Department of Emergency Medicine Faculty Papers

BACKGROUND: Data are limited regarding how to effectively and efficiently identify patient priorities for research or clinical care. Our goal was to compare the comprehensiveness and efficiency of group concept mapping (GCM), a group participatory method, to interviews for identifying patient goals when seeking care.

METHODS: We engaged patients with moderately- to poorly-controlled diabetes mellitus in either GCM or an individual interview. The primary outcome was the comprehensiveness of GCM brainstorming (the first stage of GCM) as compared to interviews for eliciting patient-important outcomes (PIOs) related to seeking care. Secondary outcomes included 1) comprehensiveness of GCM brainstorming and interviews compared …

Sigma-1 receptors (Sig-1Rs) are integral ER membrane proteins. They bind diverse ligands, including psychoactive drugs, and regulate many signaling proteins, including the inositol 1,4,5-trisphosphate receptors (IP3Rs) that release Ca2+ from the ER. The endogenous ligands of Sig-1Rs are unknown. Phospholipase D (PLD) cleaves phosphatidylcholine to choline and phosphatidic acid (PA), with PA assumed to mediate all downstream signaling. We show that choline is also an intracellular messenger. Choline binds to Sig-1Rs, it mimics other Sig-1R agonists by potentiating Ca2+ signals evoked by IP3Rs, and it is deactivated by metabolism. Receptors, by stimulating PLC and …

HLA typing in solid organ transplantation (SOT) is necessary for determining HLA-matching status between donor-recipient pairs and assessing patients' anti-HLA antibody profiles. Histocompatibility has traditionally been evaluated based on serologically defined HLA antigens. The evolution of HLA typing and antibody identification technologies, however, has revealed many limitations with using serologic equivalents for assessing compatibility in SOT. The significant improvements to HLA typing introduced by next-generation sequencing (NGS) require an assessment of the impact of this technology on SOT. We have assessed the role of high-resolution 2-field HLA typing (HR-2F) in SOT by retrospectively evaluating NGS-typed pre- and post-SOT cases. HR-2F …

The impact of occupational therapy interventions focused on stress and anger management, sleep hygiene, anxiety and depression management, relapse prevention for addiction, vocational and academic skill building, money management, healthy eating, and relationship and leisure skill development. Participant satisfaction and performance scores regarding individual goal setting and attainment was collected and analyzed via retrospective review of pre/post outcome scores (n = 44) obtained through the Canadian …

Writing For Nonprofits In A Professionally-Oriented Institution: Using Rhetorical Genre Studies To Teach Flexibility, Kathryn J. Gindlesparger Jan 2019

Writing For Nonprofits In A Professionally-Oriented Institution: Using Rhetorical Genre Studies To Teach Flexibility, Kathryn J. Gindlesparger

College of Humanities and Sciences Faculty Papers

Teaching rhetorical flexibility within a nonprofit environment to professionally-oriented students can be challenging because the seemingly transactional genres of nonprofit communication, such as grant applications,do not appear to invite improvisation. This genre analysis assignment from a Writing for Nonprofits course asks students to reflect on the intersections oftheir own values as emerging communications professionals and the rhetoricalchoices they made while writing in a nonprofit genre of their choice. To complete the assignment described here, students created a “personal code”that describes their professional values and used the code to write a genreanalysis that examines the rhetorical choices made in a nonprofit …

Wills Eye Hospital Papers

Purpose: To detect visual field (VF) progression by analyzing spatial pattern changes.

Methods: We selected 12,217 eyes from 7360 patients with at least five reliable 24-2 VFs and 5 years of follow-up with an interval of at least 6 months. VFs were decomposed into 16 archetype patterns previously derived by artificial intelligence techniques. Linear regressions were applied to the 16 archetype weights of VF series over time. We defined progression as the decrease rate of the normal archetype or any increase rate of the 15 VF defect archetypes to be outside normal limits. The archetype method was compared with mean …

De-Stigmatizing Mental Illness Through Graphic Medicine, Megumi Tsuda Jan 2019

De-Stigmatizing Mental Illness Through Graphic Medicine, Megumi Tsuda

Phase 1

Graphic medicine - or the communication of health-related narratives through images and texts, such as comics - has been increasingly recognized as a powerful educational tool. My project investigates the value of integrating graphic medicine to medical education, specifically to improve mental health literacy and de-stigmatize mental illness. As a medical student, I have encountered several instances where fellow medical students and even doctors casually throw around diagnostic terminology, especially those used to describe mental health disorders, in a colloquial and insensitive manner that seem to indicate that they do not take mental illness seriously, or even ignorant of what …

Alexander Koppel: Pioneer - Physician - Provider, Max Koppel

Jefferson Biographies

Alexander Koppel was born to immigrant parents on the Lower East Side of New York City in 1905. Early in his life, his mother was forced to return to Austria to her tenant farmer parents with Alexander and his two sisters because of a severe downturn in the American economy. A few years later, his mother brilliantly sensed the upcoming disastrous World War in 1913, and returned with the three children to Wilmington, Delaware where by that time, her husband, Samuel Koppel, had established the Wilmington Window Cleaning Company.

Alexander Koppel seized the opportunity for higher education made available to …

Cardeza Foundation for Hematologic Research

Though congenital hydrocephalus is heritable, it has been linked only to eight genes, one of which is MPDZ Humans and mice that carry a truncated version of MPDZ incur severe hydrocephalus resulting in acute morbidity and lethality. We show by magnetic resonance imaging that contrast medium penetrates into the brain ventricles of mice carrying a Mpdz loss-of-function mutation, whereas none is detected in the ventricles of normal mice, implying that the permeability of the choroid plexus epithelial cell monolayer is abnormally high. Comparative proteomic analysis of the cerebrospinal fluid of normal and hydrocephalic mice revealed up to a 53-fold increase …

The Complex Affective Scene Set (Compass): Solving The Social Content Problem In Affective Visual Stimulus Sets, Mariann R. Weierich, Olena Kleshchova, Jenna Rieder K. Rieder, Danielle M. Reilly

College of Humanities and Sciences Faculty Papers

Social information, including faces and human bodies, holds special status in visual perception generally, and in visual processing of complex arrays such as real-world scenes specifically. To date, unbalanced representation of social compared with nonsocial information in affective stimulus sets has limited the clear determination of effects as attributable to, or independent of, social content. We present the Complex Affective Scene Set (COMPASS), a set of 150 social and 150 nonsocial naturalistic affective scenes that are balanced across valence and arousal dimensions. Participants (n = 847) rated valence and arousal for each scene. The normative ratings for the 300 …

A Systemic Approach To The Development And Improvement Of Trauma Education And Training, Yoon Suh Moh Jan 2019

A Systemic Approach To The Development And Improvement Of Trauma Education And Training, Yoon Suh Moh

Counseling and Behavioral Health Faculty Papers

A number of individuals are exposed to adversity (e.g., child abuse and neglect, parental substance abuse, mental illness, incarceration) early in life (Felitti et al., 1998; Centers for Disease Control [CDC] and Prevention, 2019). Moreover, over 70% of individuals worldwide will experience a traumatic event at some point in their lives (Benjet et al., 2016). Mounting research has documented that the effects of trauma exposure in childhood are not only immediate but also enduring, leading to lifelong health problems. At the same time, studies have suggested that there is a lack of systemic trauma education and training. This article will …

The last decade has seen a sharp increase in the number of scientific publications describing physiological and pathological functions of extracellular vesicles (EVs), a collective term covering various subtypes of cell-released, membranous structures, called exosomes, microvesicles, microparticles, ectosomes, oncosomes, apoptotic bodies, and many other names. However, specific issues arise when working with these entities, whose size and amount often make them difficult to obtain as relatively pure preparations, and to characterize properly. The International Society for Extracellular Vesicles (ISEV) proposed Minimal Information for Studies of Extracellular Vesicles (“MISEV”) guidelines for the field in 2014. We now update these “MISEV2014” guidelines …

MicroRNAs, non-coding regulators of gene expression, are likely to function as important downstream effectors of many transcription factors including MYB. Optimal levels of MYB are required for transformation/maintenance of BCR-ABL-expressing cells. We investigated whether MYB silencing modulates microRNA expression in Philadelphia-positive (Ph+) leukemia cells and if MYB-regulated microRNAs are important for the "MYB addiction" of these cells. Thirty-five microRNAs were modulated by MYB silencing in lymphoid and erythromyeloid chronic myeloid leukemia-blast crisis BV173 and K562 cells; 15 of these were concordantly modulated in both lines. We focused on the miR-17-92 cluster because of its oncogenic role in tumors and found …

Health Care Evolves From Reactive To Proactive., Scott A. Waldman, Andre Terzic

Department of Pharmacology and Experimental Therapeutics Faculty Papers

Decoding health and disease pathways drives healthcare evolution. Historically, therapeutic paradigms have relied on interventions that mitigate symptoms of established diseases. Increasingly, molecular insights into pathophysiology now provide unprecedented opportunities to offer curative solutions or even prevent disease and thereby secure longitudinal wellness. These opportunities extend past individual patients to entire populations and geographies. Moreover, they optimize prospective healthspan across lifespan. Linking discovery science and its translatable innovations beyond reactive disease intervention to proactive prevention will maximize society’s returns creating the greatest benefit for the greatest number of people globally.

3d Printed Biliary Anatomy For Surgical Planning, Jason Core, Erik Massenzio, Tamar Wasserman, Kevin Xie

Phase 1

Background: Studies demonstrated that 3D-printed livers from CT or MRI data can be accurate models of actual patient anatomy. However, it has yet to be established if 3D printing offers improvements to clinical outcomes in surgery. This project seeks to optimize these applications for use at Jefferson by producing 3D-printed models from patient CT scans to guide liver resection surgeries.

Methods: A liver transplant attending was interviewed about challenges encountered during hepatectomies. A publicly available abdominal computed tomography scan was used to render a liver and its vasculature in 3DSlicer. The liver surface was cut into two halves in MeshMixer …

Portable Pelvic Exam Positioning Device For Enhanced Workflow And Patient Comfort, Hannah Levy, Elliott Perow, Sean Haynie Jan 2019

Portable Pelvic Exam Positioning Device For Enhanced Workflow And Patient Comfort, Hannah Levy, Elliott Perow, Sean Haynie

Phase 1

Background: Pelvic exams are often performed in settings other than designated obstetrics and gynecology suites. In these other settings, the equipment that is traditionally used to position the patient, such as a bed fitted with stirrups, may not be available. Therefore, physicians often improvise a solution to elevate the pelvis, such as using inverted bedpans, pillows or blankets. However, the lack of an appropriate device to position patients decreases patient and physician comfort, causes workflow inefficiency and leads to suboptimal examination due to inadequate provider visualization.

Methods: Over the course of five months, input from patients and providers alike was …

Improving Midstream Urine Collection For Urine Culture Through The Use Of A Novel Device, Flipcatch, Christopher Neely, Dante Varotsis, Helen Xu

Phase 1

Background : A midstream, clean-catch urine sample is the gold standard for diagnosing urinary tract infections and determining treatment. The current method of collection is not standardized, frustrating both patients and providers. The current method results in a high rate of contaminated samples (30-40%), which often need to be repeated in order to obtain a diagnosis and antibiotic susceptibility data. A design project was conducted in order to create a more effective urine collection device with the goals of decreasing contamination and improving patient and provider experience.

Methods : We conducted interviews of patients, allied health professionals, physicians, and clinical …

Cross-Sectional Analysis Of Sound Levels In The Neonatal Intensive Care Unit (Nicu) At Thomas Jefferson University Hospital, Caroline Komlo, Martin Morris, Robert Ries, Michael Velez Jan 2019

Cross-Sectional Analysis Of Sound Levels In The Neonatal Intensive Care Unit (Nicu) At Thomas Jefferson University Hospital, Caroline Komlo, Martin Morris, Robert Ries, Michael Velez

Phase 1

Introduction . Infants in the NICU are considered at greater risk of developmental delay. It is now known that excessively loud noise can have a negative impact on parameters such as blood pressure, breathing, heart beat and oxygen saturation. Previous research has concluded that the optimal decibel (dB) level for proper growth of neonate hair cells rests around 45dB. Consequently, the American Academy of Pediatrics recommends that noise levels in the NICU be maintained to a maximum of 45dBA. However, little research has focused on designing new noise-altering products and their impact on neonatal outcomes.

Methods. This was a cross …

Center for Translational Medicine Faculty Papers

BACKGROUND/AIMS: Diabetic cardiomyopathy (DCM) is characterized by structural and functional alterations that can lead to heart failure. Several mechanisms are known to be involved in the pathogenesis of DCM, however, the molecular mechanism that links inflammation to DCM is incompletely understood. To learn about this mechanism, we investigated the role of inflammatory serine proteases (ISPs) during the development of DCM.

METHODS: Eight weeks old mice with deletion of dipeptidyl peptidase I (DPPI), an enzyme involved in the maturation of major ISPs, and wild type (WT) mice controls were injected with streptozotocin (50 mg/kg for 5 days intraperitoneally) and studied after …

The Healing Power Of Live Music, Brianna Casey

Phase 1

Music has long been recognized for its ability to promote healing and wellbeing in patients across the entire spectrum of healthcare settings. Thanks to recent research into its therapeutic effects, music is now widely accepted as an established part of a holistic healthcare regime. However, more work needs to be done in defining the aspects inherent to music that make it such an effective agent of healing. Notably, little research has been done on whether live musical performance is more effective than recorded music in bringing about therapeutic benefits. The current project sought to explore the differential perceptions of and …

Burnt: A Narrative Exploration Of Burnout In Medicine, Aaron Chambers Jan 2019

Burnt: A Narrative Exploration Of Burnout In Medicine, Aaron Chambers

Phase 1

Burnout has become a central social issue within the culture of medicine. Estimates project that more than half of all physicians experience burnout in a given year. This results in lower job satisfaction among physicians as well as diminished patient outcomes. The topic’s literature suggests that burnout susceptibility is multifactorial, but includes extrinsic (debt, variable schedule, hierarchical workplace, EMR) as well as intrinsic (uncertainty, breaking difficult news, secondary trauma) contributors. To better understand this complicated topic, this project sought to synthesize competing factors within a narrative that explores the good and bad of contemporary medicine. The narrative is informed by …
