Did Maryland’S All-Payer Global Budget Revenue Model Change Hospital Margins And Revenue Per Discharge? A Difference-In-Differences Analysis Of Maryland And Massachusetts, 2010–2023, Kola Adegoke
Health & Biomedical Sciences Faculty Publications
Background: Maryland implemented an all-payer hospital global budget revenue (GBR) model in 2014 to decouple hospital revenue from volume and improve revenue predictability. This study evaluated whether Maryland’s 2014 GBR implementation was associated with differential changes in hospital total margins, inpatient revenue per discharge, and COVID-period margin volatility relative to Massachusetts.
Methods: A matched hospital-year panel for Maryland and Massachusetts spanning 2010–2023 (2,261 matched observations) was constructed by merging Medicare cost report-based financial measures with provider characteristics using CMS certification identifiers. Two-way fixed-effects difference-in-differences models with hospital and year fixed effects and hospital-clustered standard errors estimated post-2014 changes in outcomes …

OSC under RPAPL §749(3) denied for lack of good cause—the application has been pending since April 2026 with no movement. Court stays warrant to September 30, 2026 in the interest of justice, accounting for the GAL's good-faith efforts, the 2023 index number case with $14,843.20 outstanding, and the bureaucratic maze encountered by a court that joined both APS and HRA as co-respondents under CCA §110(d) only to learn that DSS could give no case-specific information on either application.

Intrawound Vancomycin For Prevention Of Surgical Site Infection: Meta-Analysis Of Randomized Clinical Trials, Shaan Patel, Shiva Nischal, Kush Kale, Angelette Mendonca, Nelson Sofoluke, Srinivas Prasad, James Harrop, Daniel Refai
Department of Neurosurgery Faculty Papers
BACKGROUND: Surgical site infection remains a major cause of postoperative morbidity. Intrawound vancomycin has been proposed as an adjunct to standard prophylaxis, but its effectiveness across surgical settings remains uncertain. This systematic review and meta-analysis of randomized clinical trials aimed to evaluate the association between intraoperative intrawound vancomycin and surgical site infection in adults undergoing surgery.
METHODS: MEDLINE, Embase, and CENTRAL were searched from database inception to March 2026 for randomized clinical trials comparing intrawound vancomycin with standard care in adults undergoing operative procedures. Two reviewers independently performed screening, data extraction, and risk-of-bias assessment using the Cochrane Risk of Bias …

College of Population Health Faculty Papers
BACKGROUND: Alzheimer's disease and related dementias (ADRD) affect nearly 6.9 million Americans, with the number expected to triple by 2050, while disease-modifying therapies remain unavailable. Drug repurposing, which identifies new indications for already approved medications, offers a more efficient and cost-effective pathway to accelerate development of effective therapies for ADRD. The aim of this study is to identify potential drug repurposing signals by systematically screening routinely prescribed drugs for associations with progression from mild cognitive impairment (MCI) to ADRD.
METHODS: We conducted a multi-site target trial emulation using electronic health record (EHR) data from four decentralised databases: INSIGHT Clinical Research …

Toward Reliable Compressive Strength Prediction Of Marble Powder-Blended Concrete: Comparative Machine Learning And Shap-Based Interpretability, Shamal Abdullah Mohammed, Aryan Far H. Sherwani, Nzar Shakr Piro
Polytechnic Journal
The production of conventional concrete has a significant impact on the environment, primarily due to the cement (C) component. To mitigate this impact, binders with reduced environmental footprints were used in concrete mixtures as a partial replacement for C, including powders of industrial byproducts. Waste marble powder (WMP) is one such alternative; however, its incorporation causes complicated interactions with cementitious materials, making the compressive strength (CS) prediction difficult. This study developed and evaluated four predictive models (multilinear regression (MLR), full quadratic regression (FQ), M5P tree, and artificial neural network (ANN)) using a dataset of 306 WMP-blended concrete mixes from 16 …
Caldwell Medical‑System Stability & Thermal‑Power Upgrade Engine (Cms‑Stpue‑1.0), Michael Victor Caldwell Mr.
Caldwell Medical‑System Stability & Thermal‑Power Upgrade Engine (Cms‑Stpue‑1.0), Michael Victor Caldwell Mr.
Defensive Publications Series
Medical devices — including neural implants, biosignal processors, dementia‑research chips, microfluidic analyzers, wearable diagnostic systems, and many other forms of medical hardware — require extremely stable electrical power, predictable thermal behavior, low‑noise operation, and long‑term reliability. Traditional medical systems struggle with voltage droop, thermal drift, microfluidic instability, and resonant interference, all of which degrade biological signal fidelity and reduce device lifetime.
This invention introduces the Caldwell Medical‑System Stability & Thermal‑Power Upgrade Engine (CMS‑STPUE‑1.0), a unified framework that applies four existing Caldwell engineering inventions — CVGTC‑3.1, CCVTE‑3.x, CVGTC‑2.x, and the Multi‑Core Thermal‑Hydraulic Upgrade Suite — to significantly enhance the stability, reliability, …
Safe Transactional Execution And Reversible Staging For Autonomous Agent Actions, N/A
Safe Transactional Execution And Reversible Staging For Autonomous Agent Actions, N/A
Defensive Publications Series
This specification outlines a safety-centric runtime infrastructure designed to facilitate secure, transactional execution for autonomous computer systems. Autonomous software agents operating under large language models frequently interact with local and remote filesystems, execution environments, and external program interfaces. Because conventional execution environments process such operations immediately under a fire-and-forget paradigm, a failure or interruption during a multi-step task can leave the host environment in an inconsistent or partially modified state. The runtime architecture disclosed herein resolves these reliability concerns by wrapping agent operations in a transaction coordinator. This framework provides isolated, rollback-capable staging for local operations, records undo logs for …

Defensive Publications Series
Defensive publication and enabling prior-art disclosure of a system in which an unmodified consumer smartphone lying on a turntable platter — or held in the hand — acts simultaneously as rotation sensor, control-signal generator, display, and self-contained playback deck. Discloses: gyroscope-primary sensing with absolute anchors capped below perceptual thresholds; a world-stabilized display and world-coordinate touch on a physically rotating screen; spindle-offset, shove, lift, platter-warp, and resting-plane estimation from inertial data; optional motor-field witnesses; on-device control-signal synthesis and angle/tick transport; and a method for separating an intended in-band oscillatory gesture from pathological tremor for assistive input. Comparative Experimental Investigation Of Corrugated Solar Air Heater Using Porous Media And Twisted Tapes, Hussam J. Rashid, Mohammed K. Al-Saadi, Ameer Jaddoa
Engineering and Technology Journal
This experimental work examines the thermal, thermohydraulic, and exergetic performance of a modified revised multi duct solar air heater (SAH) under real outdoor conditions. Four configurations were examined, including a smooth collector, ducts with corrugation, porous-lined ducts, and corrugated ducts equipped with inserted twisted tapes over the investigated Reynolds number (Re) interval (2000–10,000). The airflow was supplied by a DC fan powered directly by photovoltaic modules. Introducing internal flow-modifying elements resulted in a marked enhancement of the SAH heat transfer capability, as confirmed by the experimental observations. When compared to the smooth absorber, among the tested configurations, the twisted tape …
