The Older Population And The Aged Patient, Ewald W. Busse Jan 1969

The Older Population And The Aged Patient, Ewald W. Busse

MCV/Q, Medical College of Virginia Quarterly

Modern medical practice requires that the physician and other members of the health team have full appreciation of the impact of disease and disability upon identifiable groups of patients and recognize how nonmedical factors affect treatment and prognosis. It is important for the physician to have some idea of the patient's socioeconomic status and the actual and potential number of patients with not only a common diagnosis, but also similarities in their living conditions. With such knowledge, the physician can be much more realistic and efficient in his therapeutic efforts. Persons 65 years of age and over are commonly referred …

Anxiety, Defense And Cognition: A Theoretical Basis For Practical Handling Of The Surgical Patient, Stanley L. Block Jan 1969

Anxiety, Defense And Cognition: A Theoretical Basis For Practical Handling Of The Surgical Patient, Stanley L. Block

MCV/Q, Medical College of Virginia Quarterly

Both the anxiety-defense model and the cognitive model provide a scientific basis for practical handling of the surgical patient. Emerging from both models is a humanistic approach to the patient based on understanding and a nonjudgmental attitude as well as on a knowledge of where the patient is psychologically and a willingness to meet him on his ground. The rewards in terms of a reduction of anxiety, depression, and human suffering can be enormous.

Sexual Counseling In Medical Practice, George Kriegman Jan 1969

Sexual Counseling In Medical Practice, George Kriegman

MCV/Q, Medical College of Virginia Quarterly

In summary, sexual relationships involve two factors: lustful gratification and emotional and physical closeness. Lustful gratification involves sexual identity which is determined by: 1) biological sex; 2) gender identity; 3) child-parent relationships; 4) cultural value systems; and 5) sexological knowledge. Sexual problems are due to ignorance of the sexual facts of life and/or to contamination of the sexual area by other needs, wishes, or fears. A detailed understanding of the presenting complaints and the sexual history of the patient will enable the physician to make a differential diagnosis. The physician can then clarify the problem and assist the patient in …

Isolated Organ Perfusion: Physiology And Application, William E. Gayle Jr. Jan 1969

Isolated Organ Perfusion: Physiology And Application, William E. Gayle Jr.

MCV/Q, Medical College of Virginia Quarterly

Isolated organ perfusion provides us with an excellent tool for investigating the physiology and the function of a variety of individual organs. It enables us to explore these organs, their pharmacologic reactions, their ability to withstand various stresses, their pathology after stress and their suitability for preservation and transplantation. Isolated perfusion has allowed us to better understand hemodynamics, effects of various perfusates, and toxicity of drugs on individual organs. We have recently been enlightened about the vital role of platelets and endothelial integrity. We now have a model for assessing individual hormonal systems and qualitative and quantitative endocrine and exocrine …

Unilateral Aphakia And Contact Lenses, Herbert Wiesinger Jan 1969

Unilateral Aphakia And Contact Lenses, Herbert Wiesinger

MCV/Q, Medical College of Virginia Quarterly

The binocular functions of a group of 60 patients with unilateral aphakia were tested after fitting with a contact lens. Results showed a high degree of extramacular fusion in the group. However, only 50% of these patients obtained satisfactory stereoacuity levels. There was no relation between the time elapsed from cataract extraction to contact lens fitting and the amount of ultimate stereoacuity obtained. Some factors presenting common obstacles to successful fitting of aphakic contact lenses are discussed.

The Role Of Preservation In Transplantation, Folkert O. Belzer Jan 1969

The Role Of Preservation In Transplantation, Folkert O. Belzer

MCV/Q, Medical College of Virginia Quarterly

Although it is too early to draw any definite conclusions from these results, as a period of two years is probably necessary to evaluate cadaver kidney transplantation in humans, it appears that our well-matched patients have much fewer problems than our poorly matched patients. It is hoped that by preservation methods such as those discussed, improved methods of tissue typing, and better immunosuppressive therapy, we will be able to raise the survival level of cadaver kidney transplantations to the level between 80 and 85% presently being obtained in transplantations using living, related donors.

Histamine And Thyroid Hormone Function, Richard W. Schayer Jan 1969

Histamine And Thyroid Hormone Function, Richard W. Schayer

MCV/Q, Medical College of Virginia Quarterly

Briefly, it will be postulated that a major function of thyroid hormone is for slowly developing adaptations of the circulatory system to environmental changes, and that many thyroid hormone effects are secondary to altered perfusion of tissues. There is no known in vitro effect of thyroid hormone which can reasonably explain any major number of the in vivo effects (Tata, 1964).

Carbon Monoxide Poisoning: Some Aspects Of The Problem In Great Britain, Keith A. Mant Jan 1969

Carbon Monoxide Poisoning: Some Aspects Of The Problem In Great Britain, Keith A. Mant

MCV/Q, Medical College of Virginia Quarterly

In Great Britain, until recently, domestic gas was made from coal and was always referred to as "coal gas." Today we are beginning to use natural gas, and the term coal gas is being replaced by "town gas." Most of the domestic supply, however, is still coal gas, which has a high though variable percentage of carbon monoxide (Mant, 1964). In any event it is always present in a highly lethal concentration. As coal gas is so readily available, it has been the most popular suicidal poison for many years, although the recent figures show that barbiturates are overtaking it …

What Referring Physicians Can Expect From The Psychiatrist, David R. Hawkins

MCV/Q, Medical College of Virginia Quarterly

I hope that I have persuaded you that the psychiatrist can be a very useful colleague to non-psychiatric physicians. He can be most useful if the referring physician works at developing a mutual learning relationship with him and keeping all lines of communication open.

Psychiatric Issues In Therapeutic Abortion, Henry D. Lederer Jan 1969

Psychiatric Issues In Therapeutic Abortion, Henry D. Lederer

MCV/Q, Medical College of Virginia Quarterly

For the purpose of this discussion I should like to focus your attention on the psychiatric involvement, questions, dilemmas, and opinions concerning therapeutic abortion. Under most of the existing statutes, therapeutic abortion is only permitted to preserve the life of the mother. Therefore, within the context of strict interpretation, the sole psychiatric indication is a high probability of suicide by the pregnant patient.

Relation Of Tissue Typing To Results Of Clinical Transplants, Hyung M. Lee Jan 1969

Relation Of Tissue Typing To Results Of Clinical Transplants, Hyung M. Lee

MCV/Q, Medical College of Virginia Quarterly

Current data reveal that, when matching is very good, there is also very good correlation in living related donor results, particularly in the sibling donor source. When there is slight mismatching, correlation is not as good; however, many patients seem to do well in spite of some mismatch. Correlation in the unrelated cadaver group is not as precise or significant. The current status of tissue typing is still not as precise as it should be, in order for us to rely on it as a selection criterion for renal homotransplantation. However, with rapidly advancing knowledge of antigen groups in the …

Investigations On Congenital And Induced Osteopetrosis, Donald G. Walker Jan 1969

Investigations On Congenital And Induced Osteopetrosis, Donald G. Walker

MCV/Q, Medical College of Virginia Quarterly

Osteopetrosis, or marble bone disease, is a disturbance of skeletal development in which the rate of bone resorption fails to keep pace with the rate of bone formation. Bone matrix accumulates excessively throughout the skeletal system, causing damage to neighboring tissues--particularly the dental, hematopoietic and nervous tissues. The line of investigation on osteopetrosis which has led to the current point of view that the thyroid gland represents the primary side of the disturbance was initiated soon after an experimental animal became available.
