Partial And Total Tonsillectomy For Pediatric Sleep-Disordered Breathing: The Role Of The Cas-15, Jacob Garn Mabey Jan 2024

Partial And Total Tonsillectomy For Pediatric Sleep-Disordered Breathing: The Role Of The Cas-15, Jacob Garn Mabey

Yale Medicine Thesis Digital Library

The Clinical Assessment Score-15 (CAS-15) is an office-based tool for assessing the riskof sleep disordered breathing (SDB), a relatively common condition in the pediatric population. Change in CAS-15 following total tonsillectomy (TT) has been shown to have a large effect size, but it is unclear how it varies following partial intracapsular tonsillectomy (PIT). Thus, the objective of the present study is to evaluate the utility of the CAS-15 score in assessing the effectiveness of PIT and how this compares to change after TT. Children ages 2-18 undergoing PIT (N=16) or TT (N=8) with or without adenoidectomy for SDB completed the …

Relationship-Centered Communication In Breast Cancer Screening: An Interdisciplinary Training Model, Patricia Aideen Ducharme Jan 2024

Relationship-Centered Communication In Breast Cancer Screening: An Interdisciplinary Training Model, Patricia Aideen Ducharme

Yale School of Nursing Digital Theses

Breast cancer screening is essential for early detection and reducing disease progression. However, its underuse contributes to late-stage diagnoses, adverse health consequences, and reduced survival rates. Research has shown that high-quality communication significantly influences women's adherence to breast cancer screening, leading to earlier detection and better health outcomes. However, many healthcare providers receive no formal communication skills training. Healthcare providers who undergo relationship-centered communication (RCC) training demonstrate stronger interpersonal relationships, which result in increased provider-patient satisfaction, more efficient healthcare teams, and improved quality of patient care. To date, most RCC studies have focused predominantly on the physician-patient relationship rather than …

Improving Outpatient Medication Reconciliation And Reducing Adverse Drug Events: Development Of A New Process, Marisa Kristine Blake-Raut Jan 2024

Improving Outpatient Medication Reconciliation And Reducing Adverse Drug Events: Development Of A New Process, Marisa Kristine Blake-Raut

Yale School of Nursing Digital Theses

AbstractImportance: Prescribed medications can pose risks to patient safety. In the United States, persons aged over 65 account for nearly 450,000 annual emergency department (ED) visits, and they are seven times more likely to require hospital admission than younger persons (Centers for Disease Control and Prevention, 2022). Medication reconciliation (MR) errors are more likely to affect those older than 65, but can be prevented by using standardized MR criteria that identify medications with higher risks for adverse drug events (ADEs) (O’Mahoney, 2020). Objective: The DNP project goal was to create and implement a risk stratified medication reconciliation (RSMR) intervention process …

Virtual Care Navigation To Reduce Disparities And Improve Quality: A Pilot Program For Colorectal Cancer Screening, Robert Freeman Jan 2024

Virtual Care Navigation To Reduce Disparities And Improve Quality: A Pilot Program For Colorectal Cancer Screening, Robert Freeman

Yale School of Nursing Digital Theses

Purpose: This DNP project piloted a virtual care navigation program for primary care patients inan urban population, aimed at improving compliance with colorectal screening and reducing disparities in access to care. Background: Despite the high prevalence and mortality, colorectal cancer screening remains low in the United States. This is especially true for patients who are uninsured, who are younger and for racial minorities. Colorectal Cancer can be largely prevented when appropriate screening is done. Digital navigation tools have demonstrated effectiveness in helping patients complete preventative health measures like cancer screening. Methods: This quality improvement project implemented a virtual navigation program …

Implementation And Evaluation Of Perinatal Educational Video Series – Postpartum Hemorrhage, Daileann Lamara Hemmings

Yale School of Nursing Digital Theses

Abstract

Implementation and Evaluation of Perinatal Educational Video Series –Postpartum Hemorrhage Per the Centers for Disease Control and Prevention (2021), 700 women die each year due to pregnancy related complications in the United States. Black women (Erickson, 2020) have a 26.6% higher risk for morbidity resulting from postpartum hemorrhage (PPH) and a five times higher risk of mortality. Education can serve as a tool to increase knowledge, promote a mother’s ability to identify the onset of a risk and report signs and symptoms of PPH that could otherwise lead to complications or death. This DNP pilot project developed and …

The California Bridge Model For Emergency Department-Initiated Medication-Assisted Treatment For Opioid Use Disorder: Adaptations For Perinatal Hospital Services, Devin C. Thomas Jan 2024

The California Bridge Model For Emergency Department-Initiated Medication-Assisted Treatment For Opioid Use Disorder: Adaptations For Perinatal Hospital Services, Devin C. Thomas

Yale School of Nursing Digital Theses

Background: Poor access to gender-specific, pregnancy-focused medication-assisted treatment for perinatal opioid use disorder greatly contributes to adverse outcomes for pregnant patients, infants, and society, at large. It is well established that Emergency Department-initiated pharmacotherapy for opioid use disorder is safe, effective, and can be rapidly adopted by hospital systems to improve access to care for people who misuse opioids. Like the Emergency Department, perinatal hospital services are well positioned to serve as critical access points in the treatment of perinatal opioid use disorder. However, concerns such as lack of training, workflow inefficiencies, and bias act as barriers to co-located, integrated …

Effect Of The Lifestyle, Exercise, And Nutrition (Lean) Weight Loss Intervention On Anxiety Among Breast Cancer Survivors, Faiad Alam Jan 2024

Effect Of The Lifestyle, Exercise, And Nutrition (Lean) Weight Loss Intervention On Anxiety Among Breast Cancer Survivors, Faiad Alam

Public Health Theses

Background: Lifestyle interventions can be an important tool in improving quality of life (QOL) among breast cancer survivors. Anxiety has been improved in some, but not all, exercise or lifestyle interventions that have investigated this component of QOL. Therefore, we sought to examine the impact of the 6-month Lifestyle, Exercise, and Nutrition (LEAN) intervention vs. usual care on anxiety among women who completed treatment for breast cancer. Methods: A total of 151 breast cancer survivors participated in the LEAN study, and 116 participants were included in this secondary analysis. State anxiety was assessed via the state anxiety portion of the …

Unveiling Chagas Disease: An In-Depth Analysis Of Epidemiological Patterns In High Burden Latin American Nations, Nicole Del Castillo

Public Health Theses

Background: Chagas Disease, endemic to 21 countries in Latin America, is transmitted by triatominae insects infected with parasites known as Trypanosoma cruzi.1–3 It is the most prevalent tropical disease in Latin America, affecting about 6 million individuals across the Americas.1 This literature review aims to summarize unique epidemiological patterns over a 9-year time period from 2010 to 2019 across three nations in South America that experienced the highest Chagas Disease burden in 2019: Bolivia, Venezuela, and Argentina.4

Methods: For this rapid literature review, one independent reviewer extracted all data for publications reviewed within a 9-year time period: 2010 to 2019. …

The Roads Less Traveled: A Metaresearch Analysis Of Local Histories In Racial Health Equity Research, Devin Trévion Brown Jan 2024

The Roads Less Traveled: A Metaresearch Analysis Of Local Histories In Racial Health Equity Research, Devin Trévion Brown

Public Health Theses

While it is important that racial health equity research receive growing attention in theUnited States, doing so must involve a thorough recognition and interrogation of the role that structural racism plays in creating different outcomes for Black communities. History aids in making clear the process through which race can become biology as structural racism is embodied by members of Black communities. Guided by Táíwò’s conceptualization of “resource distribution and accumulation” as well as Carter’s discussion of “racialized local biologies”, this cross-sectional meta-research analysis sought to investigate when and how racial health equity research after 2020 utilized local history to understand …

Prep-Aring For Prevention: Mental Health, Internalizations, And Contextual Factors As Barriers To Prep Uptake Among Sexual Minority Men And Nonbinary Individuals, Amanda Glatter

Public Health Theses

Pre-exposure prophylaxis (PrEP) has revolutionized HIV prevention in the last ten years, as it is highly effective at preventing infection. PrEP is a strong preventive option for those at risk of HIV, but only 30% of those who could benefit from it were prescribed PrEP in the United States in 2021.2 The purpose of this study is to investigate how mental health, internalizations, and greater contextual factors unique to sexual minority men and nonbinary individuals (SMMNI) impact PrEP knowledge, uptake, and adherence. Survey data from the #MVMNT (Men’s Voices on Mapping, Networks, and Technology) project was utilized to examine the …

Measuring The Differential Effect Of Internalized Homonegativity On Hiv Prevention Outcomes By Sexual Orientation Disclosure Status Among Ymsm Across Two Us Cities, Liv Gotte Jan 2024

Measuring The Differential Effect Of Internalized Homonegativity On Hiv Prevention Outcomes By Sexual Orientation Disclosure Status Among Ymsm Across Two Us Cities, Liv Gotte

Public Health Theses

Human Immunodeficiency Virus (HIV) is highly preventable with the right resources. Nonetheless, young men who have sex with men (YMSM) face disproportionately high rates of HIV acquisition, indicating barriers to effective implementation of prevention strategies. To better understand these implementation barriers, this research explores the relationship between internalized homonegativity and HIV prevention outcomes with a focus on the mediating effect of sexual orientation disclosure to healthcare professionals among YMSM. The data used in this research were collected in a previous prospective cross-sectional study that enrolled YMSM seeking HIV prevention care in two U.S. cities ( Nashville, TN and Buffalo, NY) …

Evaluation Of One Health Education Materials: A Scoping Review And Quality Assessment, Sydney Jones

Public Health Theses

The increasing risk of zoonotic spillover, driven by climate change and evolving human behaviors, requires a systematic shift in pandemic prevention strategies towards a One Health approach. Central to this approach is effective health communication intended to translate knowledge into actionable recommendations for the most vulnerable populations. This thesis aims to systematically evaluate the quantity and quality of existing One Health communication materials through a scoping review and two distinct methods of quality evaluation: the CDC Clear Communication Index and content analysis. Among 82 communication materials identified in the scoping review, the median overall CDC Clear Communication Index score was …

Representation In The Endocrinology Literature And The Effect Of Double-Blinded Review, Gal Hodish Jan 2024

Representation In The Endocrinology Literature And The Effect Of Double-Blinded Review, Gal Hodish

Public Health Theses

Background: There is an open question as to whether authorship in peer-reviewed scientific journal articles is representational. Part of this discrimination has been attributed to bias in the review process of manuscripts, with the suggestion that a double-blind review process ensures equitable outcomes due to complete deidentification of both authors and reviewers. Methods: We performed an automated review of 4,191 manuscripts published at three endocrinology journals with comparable impact factor (two single-blind journals; one double-blind journal) in the years 2010-2023; manuscripts were included in the analysis if they were published from institutions based in the United States. Gender and race/ethnicity …

Indoor Air Pollution In Nomadic Himalayan Highlander Communities, Charles Minicucci Jan 2024

Indoor Air Pollution In Nomadic Himalayan Highlander Communities, Charles Minicucci

Public Health Theses

Indoor air pollution (IAP) from burning solid biomass fuel is a major global health challenge, especially in resource-limited settings without access to electricity or reliable fuel sources. In the Kingdom of Bhutan, where, despite a reported 99% of households having access to electricity, the vast majority of rural households still use biomass fuels for cooking and indoor heating. Given the resource limitations and access challenges associated with Bhutan’s remote highlander and herding communities, few solutions for or details about their exposure to IAP are available – especially during nomadic periods where families may spend months beyond the reach of infrastructure …

Hybrid Medical Practices In Malaria Management Among Sao Tome And Principe’S Elderly, Yihan Shi Jan 2024

Hybrid Medical Practices In Malaria Management Among Sao Tome And Principe’S Elderly, Yihan Shi

Public Health Theses

Africa has the global heaviest burden of malaria. Sao Tome and Principe, a low-transmission island nation in the Gulf of Guinea with a population of about 200,000, has entered the malaria elimination phase. Despite this, the prevalence and mortality rates of malaria among the elderly in Sao Tome and Principe remain high compared to high transmission areas. This study employed a mixed-methods approach to explore the association between age and recommended malaria management strategies for adults in Sao Tome and Principe, with a focus on examining samples older than 50 years. All participants (N = 274) completed surveys, with in-depth …
