Management Of Advanced Endodermal Sinus Tumor Of The Ovary With Preservation Of Reproductive Function, Michael L. Hicks, Shari L. Maxwell, Wooshin Kim Mar 1990

Management Of Advanced Endodermal Sinus Tumor Of The Ovary With Preservation Of Reproductive Function, Michael L. Hicks, Shari L. Maxwell, Wooshin Kim

Henry Ford Hospital Medical Journal

Endodermal sinus tumor of the ovary occurs primarily in children and young adults. This tumor is classified as a germ cell tumor which comprises approximately 5% of all ovarian tumors. In the past, patients with this highly malignant tumor had low survival. However, with the evolution of effective adjunctive chemotherapeutic regimens, specifically vincristine, actinomycin D, and cyclophosphamide, survival rates have improved. In an attempt to continuously improve survival rates, other combination chemotherapeutic regimens are currently under investigation. Unlike epithelial ovarian malignancies, endodermal sinus tumor can be monitored by a specific tumor marker, alpha-fetoprotein (AFP). Serum levels of AFP can be …

Home Safety-The Challenge To Public Health, Ray Ranson Mar 1990

Home Safety-The Challenge To Public Health, Ray Ranson

The Journal of Sociology & Social Welfare

Home accidents are now a leading cause of death and injury particularly in young children and the elderly. For example, 3.1 million accidents occur in the home every year in the United Kingdom with no signs of diminishment. More effective systems of accident recording, monitoring, investigation, intercollaboration and education are urgently needed to redress this epidemic. More attention needs to be given to preventive safety design in architect training and adoption of home safety design standards and legislation. The major threat to public health which home accidents represent must not go unchallenged.

Health and Wellness

Research indicates that 74% of all nursing home patients experience eating difficulties sometime during their stay (Trupe, Siebens, & Siebens, 1984). Additionally, 59% of patients suffering from stroke experience some degree of dysphagia and aspiration difficulties (Echelard, Thoppil, & Melvin 1984). A significant number of the high risk dysphagia patients described above suffer from life threatening aspiration pneumonia. Consequently the management of swallowing disorders (Dysphagia) is of critical concern to hospital and nursing home personnel. Patients specficially at risk for dysphagia, according to recent studies, include those with head injury, stroke (CVA), and cerebral palsy. Also, patients experiencing cancer of …

Domestic Violence: Physicians A Link To Prevention, Mary Guinan Jan 1990

Domestic Violence: Physicians A Link To Prevention, Mary Guinan

School of Public Health Faculty Research

Domestic violence may result in more injuries and fatalities for women than automobile crashes, muggings, and rapes combined, but according to a recent editorial, physicians largely ignore the problem. Evidence showed that a majority of women treated at an emergency facility for injuries resulting from violent acts of spouses or other persons known to the victims were discharged without any arrangement being made for their future safety. Physicians rarely inquired about an injury victim's past sexual or physical abuse, living arrangements, or future prospects of safety.

Amwa Endorses Uniform Mammography Screening, Mary Guinan

School of Public Health Faculty Research

Breast cancer is gaining on us at an increasing rate each year. In 1990 in the United States alone we expect about 145,000 new cases and 45,000 breast cancer deaths in women. What are we doing about it? What is AMWA doing, and what are women physicians doing- for themselves and their patients? Can we do anything? The answer is yes, we can. We can reduce mortality considerably if the cancer is detected early. The five-year survival rate for women with breast cancer diagnosed in its early (localized) stages is 90%, but for women with cancer diagnosed after it has …

Man/Hombre/Homme: Meeting Male Reproductive Health Care Needs In Latin America, Debbie Rogow, Judith Bruce, Ann Leonard Jan 1990

Man/Hombre/Homme: Meeting Male Reproductive Health Care Needs In Latin America, Debbie Rogow, Judith Bruce, Ann Leonard

Poverty, Gender, and Youth

This edition of Quality/Calidad/Qualité answers the following question: how can family planning programs understand and better serve the interests of men? The proposals in the article are derived from the experience of PRO-PATER, in São Paulo, Brazil, and briefly, from the activities of the Profamilia Men’s Clinic in Colombia. The experiences of both PRO-PATER and Profamilia suggest that, although the reproductive health needs of men may be different from those of women, they are still very interested. It is clear that there is abundant demand for high-quality services that offer convenience, confidentiality, information, attentive providers, and attention to reproductive health …

A Decade Of A Maturing Epidemic: An Assessment And Directions For Future Public Policy, Lawrence O. Gostin Jan 1990

A Decade Of A Maturing Epidemic: An Assessment And Directions For Future Public Policy, Lawrence O. Gostin

Georgetown Law Faculty Publications and Other Works

The author's goal in this article, is not merely to propose public health strategies for the future, but also to examine why government has been so slow, so equivocal, in its public health response to the acquired immune deficiency syndrome (AIDS) epidemic. He argues that there has been a fundamental ambivalence in perceptions of the epidemic. For some, AIDS is perceived as a disease, with sympathy for sufferers. Once AIDS is viewed as a disease, like other catastrophic diseases, it follows that public policy will be based upon science and epidemiology--health education, research and treatment.

For others, AIDS is caused …

Assisted Noncoital Reproduction: A Comparative Analysis, George P. Smith Ii Jan 1990

Assisted Noncoital Reproduction: A Comparative Analysis, George P. Smith Ii

Scholarly Articles

This article, written in the last ten years of the 20th century, presents an historical metric of geopolitical, socio-legal, ethical, medical, and religious responses to noncoital reproduction in Australia, Britain, Germany, and the United States — as well as the various efforts of the United Nations — to protect reproductive freedoms as human rights. From this analysis will come an understanding of how these responses shape normative values, principles, regulatory policies and laws which in turn seek — at various levels — to monitor and even control scientific inquiry, advance social justice and safeguard procreative liberties for women. The U.S. …

The Safety Of Maximal Exercise Testing, Larry W. Gibbons, Steven N. Blair, Harold W. Kohl Iii, Kenneth H. Cooper

Faculty Publications

Previous reports on the safety of exercise testing have been based on surveys from different testing facilities with a variety of testing protocols and patient types. From 1971 through 1987, 71,914 maximal exercise tests conducted in a population with a low prevalence of known coronary heart disease under uniform conditions at a single medical facility resulted in six major cardiac complications including one death. No complications have occurred in the past 10 years in 45,000 maximal tests. The overall cardiac complication rate in men and women is 0.8 complications per 10,000 tests with 95% confidence intervals of 0.3-1.9 complications per …

The Importance Of Screening For Medullary Thyroid Carcinoma In Families Of Patients With Men 2, A. Frilling, P. E. Goretzki, L. Bastian, H. D. Roeher Sep 1989

The Importance Of Screening For Medullary Thyroid Carcinoma In Families Of Patients With Men 2, A. Frilling, P. E. Goretzki, L. Bastian, H. D. Roeher

Henry Ford Hospital Medical Journal

Family .screening for medullary thyroid cancer (MTC) is important for detecting members of multiple endocrine neoplasia type 2 (MEN 2) families who may be gene carriers but show no clinical evidence of the disease. Most members of our MEN 2 families are screened yearly by measuring basal and pentagastrin-stimulated calcitonin (CT) levels. A 15-year-old first-degree relative of an affected member of the D-kindred showed a normal basal and an elevated stimulated CT level. Clinical examination, ultrasonography, and scintigraphy were normal. Thyroidectomy and bilateral neck dissection revealed a multicentric MTC with no lymph node involvement. In the O-kindred we detected elevated …

Men 2a: Update On The Northern Ireland And Australian Family, Patrick J. Morrison, David R. Hadden, Colin J. Russell, Norman C. Nevin

Henry Ford Hospital Medical Journal

The Northern Ireland/Australian family with multiple endocrine neoplasia type 2A (MEN 2A) originally described in 1987 is presented with a revised and enlarged pedigree. Four members of the first generation studied have died. A seventh member of the second generation studied has developed medullary thyroid carcinoma and has progressed to surgery. None of the third generation members studied has shown any conclusive abnormality in metabolic screening tests. Each member of the third and fourth generations has had genetic counseling and (if appropriate) DNA analysis with gene probes close to the MEN 2A gene locus on chromosome 10. All members of …

Screening For Pheochromocytoma In The Men 2 Syndrome, J. Kotzerke, C. Stibane, H. Dralle, H. Wiese, W. Burchert

Henry Ford Hospital Medical Journal

Twenty-two patients with the multiple endocrine neoplasia type 2 (MEN 2) syndrome were screened for pheochromocytoma since it is a major cause of morbidity in MEN 2 families. Clinical symptoms, biochemical parameters, ultrasound, computed tomography or magnetic resonance imaging, and meta-iodo-benzylguanidine (MIBG) scintigraphy were evaluated for detection of adrenal tumors. Clinical symptoms and plasma or urine catecholamines appeared to be nonspecific, whereas MIBG scintigraphy was highly specific and the most sensitive parameter. Patients older than age 30 should be scintigraphically screened at least once despite the radiation exposure. Demonstration of only slight uptake is not an indication for surgery but …

Closing In On The Men2a Locus, Nancy E. Simpson, Kenneth K. Kidd Sep 1989

Closing In On The Men2a Locus, Nancy E. Simpson, Kenneth K. Kidd

Henry Ford Hospital Medical Journal

The mapping of the locus for multiple endocrine neoplasia type 2A (MEN 2A) to chromosome 10 using linkage is briefly reviewed including a discussion of linkage strategy and reference to some of the exclusions before the assignment. The subsequent development of the map of the centromeric region of the chromosome and the linking of what appear to be the four closest flanking markers and the centromeric alphoid sequence to the disease locus are reviewed. To date no recombination has been observed between the centromeric marker and the MEN2A locus among, at least, 26 informative meioses, 11 of which are phase …

Postoperative Pentagastrin-Stimulated Serum Calcitonin Concentrations In Patients With Medullary Thyroid Carcinoma: Reoperations In Patients With Concentrations Bordering The Detection Limit, Hinrich Meybier, Heinrich Schmidt-Gayk, Heinz Buhr, Friedhelm Raue

Henry Ford Hospital Medical Journal

The case reports on two patients with medullary thyroid carcinoma show that even postoperatively stimulated serum calcitonin (CT) concentrations near the detection limit (using a polyclonal antibody against synthetic CT) can demonstrate persistent disease. Stimulated CT concentrations can be lowered to nondetectable levels by a second and third operation if a meticulous technique is used for dissection of the lymph compartments. The patient can then be assumed lo be cured. Diagnostic accuracy at very low CT concentrations can be improved by selective venous catheterization with blood sampling for CT after stimulation.

Henry Ford Hospital Medical Journal

Multiple endocrine neoplasia type 2A (MEN 2A) is a rare hereditary disease transmitted in families as an autosomal dominant trad. We have identified a family in which the expression of a rare autosomal dominant form of cutaneous lichen amyloidosis appears to cosegregate with MEN 2A. In this family the skin lesion presented as multiple infiltrated papules overlying well demarcated plaques over the scapular area (right or left). Immunohistochemical studies demonstrated amyloid which stained for keratin but not calcitonin. A total of 19 members were screened. Three members of the family have the characteristic skin lesion and MEN 2A; two additional …
