The state of Florida has proposed the establishment of the dental therapy mid-level provider to alleviate the lack of access to oral health care of the underserved population. Despite the success in other states opposition exists this workforce model. The purpose of this research is to evaluate the attitudes of Florida school-based nurses regarding the development of dental therapists to address the states oral health access issues. A survey queried a random sample of 300 Florida school-based nurses regarding dental therapy and access to oral health care issues among the school age children population. Host-parasite systems exist across complex and ecologically heterogeneous landscapes, and may occur across taxonomically and ecologically disparate host species. Under these conditions, mechanisms underlying microevolutionary processes (i.e. gene flow, genetic drift) are not always clear, and may be mediated by numerous co-occurring factors specific to individual hosts. Host traits such as host immunology, demographics, phylogeny and ecology may act in concert to shape host-parasite relationships, and ultimately evolutionary processes. Avian haemosporidians are protist parasites that are distributed worldwide. We investigated these parasites in a tropical South Pacific archipelago to answer two questions: 1) Which community characteristics influence haemosporidian diversity; 2) Does host specificity vary across islands? To answer these questions we sequenced a mitochondrial gene and created linear models to establish relationships between select variables and diversity. In addition, we calculated host breadth of each lineage and constructed models to infer predictors of diversity. The percent of infected individuals on each island was correlated with island area. Background:Our institution’s rate of hospital acquired Clostridioides difficile (C. diff) infection (CDI) is higher than expected based on documented severity of illness of our patients. The C. diff testing algorithm at our institution utilizes a multi-step approach. It begins with two tests (antigen EIA and toxin A/B EIA). If the results of these two tests are indeterminate, the algorithm then reflexes to a third test (NAAT-PCR). Patients who are diagnosed with HIV but not retained in medical care have been identified as one of the highest risks for new HIV transmissions and have higher rates of all cause mortality 1,2. Introduction:

In 2018, there were 2,943 surgical site infections (SSIs) among patients who underwent abdominal hysterectomies in the U.S. and these account for substantial morbidity and increased healthcare costs. University of New Mexico Hospitals (UNMH) has had elevated rates of abdominal hysterectomy SSIs compared with national benchmarks. One of these benchmarks includes the Standardized Infection Ratios (SIRs). UNMH’s SIR is often >1 indicating more infections than predicted. In response, a multidisciplinary team created and then implemented a bundle to prevent SSIs. Our objectives were to assess SSI bundle compliance and its impact on SSI rates and numbers.

BACKGROUND: Although the association between diabetes mellitus (DM) and tuberculosis (TB) has been well-documented for centuries, evidence of the link between diabetes and drug resistance among previously treated TB patients remains limited and inconsistent.

METHODS: An observational study was performed that involved 1791 retreated TB-no DM patients (refers to TB cases without diabetes) and 93 retreated TB-DM patients (refers to TB cases with diabetes) in Shandong, China from 2004 to 2017. Baseline data including demographic and clinical characteristics, drug susceptibility test (DST) results, and diabetes status were collected. Understanding case identification practices, protocols, and training needs of medical examiners and coroners (MEC) may inform efforts to improve cause-of-death certification. We surveyed a U.S.-representative sample of MECs and described investigation practices and protocols used in certifying sudden unexpected infant deaths (SUID). We also identified MEC training and resource needs. Of the 377 respondents, use of the SUID Investigation Reporting Form or an equivalent was 89% for large, 87% for medium, and 52% for small jurisdictions. Routine completion of infant medical history, witness interviews, autopsy, photos or videos, and family social history for infant death investigations was ≥80%, but routine …

OBJECTIVE: Subtherapeutic vancomycin trough concentrations are common in children and may be associated with suboptimal therapeutic response. Our objective was to determine if vancomycin loading doses safely increase the frequency of target trough attainment in hospitalized children.

METHODS: Patients (≥6 months and15 mg/dL for invasive infections and >10 mg/dL for non-invasive infections.

RESULTS: A total of 151 patients were enrolled, with 77 in the control arm and 74 in the loading dose arm. There was no significant difference in the frequency of comorbidities or need for intensive care unit admission between the two arms. Those receiving a vancomycin loading dose …

The increased incidence of opioid use during pregnancy warrants investigation to reveal the impact of opioid exposure on the developing fetus. Exposure during critical periods of development could have enduring consequences for affected individuals. Particularly, evidence is mounting that developmental injury can result in immune priming, whereby subsequent immune activation elicits an exaggerated immune response. This maladaptive hypersensitivity to immune challenge perpetuates dysregulated inflammatory signaling and poor health outcomes. As part of the State Targeted Response to the opioid epidemic, California has adopted the Hub and Spoke model to expand access to medications for opioid use disorder, particularly buprenorphine, throughout the state. By aligning opioid treatment programs as hubs with primary care, office-based practitioners, and other health care settings as spokes, a broader treatment model can reach more people with opioid use disorder, improve access to medications for opioid use disorders, and decrease overdose deaths. The three main causes of vaginitis are bacterial vaginosis (BV), vulvovaginal candidiasis (VVC), and trichomoniasis (TV). Two multiplex assays are commercially available for detection of DNA from organisms associated with vaginitis: BD Affirm™ VPIII Microbial Identification Test (Affirm) and BD MAX™ Vaginal Panel (MAX VP). Here, the performance of MAX VP was compared to that of Affirm, which was considered the standard of care. We evaluated the performance of the Luminex ARIES® C. difficile Assay on 984 stool specimens prospectively collected from patients being tested for CDI at 4 clinical laboratories in the United States. Results were compared to direct and enriched toxigenic culture. Positive percent agreement (PPA) of the ARIES® C. difficile Assay was 98.1% versus direct toxigenic culture, and sensitivity versus direct plus enriched toxigenic culture was 90.5%. Negative percent agreement (NPA) of the ARIES® C. difficile Assay against direct culture was 92.6%, and specificity versus direct plus enriched toxigenic culture was 95.8%. BACKGROUND: Programs for high-need, high-cost (HNHC) patients can improve care and reduce costs. However, it may be challenging to implement these programs in rural and underserved areas, in part due to limited access to specialty consultation.

AIM: Evaluate the feasibility of using the Extension for Community Health Outcomes (ECHO) model to provide specialist input to outpatient intensivist teams (OITs) dedicated to caring for HNHC patients.

However, a truly multipurpose laser therapy method is very rarely available. Here we introduce a multipurpose laser therapy device in Ukraine, which is capable to take care a multitude of diseases. It is called “Helios”, by one of us (VK). We also give a case where a patient who suffered from Covid-19 has been treated successfully until he is recovered to healthy condition. In the last section we also discuss potential future application of Helios for other fields; i.e. neurology disorders.

West Virginia (WV) is situated at the epicenter of the opioid epidemic with the highest rates of overdose deaths and some of the lowest rates of access to life saving evidence-based medication assisted treatment (MAT) for patients with opioid use disorder (OUD). With the goal of eliminating hepatitis C virus (HCV) as a public health problem in Washington State, Public Health–Seattle & King County (PHSKC) designed a Hepatitis C Virus Test and Cure (HCV-TAC) data system to integrate surveillance, clinical, and laboratory data into a comprehensive database. The intent of the system was to promote identification, treatment, and cure of HCV-infected persons (ie, HCV care cascade) using a population health approach.

The fundamental currency of life is energy. Organisms need energy to grow, to survive and to reproduce. Understanding the acquisition of energy by consumers is thus a foundational aspect of biological research. This is especially important in the modern era, as impacts of ongoing anthropogenic effects will be mediated or amplified through food webs. Here, I explore how isotopic analysis of individual amino acids – a technique new to ecological studies – can be used to trace energy flow through animal communities in modern and ancient time periods. BACKGROUND: Extension for Community Healthcare Outcomes (ECHO) and related models of medical tele-education are rapidly expanding; however, their effectiveness remains unclear. This systematic review examines the effectiveness of ECHO and ECHO-like medical tele-education models of healthcare delivery in terms of improved provider- and patient-related outcomes.

METHODS: We searched English-language studies in PubMed, Embase, and PsycINFO databases from 1 January 2007 to 1 December 2018 as well as bibliography review. Two reviewers independently screened citations for peer-reviewed publications reporting provider- and/or patient-related outcomes of technology-enabled collaborative learning models that satisfied six criteria of the ECHO framework.
