Cytomegalovirus gastroenteritis can be a life-threatening infection in patients with the acquired immunodeficiency syndrome. Although gastrointestinal ulcerations from cytomegalovirus have been widely reported, our patient is only the second case reported with actual perforations in the small bowel.

Multiple specimens from 124 patients from our clinic were evaluated for T4/T8 cell ratio over three to 28 months. Twenty-nine of 30 patients with the acquired immunodeficiency syndrome (AIDS), 58 of 71 patients with AIDS-related complex (ARC), and four of 23 patients at risk showed initial T4/T8 cell ratios of less than 0.8. which either remained unchanged or decreased. Seventeen of 23 AIDS patients and two of 20 ARC patients with a consistently low ratio (0.3 or less) died during the follow-up period. This indicated that the severity of disease could be predicted by periodical T4/T8 cell ratio studies. These …

The acquired immunodeficiency syndrome (AIDS) is a significant health problem. Patients with this disease develop unusual infections and neoplasms, many within the gastrointestinal tract. Of the 66 AIDS patients seen over a three-year period at Henry Ford Hospital, 25 patients had AIDS-related gastrointestinal disease, with one death attributed to this cause. The radiologic findings and pathologic correlation of this experience are presented.

Creutzfeldt-Jakob disease, a transmissible, rapidly progressive dementia of unknown etiology, mimics Alzheimer's disease, presents in middle life, and affects many central nervous system structures. The disease progresses in three distinct stages, culminating in death. Its occurrence is sporadic and its distribution worldwide. Pathological changes are varied, but spongy degeneration of the neuropil is classic. Research on scrapie, the animal model of Creutzfeldt-Jakob disease, has demonstrated that this unconventional, slow disease is transmitted via a small (less than 50,000 mW) particle, which is composed principally of protein. Acute renal failure developing during the clinical course of the acquired immunodeficiency syndrome (AIDS) has been related to complications of sepsis, nephrotoxic antibiotics, and recently to the development of glomerular lesions. Of 114 AIDS patients admitted to our hospital between January 1985 and June 1986. II patients (9.6%) developed acute renal failure. None of these II patients had a history of intravenous drug abuse or hypertension. All patients were male with an average age of 35 years old, 81% were black, and all were bisexual or homosexual. We have retrospectively reviewed our bronchoscopic experience in patients with suspected pulmonary complications of the acquired immunodeficiency syndrome (AIDS) or AIDS-related complex at Henry Ford Hospital between 1984 and 1986. The charts of 52 patients, representing a total of 87 bronchoscopic examinations, were available for review. The bronchoscopic findings as well as correlative chest roentgenograms and arterial blood gases are reviewed, and implications for therapy are discussed.

To evaluate the effectiveness of ultrasound in the management of postpartum problems, ultrasonographic examination was performed on 98 patients with suspected postpartum infection. The study includes only those patients who did not respond to antibiotic treatment for at least 48 hours and those patients who were readmitted to the hospital with suspected postpartum infection. Echogenic tissue was found in the uterine cavity in 12 patients, and significant fluid collection was found in 24 patients. Gas collection appeared in the uterine cavity in five patients and around the uterine incision in four patients. Fluid collection was found around the uterine incision …

Bilateral Anterior Cerebral Artery Occlusion In An Alcohol Abuser With Sickle-Cell Trait, Thomas H. Swanson, John L. Zinkel, Patti L. Peterson

The contribution of ethanol ingestion to brain infarction is unclear, although many studies suggest that the two may be causally related. We report an unusual case of bilateral anterior cerebral artery occlusion in a young female ethanol abuser with sickle-cell trait whose platelets showed hyperaggregability during epinephrine and adenosine diphosphate-induced aggregation experiments. It is concluded that ethanol withdrawal and dehydration along with direct effects of ethanol on platelet aggregation may result in cerebral artery thrombosis. Sickling of red blood cells in the distal circulation also may be a compounding factor, but this is not proven.

A growing body of literature attests to the value of employer-supported child care initiatives. This descriptive research documented the characteristics and relationship of Connecticut employer-supported child care centers with their sponsoring organizations. Data concerning the factors that lead to their implementation, aspects of company operation perceived as having been affected by the child care program, and attitudes of employer-sponsored child care center directors, regarding their company's sensitivity to employee issues, were collected· by interviews with 14 day center directors. Results indicated that programs were initiated to improve a variety of work-related productivity measures and achieved the effects for which they …

This issue of SEEDS focuses on ways in which women have been involved in a government forest conservation and restoration program in Nepal. As is common with many large-scale projects with a general impact, women were not a direct focus of the project's original design. As activities got underway, however, both the Nepali staff and their expatriate colleagues quickly realized that the direct involvement of women was crucial to the success of the project's participatory strategy. Over the initial five years, 1980 to 1985, a number of approaches to addressing women's needs and generating their active participation were tried. The …

More than 500 cases of acquired immune deficiency syndrome (AlDS) in children (ages I3 and under) had been reported to the Centers for Disease Control as of August , 1987. Of these children, 78% were infected perinatally by their mothers. The ratio of AIDS cases among women to AIDS cases among ; i children has remained stable at approximately 5:1 for several years. So, for the foreseeable future, we can expect that for every five reports of women with AIDS, one child with AIDS will be reported. Developments in medicine and constitutional law dictate modification of public health legislation in the United States. Traditionally overlooked by legislators, present public health laws provide inadequate decision-making criteria and inappropriate procedures for dealing with issues. Revised legislation should provide health care officials and agencies with the tools to balance individual rights against public health necessities. This article makes four recommendations for legislative reform: (1) remove artificial legislative distinction between venereal and other communicable diseases; (2) provide criteria defining "public health necessity" to limit discretionary exercise of police power by health officials; (3) provide strong confidentiality protections in the collection and …

Examination of coprolites excavated from archaeological sites in the Americas demonstrates excellent preservation of helminth eggs and, in some cases, larvae. To gain an understanding of helminth parasitism in prehistory on the Colorado Plateau of Arizona, New Mexico, and Utah, 319 coprolites from 5 archaeological sites were analyzed. Helminth eggs and larvae were recovered after the coprolites were rehydrated, screened, and sedimented. At a sixth site, soils excavated from 5 rooms used as latrine areas were processed with palynological techniques. The results indicate that all but 1 of the prehistoric populations examined were infected with intestinal worms. The helminths implicated …

This essay serves as the foreword to Public Health & the Law, a symposium dedicated to Professor William J. Curran held in 1987.

During his career, Professor Curran chaired the Harvard School of Public Health Committee on Human Research; he directed the Program in Law and Public Health; and he was co-director of the Harvard Interfaculty Program in Medical Ethics from 1973 to 1980. He was also an advisor to the World Health Organization and spent two sabbatical periods in Europe with WHO organizations. He advised and lectured in countries throughout the world.

Mission To collect and report complete, high-quality, and timely population-based cancer data in Louisiana to support cancer research, control, and prevention. Vision To reduce suffering and death from cancer using information collected by the Louisiana Tumor Registry. History Cancer registration in Louisiana began in 1947 at the Charity Hospital Tumor Registry in New Orleans. In 1974, as part of its Surveillance, Epidemiology and End Results (SEER) Program, the National Cancer Institute provided funds for a population-based cancer incidence and survival registry covering Jefferson, Orleans, and St. Bernard parishes.
