The spread of Human Immunodeficiency Virus (HIV) is one of the greatest threats to stability and development in Kenya. The country's infection rate is estimated between seven and eight percent, making it one of the highest in the world. Several studies have been done that examine subpopulations such as truck drivers, sex workers, and migrants. However, to date there is little research that examines who, across the country, is contracting HIV. Kenya's 2003 Demographic and Health Survey includes data that make it possible to determine which groups have the highest odds of being infected. The question of who is contracting …

According to the 2000 Census, 1 of every 5 children in the United States is a child of immigrants – either a child who is an immigrant or has at least one immigrant parent. While most children who experience mental health problems have limited access to help, children who have migrated to this country, especially under difficult circumstances, face particular challenges. Background - Physical activity behavior is influenced by a person's physical environment, but few studies have used objective measures to study the influences of the physical environment on physical activity behavior in youth. The purpose of this study was to examine the relationship between selected neighborhood physical activity resources and physical activity levels in high school girls.

Methods - Participants were students in schools that had participated in a large physical activity intervention trial. The 3-Day Physical Activity Recall was completed by 1506 12th-grade girls. Race Differences In Activity, Fitness, And Bmi In Female Eighth Graders Categorized By Sports Participation Status, John R. Sirard, Karin A. Pfeiffer, Marsha Dowda, Russell R. Pate

The purpose of this study was to identify racial differences in physical activity (PA), fitness, and BMI in female 8th-grade sports participants and nonparticipants. Girls from 31 South Carolina middle schools (N=1,903, 48% White; mean age=13.6 ± 0.63) reported PA and previous year sports-team participation, completed a submaximal fitness test, and had height and weight measured. Sports team participation was positively associated with PA and negatively associated with television viewing and BMI, in a dose-response manner. Compared with Whites, African-Americans reported less PA and more television viewing, and had greater BMI scores. Whereas PA intervention programs that incorporate …

The purpose of this study was to examine the relationships among self-efficacy levels, physical activity, aerobic fitness, and body composition (relative body mass index; RBMI) and to determine whether a school-based pedometer intervention program would improve those variables and prevent weight gain in sixth and seventh graders in rural mid-South middle schools. Bandura’s Social Cognitive Theory served as the theoretical basis for the study.Methods: A quasi-experimental pre-test, post-test design was used. The sample, recruited from two rural middle schools in Kentucky, consisted of 116 sixth and seventh grade students (98% Caucasian, 51% male, age=11.65±0.71). This descriptive correlation study had four purposes: describe depressive and anxiety symptom levels in pregnant and non-pregnant African American (AA) HIV-positive women, examine the association between underlying vulnerabilities and depressive symptoms in HIV-positive AA women, determine the association between underlying vulnerabilities and anxiety symptoms in AA HIV-positive women, and determine the predictors of depressive and anxiety symptoms among African American HIV-positive women. A convenience sample of 80 African American HIV-positive women was recruited from university-based obstetrics/-gynecology and infectious disease practices in a large metropolitan city in the Mid-South. Carbohydrates are a major energy source for the human body and particularly glucose is the only energy source for the brain. Thus glucose metabolism is important to maintain normal brain function. Evidence showed insulin resistance and diabetes are associated with cognitive decline and a large amount of highly processed carbohydrate intake; in other words, a high glycemic load diet, which increases blood glucose faster and insulin demand, is associated with increased risk of insulin resistance and diabetes.

Di kelurahan Kampung Melayu, penyakit infeksi amuba menempati urutan 9 dari 10 besar penyakit di wilayah tersebut. Diduga di sana banyak terdapat kasus infeksi amuba asimptomatik yang dapat menular ke orang lain akibat kondisi sanitasi lingkungan dan higiene yang buruk. Penelitian ini bertujuan untuk menilai hubungan antara kontaminasi air oleh E.histolytica dengan kejadian infeksi amuba asimptomatik pada anak usia sekolah. Penelitian dengan desain penelitian case control ini dilakukan terhadap 46 kasus dan 46 kontrol. Data dikumpulkan melalui pemeriksaan laboratorium, wawancara dan observasi. Bencana tsunami melanda Nanggroe Aceh Darussalam pada tanggal 26 Desember 2004, selain meningkatkan kejadian luar biasa (KLB) malaria juga memunculkan daerah-daerah endemis malaria. Penelitian ini dilakukan untuk mendapatkan prevalensi penderita malaria di Pulau Weh, pasca Tsunami pada akhir 2004. Penelitian dilakukan secara ‘cross sectional’. Diagnostik malaria ditegakkan berdasarkan pemeriksaan apusan darah (mikroskopik). Ditemukan penurunan kasus malaria di Pulau Weh. Prevalensi penderita malaria yang diperoleh adalah 15,3%. Dari semua penderita malaria, 41,4% tanpa gejala klinis demam. Penderita yang tidak mengalami gejala klinis demam tersebut, umumnya mempunyai gejala klinis badan pegal, pusing, gangguan pencernaan dan lemas. Penurunan prevalensi malaria dalam penelitian ini …

Pengaruh bahaya bising dan kimia terhadap timbulnya gangguan pendengaran diketahui dengan baik, tetapi pengaruh penyakit kronis hipertensi terhadap timbulnya gangguan pendengaran sensorineural belum banyak timbulnya gangguan pendengaran sensorineural. Penelitian ini dilakukan di Perusahaan minyak A di Kalimantan Timur, Indonesia. Subyek penelitian adalah pekerja terpajan bahaya bising dan kimia. Desain studi yang digunakan adalah Kasus Kontrol. Kasus adalah penderita gangguan pendengaran yang dilihat pada data medical check up tahun 2003. Hasil pengukuran dosimetri kebisingan rata-rata 87,12 dBA (150,98 %) yang merupakan TWA 12 jam. Kholinesterase darah merupakan salah satu indikator keracunan pestisida yang masih menjadi masalah kesehatan masyarakat, khususnya diwilayah pertanian. Pada tahun 2005, di Kabupaten Majalengka, hasil pemeriksaan mendapatkan angka keracunan ringan (21,7%) keracunan sedang (32,5%) dan keracunan berat (3,6%) Penelitian ini bertujuan untuk mengkaji berbagai faktor determinan aktivitas kholinesterase darah pada petani penyemprot hama tanaman holtikultura. Penelitian yang menggunakan desain potong lintang dengan sumber data sekunder hasil pemeriksaan aktivitas kholinesterase darah yang dilakukan oleh Dinas Kesehatan Kabupaten Majalengka tahun 2007. Pencapaian kegiatan Usaha Kegiatan Sekolah dari Standar Pelayanan Minimal di Kabupaten Purwakarta, tahun 2005 masih jauh dibawah target standar pelayanan minimal secara nasional. Cakupan tersebut meliputi deteksi dini tumbuh kembang anak balita dan prasekolah (74,57%), pemeriksaan kesehatan siswa oleh tenaga terlatih (61,82%) pelayanan kesehatan remaja (51,17%). BACKGROUND: Decisions regarding whether to administer intensive care to extremely premature infants are often based on gestational age alone. However, other factors also affect the prognosis for these patients.

METHODS: We prospectively studied a cohort of 4446 infants born at 22 to 25 weeks' gestation (determined on the basis of the best obstetrical estimate) in the Neonatal Research Network of the National Institute of Child Health and Human Development to relate risk factors assessable at or before birth to the likelihood of survival, survival without profound neurodevelopmental impairment, and survival without neurodevelopmental impairment at a corrected age of 18 to …

A large literature in medicine documents variation across areas in the use of surgical treatments that is unrelated to outcomes. Observers of this phenomena have invoked “flat of the curve medicine” to explain these facts, and have advocated for reductions in spending in high-use areas. In contrast, we develop a simple Roy model of patient treatment choice with productivity spillovers that can generate the empirical facts. Our model predicts that high-use areas will have higher returns to surgery, better outcomes among patients most appropriate for surgery, and worse outcomes among patients least appropriate for surgery, while displaying no relationship between …

For decades, the federal government has targeted health care funding, resources and staff to meet the health care needs of areas designated as "medically underserved areas" and "health professional shortage areas." Areas that qualify may, for example, receive federal funding to support the establishment and operation of community health centers, or receive National Health Service Corps (NHSC) physicians and clinicians. In addition, physicians who practice in these health shortage areas may receive higher payments under Medicare. Numerous safety net programs and health care providers depend on Medically Underserved Area and Population (MUA/P) and Health Professional Shortage (HPSA) designations to qualify for federal funding, physician subsidies and placement, and health-related investments to improve access to care for communities and populations at high risk of poor health. These resources are particularly critical for federally-qualified health centers at a time when the number of uninsured is growing and the capacity of the safety net shrinking. On February 29, 2008, the Department of Health and Human Services (HHS) released a proposed regulation to alter the way these designations are made. …

In 1965, Congress enacted the Older Americans Act, establishing a federal agency and state agencies to address the social services needs of the aging population. The mission of the Older Americans Act is broad: to help older people maintain maximum independence in their homes and communities and to promote a continuum of care for the vulnerable elderly. In successive amendments, the Act created area agencies on aging and a host of service programs. The “aging services network,” broadly described, refers to the agencies, programs, and activities that are sponsored by the Older Americans Act. The Act’s funding for services is …
