This report documents the findings of a strategic assessment of reproductive health in the Dominican Republic (DR), carried out by the Ministry of Health and Social Welfare (SESPAS) and the Dominican Social Security Institute with support from the Population Council’s Expanding Contraceptive Choice (ECC) program and its Latin American and Caribbean regional offices, and the United States Agency for International Development (USAID). The USAID/DR has been working closely with SESPAS to understand the major reproductive health problems in the DR. Community-based distribution (CBD) is the use of nonprofessional local distributors or agents to provide family planning (FP) methods—typically condoms, pills, and spermicides—and referral for other services. FP programs in Africa, Asia, and Latin America have implemented CBD programs for the past 30 years. There is a large body of evidence on the effectiveness, cost, and sustainability of CBD models. Most evidence supports using CBD where appropriate conditions exist. However, major changes have taken place in the context in which programs operate, including the onset of the HIV/AIDS pandemic, enhanced access to FP services, and increased demand for related reproductive health …

The Frontiers in Reproductive Health program, a USAID-funded project of the Population Council is conducting an operations research (OR) study that investigates the effects of male participation in a new model of maternity care that is gender sensitive and provided at the primary-care level. The immediate objectives are to increase the use of family planning methods in the postpartum period and to promote STI primary preventive practices in men and women. The HIV/AIDS epidemic has led program managers to seek approaches to family planning (FP) that will also help combat HIV/AIDS and other sexually transmitted infections (STIs). These approaches include use of simplified STI diagnosis and treatment protocols and promotion of male and female condoms. One aspect of condom promotion that is receiving increased attention is the idea of adding men to community-based distribution (CBD) programs. The male latex condom is the only contraceptive method that, when used correctly and consistently, provides protection against STIs, including HIV/AIDS. This operations research study aimed to develop and test a job aid to provide comprehensive care to mothers and children during the first year postpartum; to train physicians, nurses, auxiliary nurses, and social workers in its use; and to develop and test strategies and materials for training community health agents to promote services for new mothers during the first year postpartum. This document contains reports on three activities undertaken by the Pilot Health Project (PHP) in order to upgrade antenatal and postpartum services for Palestinian women. Specifically, it includes results of a survey and a comprehensive assessment of the antenatal and postpartum care services, including family planning, that are currently provided at the project clinics. This research was Wave II of a longitudinal, qualitative study designed to describe the outcomes of welfare reform legislation on rural families in Louisiana as they tried to comply with provisions of the Personal Responsibility and Work Opportunity Reconciliation Act of 1996. The accelerated rate of bone loss that occurs after menopause may be reduced by consuming rice bran oil (RBO), which contains natural oryzanol. Three different forms of oryzanol (natural oryzanol, NO; crystalline oryzanol, CO; and solubilized oryzanol, SO) were evaluated in a rat model of postmenopausal women. Retired Sprague-Dawley breeder rats were stratified by weight and assigned randomly to one of five groups for a thirteen week study. Food security, nutritional adequacy, and anthropometrics were assessed in 30 low-income women living in rural Louisiana. For food stamp recipients, a 24-hour-diet recall was collected at the beginning (Day 1) and another at the end (Day 2) of their monthly resource cycle; for non-food stamp recipients, the first 24-hour diet recall was collected at a time that was specified by participants (Day 1) and the second was collected approximately 31/2 weeks later. Twenty-one of the 30 participants received food stamps. Ten of the 30 participants were food insecure. Of the 10 food insecure participants, seven received foods stamps. As a …

The purpose of this study was to investigate medfly longevity and reproduction across a broad spectrum of diet restriction using a protocol similar to those applied in most rodent studies. Age-specific reproduction and age of death were monitored for 1,200 adult males and 1,200 females, each individually maintained on one of 12 diets from ad libitum to 30% of ad libitum. Diet was provided in a fixed volume of solution that was fully consumed each day, ensuring control of total nutrient consumption for every fly. The purpose of this study was to examine the effectiveness of women inmates in maintaining communication with the outside world. Specifically, the investigator sought to determine how certain independent variables affect particular dependent variables. The independent variables used in the study were, (1) race/ethnicity, (2) remaining time to serve in prison, (3) age, (4) number of children, if any, the inmate has, and (5) education. The dependent variables used in the study were, (1) number of people on the outside world, (2) volume of correspondence, (3) number of visitations, and (4) volume of telephone calls.

The effects of homeless assistance services at the local level are tremendously difficult to ascertain. In this study, a four-month sample of homeless persons served by a local homeless shelter and case management program were contacted nine to eleven months after receiving services. The findings suggest that the program had some initial success in assisting the homeless clients to locate housing within the first year after leaving the shelter. However, the housing costs paid by these formerly homeless were quite high, with nearly three-quarters of them spending forty percent or more of their income on housing.

Deviant Behavior And Victimization Among Homeless And Runaway Adolescents, Les B. Whitbeck, Dan R. Hoyt, Kevin A. Yoder, Ana Mari Cauce, Matt Paradise

This study used a high-risk population of runaway and homeless adolescents to investigate the effects of history of caretaker abuse and deviant subsistence strategies on victimization among adolescents. Based on a multisite sample of 974 homeless and runaway adolescents, logistic regression models were used first to examine factors predicting involvement in sexual and nonsexual deviant subsistence strategies and then to investigate the effects of deviant subsistence strategies on physical and sexual victimization when adolescents were on the streets. Food banks and other private feeding programs have become an institutionalized component of the social welfare system in over 190 urban areas in the U.S. More recently, private food assistance has gained importance in rural areas as well. The density and capacity of agencies to serve the poor is higher in urban areas than in sparsely populated rural locales where distance and dispersal tend to be barriers to supplying and accessing donated food. Rural food distribution strategies thus must be qualitatively different than those in larger communities, because of the smaller-scale, more informal distributional system. This report summarizes an evaluation of the effectiveness of a welfare-to-work program that is operated by the Montachusett Opportunity Council (MOC) in Central Massachusetts. The study looks at how effective the program was in helping clients achieve goals towards self-sufficiency and tries to understand the particulars of the trusting relationships between staff and clients that provided the base from which they worked to address multiple concerns impeding family financial self-sufficiency.

Objectives. To estimate the effects of maternal and birth characteristics on the decision to breastfeed and to relate breastfeeding practices to racial differences in infant mortality.

Methods. Using a sample of women with young children from the National Survey of Family Growth (NSFG), Cycle V, 1995, the likelihood of breastfeeding was modeled using logistic regression techniques. In addition, single, live births from the NSFG 1988 and 1995 surveys were analyzed to model the effects of race and breastfeeding on infant mortality using Cox regression methods.

We hypothesized that Chinese Americans with disabilities may remain culturally attached to their ancestors' homeland and this cultural attachment may have influences on the concept of Quality of Life (QOL). That is, QOL may be perceived, by Chinese Americans with disabilities, not only as an individual's satisfaction with his or her life, but also the person's fulfillment of his or her responsibilities to his or her family and community. Of course, this hypothesis needs to be examined. The research also attempts to identify the elements that contribute to life satisfaction among active older persons. Globalization, spurred on by capitalist principles, seeks the integration of countries and their economies into a single global market. Global integration has impacted countries in many ways, often negatively. Phenomena such as the illicit trade in weapons, drugs, and people for their sexual and/or physical labor, have also been facilitated by globalization. Specifically examined in this study were a) patterns of sex trafficking of women and girls in four countries: India, Nepal, the Philippines, and Thailand, and b) the various ways globalization has both impacted women's status within these countries and facilitated the sex trade.

No study, to date, has systematically examined the interplay of social contact, depression, functional disability, and cardiovascular health when examining the relation between religious activity and all -cause mortality. This study used Cox regression models as well as a series of structural equation models to elucidate these relations and resultant mortality over a 5-year period. This sample included 3,607 persons, age 65 and older, who participated in the Cache County Study on Memory in Aging, and who were not demented. Results indicate that when using Cox regression modeling, after controlling for other variables related to mortality, both religious activity and …
