With the International Classification of Functioning, Disability and Health (ICF) the World Health Organization (WHO) has prepared the ground for a comprehensive understanding of Human Functioning and Rehabilitation Research, integrating the biomedical perspective on impairment with the social model of disability. This poses a number of old and new challenges regarding the enhancement of adequate research capacity. Here we will summarize approaches to address these challenges with respect to 3 areas: the organization of Human Functioning and Rehabilitation Research into distinct scientific fields, the development of suitable academic training programmes and the building of university centres and collaboration networks.

Bereavement is the process of suffering that follows the loss of a living being that is significant to someone. When one suffers, she or he has to endure an unpleasant experience, in the case of bereavement, the loss of something special to the person. This loss most often is a loved one but could also include the loss of a pet, relationship, or physical or mental capability. This state of suffering is called grief. In describing his grief, C. S. The Tennessee Division of Mental Retardation Services (DMRS) implemented the Employment First! initiative in 2002. The goal of Employment First was to make employment the first day service option for adults receiving supports funded by DMRS, Medicaid, or the state. Employment First set the standard that employment was the preferred service option for adults with mental retardation and developmental disabilities (MR/DD).

Over six years, regulations in Vermont gradually restricted and eventually prohibited the use of state funds for sheltered workshops or enclaves. Concurrent with the change in funding regulations, the state worked with providers to convert the remaining sheltered workshops in Vermont.

Vermont’s Division of Disability and Aging Services (DDAS) and Division of Vocational Rehabilitation (DVR) worked with a local service provider to convert its congregate day services to community employment. Implementation Between 1987 and 2002, DDAS and DVR worked with several providers to close down sheltered workshops and move people into community supports.

New Hampshire implemented an innovative technical assistance model that promoted organizational change to expand individual employment opportunities. This person-to-person change began at the micro level but “trickled up” through organizations across the state. In 2000, realizing that the state’s growth in integrated employment had stalled, the Bureau of Developmental Services invested aggressively in expanding its intervention strategy by recruiting a community provider to work directly through the bureau. The bureau hired John Vance-the director of ACCESS, a small individualized support organization in New Hampshire-half-time to provide individualized technical assistance to providers across the state. Washington’s Division of Developmental Disabilities (DDD) has recently issued a new policy which went into full effect on July 1, 2006. This policy “designates employment supports as the primary method of furnishing state-financed day services to adult participants.” Emphasizing community employment as the primary service option, the policy further states that: “services for persons under the age of 62 that do not emphasize the pursuit or maintenance of employment in integrated settings can be authorized only by exception to policy” (WA DHSH, DDD, “County Services for Working Age Adults” Policy 4.11). The Tennessee Employment Consortium (TEC) is a statewide organization focused on increasing the number of Tennesseans in integrated employment. The consortium comprises volunteers from the state’s Division of Mental Retardation Services (DMRS) and Division of Rehabilitation Services (DRS), the Tennessee Council on Developmental Disabilities, the ARC of Tennessee, the Center on Disability and Employment at the University of Tennessee, community rehabilitation providers (CRPs), family members, and other stakeholders. TEC’s ability to organize collaborative activities across state agencies, advocacy organizations, and CRPs has played an important role in increasing integrated employment outcomes.

This project was designed to target the needs of students with psychiatric disabilities pursuing higher education. Kessler, Foster, Saunders and Stang (1995), noted that early-onset psychiatric disorders often disrupt attainment of higher education. According to Mowbray et al. (2006) epidemiological studies indicated that youth ages 15-21 years old had the highest prevalence rate of mental illness in 2005. These ages correlate with the conventional years that an individual may attend college. Consequently, the initial psychiatric occurrence of many young adults is experienced during college years. Beliefs and practices concerning death have changed throughout human history. In pre-modern times, death at a young age was common due to living conditions and medical practices. As medical science has advanced and helped humans live longer, attitudes and responses to death also have changed. In modern Western societies, death is often ignored or feared. Changes in lifestyles and improved medical science have depersonalized death and made it an encroachment on life instead of part of life. This has left many people ill equipped to deal with death when it touches their lives.

This monograph presents the results of secondary analysis of the RSA-911 database from the Rehabilitation Services Administration. All successful VR closures for individuals with mental retardation, cerebral palsy, and epilepsy for six data points between 1985 and 1998 were investigated. Trends in competitive labor market and extended employment (sheltered workshops) closures were examined. The use of supported employment in the VR system and its outcomes were also discussed. Findings include increased incidence of competitive labor market closures and supported employment services, with a decrease in extended employment closures.

As the specialization of life care planning continues to evolve, a greater emphasis is occurring from both within the discipline and externally for empirically driven data to support life care planner opinions. One area remaining unresolved pertains to life care planners’ knowledge of how, or if, their life care plan has been implemented post-settlement. A related area pertains to the daily physical and psychosocial health issues of persons with spinal cord injury which can impact the development of their life care plan. The Effectiveness Of Personal Data Assistants In Improving Daily Function Of Adults With Mild Traumatic Brain Injury, Connie Lee Budiwarman

This study evaluated the effectiveness of using Personal Data Assistants to help with memory and/or organizational difficulties with adult patients who had survived mild Traumatic Brain Injury. Utilizing an AB single subject design method with a training period, one subject ultimately completed the study. The study demonstrated mixed results. Through the use of daily record sheets, the subject reported improvement in three areas of daily function but reported negative improvement in three areas of daily function. This study suggests, but does not completely support, the potential use of Personal Data Assistants with patients with Traumatic Brain Injury.

This qualitative study examines the influence of occupational therapists’ worldview on clinical reasoning and action. Recent clinical reasoning research has determined that this complex and multi-faceted process is more than applied theory. Some scholars in occupational therapy have identified intrapersonal factors as influencing clinical reasoning. However, the nature and role of the intrapersonal factors in clinical reasoning remains unclear. To increase understanding of this phenomenon, semi-structured interviews were conducted with expert therapists. Attitudes Toward People With Disabilities In The Social Context Of Dating And Marriage: A Comparison Of American, Taiwanese, And Singaporean College Students, Roy K. Chen, M. G. Brodwin, E. Cardoso, F. Chan

The purpose of this study is to examine the attitudes of American, Taiwanese, and Singaporean students toward people with disabilities in the general context, as well as the specific social context of dating and marriage. The participants include 212 Taiwanese, 115 Singaporean, and 186 American students. They were given the Attitudes Toward Disabled Persons Scale, Form A (ATDP-A) and the Attitudes Toward Dating and Marriage Scale (ATDMS); the results indicated significant differences between American and Asian students in both the general and specific social contexts. We hypothesized that Chinese Americans with disabilities may remain culturally attached to their ancestors' homeland and this cultural attachment may have influences on the concept of Quality of Life (QOL). That is, QOL may be perceived, by Chinese Americans with disabilities, not only as an individual's satisfaction with his or her life, but also the person's fulfillment of his or her responsibilities to his or her family and community. This monograph reports on a study investigating the characteristics of effective state service systems. Findings are based on the experiences of individuals with disabilities who have used a state agency (Vocational Rehabilitation, Department of Mental Retardation, Department of Mental Health, or One Stop Center) to find employment. Interviews were conducted to examine individuals' experiences with employment services including job search, job entry, strategies that facilitated involvement, supports provided, and barriers experienced. Findings indicated five key components to effective service delivery, including agency culture, consumer-directedness, access to resources, quality personnel, and coordinated services.
