Background: The trauma intensive care unit (ICU) at University of Kentucky (UK) Healthcare uses a ventilator separation protocol which provides specific guidelines on the successful weaning of patients from mechanical ventilation. However, many of the nurses are not aware of this protocol as formal education is not included in their orientation or training. This lack of knowledge can lead to risks such as increased patient morbidity, mortality, ICU length of stay, and healthcare-related costs. Research shows that using structured mechanical ventilator weaning tools in the ICU can shorten the duration of mechanical ventilation, which in turn, lowers these associated risks. …

Abstract

Background: Minimally invasive esophagectomy surgery is commonly performed by thoracic surgeons on patients with esophageal cancer. The post-operative care of these patients is meticulous and follows a protocol developed by the thoracic surgeons at the University of Kentucky. At UK Chandler Medical Center, concerns have been raised regarding nurses’ lack of education of the protocol, and therefore decreased implementation. Increasing nursing knowledge pertaining to this protocol by performing educational in-services periodically and standardizing and implementing formal education as part of nursing orientation will decrease the knowledge gap and improve patient outcomes and increase utilization of the post-operative protocol.

Background: In the United States, it is estimated that there are more than 2 million cases of Hepatitis C (HCV), with over 66,000 new cases since 2020 However, an estimated 75% of those with HCV go undiagnosed and less than 35% receive treatment within a year of diagnosis. In 2018, UK Healthcare (UKHC) implemented a universal HCV screening method using a Best Practice Advisory (BPA) for anyone 18 and older who presented to the Emergency Department and had labs ordered. Background: Adherence to diet is a challenging part of managing type 2 diabetes mellitus (T2DM). Guidelines recommend including a registered dietician and/or a diabetes educator. While this is the gold standard, there are barriers to implementation in primary care. The Starting the Conversation (STC) diet recall tool can aid primary care providers (PCPs) in providing brief nutrition counseling for patients with T2DM. Background: The benefits of breastfeeding for both mother and baby are known. It is recommended that all infants be exclusively breastfed until six months (Meek & Noble, 2022; North et al., 2021). Despite this, breastfeeding rates remain low, especially in medically fragile infants requiring care in the neonatal intensive care unit (NICU) (Rollins et al., 2016). Receipt of evidence-based breastfeeding support by healthcare providers has been shown to improve breastfeeding success; however, many healthcare providers report not being comfortable or knowledgeable enough to offer support consistently (Mulcahy et al., 2022; Rollins et al., 2016).

A two-year field experiment (2022-2023) was conducted at the University of Kentucky Horticulture Research Farm in Lexington, KY to study the effects of potassium (K2O) fertilization rates on tomato yield and ripening disorders in determinate and indeterminate varieties (‘BHN-589’ and ‘Big Beef’, respectively). In the first year, K2O rates representing the soil test result-based commercial recommendation (0 lbs/acre) and sequentially greater rates (150, 300, and 450 lbs K2O/acre) were applied. The objective of the alfalfa survey was to determine the role of soil fertility in the alfalfa yield plateau. Soil samples at depths of 10, 15, and 30 cm and tissue samples were collected from 114 alfalfa stands across Kentucky and surrounding states. Results indicate that soil fertility is likely contributing but not driving the alfalfa yield plateau. The main objective of the polyhalite incubation study is to determine if POLY4 was soluble enough to support potassium (K) and sulfur (S) for plant growth. Maize stalk lodging reduces grain yields due to the permanent breakage or deformation of stalk tissue before grain harvest. To reveal new insights into the complex problem of maize stalk lodging, this dissertation will examine three primary objectives. First, the direct, diurnal influence of turgor pressure will be shown to have only marginal effects on stalk bending strength, a key trait associated with stalk lodging resistance. Previous studies of the benefits of silicon (Si) supplementation in turfgrass have yielded inconsistent results. This dissertation aimed to assess the benefits if Si supplementation in cool-season turfgrass under more well controlled conditions than many previous studies. The first study assessed the effect of Si supplementation on biomass production and Si accumulation of tall fescue (Festuca arundinacea) and bentgrass (Agrostis stolonifera) in buffered Hoagland’s media (pH 6) with four sodium silicate (Na2SiO3) treatments (0, 0.5, 1, 2, and 4 mM Si). The objective of this study was to determine the dose-dependent response of one-carbon metabolite (OCM: methionine, choline, folate, and vitamin B12) supplementation on heifer dry matter intake on fixed gain, organ mass, hematology, cytokine concentration, pancreatic and jejunal enzyme activity, and muscle hydrogen peroxide production. Zinc (Zn) supplementation has proved to mitigate the effects of heat stress with varying effects evident with Zn source during acute heat events. However, the effects of Zn supplementation during long-term summer weather patterns have yet to be explored. Therefore, the objective of this study was to identify the effects of supplementation source and level of Zn to mitigate the negative effects of long-term, cyclic heat stress in finishing swine. A total of 425 weaned pigs (Exp. 1: 225 pigs [5.8 ± 0.9 kg]; Exp. 2: 200 pigs [6.1 ± 1.2 kg]) were used to determine the optimal dietary standardized ileal digestible (SID) arginine (Arg) level in early nursery diets based on growth and health responses. The basal diet in Exp.1 was formulated to meet SID Arg recommendation (0.66%; NRC, 2012) and in Exp. 2, SID Arg was set to simulate current industry practices for feeding nursery pigs (1.15 %). Egg whites contain a variety of biologically active proteins and are a good source of bioactive peptides. Egg white hydrolysate prepared by thermoase (EWH) possesses antihypertensive effects. Our recent study showed that egg white proteins, after depleting ovotransferrin, also contained many angiotensin-converting enzyme (ACE)-inhibitory peptides after hydrolysis using thermoase. In the present study, we aimed to explore the antihypertensive effect of the ovotransferrin-depleted egg white hydrolysate prepared by thermoase (OD-EWH) in spontaneously hypertensive rats, using EWH as the reference. The effects of different concentrations of ε-Poly-lysine (ε-PL: 0.005%, 0.01%, 0.02%, 0.04%, and 0.06%) on the marine fish-egg white protein compound gels, treated with 0.4% TGase induced cross-linking were systematically investigated under low salt and phosphorus-free conditions (0.5% NaCl). The results showed the combination of ε-PL and TGase had a synergistic effect on improving sectional gel properties of composite surimi samples. Nicotine replacement therapy (NRT) is a safe, efficacious smoking cessation treatment for cancer patients; however, equitable access is limited due to insurance gaps. Offering free NRT to patients with cancer has potential to increase tobacco treatment engagement and promote equity. This mixed-methods study aims to describe how some Cancer Center Cessation Initiative (C3I) sites implemented free NRT within their tobacco treatment programs (TTPs) to inform future development of equitable TTPs. Background: Clinicians and researchers often assume that symptom burden is associated with self-care management (SCM) in patients with heart failure (HF). However, that association is often not borne out in simple regression analyses and may be because another variable mediates the association. Fatalism is an appropriate candidate for mediation and is the belief that circumstances are predetermined without opportunity for control by individuals.

Objective: Our objective was to determine whether fatalism mediated the relationship of symptom burden with SCM among adults with HF.

Introduction: Residents of Appalachia experience elevated rates of morbidity and mortality compared to national averages, and these disparities are associated with inequitable exposures to various determinants of population health. Social and environmental determinants of health are a useful lens through which to develop and evaluate programs to mitigate regional health disparities.

Methods: This 2023 scoping review was conducted of studies linking determinants of Appalachian health with leading causes of regional mortality and morbidity. Crabgrass (Digitaria spp.) is an annual summer grass that can provide high-quality forage, but optimal management strategies are unclear. Our objective was to compare the yield and quality of crabgrass (Mojo and Quick-N-Big) under different nitrogen rates and harvest management. The experimental design was a randomized complete block with five treatments and three replications for each crabgrass variety, totaling fifteen experimental units for both Mojo and Quick-N-Big, in adjacent sites. The objective of this study was to evaluate the chemical composition and microbial diversity of the silage of forage peanut harvested at 40 and 80 days of regrowth and treated with microbial inoculants. The inoculants evaluated were commercial inoculant (CI) Sil-All 4×4 (Lallemand®, Brazil); the AV14.17, Lactiplantibacillus pentosus strain (ISO) isolated from alfalfa silage; and control (CTRL), without inoculant. A total of 500 g of forage was ensiled in nylon-polyethylene bags (25.40 × 35.56 cm). In grass-legume pastures, 2,4-D-amine + florpyrauxifen-benzyl controls broadleaf weeds but red clover (Trifolium pratense L.) is eliminated. Grazing within a week prior to herbicide application is likely to reduce leaf surface area and may reduce red clover injury and maintain productivity. Experiments were conducted in 2020 and in 2021 to determine if red clover could be productive when 560 g ae ha-1 2,4-D-amine + 6.3 g ae ha-1 of florpyrauxifen-benzyl with 1% v/v methylated seed oil applied in the spring was grazed before or after herbicide application. The ability of breeders to select for traits that impart adaptability to climate resilience will be critical for the future of grasslands. Near Infra-red Spectroscopy (NIRS) is an attractive option for forage analysis. NIRS is less labor intensive, nondestructive, rapid, environmentally friendly and provides accurate and precise results. However, many nutritionists are quick to brush off NIRS, citing ‘poor accuracy’. Teosinte is highly admired by the Nepalese farmers as a summer forage crop. It is also an important crop for milking animals in the tropical world where dairy husbandry is practiced. The only Sirsa cultivar of teosinte currently grown in Nepal takes 8 to 9 months from sowing to seed harvesting, which makes growing this crop difficult for farmers with small land holdings wanting to establish winter crops. Therefore, a diversity study on eighteen teosinte accessions was conducted in CRBD design. The potato leafhopper [PLH, Empoasca fabae (Harris)], which affects several crops including alfalfa (Medicago sativa L.), is a recurrent problem in several regions of Quebec. The objective was to evaluate alfalfa management tools in order to reduce yield losses caused by this pest. An experiment was conducted at two sites in Quebec over three field seasons to evaluate the impact of insecticide applications and the use of PLHtolerant cultivars on forage yield and PLH populations. The low ratio of sugars (S) to crude proteins (CP) in alfalfa (AL, Medicago sativa L.) leads to inefficient use of nitrogen by ruminants. The objective was to determine if adding red clover (RC, Trifolium pratense L.) or birdsfoot trefoil (BT, Lotus corniculatus L.) with or without a grass species to AL improved the forage S/CP ratio. A field trial on Co4 was conducted at ICAR-IVRI, Izatnagar during 2013-14 to 2017-18 to study the effect of harvesting season/time on green, dry matter yield and morphological parameters. Data shows that yield of Co4 was increased from first year 2013-14 to third year 2015-16 and decreased in fourth year of plantation of crop. Regrowth of ratoon crop was good as it was in first year crop, it may be concluded that crop can be grown upto 5-6 years. The crop yield of first year and second year as par with fifth year. Interest in ‘AU-Grazer’ sericea lespedeza (Lespedeza cuneata) as an anti-parasitic bioactive forage has been growing in the United States, but the concentration and bioactivity of tannins and other phenolics from additional accessions of L. cuneata, as well as other Lespedeza species, have not been evaluated. A study was completed to determine the concentration of extractable condensed tannins (ECT), total CT (TCT), total phenolics (TP), and protein-precipitable phenolics (PPP) of 32 accessions of L. cuneata and 16 additional Lespedeza species from a germplasm collection. Introduction: Assessing how well a hospital adheres to the Ten Steps to Successful Breastfeeding is the key to outlining necessary modifications in mother breastfeeding support. This study aimed to assess Latinx mothers’ perception of how well a hospital adheres to the Ten Steps to Successful Breastfeeding and its influence on exclusive breastfeeding (EBF) rates at hospital discharge.
