BACKGROUND: Patients 65 years old and older largely represent (>50%) hospital-admitted patients with acute coronary syndrome (ACS). Data are conflicting comparing efficacy of early routine invasive (within 48-72 hours of initial evaluation) versus conservative management of ACS in this population.
OBJECTIVE: We aimed to determine the effectiveness of routine early invasive strategy compared to conservative treatment in reducing major adverse cardiovascular events in patients 65 years old and older with non-ST elevation (NSTE) ACS.
DATA SOURCES: We conducted a systematic review of randomized controlled trials (RCTs) through PubMed, Cochrane, and Google Scholar database.

Heparin-induced thrombocytopenia (HIT) can occur following exposure to heparin and is characterized by thrombocytopenia with increased risk for thrombosis. This condition is mediated by formation of immunoglobulin G antibodies against platelet factor 4/heparin complexes that can subsequently lead to platelet activation. Herein, we detail the clinical and laboratory findings, treatments, and outcomes of two patients who developed HIT and thrombosis after undergoing collection of hematopoietic progenitor cells by apheresis (HPC-A) for autologous HPC transplant. We evaluated the performance of the Luminex ARIES® C. difficile Assay on 984 stool specimens prospectively collected from patients being tested for CDI at 4 clinical laboratories in the United States. Results were compared to direct and enriched toxigenic culture. Positive percent agreement (PPA) of the ARIES® C. difficile Assay was 98.1% versus direct toxigenic culture, and sensitivity versus direct plus enriched toxigenic culture was 90.5%. Negative percent agreement (NPA) of the ARIES® C. difficile Assay against direct culture was 92.6%, and specificity versus direct plus enriched toxigenic culture was 95.8%. Hematopoietic stem and progenitor cells (HSPCs) are responsible for the development, maintenance, and regeneration of all the blood forming cells in the body, and as such, are critical for a number of patient therapies. For successful HSPC transplantation, stem cells must traffic through the blood and home to the bone marrow (BM) microenvironment or "niche," which is composed of soluble factors, matrix proteins, and supportive cells. HSPC adhesion to, and signaling with, cellular and extracellular components of the niche provide instructional cues to balance stem cell self-renewal and differentiation. Objective: Buprenorphine, a medication for opioid use disorder, remains vastly underutilized despite its proven efficacy. This study sought to evaluate which strategies, within a system that employed a variety of concurrent strategies, effectively increased access to buprenorphine.
Methods: Over the course of 15 months, 25 federally qualified health centers were invited to participate in four commonly used implementation strategies. This study examines the impact of clinic attendance at strategy events on change in numbers of patients prescribed buprenorphine and numbers of buprenorphine-waivered providers by clinic.
Results: There was a nearly three-fold increase (2.84) in patients on buprenorphine and two-fold increase …

BACKGROUND: Programs for high-need, high-cost (HNHC) patients can improve care and reduce costs. However, it may be challenging to implement these programs in rural and underserved areas, in part due to limited access to specialty consultation.
AIM: Evaluate the feasibility of using the Extension for Community Health Outcomes (ECHO) model to provide specialist input to outpatient intensivist teams (OITs) dedicated to caring for HNHC patients.
SETTING: Weekly group videoconferencing sessions that connect multidisciplinary specialists with OITs.

However, a truly multipurpose laser therapy method is very rarely available. Here we introduce a multipurpose laser therapy device in Ukraine, which is capable to take care a multitude of diseases. It is called “Helios”, by one of us (VK). We also give a case where a patient who suffered from Covid-19 has been treated successfully until he is recovered to healthy condition. In the last section we also discuss potential future application of Helios for other fields; i.e. neurology disorders.

Plithogenic n – Super Hypergraph introduced by Smarandache is a novel concept and it involves many attributes. This article aims to bridge the concept of Plithogenic n-Super Hypergraph in the vicinity of optimal decision making. This research work introduces the novel concepts of enveloping vertex, super enveloping vertex, dominant enveloping vertex, classification of the dominant enveloping vertex (input, intervene, output dominant enveloping vertices), plithogenic connectors. Throughout the United States dams are approaching the terminus of their original licensing periods and are undergoing re-licensing by the Federal Energy Regulatory Commission. This period of review has prompted extensive studies in these basins to determine the cost-benefits associated with keeping these dams, versus removing and restoring the natural ecosystems that are currently inundated. Climate change is a multi-generational problem, but it does not impact all generations in the same way. Correspondingly, older Americans and younger ones differ greatly in how they perceive the issue and how they respond. The wave of youth activism epitomized by Greta Thunburg is on one side of this generation gap. Donald Trump’s climate skepticism is on the other. This is supplementary material for the forthcoming publication entitled “Overlapping roles of NADPH Oxidase 4 (Nox4) for diabetic and gadolinium-based contrast agent-induced systemic fibrosis”. This supplementary material utilizes a model for gadolinium retention and renal elimination of pharmaceuticals, interspecies allometric scaling to determine animal equivalent dosing, label dosing and published studies of varying and repetitive dosing of gadolinium-based contrast agents used to determine dosing of gadolinium-based contrast agents in our preclinical model. Supplementary Figure 1 demonstrates modeling plasma gadolinium-based contrast agent concentrations based on the Hirano Suzuki equation.

New Deal cooperatives succeeded in electrifying rural America when for-profit utilities would not. Today, however, rural electric cooperatives are lagging behind when it comes to meeting the challenge of climate change. Cooperatives have collectively been slower to embrace the shift to low-carbon electricity than for-profit and municipal utilities and have served as a drag on state and federal clean energy and climate policies. This is partially because of the structural differences between cooperatives and other utilities, but also because of a weak and under-determined federal and state regulatory structure. West Virginia (WV) is situated at the epicenter of the opioid epidemic with the highest rates of overdose deaths and some of the lowest rates of access to life saving evidence-based medication assisted treatment (MAT) for patients with opioid use disorder (OUD). WV used a modified hub-and-spoke model to build organizational capacity for facilities to use buprenorphine to treat patients with OUD and to provide ongoing case consultation.
