Therapeutic injection of facet joints is now widely practised, but Chinese experience has been infrequently reported. 21patients with suffering from the ”facet syndrome” have been taken from OPD patients, Our series including total 30 intra-articular injections C 2% Xylocaine & Kenacort A preparation provide a diagnostic-therapeutic block; Certainty of joint penetration can be ensured only by the routine use of joint arthrography ¢ Metrazamide. Results indicate that immediate effective is 61.9% for releiving pain. It means this block is a good diagnostic-therapeutic procedure in low back pain patients.

Summary In order to establish normal data of the isokinetic (IK) and isometric (IM) contraction of the quadriceps (Q) and hamstring (H) muscles, 22 normal young men and 24 normal young women were tested with a Cybex II Isokinetic Dynamometer at the speed of 0,5,10,20,and 30 rpm (0 rpm= isometric contraction). The IM contraction was tested at 45, 60, and 90 degrees of knee flexion in the Q muscles, and at 30, 45, and 60 degrees in the H muscles. Pneumatic pylon is used for the lower extremity amputees to allow early ambulation and to reduce stump edema postoperatively. The results of pneumatic pylon was studied in 10 patients who received lower extremity amputation form May 1987 to October 1987 at Chang Gung Memorial Hospital.Among them, two cases received above-knee amputation and eight cases received below-knee amputation. No stump pain and skin break-down was noted in all cases studied. Phantom sensation was improved in eight patients. Nine patientshowed complete confidence in the future fitting of permanent prosthesis. They needed shorter training time using permanent prosthesis. Wound infection is high in …

The functional outcome of 34 cases of digital ampution with toe-to-digit reconstruction in Chang-Gung Memorial hospital, from Jan 1984 to Jan 1987, were evaluated by questionaires.The age of the patients were from 16 to 48 years old, with male in majority (82.3%). Nearly all of them were occupational injury. There were 20 cases with right hand injury, most of them should change hand dominance even after reconstruction surgery (63.2%). The purpose of this study was to estimate the number of motor unit in the first dorsal interosseous muscle by using the routine electrodiagnostic techniques. 34 young adult volunteers was included in this study, each subject recevied the following procedures on the first dorsal interosseous muscle: 1. Surface recording of the total surface area, the area under the negative phase, peak amplitude and the base line to negative peak amplitude of the maximal evoked muscle action potential. 2. This study was carried out on 67 children with obstetric palsy in the Department of physical Medicine and Rehabilition of the National Taiwan. University. It revealed that birth weight of more than 3500 gm would have higher prevalence of the palsy. There was no significant evidence to support the palsy had any relationship to patient's sex, presentation, parity, mode of delivery and lateralization of the lesion.The follow up intervals ranged from 3 months to 6 years, with the mean 2 years and 3 months. The prosthetic workshop of National Taiwan University Hospital was established in 1970. There were 665 amputees recieved prosthetic fitting at the workshop from the beginning of 1971 to the end of 1985. Among these amputees, 307 patients were the victims of black-foot-disease, and has ben reported previously. Therefore, the present study included the rest of 358 amputees, 278 males and 80 females with male/female ratio 3.5: 1. The site of extremity involved were 160 in right and 198 in left, which revealed no significant difference statistically (p>0.05). Hearniation of lumbar disc (HIVD) causing compression of the cauda equina is rare but needs urgent diagnosis and surgical treatment. Cauda equina syndrome (CES) has been described as a complex of low back pain, bilateral or unilateral sciatica, saddle anesthesia and motor weakness in the lower extremities that may progress to paraplegia with bowel and urinary bladder incontinence. The onset of bladder and rectal paralysis with saddle anesthesia should be viewed with a high index of suspicion in a patient with backache and sciatica. Theoretically, perceptual information from the interaction with an object and its surfaces (an affordance) facilitates the development of motor skill. This study investigated the use of an affordance in learning control of flexion and extension of an above-elbow training prosthesis. Fifty-two male college students were randomly assigned to two training procedures: two, 1.5-minute periods of a joining dots activity with a flashlight attached to the hook of the prosthesis (affordance group), and practice moving the prosthesis at the elbow joint (no-affordance group) for the same amount of time. A discussion of the incidence and prevalence of Traumatic Brain Injury (TBI) is presented. A focus on the cognitive deficits and vocational rehabilitation is maintained throughout the paper. Cognitive deficits such as language, memory, and communication skills are discussed. Common rehabilitation appraches are presented for cognitive rehabilitation. Vocational rehabilitation and components of the services network utilized for IBI rehabilitation are described. The cost effectiveness of the TBI rehabilitation model is discussed. Exemplary programs for both cognitive and vocational rehabilitation are described.

Despite extensive research, there has been no way to predict before drug administration which children with attention deficit hyperactivity disorder (ADHD) will respond to medication intended to calm them. A drug trial is the current method used. This paper discusses the action of stimulant medications and presents a hypothesis as to why they work on some children and not others. Sensory integration theory, particularly on vestibular system measures, that involves differential diagnosis of certain types of ADHD children, is used to explain why some children respond to stimulant medications.

The purpose of this study was to determine whether high voltage electrical stimulation accelerates the rate of healing of dermal ulcers. Sixteen patients with stage IV decubitus ulcers, ranging in age from 20 to 89 years, participated in the study. The patients were assigned randomly to either a Treatment Group (n = 9) or a Control Group (n = 7). Patients in the Treatment Group received daily electrical stimulation from a commercial high voltage generator. A simulated work environment was utilized to study the effects of group incentive plans on individual performance on an assembly task. The subjects in the study were college students who worked in groups of a small size, a medium size, or a large size. A reversal design was utilized in which the first condition was an individual incentive condition, the second was a group incentive condition, and the final was the return to an individual incentive condition.
The results showed: (a) individual performance did not significantly change when individuals were switched from an individual to a group incentive plan; (b) …

This study examines the relationship between life satisfaction and the volition subsystem of 32 older adults who reside in nursing homes. Specifically, it attempts to measure congruence or the perception of the individual that the inherent characteristics of an activity "fit" his or her sense of personal causation, values and interests. The study examines the relationship between this congruence and life satisfaction.
Results of the study identified positive correlations between congruence and life satisfaction. Correlations between congruence of volition subsystem components and activity were at a statistically significant level (p <.05) for personal causation, values and total volition. Correlations between congruence of volition subsystem components and some specific activities were also found.

The somatosensory evoked potentials following tibial nerve stimulation were recorded in 18 patients with acute traumatic spinal cord injuries at 3 to 5 weeks. The results were correlated with the patient's neurological recovery.One cervical case had absent transmission in unilateral cortical SEP but still with good neurological recovery later on, one lumbar case had no recorded SEP either at L2 or cortical level got complete recovery within 3 months. Two thoracic SCI had only prolong latencies in SEP, but no any neurological recovery at all. There was no constant rule between prolong latencies to severity of dysfunction or degree of …

The purpose of this study was to determine the influence of the reference electrode to the motor unit action potential parameters including amplitude, duration, surface area, phases and turns by reading the same recording area by monopolar and cocentric needles. A concentric needle was stuck into the subject's anterior tibialis muscle, online motor unit analysis was done by EMG-computer combinations. The sinal from the active recording electrode was shunted to another EMG-computer set for analysis of the same motor unit by adding a surface electrode as a reference electrode (monopolar needle recording pattern). Crossed aphasia is defined as the combination of left hcmiplegia (-paresis) and aphasia in a right-handed patient. The incidence of such cases is very low, reported as 0.4%-3.75% in all aphasics. Henderson (1983) reviewed that for the crossed aphasics, the correlation between fluency and infaret localization was similar to that of right-handed aphasics with left hemisphere lesion.A fluent jargon aphasia following a right cerebral infraction developed in a 52-year-old right-handed Chinese woman. Long term hemodialysis of uremic patients can keep on their living, but they still have many problems like severe anemia, atherosclerosis, and decreasing physical fitness, etc.In this report, a symptomatic maximum bicycle execise test was given to 19 male and 11 female unremic patients who were in stable condition. This test was administered some 18-36 hours after hemodialysis. The patients experienced a progressive increase of resistance on the bicycle until the patient couldn't tolerate it. Six hundred cases of poliomyelitis during the last burst out episode in Taiwan of 1982 were analysed. The age distributions of them were very young in majority, 267 cases were below 1 year old (44.5%), 108cases were 1 year old (18.0%), 67 cases were 2 years old (11.2%).Among them, 253 cases were never received any polio vaccination (42.2%), 186 cases only received vaccination for one time (31.0%), 72 cases received twice (12.0%), 52 cases received 3 times (8.7%), only 20 cases accept complete 4 times vaccination (3.3%). Patients with incomplete spinal cord injury admitted to Chang Gung Memorial Hospital from July 1982 to December 1985 were analysed. Totally , 68 male and 9female patients could be followed up by questionaires. The average age of the was 39.6±9.5years. Among them, 34 cases (44.2%)received primary school education. Traffic accident was the leading cause of injury (37.3%), and falling from height was the second major cause(27.3%). The severity of injury was denoted by Yale Score. The purpose of this study were to evaluate the acromicolavicular joint factor eliciting the shoulder pain and limitation of movement. We collected 30cases with suspected the A-C joint lesion disclosed by physical examination from O.P.D of the Rehabilation Dept. V.G.H.-T.C sine Sept. '86 to March. '87. The physical findings were (1) local tenderness at the A-C joint area, (2) painful arc at 150 to 180 degree, (3) limited R. O.M. during horizontal adduction and elicited pain at the end range. Dancing population has been increaseing in Taiwan for the past 15years, but report of studing dance injuries are still few. Eighty-one theatrical dancing students (Male 18, Female 63 ) at National Institute of the Arts and Chinese Culture university were diagnosed and followed in our OPD to study the patterns, severity & mechanisms of dance injuries. A total of 260injuries in the students were reported. We found that the major injury sites by rate were back: 23.46%;ankle: 22.3%;knee: 21.45%; foot:9.23%; hip & thight: 9.6%; other site including: leg, shoulder, elbow, wrist were 14.2%.
