Subjective semi-quantitative method is often used to evaluate the severity of spasticity in clinical condition. The purpose of this study is to develope an objective and quantitative model to evaluate the severity of spasticity for hemiplegic patients. We utilize Kin-Corn III isokinetic machine to evaluate 16 hemiplegic patients with 8 velocities from 5 to 210 degrees per second. The elbow joints of subjects are pulled when subjects totally relaxed and under no influence of gravity. The curve of resistance force was recorded and analyzed. We found 150 degrees per second is the best velocity for the evluation. And for elbow …

The purpose of this study was to evaluate the effects of different weight bearing on reducing spasticity in stroke patients. Quantity of spasticity was defined as level of associated reaction during exercise. It can be measured by surface integrated EMG levels.Thirty cerebrovascular accident patients (Brunnstrom's stage II-V in upper extremity) performed forward target-touching exercise in sitting position. The exercise was performed with affected shoulder flexion and keeping elbow straight while the associated reaction in same side biceps brachii was monitored by integrated EMG levels and the weight bearing of both feet was measured by 2 weights. In order to study the effects of hip and knee angle to knee flexion and extension torque and other measurements obtained from Cybex 340 isokinetic dynamometer in isometric and isokinetic exercises, twenty normal young adults were tested at various hip position(SITTING:hip 85°,SEMIRECLINED:hip 135°, SUPINE: hip 180°) and different speeds (ISOMETRIC:0° /sec, LOW SPEED ISOKINETIC:60°/sec, HIGH SPEED ISOKINETIC:180°/sec). Adequate warm-up exercise and rest was requested between each test in this study.In isometric flexion exercises, the greatest torque value was obtained at supine position. The purpose of this study was to evaluate the muscular strength of five patients which were received shoulder hemiarthroplasty. Isokinetic test used by Lido Isokinetic Rehabilitation System included shoulder external rotation, internal rotation, abduction, adduction, flexion and extension at the speed of 60, 120, 180 and 240 degree/sec. There was significant difference (p<0.05) between involved and non-involved shoulder on the peak torque in external rotation, internal rotation, abduction and flexion. Ther peak torque to body weight ratio also showed significant difference (p<0.05) in internal rotation, abduction and flexiom. Bicycle injuries are the most common sports injuries in U.S. There are different causes, types, and sites of sports injury between amateur and professional cyclist. 16 Asian Games cyclists of our country participated in this survey study of cyclic sports injuries. We used questionaire and physical examination to complete this study. According to the result of questionaire, the most frequently lesion sites were: knee (87.5%), low back (81.3%), hand (56.3%). A 5 year-old boy had non-progressive myopathy since 2 years of age. Muscle enzymes were normal. Electromyography showed a myopathic feature. Prominantly disseminated nemaline bodies and predominant type I muscle fibers were found in muscle biopsy specimen. The clinical and morphological findings supported the existence of a rare form of congenital myopathy.

Currently, the information for successful integration must be referenced from several sources. By having the key elements of integration in one volume, users attempting to layout and/or design a publication using these programs will be able to proceed quickly with fewer technical problems. Rapid growth is predicted for computer-based tools in graphic design studios, advertising agencies, and magazines which normally use computer design stations. Current trends in the printing industry are shifting towards rotogravure printing processes. Today, rotogravure printing is responsible for 20% of the printing industry and is growing rapidly. The printing industry is moving towards rotogravure processes for high print quality for long run jobs.

The most commonly found examples of the rotogravure printing process are: "cheaper" catalogues, periodicals, tabloids, directories, newspaper inserts and the "Sunday supplement." Hydrogen peroxide is an oxidizing bleaching agent that is utilized extensively in the pulp and paper industries to bleach cellulosic fibers to high brightness levels. The main advantage of hydrogen peroxide bleaching of mechanical pulps is the high yield. Peroxide used under mild alkaline conditions can significantly increase brightness 10-20 points by oxidizing lignin constituents without actual lignin degradation. Due to peroxides' inherent quality of lignin preservation, it is an excellent bleach for application to CTMP. This allows a high yield process like BCTMP to compete with kraft pulp in application to many grades of paper.

This paper describes a novel method of character recognition targeted for extracting complex annotations found in engineering documents. The results of this work will make it possible to capture the information contained in documents used to support facilities management and manufacturing. The recognition problem is made difficult in part because characters and text may be expressed in arbitrary fonts and orientations. Our approach includes a novel incremental strategy based on the multi-scale representation of wavelet decompositions. Our approach is motivated by biological mechanisms of the human visual systems. Stress is a response to change or conflict. It is usually considered to be negative and damaging. However, not all forms or levels of stress are bad. Competing in sports and achieving in school or at work are examples of positive stressors. Stress becomes negative when the pressures surrounding these and other situations become too great or when several small stressors occur at once, and one can no longer adjust. Reports of abnormalities of regional cerebral blood flow (rCBF) in patients with schizophrenia and major affective disorder are contradictory. To gain a better understanding of the possible sources of these contradictions, the rCBF and neuropsychological functioning of 15 schizophrenics, 15 patients with major affective disorder, and 15 age-matched controls were studied. No group differences in overall rCBF, percent frontal devialion in flow, or rCBF asymmetries were observed. Both overall rCBF and percent frontal deviation of rCBF were reduced with greater age and with increasing neuropsychological impairment. Dosage of neuroleptic medication was associated with reduced whole brain flow, not with percent …

Ninety-two consecutive patients with obstructive sleep apnea syndrome (OSAS) were studied before and six weeks after treatment with either nasal continuous positive airway pressure (CPAP) or uvulopalatopharyngoplasty (UPPP) (n = 46 per group). Assignment of patients to treatment was based on clinical considerations and patient preference. Patients were assessed by nocturnal polysomnography and performance on the Multiple Sleep Latency Test (MSLT) the following day. Before treatment, the CPAP and UPPP groups did not differ in sleep-related respiratory disturbance, oxygenation during sleep, fragmentation of sleep, or the level of excessive daytime sleepiness indicated by the MSLT. Recent research suggests that the most potent feature of the Type A behavior pattern for prospectively predicting cardiac disease is aggravation, irritation, anger, and impatience (AIAI). The present study examines psychometric properties of a new AIAI measure and its relationship to the severity of coronary artery disease (CAD). Subjects included 61 males undergoing coronary angiography. Comparisons were made of mean psychometric scores across groupings, defined by number of vessels occluded. These comparisons showed that the normal or subclinically occluded coronary angiographic group had high levels of depression and anxiety. Investigation of the neuropsychological sequelae of brain tumors is extremely complex largely because the neurobehavioral consequences of brain tumors depend upon complex interactions among disease and treatment variables as well as patient characteristics. To illustrate some of these complexities, we present case studies of two patients in whom the behavioral outcome was not easily predictable on the basis of our current understanding of brain-behavior relationships in tumor patients. The case studies do illustrate how neuropsychological evaluation aids in identifying cognitive deficits which have implications for subsequent quality of life. Malnutrition is a common problem of patients undergoing liver transplantation. To treat malnutrition, it must first be identified through a nutritional assessment. Because many objective nutritional assessment parameters have limitations in end-stage liver disease, subjective nutritional indicators may be used as an alternative. Nutritional needs following transplantation are categorized as short and long term. The short-term nutritional goal, anabolism, can be complicated by the nutritional status of the patient, surgical procedures, and necessary medications. The increased nutrient needs during the early posttransplant phase require particular nutritional support. Lipohyperplasia of the ileocecal valve is a fairly common entity that occasionally behaves as an intestinal tumor, causing obstruction, sometimes with intussusception, or bleeding, which may be acute or chronic. Most often occurring in middle-aged or elderly women, it may be mistaken clinically and radiologically for carcinoma or other neoplasms. Its differentiation is best made by endoscopy, confirmed if possible by biopsy. Differentiation is important, as the treatment is a limited ileal and cecal resection, instead of a blind hemicolectomy. A recent study suggests possible associations with other medical conditions. Rectal Non-Hodgkin's Lymphoma: A Clinicopathologic Study And Review, Cary A. Gottlieb, Eyal Meiri, Koichi Maeda

A total of 11 patients with lymphomatous involvement of the rectum were seen at Henry Ford Hospital between 1964 and 1987. Eight of these patients had primary rectal lymphoma and three had secondary rectal lymphoma. These patients' clinical presentation and course as well as pathological findings are described and compared with cases previously reported. As identified in ours and others' series, rectal lymphomas are associated with the acquired immunodeficiency syndrome and tend to have a B-cell phenotype.

A clinical whole organ pancreas transplantation program for patients with insulin-dependent diabetes mellitus complicated by end-stage renal disease was initiated at Henry Ford Hospital in 1987. Five patients have received pancreatic allografts after a previous kidney transplant (phase 1), and six patients had simultaneous pancreas-kidney transplants (phase 2). Ten patients had functioning pancreatic grafts after surgery, and all of them had normal carbohydrate tolerance with appropriate plasma free insulin responses to an oral glucose tolerance test three months after transplantation. As long as 28 months postsurgery six patients remained free of insulin requirements; however, one patient rejected the pancreatic allograft, …
