A cancer other than non-melanoma skin cancer will occur in one of every seven persons. Individuals with higher risks for cancer are those with increasing age, hereditary disorders, and a family history of cancer. Lifestyle, especially the use of tobacco and alcohol, diet, and perhaps behavioral patterns influence the risk. Less well understood but clearly pertinent risk factors include environmental and occupational exposures. The risk factors specific for breast cancer include a positive family history of breast cancer, history of mastopathy, age at first pregnancy greater than 35 years, early menarche with late menopause, and history of carcinoma of the …

We are reporting a case of acute myeloid leukemia in a child with pericardial effusion and cardiac tamponade, with a review of the literature. Morphologic and cytochemical evidence was used to diagnose acute myeloid leukemia, and radiologic, electrocardiographic, echocardiographic, cytologic, and histologic evidence confirmed the diagnosis of tamponade and pericardial leukemic involvement. Pericardiotomy and pericardial window drainage successfully relieved the tamponade, and conventional chemotherapy was used to induce a remission.

Criteria for the diagnosis of pituitary carcinoma are subject to controversy. Some authors do not accept the demonstration of cellular anaplasia alone as sufficient to establish the diagnosis but require evidence of metastatic disease in order to label a pituitary tumor as malignant. However, pituitary tumors can metastasize even in the absence of anaplasia. The present case of Cushing's disease and pituitary carcinoma had no clinical evidence of metastases when first reported. Partial surgical removal of the tumor followed by irradiation resulted in clinical remission of the Cushing's syndrome, but two years later progressive uncontrollable recurrence of the disease ended …

Peritoneal Lavage Red Blood Cell And White Blood Cell Count: A Prospective Comparison Of Usefulness, Eugene Meyer, James C. Gruenberg

Prospectively, 637 patients with blunt or penetrating trauma to the lower chest or abdomen had quantitative peritoneal lavage red blood cell (RBC) and white blood cell (WBC) counts as part of their initial diagnostic evaluation. Statistically, the lavage RBC count (error rate: 6.8%) was a superior test to the lavage WBC count (error rate: 9.7%) (chi sq=9.2564, p=0.0024). See Appendix. When the lavage RBC and WBC tests agreed, the predictive value was high: 94% (46/49) when both were positive and 95% (521/549) when both were negative. Cancer of the breast and colon are well known to occur with high frequency in families with certain inherited cancer syndromes. These cancers also occur with increased frequency in families which do not have a recognizable genetic cancer syndrome. The fact of familial occurrence can be used to identify people with increased risk of cancer. In high-risk people methods for early detection can be developed which may reduce morbidity and mortality from these cancers, as has been done for certain inherited cancer syndromes.

External Beam Treatment Of Inoperable Non-Small Cell Carcinoma Of The Lung, H. Gunter Seydel

External beam radiation therapy for unresectable non-small cell carcinoma of the lung has produced significant survival in patients with disease limited to one hemithorax. New approaches to improve the results of radiation treatment of these patients include interstitial implantation, the use of hypoxic cell sensitizers, biologic response modifiers, as well as changes in the technique of radiation therapy. This includes the use of modern computerized tomographic scanning technology for better definition of tumor volume as well as the use of two fractions per day in the treatment of non-small cell carcinoma of the lung. The development of effective chemotherapy and improved techniques of radiation therapy offer the possibility of managing soft tissue sarcomas so that surgery may need to be less radical than in the past and amputation can be avoided. The low rates of local-regional and systemic recurrence that have resulted from using innovative combinations of chemotherapy, radiation therapy, and conservative surgery encouraged us to use preoperative intraarterial adriamycin (doxorubicin) with or without radiation therapy for three cases of soft tissue sarcomas of the lower extremities. The rationale for this therapy is also discussed.

For patients in whom unexplained elevations in SGOT and SGPT concentrations are found, determination of the serum iron level is recommended to screen for asymptomatic precirrhotic hemochromatosis. Care must be taken to differentiate between hereditary hemochromatosis and secondary iron accumulation due to liver disease.

The immunosuppressive properties of corticosteroids are well established and cause a depression of pulmonary defense mechanisms. We report the case of a patient treated with short-term corticosteroids for an exacerbation of chronic obstructive lung disease who developed a multiple organism pneumonia.

Recent technological advances in diagnostic ultrasound have resulted in the development of high-resolution, portable real-time scanners. In neurological ultrasound, these devices have been particularly useful for detecting intracranial hemorrhage in premature infants. Sonography is now being used during neurosurgical procedures to help in localizing masses for resection and biopsy, as well as in the placement of shunt catheters. The applications of neurosonology and our experience with intraoperative neurosonology are reported in this review.

Mosquitoes As Vectors Of Disease In Minnesota, John W. Washburn

Diseases due to mosquito-borne viruses occur every summer in Minnesota. The incidence of Western encephalitis and LaCrosse encephalitis is usually low, but outbreaks of Western encephalitis have occurred in the past. Evidence of Jamestown Canyon virus activity has been found in Minnesota. This virus may represent a newly-recognized cause of central nervous system disease and encephalitis. The epidemiology of the mosquito-borne encephalitis viruses found in Minnesota and the methods of disease surveillance and control are discussed.

Homosexual and heterosexual patients with the acquired immunodeficiency syndrome were compared by risk group. Race; diagnoses; history of sexually transmitted diseases, sexual behavior, and drug use; and socioeconomic indicators differed considerably among the risk groups, suggesting different risk factors for acquisition of the syndrome. Patients in the homosexual, intravenous drug user, and Haitian risk groups differed in their serologic response to cytomegalovirus and syphilis testing, presumably due to lifestyle-related exposures. Differences in the rate of recovery of cytomegalovirus, serum levels of IgA and IgG, and antibody titers to Epstein-Barr virus were noted among patients with different diagnoses. We conclude that …

Antimalarial activities ofchloroquine, mefloquine, amodiaquine, and quinine in vitro against Plasmodium falciparum were diminished as a consequence ofmembranefiltration. Filtered drug solutions gave ID₅₀ values up to 25-fold greater than those of non-filtered (ethanol-sterilized) drug solutions. Loss ofactivity byfiltration was overcome by increasing the drug concentration prior to filtration. Water solutions filtered through Millex-GSfilter units consistently showed an absorbance maximum at 277 nm, accompanied by a lesser peak at 225 nm. Water filtrates from Nucleopore and Millex-G Vfilters showed no absorbance at 277 nm and only slight absorbance was evidentfor the Gelmanfilter unit. Too often when development planners or practitioners plan income-generating schemes for women they consider only handicrafts. While in some situations craft production may provide a secure source of income for women, in many cases it results in poor returns and proves more complicated an undertaking than expected. The Bangladesh Rural Advancement Committee (BRAC) is one agency that has developed a successful program of non-craft employment opportunities for women. Some 10,000 poor women have been engaged by BRAC in viable economic schemes: 9,000 in non-craft production. The Minnesota Mosquito Research Program (MMRP) was established in September 1984 in response to a request by Governor Perpich. The Minnesota Department of Health (MDH), as the lead agency, brought together a Working Group of distinguished North American experts to discuss the problems related to mosquito research and control. This Working Group- with advice from legislators and representatives from the community, environmental groups, and state and federal agencies- made recommendations for the development of a mosquito research and control program in Minnesota.

Some units were equipped with a new drainage system called SIRDO (Integrated System for Recycling Organic Wastes), and families interested in living in the experimental block where the SIRDO was to be installed could be given housing right away.
