This report, co-published by the Population Council and the International Center for Research on Women, describes a workshop convened in Cairo in 1999 to learn more about the nature of both younger and older adolescents' work experience, differentiate the particular needs and potentials of adolescent girls, and identify programs and policies that might have promise for supporting them.

In the United States and other high-income countries, where most people live in cities, there is intense scholarly and program interest in the effects of household and neighborhood living standards on health. This paper investigates whether in these cities the health of women and young children is influenced by both household and neighborhood standards of living. To judge from our results, it appears that as a rule, poor urban households do not tend to live in uniformly poor communities; indeed, about one in ten of a poor household’s neighbors is relatively affluent, belonging to the upper quartile of the urban …

Social programs need to balance volume (coverage) and revenue (sustainability). The law of demand says that we cannot get both coverage and sustainability at the same time—as prices go up, demand will come down. Client loss with increasing prices is inevitable, except in those cases where starting prices are so low or demand is so high that demand is insensitive to price changes. Willingness to pay surveys allow program managers to simulate price-related changes in demand without actually changing prices, giving them a way to make pricing decisions based on empirical information. In 1996, the government of India decided to provide a package of reproductive and child health services through the existing family welfare program, adopting a community needs assessment approach (CNAA). To implement this approach, the government abolished its practice of setting contraceptive targets centrally and introduced a decentralized planning strategy whereby health workers assessed the reproductive health needs of women in their respective areas and prepared local plans to meet those needs. They also involved community leaders to promote community participation in the reproductive and child health program. Since 1998, several evaluation studies have assessed the impact of CNAA on …

An operations research project was launched in northwestern Bangladesh with the objective of preventing adverse outcomes and promoting healthy lifestyles among adolescents by providing reproductive health education and services. The Population Council, in collaboration with the Urban Family Health Partnership and its three nongovernmental service delivery partners, worked in three urban areas of the country. On the basis of its findings, the study first recommends implementation of a combination of reproductive health interventions at the school, community, and health-facility levels, accompanied by community sensitization, to effectively respond to adolescent reproductive health needs. The purpose of this study was to strengthen community strategies to improve quality-of-care services first implemented as part of a Bolivian Ministry of Health and Prevention and World Health Organization (WHO) project in 1997–98. To reach groups that do not regularly attend health facilities and improve community demand for services, the Centro de Información y Desarrollo de la Mujer (Center for Information and Development of Women) implemented a community education intervention focusing on sexual and reproductive health and rights targeting adolescents, men, and women with older or no children. The Population Council’s Frontiers in Reproductive Health (FRONTIERS) program and Policy Research Division, in collaboration with CARE India, conducted an operations research study in Allahabad, Uttar Pradesh to examine the feasibility and impact of adding livelihood counseling and training, savings formation activities, and follow-up support to an ongoing reproductive health program for adolescents. The short-term objective of the study was to foster development of alternative socialization processes for adolescent girls that encourage positive sexual and reproductive health behaviors. In order to expand the benefits of breastfeeding as a birth spacing option for women in the Republic of Kazakhstan, the Kazakh Academy of Nutrition at the Academy of Preventive Medicine in Almaty conducted an operations research study to test the effectiveness of lactational amenorrhea method (LAM) promotion among women in Kazakhstan, as part of the Population Council’s Frontiers in Reproductive Health Small Grants Program. Results show that the intervention programs implemented in experimental baby-friendly hospitals (BFHs) and ordinary hospitals (OHs) improved women’s knowledge, skills, and desire to breastfeed The report concludes that training activities should be strengthened and all …

To support new health reform initiatives undertaken by the Romanian Ministry of Health and Family, and in support of the USAID Romania strategy for improving women’s reproductive health, Project Concern International/Romania (PCI/R) was awarded a two-year grant to implement a reproductive healthcare project, Healthy Parents-Healthy Children (HP-HC). This project trained family physicians and nurses in rural dispensaries in techniques to improve pre- and postnatal care, including training on modern contraception, cervical and breast cancer screening, treating menopause and its symptoms, and the encouragement of healthy lifestyles. The purpose of this study was to investigate the response of participants in the first three Stages of Change (SOC) to a preparation stage-tailored intervention to increase fruit and vegetable (F&V) consumption. An on-line survey was delivered to 400 university employees, and included questionnaires about their demographic information, SOC, self efficacy (SE), and decisional balance (DB). One hundred fifty six (39%) responded to the survey. Anorexia nervosa is the third most common illness among adolescent females. Approximately one half the cases of anorexia nervosa have been suggested to be activity-induced. Various animal studies have been used to study human anorexia, particularly the activity-based anorexia model (ABA). The ABA paradigm consists of diet restriction and liberal access to activity, which ultimately results in a rapid decrease in both body weight and food intake paradoxical to the significant increase in activity. The Louisiana Family Nutrition Program (FNP) reaches up to 120,000 food-stamp recipients and other low SES individuals per year through direct and indirect nutrition education methods. To be effective in eliciting behavior change, a nutrition education program must be developed to be consistent with the needs, motivations, and concerns of the target audience; therefore, it is important that the needs, motivations, and concerns are being met by FNP. Many food recovery agencies depend on donated food, and its safety is critical for the health of vulnerable populations. A food safety curriculum was developed for agency volunteers and paid staff of the Lower Mississippi Delta region. Examples of topics in the curriculum included: personal hygiene, food storage, transporting food safely, and HACCP. Food Safety Knowledge Pre- and Posttests (20 questions) were identical, and validity and reliability were established prior to use. Paired t-tests were performed to determine the effectiveness of the curriculum (n=190). This is a phenomenological study of the lived experiences of homeless young people in Western Australia. Its focus is the reasons why homeless young people leave education, although many of them make a serious effort to complete post-compulsory schooling. The study was qualitative, and was designed as an in-depth analysis of the experiences of five young people aged between 15 and 17, all attending the same school. Data collection consisted of two in-depth interviews with each participant, and a focus group discussion with all five. The 2000 U.S. Census brought confirmation of the increase of the Latino population and of the growing diversity of Latino national groups that now make this region their home. Latinos now number 428,729, a 55% increase over their numbers in 1990. In 30 years, the Latino population has increased six-fold, and from its initial concentrations in Springfield, Holyoke, and Boston its presence is now a fact across the Commonwealth.
Massachusetts Latinos are also showing increasing diversity, matching that of the Northeast region and exceeding that of the nation. At the national level, Mexicans have a dominance that dwarfs all other …

In cooperation with the Human Relations Foundation of Chicago (HRF), CURL and the Jane Addams Hull House examined inequalities among racial and ethnic groups in Chicago. Drawing from a broad range of existing data sources, researchers documented areas of reduced inequality as well as other areas of persistent inequality.
Minding the Gap: An Assessment of Racial Disparity in Metropolitan Chicago examines seven quality of life measurements: income, wealth and employment, education, housing, transportation, health, the lives of children and the criminal justice system. The extent to which means-tested transfers, social insurance, and tax credits fill the gap between a family’s private resources and the poverty threshold is a periodic barometer of the social safety net. Using data on families from the Current Population Survey I examine how the level and composition of before- and after-tax and after-transfer poverty gaps changed in response to changes in the policy and economic landscapes over the past two decades. The estimates presented here indicate not only dramatic changes in the level and sources of income maintenance programs filling the poverty gap, but also dramatic changes in which …

Currently 16% of Americans breastfeed their children for at least 12 months as recommended by the AAP, which is well below the HP 2010 goal of 25%. Breastfed infants receive benefits that can improve their health throughout their lives. The benefits of breastfeeding for children include increased resistance to infectious diseases, such as gastroenteritis, respiratory tract infections, and ear infections. Breastfed children also display lower rates of chronic diseases including diabetes, obesity, asthma, and leukemia. The choice to breastfeed results in economic benefits from lower health care costs and from reduced spending on infant formula. The Healthy People 2010 targets …

Increasingly the term epidemic is being used to describe the current prevalence of fatness in the United States. Skyrocketing rates of obesity among all groups of Americans, particularly children, the poor, and minorities, have become a major public health concern. Indeed, it is difficult to open a newspaper or magazine without encountering a discussion of the expanding American waistline and the health problems associated therewith. In this paper I use 751 New York Times articles on obesity to examine the media construction of the obesity epidemic. The voices we hear describing the lives of mothers who come into contact with child welfare agencies are usually those of service providers and researchers. How do mothers make sense of their own lives and what happened to their families when they became involved with child welfare? This report provides an opportunity to listen to what 16 of these mothers had to say over conversations averaging 5 - 6 hours with each woman. During the past decade, there has been increased awareness of issues facing grandparent caregiver families on the part of policymakers and service providers. This awareness has prompted efforts to document the numbers of children being raised by grandparents, to identify challenges faced by grandparents raising grandchildren, and to provide services to meet the needs of these families. National estimates suggest that the numbers of grandparent caregiver families are increasing. An intensive, experiential travel course in Agroecosystems Analysis was conducted in Iowa, Minnesota, and Nebraska (United States) during summers of 1998 and 1999. The intended student audience was advanced undergraduate and beginning graduate students. Pretravel readings and a week-long series of farm visits, which consisted of in-depth interviews with the farmers and their families, prepared student teams to analyze and evaluate the production, economic, environmental, and social sustainability of 10 farms. Students shared their analyses both orally and in written reports. Based on a multifaceted student evaluation process, we found that participants were highly motivated, strongly engaged with the course …
