Tap and Reposition Youth (TRY) was a multiphase initiative undertaken by the Population Council and K-Rep Development Agency (KDA), the oldest and largest microfinance institution in Kenya. The overall aim of the project was to reduce adolescents’ vulnerabilities to adverse social and reproductive health outcomes, including HIV infection, by improving their livelihoods options. The project was launched in low-income and slum areas of Nairobi, Kenya, where rates of HIV infection are alarming and where young women are disproportionately affected.

The usual lessons drawn from East Asia’s striking experience of health and fertility transition concern the efficacy of well-designed government programs catering to an existing or ideationally stimulated demand. An alternative interpretation sees the demographic change—and the uptake of services—as a by-product of social and economic development together with, in some cases, strong government pressures. This Population Council working paper probes more deeply into this experience, seeking to identify common features of development design and administration that underlies it and to derive lessons for policies elsewhere. Nairobi, Kenya is home to one of the largest slum populations in sub-Saharan Africa. Adolescents living in these slums, especially girls, are cut off from the potentially protective environments of school and family and lack safe economic opportunities. Unemployment is a major concern for adolescent girls, and desperate circumstances force them to survive however they can: 21 percent of sexually active girls aged 15–19 report exchanging sex for money or gifts. This heightened vulnerability is reflected in rates of HIV infection. The Self Employed Women’s Association (SEWA), a key Indian micro-finance nongovernmental organization, in partnership with the Population Council, undertook a pilot livelihoods skill-building program to support adolescent girls aged 13–19 years (mostly unmarried, in school and out of school). Like other such programs, this livelihoods program focused on exposing girls to new ideas, building social networks, exposing them to the world around them, and providing the technical skills needed to enhance their access to income generating opportunities. The first study was conducted to determine if aminophylline cream application to the waist will reduce waist circumference compared to a control. Topical fat reduction from the thigh in women has been demonstrated, but local fat reduction in other areas or in men has not. Fifty men and women were randomized to 0.5% aminophylline cream to the waist twice a day or to no treatment to the waist. At week 12 there was a significant reduction in BMI from baseline that was not different between the groups. The reduction in waist circumference was 11 ± 1.0 cm in the aminophylline …

Prices of a market basket of 149 foods that could be combined to meet the 2005 Dietary Guidelines for Americans (DGA) were surveyed in 29 stores in an eight parish area of Southeast (SE) Louisiana. Two-week cycle menus and recipes were planned to meet the 2005 DGA in a culturally acceptable way. Linear Programming (LP) analysis was also performed on the market basket to determine the cost-minimizing combination of foods that will sufficiently meet the 2005 DGA. Evaluation Of The Effectiveness Of A Body Mass Index-For-Age Percentile Health Report In Raising Parent Awareness Of Their Child's Weight Status, Anantha Padmaja Lakkakula

Overweight in children has become a major health concern. Research suggests that many parents may not be aware of their child’s actual weight status. The objectives of this study were to test the effectiveness of a body mass index (BMI)-for–age percentile report in raising parent awareness of their child’s weight status. Eighteen public elementary schools in southeast Louisiana were pair matched and divided into nine intervention and nine control schools. Children in the intervention and the control schools were divided into two groups 1) healthy weight (BMI ≥ 5th to <85th percentile) and 2) at risk and overweight (BMI ≥ 85th percentile). Forty children were randomly selected from each of the two weight groups from the intervention and the control schools. Parents of children in the intervention group received a BMI-for-age percentile health report along with a short questionnaire. Parents of children in control schools received the questionnaire only. Parents in the intervention group have 4.7 times more accurate perception about their child’s weight compared to the control group (OR: 4.7, 95% of CI: 0.89-24.86, p=0.00 ). After receiving the report, more parents of at risk or overweight children were concerned and only fewer parents of healthy weight children were anxious about their child’ weight. When parents were compared based on their child’s weight regardless whether they got the report, parents of at risk or overweight children were more than five times less likely to perceive the correct weight classification of their child (OR: 1.8, 95% of CI: 0.05-0.62, p=0.00) and less concerned about their child’s weight (OR: 0.98, 95% of CI: 0.32-2.93, p=0.00) when compared to parents of healthy weight children. All parents were willing to help their child follow healthy behaviors regard less of the report and their child’s weight status. A BMI-for-age percentile report appears to be an effective way to increase parent awareness and concern regarding their child’s weight status. With increased awareness, parents may be more likely to encourage their children to achieve a healthy weight.

The objectives of this study were to explore the relationships among depression and food security status, dietary nutrient intakes, sociodemographic characteristics, body mass index (BMI), and the presence of chronic diseases in a female food stamp recipient population (n = 66) in Southeast Louisiana. Women were dichotomized by stated depression and descriptive statistics on socioeconomic characteristics and mean nutrient intakes were presented for each group. Purchase and consumption of energy-dense nutrient poor [EDNP] foods by low-income individuals contributes to the lower diet quality and increased risk of chronic disease (e.g. obesity, type 2 diabetes) in this population. Food purchasing behaviors, dietary energy density [ED], perceived health status, and perceived nutrition knowledge were assessed in 22 female food stamp recipients in Southeast Louisiana. Two 24-hour recalls (at the beginning and at the end of the month) and food expenditure receipts were collected; participants also completed several questionnaires. Systematic screening is a strategy to integrate reproductive health services at the provider level. Integration is the proactive provision of multiple reproductive health services in the same facility at the same time. Systematic screening is a simple intervention to increase the number of services received at a single client visit. In this strategy, providers use a checklist or questionnaire to identify each client’s needs and desires for reproductive health services. The Somali community living in Kenya has practiced the severest form of female genital mutilation/cutting (FGM/C), infibulation, for centuries. To understand the context within which the practice takes place, and how its complications are managed, FRONTIERS undertook a diagnostic study in North Eastern Province and in the Eastleigh area of Nairobi. The study found that the health system is ill equipped to serve women who have been cut, and particularly infibulated women who are pregnant and delivering, stemming from an overall weakness in the availability and quality of maternal and neonatal heath services. The Population Council’s Frontiers in Reproductive Health (FRONTIERS) program, in collaboration with the Bangladesh Ministry of Health and Family Welfare, the Urban Family Health Partnership, and two nongovernmental service delivery partners, carried out the Global Youth project in northwestern Bangladesh from 1999–2003. The important lesson learned from that project was that reproductive health education could increase reproductive health knowledge in adolescents, particularly in areas related to reproductive biology, family planning, pregnancy, sexually transmitted infections, HIV, and AIDS. The Christian Health Association of Ghana (CHAG) is a large faith-based NGO which currently serves an estimated 35 percent of the Ghanaian population, mainly in remote rural areas. This study built capacity within the CHAG secretariat to calculate the economic cost and cost recovery levels of selected reproductive health services in four CHAG-affiliated hospitals. Techniques learned in the study are applicable to most costing problems, not just to reproductive health. Information obtained in the study forms the basis for negotiating reimbursement under the Ghana National Health Insurance Scheme, and for setting cost recovery and containment policies. This report outlines results of an in-depth assessment carried out in Ghana in order to provide a better understanding of key factors affecting reproductive health (RH) prioritization at the district level; and to make recommendations for policy dialogue, advocacy, resource allocation, and RH program implementation. In particular, the study examined whether or not districts are connecting to the central process of priority setting and reasons for not doing so.
