To investigate the nature of exposure factors, and effectiveness of countermeasures in head injuries in Taiwanese children with head injuries consecutively admitted to Chang Gung Memorial Hospital from Jan. 1985 to Dec. 1989. We analyzed the data by chart review. The analysis confirmed a boy to girl ratio of about 2.3 to 1. The largest number of head injuries was in children under 1 year, 4 to 5 years and more than 15 years of age. The most common place for head injuries to occur was outdoors, with the road contributing the most, while the bedroom was the most common …

The conscious perception of pain that is evoked by a noxious stimulus is a multidimensional process that involves both a subjective evaluation of the sensory aspects of the stimulus (intensity, duration, location) as well as an affective or emotional response to that stimulus.The purpose of this invesigation was to develop a simple, reproducible, systemic measurement to realize the relationship between subjective and objective evaluation. Low back pain is a frequent complaint in PM&R OPD. Tumors of the cauda equina play an uncommon but nevertheless important role in the ”sciatic syndrome”. Early detection will lower the risk of neurological sequelae. However, on clinical grounds alone, it is sometimes difficult to differentiate between a cauda equina tumor and a prolapsed interver-tebral disc. Recent advances in imaging techniques had increased the diagnostic rates. This study was based on 6 cases of ependymoma of the cauda equina, collected from January 1986 to May 1992 at VGHTC. Sarmiento introduced the functional fracture brace for the management of tibial shaft fracture in 1963. However, tibial angulation with varus deformity may still occur, especially in the presence of an intact fibula. We report here a case of tibial shaft fracture with varus angulation treated with a modified below knee fracture brace. Our modification included a wooden rod embedded in the under-side of the anterior shield with nod positioned laterally between tibia and fibula. Trimming of rod was done for fitting comfortably against the varus angulation. Good bridging callus formation with sound alignment was noted at the twentieth week. The …

Capsular tightening at the shoulder is a common disorder, usually referred to as frozen of cases seen by physiatrist are those in which no specific cause can be determined for the stiffening. It may also be a sequela of other diseases, such as stroke, fracture, malignancy, etc. We present four cases of secondary frozen shoulder without conspicuous etiology. Intoxication of lithium carbonate may present as a multiple systematic dysfunction, but central and peripheral nervous involvement is rare and more troublesome. In the past, there was only one case of lithium intoxication with polyneuroparhy reported by Chang et al in Taiwan. We report here an additional 49 year-old man with manic-depressive psychosis. He received lithium carbonate medication treatment for 5 years and developed a severe cerebellar dysfunction and a mild sensori-motor polyneuropathy. Blood lithium level was 1.80meq/l two days after quitting lithium. Nerve conduction velocity studied, electromyography and sural nerve biopsy confirmed the peripheral neuropathy with marked evidence of …

This 67 years old, male, DM patient suffered from a falling accident, resulting in traumatic cervical disc herniation with epidural abscess. After discectomy, the patient sustained quadriplegic condition and needed mechanical ventilation due to breathing difficulty. In addition to the usual rehabilitation program, we also used transcutaneous electrical stimulation on this patient's various muscle groups in order to stimulate his muscles and to help his weaning from ventilator. Diurnally fluctuating hereditary progressive dystoria is a very rare disorder. In 1975, Segawa regarded this disorder as an entity, and there are 46 cases reported till 1986 since then. The clinical manifestation of this disease are: normal development, early onset, dyotonia, diurnal change, excellent response to L-dopa. In this report, a 21-year-old female, who had had symptoms onset since 8 years old; became wheel chair bound at 15 years of age; was not diagnosed correctly until 20 years old. The Effect Of Varying Weightload Intensities On Biceps Curl Technique, Jeffrey C. Gailhouse

This study described biomechanical deviations from proper biceps curl technique specifically in the shoulder, trunk, body and knee angles, that occurred in response to varying intensities of an individual's 1 RM (repetition maximum). Ten college age males participated in the study. Subjects were required to perform one repetition at 60%, 70%, 80%, 90% and 100% of his 1 RM. The only differentiating factor was the order in which the five intensities were performed. Condition order was randomized to minimize bias. Movement was filmed with a high-speed motion picture camera, digitized and then analyzed with computer software.

The Effect Of Vestibular Stimulation On Expressive Language Of Pre-Schoolers With Down Syndrome, Tiffany Bergo

Effects of vestibular stimulation, when presented prior to language therapy, on expressive language abilities in pre-school children with Down syndrome were examined. Two subjects participated; one subject completed the study. Vestibular stimulation was therapist- and child-initiated and provided by various types of equipment. The subject's expressive language use (verbal and non-verbal) was videotaped during fifteen minutes of language therapy across a six-week, eleven-session period.

Results of this study indicate that expressive language use increased or improved across many parameters during the vestibular stimulation phase of the study. The subject displayed greatest gains in the use of multiple word utterances and …

Occupational therapists treating older people with Alzheimer disease know that they must also consider the others who are affected by the disease, the informal caregivers. Intervention is most effective when it enables both the impaired person and the primary caregiver to manage the secondary symptoms of dementia. Unfortunately, little is understood about how caregivers approach and carry out their tasks and about why male and female caregivers respond differently to their caregiving role in terms of depression, burden, stress, and substance abuse. Post-event massage therapy has been proposed to hasten muscle recovery and improve ensuing performance in multistage ultraendurance cycling events. Six elite male cyclists competed in two 4·d stage races of 161 kmd·1 on a computerized race simulator over challenging terrain in a controlled laboratory environment with an average completion time of 275 ± 28 min. In a crossover experimental design, subjects received either 30 min post-event massage therapy or a 30 min inactivated diathermy treatment serving as a blind placebo after each 161 km race. Dementia Management: An Occupational Therapy Home-Based Intervention For Caregivers., Mary A. Corcoran, Laura N. Gitlin

This paper describes an occupational therapy intervention designed for family caregivers of persons with dementia. The intervention, based on the framework of a competence-environmental press model and the principle of collaboration, was implemented during 5 home visits. Each visit was designed to build caregiving skills through collaboration in identifying problem areas, developing and implementing environmental strategies, and modifying management approaches. A case vignette illustrates the therapeutic process and outcomes. The theoretical rationale and structure of the intervention and innovative documentation for evaluation of the theoretic process are also presented.

Cultural sensitivity is a crucial component of health care provision, particularly in psychiatric settings. As society becomes more multicultural, it is essential for occupational therapists to continue to develop cultural competence, which is defined in this paper as an awareness of, sensitivity to, and knowledge of the meaning of culture. At San Francisco General Hospital, an innovative multicultural model consisting of special focus programs is used. The key to the success of such programs is a culturally competent professional staff.

The F wave or F response results form the recurrent discharge of antidromically activated anterior horn cells. F response latency represents an impulse which travels to and from the spinal cord through the central segment of a nerve. Study of the F wave is commonly done to assess proximal conduction in motor nerves, to measure motor nerve conduction velocity over a longer pathway than in orthodromic motor studies, and to assess motor neuron excitability. The purpose of this study was to report the relationship among perceived intoxication, performance impairment, and actual blood alcohol concentration (BAC) levels. Fifteen subjects, aged 21 to 40, completed both single- and double-dose sessions of alcohol consumption_ BACs, reaction and anticipation time, and perceived intoxication data were collected during both sessions. Analysis of data showed that perceived intoxication was significantly related to performance impairment, but the actual BAC was not.

The problem under investigation was to determine if strengthening the abductor and adductor muscle groups of the hip would increase hamstring flexion strength. Forty females from aerobic classes were put into two groups, control and treatment, after being pretested on the Cybex II Dynamometer. The control group participated only in aerobics, while the treatment group participated in the aerobics and treatment program. The treatment consisted of four exercises working abduction and adduction of the thigh with SPRI's Xercise Band.

After six weeks all subjects were posttested on the Cybex II Dynamometer. Both groups had significant increases in hamstring strength. These …

This was a descriptive study of a curriculum development process for occupational therapy at Saginaw Valley State University in Michigan. A four-stage process was designed and documented. These stages included: (1) feasibility , (2) curriculum design and development, (3) review and evaluation-validation for each component, and (4 ) implementation stage.

Surgical management of cerebral palsy children typically involves multiple orthopaedic surgeries in order to achieve and maintain the maximum functional level of ambulation. Orthopaedic intervention to address lower extremity spasticity often includes: muscle lengthenings, muscle releases and tendon transfers. The most recent advance in management of lower extremity spasticity has been through a neurosurgical approach; selective dorsal rhizotomy. The long term effects of dorsal rhizotomy are still being investigated. The purpose of this independent study is a literature review of the principles of training and a critique of the shoulder strength training programs for increasing the throwing velocity of the baseball pitcher. In addition, a sport specific movement analysis, based on biomechanics, physiology and kinesiology of the baseball overhand pitch will be examined. The critique will be based on the principles of training reviewed earlier and a sport specific movement analysis. Finally, a proposal of a potential training program and possible topics for research will be discussed.

Sex offenders are often required by the court to enter therapy and receive help so they can stop deviant sexual behaviors. Mental health professionals must have some means of evaluating a mandated client's progress in therapy, however, there are currently no valid criteria available. A survey form was developed containing 73 items which professionals identified as having possible utility in evaluating progress.

The purpose was to determine how these concepts were used while addressing the teaching and learning theory competency of the accreditation standards.

The modification of a new design in orthosis usually has its clinical indication. In this study, a low temperature thermoplastic AFO was designed in anterior leaf type, called MIT (metatarsal into tibial )AFO, basically to meet the need of indoors barefoot walking in Taiwan. It could be easily molded and remolded to fit the foot, as well as to adjust the position of foot and ankle.Sixty-eight patients with acute hemiplegia were fitted with this MIT-AFO during ambulation training, 46had good fitting (67.6%), 15 in fair grade (22.1%), and 7 in poor grade (10.3%).
