Helminth Remains From Prehistoric Indian Coprolites On The Colorado Plateau, Karl J. Reinhard, Richard H. Hevly, Glenn A. Anderson Jan 1987

Helminth Remains From Prehistoric Indian Coprolites On The Colorado Plateau, Karl J. Reinhard, Richard H. Hevly, Glenn A. Anderson

Karl Reinhard Publications

Examination of coprolites excavated from archaeological sites in the Americas demonstrates excellent preservation of helminth eggs and, in some cases, larvae. To gain an understanding of helminth parasitism in prehistory on the Colorado Plateau of Arizona, New Mexico, and Utah, 319 coprolites from 5 archaeological sites were analyzed. Helminth eggs and larvae were recovered after the coprolites were rehydrated, screened, and sedimented. At a sixth site, soils excavated from 5 rooms used as latrine areas were processed with palynological techniques. The results indicate that all but 1 of the prehistoric populations examined were infected with intestinal worms. The helminths implicated …

The Future Of Public Health Law, Lawrence O. Gostin Jan 1987

The Future Of Public Health Law, Lawrence O. Gostin

Georgetown Law Faculty Publications and Other Works

Developments in medicine and constitutional law dictate modification of public health legislation in the United States. Traditionally overlooked by legislators, present public health laws provide inadequate decision-making criteria and inappropriate procedures for dealing with issues. Revised legislation should provide health care officials and agencies with the tools to balance individual rights against public health necessities. This article makes four recommendations for legislative reform: (1) remove artificial legislative distinction between venereal and other communicable diseases; (2) provide criteria defining "public health necessity" to limit discretionary exercise of police power by health officials; (3) provide strong confidentiality protections in the collection and …

Echocardiographic Features Of Metastatic Pericardial And Myocardial Malignancy, Carey Green, Mohsin Alam, Howard S. Rosman, Jeffrey B. Lakier

Henry Ford Hospital Medical Journal

Clinical and echocardiographic features of a patient with extensive metastasis to the pericardium and myocardium are presented. Echocardiography revealed pericardial effusion, markedly thickened pericardium, and myocardium with increased echogenicity. These findings were subsequently confirmed during surgery and autopsy, which revealed marked tumor encasement of the heart and pericardial myocardial infiltration.

Radiographic Features Of Anterior Cruciate Ligament Reconstruction, Mark I. Burnstein, Burton I. Ellis, Robert A. Teitge, Martin L. Gross, Christopher K. Shier

Henry Ford Hospital Medical Journal

Anterior cruciate ligament disruption is a common injury that occurs in contact sports such as football. The treating orthopedic surgeon may elect any of a variety of therapeutic options. Surgical management may consist of primary repair of the torn ligament or replacement of the torn ligament with graft material, known as anterior cruciate ligament reconstruction (ACER). Many physicians, including radiologists, are unfamiliar with the surgical procedure or the expected postoperative radiographic appearance of ACER. Assessment of radiographs following ACER, as with many surgical procedures, requires understanding of the surgical procedure. We present our experience in assessing the postoperative radiographs of …

The First-Bite Syndrome, William S. Haubrich Dec 1986

The First-Bite Syndrome, William S. Haubrich

Henry Ford Hospital Medical Journal

Patients presenting with esophageal disorders often describe what can be called a "first-bite syndrome." The condition can be discerned by its characteristic clinical features. It may be a variant of diffuse esophageal spasm. While in a majority of patients it is a benign functional disturbance, it can be a harbinger of carcinoma. When of functional origin, it is amenable, in most cases, to relatively simple medical management.

Small Cell Carcinoma Of The Lung In The Intensive Care Unit, Paul Harkaway, Cynthia Glines, Michael Eichenhorn, Paul Kvale, Robert Chapman, John Popovich Jr.

Henry Ford Hospital Medical Journal

The outcome of 29 patients with a diagnosis of small cell carcinoma of the lung admitted to the medical intensive care unit (MICU) from 1980 through 1984 was reviewed retrospectively. Respiratory failure was the most common admitting diagnosis (23 patients [80%]). followed by cardiopulmonary arrest (three patients [10%]), and hypotension (three patients [10%] ). Only five patients survived to leave the MICU, and only two of these patients lived longer than two months after MICU discharge. Of the features examined, the absence of sepsis was the only statistically significant predictor of MICU survival. Treatment of the malignancy did not appear …

Water-Cover Influences On Diatom Ingestion Rates By Meiobenthic Copepods, Alan W. Decho Oct 1986

Water-Cover Influences On Diatom Ingestion Rates By Meiobenthic Copepods, Alan W. Decho

Faculty Publications

Laboratory experiments on meiobenthic copepods using 14C-diatoms were conducted to investigate whether: (1) feeding rates or (2) food sources (planktonically-suspended foods or benthic sediment-associated foods) vary in response to the presence or absence of water-cover (i.e. simulated Higher-water vs Low-water conditions). Three diatom-feeding harpacticoids were examined. Scottolana canadensis feeds at significantly higher rates (2x) during Higher-water (HW) conditions (P<0.001), at which time it consumes planktonic foods; during Low-water (LW), feeding is greatly reduced. These feeding patterns are related to its burrow-dwelling and to its subtidal habitat. Cletocamptus deitersi remains virtually unaffected by changes in ambient water-cover, feeding at nearly equal rates during HW and LW conditions but always tending to consume more benthic diatoms. Microarthridion littorale consumes food at nearly equal rates during HW and LW …

Altered Platelet Function In Patients With Severe Congestive Heart Failure, Syed M. Jafri, Jeanne M. Riddle, Sundara B. K. Raman, Sidney Goldstein Sep 1986

Altered Platelet Function In Patients With Severe Congestive Heart Failure, Syed M. Jafri, Jeanne M. Riddle, Sundara B. K. Raman, Sidney Goldstein

Henry Ford Hospital Medical Journal

Platelet function was assessed in 15 patients with severe congestive heart failure (CHF) and in 26 control subjects of similar ages. The platelet count (mm3), surface reactivity, aggregometry studies, release factors, and circulating aggregates were investigated. The mean number of circulating platelets was normal, but a hyperactive platelet response was found in 53% of the CHF patients. CHF patients had a 42% mean for the spread type platelet, and the average number of aggregates was 64: control subjects had a 12% mean for the spread type platelet, and the average number of aggregates was 40 (p < 0.05). Aggregation with all of the inducers was normal, although 27% of CHF patients showed spontaneous aggregation. The mean plasma levels of both platelet factor 4 and betathromboglobulin were abnormally elevated. No circulating platelet aggregates were detected. Our studies indicate that platelet function is abnormal in patients with CHF. The abnormal platelet reactivity found might contribute to the increased incidence of thromboembolic events observed in CHF patients.

"Congestion": An Infrequent Feature In Patients With Chronic Heart Failure On Therapy, Syed M. Jafri, Jeffrey B. Lakier, Howard S. Rosman, Sidney Goldstein

Henry Ford Hospital Medical Journal

The clinical presentation of chronic heart failure has changed with advances in therapy, and the physical findings of congestion are now inconsistently present. To determine the clinical signs of congestion, we studied the clinical findings of 44 patients who had chronic heart failure and abnormal left ventricular ejection fractions (23 ± 10%). Signs of pulmonary edema manifested as rales were absent in all patients in New York Heart Association (NYHA) classes I and II, in 66% of patients in NYHA class III, and in 14% of patients in NYHA class IV. Signs of right heart failure manifested as elevated jugular …

Echocardiographic Evaluation Of Patients With Congestive Heart Failure, Mohsin Alam Sep 1986

Echocardiographic Evaluation Of Patients With Congestive Heart Failure, Mohsin Alam

Henry Ford Hospital Medical Journal

Echocardiography is a useful noninvasive test for evaluating patients with suspected heart failure and enlarged cardiac silhouette. This test can obtain the dimensions and wall thickness of all four cardiac chambers and can evaluate left ventricular function. The test results are reproducible, and in many instances the etiology of heart failure can be determined.

Status Of Cardiac Transplantation With A Report Of The First Year's Experience At Henry Ford Hospital, Fraser M. Keith, Donald J. Magilligan Jr., Jeffrey B. Lakier, Conrad Drost

Henry Ford Hospital Medical Journal

Cardiac transplantation is now widely accepted as effective treatment for selected patients with end-stage heart disease. Improvements in immunosuppressive treatment and monitoring in the past 15 years have resulted in impressive one-year and five-year survival rates of 80% and 60%. In general, survivors have enjoyed a considerably improved quality of life. A cardiac transplantation program was initiated at Henry Ford Hospital in 1985, and a total of 15 patients received heart transplants in the first 12 months. Fourteen patients are currently alive and well at various stages posttransplant. This early clinical success has prompted the consideration of both combined heart-lung …

Early Identification Of Risk For Noncompliance In Tuberculosis Patients, Penny Klein Weismuller Sep 1986

Early Identification Of Risk For Noncompliance In Tuberculosis Patients, Penny Klein Weismuller

Loma Linda University Electronic Theses, Dissertations & Projects

In this descriptive study of early identification of risk for noncompliance to tuberculosis treatment or prevention regimens, 523 patients, from four ethnic groups, were rated by nurses trained as participant-observers of several predictive factors. The predictive factors included acceptance of diagnosis. attentiveness, eye contact, the treatment's interference with the patient's lifestyle, fear of side effects, fear of needed medical procedures, and the presence and degree of the patient's social support. An observational measure by the nurse of the patient's Health Locus of Control was also included. The patient was asked to provide a self-rating on the degree of difficulty anticipated …

Trauma In Pregnancy, Victor J. Sorensen, Brack A. Bivins, Farouck N. Obeid, H. Mathilda Horst

Henry Ford Hospital Medical Journal

Management of 12 pregnant trauma patients is reviewed. These patients had injuries to the neck, chest, abdomen, and extremities. Six patients had penetrating wounds, and six others had blunt injuries. Eight of the 12 women required surgical intervention during their hospitalization. Liberal use of diagnostic procedures such as roentgenograms, diagnostic peritoneal lavage, and angiography aided in the evaluation of these patients. None of the 12 patients died, and only two developed major complications. One fetus died as a result of shock in the mother and only one major neonatal complication occurred. Rapid resuscitation combined with thorough diagnostic evaluation and early …
