen revealed host genes essential to coronavirus pathogenesis and entry.7 The screen identified KDM6A, a histone demethylase, as an important pro-viral gene.7 Building upon this observation, in this thesis, I explore the role of Kdm6a in murine hepatitis virus (MHV), a type of mouse coronavirus. Background. Streptococcus pneumoniae is a commonly found upper respiratory tract colonizer that can progress to more severe disease forms such as pneumonia. Interactions with respiratory viruses other than SARS-CoV-2 have been previously identified such that S. pneumoniae may sometimes enhance disease or occur as a secondary or co-infection. As the primary vector for the West Nile virus in the United States, members of the Culex pipiens complex have caused illnesses and deaths in the human population since its discovery in 1937. The potential contribution of Cx. pipiens to transmission and enzootic amplification of WNV can be influenced by multiple aspects, including their interactions with various vertebrate hosts as an important factor. By using blood meal analysis and virus testing, this study further investigated the role of Cx. pipiens in WNV transmission and amplification. We propose a model of how multiple societies respond to a common crisis. A government faces a “damned-either-way” policy-making dilemma: aggressive intervention contains the crisis, but the resulting good outcome makes people skeptical of the costly response; light intervention worsens the crisis and causes the government to be faulted for not doing enough. This dilemma can be mitigated for the society that encounters the crisis first if another society faces the same crisis afterward. Our model predicts that the later society does not necessarily perform better despite having more information, while the earlier society might benefit from a dynamic counterfactual …

Chemsex, defined as the use of drugs before or during sex to enhance and facilitate the experience, is increasingly prevalent in urban populations of men who have sex with men. Chemsex is associated with higher risks of sexually transmitted infections including HIV, and it has negative effects on mental health and productivity. However, chemsex is under- or un-addressed in most primary care practices, and chemsex users rarely present to specialists in addiction medicine. Measuring Movement And Social Contact With Smartphone Data: A Real-Time Application To Covid-19, Victor Couture, Jonathan L. Dingel, Allison Green, Jessie Handbury, Kevin R. Williams

Tracking human activity in real time and at fine spatial scale is particularly valuable during episodes such as the COVID-19 pandemic. In this paper, we discuss the suitability of smartphone data for quantifying movement and social contact. We show that these data cover broad sections of the US population and exhibit movement patterns similar to conventional survey data. We develop and make publicly available a location exposure index that summarizes county-to-county movements and a device exposure index that quantifies social contact within venues. We use these indices to document how pandemic-induced reductions in activity vary across people and places.

In January 2020, the first confirmed case of COVID-19 was reported in the United States, with Connecticut recording its first case by March 8. As of the time of writing, tens of thousands of Connecticut residents have tested positive for the illness, and thousands have died, with the true number of affected individuals likely being much higher. Early data indicate that Black, Latino, and Indigenous communities are experiencing disproportionately higher rates of infection and serious complications. In Connecticut, Black and Latino individuals have died at higher age-adjusted rates than white individuals, and nursing home residents account for more than half …

People who inject drugs are at increased risk of acquiring HIV and accounted for 10% of all new diagnoses in 2018. Pre-exposure prophylaxis has been shown to reduce HIV acquisition among at-risk populations, including people who inject drugs. However, pre-exposure prophylaxis is underutilized by people who inject drugs due to limited knowledge about its existence and purpose. Hospital-acquired infections are common and represent the most frequent adverse event in healthcare delivery. Hand hygiene has been identified as the most effective intervention to prevent the spread of hospital-acquired infections. Although patients have been identified as vectors, the role of increased patient hand hygiene in the reduction of hospital-acquired infections has not been determined. We propose that the addition of a patient-centered hand hygiene intervention among hospitalized adult patients on general medicine floors will result in a 30% reduction in acquisition of hospital-acquired infections over the time period of 18-months in comparison to the standard of care focused on …

Candida endocarditis is a life-threatening, opportunistic fungal infection of the endocardium. The mortality rate of Candida endocarditis is approximately 50% and has been increasing in incidence over recent years. Historically, amphotericin B and flucytosine has been considered the standard of treatment for candida endocarditis, but is limited by safety concerns of amphotericin B, primarily nephrotoxicity and hepatotoxicity. Echinocandins, such as micafungin, have demonstrated similar efficacy in other forms of invasive candidiasis with better safety profiles, but there have been no large-scale or direct comparison trials.
