To improve access to and use of facility-based obstetric and newborn care, the Government of Bangladesh is implementing two innovative performance-based financing programs, namely demand-side financing (DSF) and pay-for-performance (P4P). With the purpose of identifying the lessons learned, limitations of the P4P and DSF models, and scopes for cross learning, a two-day workshop was organized in Dhaka. This workshop report, prepared by the Population Council, resulted in several recommendations to modify DSF and P4P schemes. The Population Council explored the possibilities of introducing a Pay-for-Performance (P4P) scheme in Bangladesh to improve maternal, newborn and child healthcare (MNCH) services. Based on the consultation and with guidance from the Government of Bangladesh, the Population Council and UNICEF provided technical assistance to the Directorate General of Health Services (DGHS) to test two P4P strategies for MNCH service providers to improve service volume and quality of care, and for poor clients to receive services subsidized through vouchers or coupons. In Kenya, recent trends in contraceptive use among currently married women aged 15–49 years show that after experiencing a stall between 1998 and 2003, the contraceptive prevalence rate (CPR) increased between 2003 and 2008–2009. Although the increase in CPR after 2003 is encouraging, there is limited understanding of how these changes affected adolescents, especially girls, given the unique sexual and reproductive health challenges that they face. Under the USAID funded FRONTIERS program, the Population Council conducted a study in six health facilities in Western Kenya that demonstrated that screening for TB within an antenatal care (ANC) setting is feasible and acceptable among the service providers. However, a major challenge remains: although TB detection is encouraged among ANC clients within the maternal and child health clinics in the country, providers in these settings fail to appreciate the need for a continuum of care from pregnancy through to the postnatal period. The objective of these studies was to determine the association between dietary intakes of selected food groups, macronutrients, and micronutrients in mother-child dyads. This was a secondary analysis of data on low-income Black, Hispanic, and White children 3-5 years-old (y) participating in Head Start (HS) (n=650), and their mothers. Mothers served as a proxy for their child, and self-reported intake during a multiple-pass 24-hour recall interview for one weekend day. Children’s food preferences play a major role in their food choices and consumption. The objectives of these studies were to examine the relationship between children’s preferences for fruits and vegetables (FV) and their weight status and to increase children’s liking of FV by offering repeated opportunities to taste less liked items. A representative sample of elementary school children (1st, 3rd, 4th, and 5th grades) were recruited in the three interventions. In previous studies, in has been observed that the addition of resistant starch (RS) in the diet of rodents, a non-digestible fermentable fiber, promotes increases in markers of fermentation and improvements in body weight and body fat analysis when incorporated into low to moderate fat diets. This study investigated whether the same beneficial effects observed with RS addition in these lower fat diets could be obtained in a high fat diet, and if the type of fatty acid consumed in the diet makes a difference in markers of fermentation and body fat. The estimation of the centre of mass position in humans is usually based on biomechanical models developed from anthropometric tables. This method can potentially introduce errors in studies involving elderly people, since the ageing process is typically associated with a modification of the distribution of the body mass. In this paper, an alternative technique is proposed, and evaluated with an experimental study on 9 elderly volunteers. Durkheim argues that an individual is more vulnerable to self-destruction the more s/he is detached from the collective. This dissertation will explore the relative impact of social integration on older adults who have transitioned into their new roles in the social structure in relationship to their physical (obesity) and psychological (stress) health status. Additionally, the dissertation examines how social integration varies in its impact from one racial group to another, and how such variations influence the health status of the older adults who are members of these groups.

This thesis links two theories, Interaction Ritual Chains (IRC) with Identity Control Theory (ICT), through a multi-method study of group training programs for beginning runners. First, group training programs are identified as IRCs. Second, we see that IRCs produce identities within individuals which reflect the IRC's activities and its group members. Third, we look at how IRCs of varied strengths produce identities of varied strengths. Fourth, we compare different types of group training programs and their abilities to develop identities. Finally, we conclude that IRCs are mechanisms which individuals are able to use as part of the self-verification process.

It has been shown that a history of GDM is associated with an increased risk of incident type 2 diabetes in women. Tiggerman and Pickering (1996) found that it was not just the amount of television viewed, but the viewing of appearance focused shows that affected body image. Limited research exists on the experiences of sedentary women as they transition through menopause. This gap creates difficulty for public health practitioners as they strive to develop resources, implement programs, or influence policy change at the community level for this group of marginalized women. Keeping women healthy throughout the aging process, including menopause, improves their quality of life and decreases the impact aging has on the health care system. The elderly population is among the fastest growing populations in the United States. Finding and consuming medications safely and effectively are challenging endeavors for this population. Nonsteroidal anti-inflammatory drugs (NSAIDs) are a widely consumed class of medications among the elderly population, with 70% of individuals consuming over-the-counter (OTC) NSAIDs once a week and 34% using them daily. Guided and influenced by a famous follow-up study in criminology focused on desistance from crime, this dissertation studies desistance from crime, homelessness, and substance abuse. In the early 1990s, The New Orleans Homeless Substance Abusers Project (NOHSAP) was founded as an experiment funded by the National Institute on Alcohol Abuse and Alcoholism (NIAAA) to uncover optimal treatment strategies for homeless alcoholics and drug addicts. The program ran for three years (1991-1993) and in those years, 670 homeless New Orleans men and women were admitted into treatment. Parental involvement has been emphasized as a mechanism for improving our public schools. In this study the author inquired into (a) the trend and status of parental involvement and (b) whether parental involvement is associated with schools meeting accountability measures. Secondary analyses were conducted on multiple waves of nationally representative data collected by the National Center for Education Statistics (NCES) School and Staffing Surveys (SASS). Descriptive statistics, discriminant function analysis, logistic regression, among others, were used for the study.

Mothers in numerous species exhibit heightened aggression in defense of their young. This shift typically coincides with the duration of lactation in nonhuman mammals, which suggests that human mothers may display similarly accentuated aggressiveness while breast feeding. Here we report the first behavioral evidence for heightened aggression in lactating humans. Breast-feeding mothers inflicted louder and longer punitive sound bursts on unduly aggressive confederates than did formula-feeding mothers or women who had never been pregnant. Maternal aggression in other mammals is thought to be facilitated by the buffering effect of lactation on stress responses. Consistent with the animal literature, our results …

Most microfinance institutions have assumed that positive second-generation effects follow involvement in microcredit, but the nature of these effects has been unclear since few scholars have directly focused their attention on them. To address this gap in the literature I conducted exploratory survey research in Western Guatemala in January 2011. I returned with 97 interviews conducted with 68 Guatemalans who had received a microloan at some point in their lives, and 29 who had never received a formal loan. This thesis traces the history of alcohol production and consumption throughout U.S. history by following the course of the four major periods of relative equilibrium in social norms that the country has experienced. It uses this socio-historiography as a platform to understand how national trends in alcohol production and consumption were experienced along the border in El Paso, Texas, in a very unique fashion. The thesis aspires to augment El Paso's pride and sense of identity by building on knowledge of local history, customs, and norms.

This research focuses on the perceptions of disability amongst the Tarahumara, known amongst themselves as the Rarámuri, in the Sierra Madre region of Northern Chihuahua. This population of individuals has typically lived in isolated areas in communities in the Sierra Madre where medical and specialized educational services were rare. The sample includes 56 participants comprised of 31 professional stakeholders and 25 patient stakeholders affiliated with the Charles R. Wood Cancer Center. This paper draws on empirical evidence collected from pro-anorexia websites and qualitative interviews with dieters to develop an analysis of the uses of medical and scientific expertise in processes of body- and self-fashioning. It builds on previous work by examining how `lay publics' refashion expertise in order to use it for new purposes, sometimes contradictory to the purposes of medicine itself. Using the "think aloud" protocol, which allows for the collection of data in real time, the researcher audio taped comments from 13 white college students from a predominately white university in the Southeastern United States and 15 black students from a predominately black university, as they explained how they searched for HIV/AIDS information on the Internet. A grounded theory analysis of the tapes revealed a three-stage model that students progressed through as they searched for HIV/AIDS information on the Internet.
