Caring for terminally ill patients requires effective communication skills. It is important that medical personnel understand appropriate ways to communicate with the patient in order to inform him about his illness, throughout its duration. Physicians must develop an understanding of how patients communicate and ways to respond to that communication. Furthermore, the physician needs to communicate with the family of the terminally ill patient. Effective communication must become a regular part of the treatment.

This paper is a review of the literature on mobile crisis services and other forms of modern crisis intervention services. It is the first part of a larger project, termed the North Central West Virginia Crisis Study. The purpose of this literature review is to identify recent published literature on crisis services, with particular attention to crisis response services. No attempt has been made to survey the voluminous literature on crisis intervention practice skills and techniques.

A Descriptive Study Of Commitment Practices In A Selected Jurisdiction, Louis A. Pesnicak

This study examines the application of commitment procedures in one urban community in Virginia. The study investigated: the concordance rates of clinical recommendation for treatment and the outcome of the commitment hearing. the time lapse between detention of an individual and the hearing outcome. the presenting symptomatology and outcome. and the pre-detention compliance to treatment and outcome. The study's findings showed a concordance rate of 64% between clinical recommendation and outcome. It provided no significant differences in symptomatology/outcome, compliance/outcome, and time lapse/outcome. The lack of any significant findings seems to suggest that application of commitment laws might benefit from institution …

Cushing's syndrome caused by ectopic secretion of adrenocorticotropic hormone (ACTH) is often a serious disease and a diagnostic dilemma. In the reported patient, the source of ACTH proved to he a benign pulmonary carcinoid tumor. The patient describes his trying experiences through the six months from initial diagnosis to definitive therapy.

The purpose of this study was to investigate the differences in physiological and subjective perceived exertion levels at maximal and submaximal treadmill exercise between three phases of the menstrual cycle in eumennorheic women.
Nine females of varying fitness levels were subjects in the study. None of the subjects were using oral contraception or hormone altering medications at the time of the study.
The subjects performed three separate maximal graded exercise tests (GXT) using the Balke protocol. During each GXT, oxygen uptake (VO2), heart rate (HR), blood pressure (BP), respiratory quotient (RQ), rate of perceived exertion, and total time …

These proceedings are from a seminar organized by the Department of Obstetrics, Gynaecology, and Perinatology, Obafemi Awolowo University, Ile-Ife (Nigeria) in collaboration with the Population Council. The primary purpose of the multidisciplinary seminar was to identify the determinants of the high rate of mortality and morbidity from unsafe abortion in Nigeria. The specific objectives were: 1) to identify measures that could be undertaken on a short- and long-term basis to reduce the rate of abortion-related mortality, and 2) to set an agenda for research into abortion in Nigeria. The effect of psychosocial factors on the immune response of HIV infected human subjects was assessed by measuring the degree of relationship to CD-4 T cell count and physical symptoms.
Twenty six participants (22 white and 2 black males) all who had been diagnosed as HIV positive completed a battery of 11 psychological tests and a social and medical history questionnaire. Fifty four psychosocial factors were selected as having the potential to have a significant effect on immune response.
Correlational analyses yielded no strong relationships between the psychosocial factors and CD-4 T cell count or physical symptoms. Moderate associations were …

This presentation to the 1991 Berzelius Symposium, Stockholm, Sweden, explores the issue of maternal risk—the probability or chance of dying or being seriously injured in pregnancy—as it is used in maternal health care. This concept of risk has been a useful tool for research and medical and epidemiological education, but its use as a tool for service delivery design has been more problematic. In order to construct a risk system, one has to have reliable data on the relationships between individual characteristics and the outcome being studied—something that is difficult to develop with regard to maternal health. This article returns to a war waged virtually throughout this century--a war between the theories of punishment and rehabilitation in curtailing the drug epidemic. Today, the terms of the war are recast as supply-side policies based upon law enforcement; destroying crops in source countries; interdiction and increased sentencing; and demand reduction based upon prevention, education, and treatment. The war on drugs has reached a feverish pitch. New policies and statutes have tightened the grip of supply-side policies, with images of battle and hate mongering which go beyond the vilified drug lords and governments which harbor them, to the middle men, …

This paper analyzes the law, ethics and public policy concerning "genetic discrimination," defined as the denial of rights, privileges or opportunities on the basis of information obtained from genetically based diagnostic and prognostic tests. The Human Genome Initiative will enhance the ability to gather and organize information that may predict a person's future potential and disabilities. Enormous human benefits may ensue from understanding the etiology and pathophysiology of genetic disorders, including disease prevention through genetic counseling, and treatment of the disorders through genetic manipulation. Drug dependence and human immunodeficiency virus (HIV) are America's two most pressing epidemics, interconnected by a cycle of urban poverty, physical dependence and a culture of sharing needles and syringes. Extant political strategies to curb these interconnected epidemics involve two traditional approaches. The first--law enforcement and interdiction--is designed to limit the supply of illicit drugs to the marketplace. This strategy is advanced by broad criminal sanctions against importing, selling, distributing, medically prescribing, or possessing illicit drugs or drug paraphernalia. The second strategy to combat the drug and HIV epidemics involves reducing the demand for illicit drugs. Education, counseling, and treatment …

Between August 1988 and January 1989 I visited niQe countries in the Western Pacific, North America and Europe to survey women's health promotion from the perspective of the women's health movement. The experience was facilitated by study leave (Professional Experience Programme) from the Western Australian College of Advanced Education (now Edith Cowan University).

In most cases there are safe art material substitutes or alternative working methods which may be used. Unfortunately, many art teachers and their students still may not be fully aware of the dangers posed by a variety of hazardous materials and processes.

Investigation of the neuropsychological sequelae of brain tumors is extremely complex largely because the neurobehavioral consequences of brain tumors depend upon complex interactions among disease and treatment variables as well as patient characteristics. To illustrate some of these complexities, we present case studies of two patients in whom the behavioral outcome was not easily predictable on the basis of our current understanding of brain-behavior relationships in tumor patients. The case studies do illustrate how neuropsychological evaluation aids in identifying cognitive deficits which have implications for subsequent quality of life. Reports of abnormalities of regional cerebral blood flow (rCBF) in patients with schizophrenia and major affective disorder are contradictory. To gain a better understanding of the possible sources of these contradictions, the rCBF and neuropsychological functioning of 15 schizophrenics, 15 patients with major affective disorder, and 15 age-matched controls were studied. No group differences in overall rCBF, percent frontal devialion in flow, or rCBF asymmetries were observed. Both overall rCBF and percent frontal deviation of rCBF were reduced with greater age and with increasing neuropsychological impairment. Dosage of neuroleptic medication was associated with reduced whole brain flow, not with percent …

Recent research suggests that the most potent feature of the Type A behavior pattern for prospectively predicting cardiac disease is aggravation, irritation, anger, and impatience (AIAI). The present study examines psychometric properties of a new AIAI measure and its relationship to the severity of coronary artery disease (CAD). Subjects included 61 males undergoing coronary angiography. Comparisons were made of mean psychometric scores across groupings, defined by number of vessels occluded. These comparisons showed that the normal or subclinically occluded coronary angiographic group had high levels of depression and anxiety. Patients with narcolepsy have more psychiatric symptoms than normal controls as measured by psychometric tests. However, it is unclear whether these findings are specific to narcolepsy, as some studies have suggested, or related to excessive daytime sleepiness (EDS) or to chronic illness. We compared a group of 56 narcoleptics to age- and sex-matched controls with EDS. A group of 48 individuals with normal sleep architecture was also used as an additional control group. Both the narcoleptic group and the EDS-control group had significantly greater scores on Minnesota Multiphasic Personality Inventory scales hut were not different from each other. Our data …

There seems little that can be done to eradicate or even control natural disasters, considering the current state-of-the-art. If nothing can be done to prevent them, we must be prepared to cope with them whenever they do occur.
One possibility for greater assurance that hospital will properly accomplish its post disaster mission is an up-to-date, well-thought-out " Hospital Emergency Preparedness Plan." A well written and rehearsed disaster plan is essential for the proper and timely care of casualties arising from an internal or external disaster.

Infants born to adolescent and young adult mothers who abuse substances, are at high risk for chemical dependency and its sequelae. There is a need to identify such infants and their mothers so that intervention will be available to them. The relationship between moderate exercise training (five 45 min sessions/week, brisk walking at 62+2% V02max for 15 weeks) and changes in nutrient intake was investigated in a group of 36 sedentary, mildly obese women, conducted using a two (exercise, EX and nonexercise, NEX groups) x three (baseline, six-, 15-week testing sessions) factorial design, with data analyzed using repeated measures ANOVA. The pattern of change over time between groups for kilocalorie intake tended to be different between groups [F(2,68)=2.50,p=0.089] with the EX group experiencing a significant decrease versus baseline by 15 weeks.
