Richmond Smith As A Clinical Investigator: His Work On Adult Periosteal Bone Expansion And Nutritional And Endocrine Aspects Of Osteoporosis In Light Of Current Concepts, A. M. Parfitt

This paper discusses Richmond Smith's contributions to the field of metabolic bone disease from 1949 to 1969 in light of current concepts. He was the first to demonstrate continuing periosteal bone expansion in adults, a phenomenon of great biologic interest which is best explained if adult bone remodelling is qualitatively similar on all surfaces. He was the first to show that persons with less bone than normal have an increased risk of subsequent fracture, and so are more likely to benefit from preventive treatment. He found that blood vitamin D levels were lower in osteoporotic subjects than in controls, but …

Osteoporosis And Epidemiology Of Fractures In Croatia: An International Comparison, Velimir Matkovic, Marija Ciganovic, Cedomil Tominac, Krista Kostial

We examined the epidemiology of osteoporotic fractures in Croatia and correlated the results with data from other countries. We analyzed annual rates of wrist fractures in the large urban community of Zagreb and hip fractures in the Republic of Croatia. In addition, we determined mortality data and measured metacarpal bone mass In patients with hip fractures. There were age and sex differences in the incidence of both types of fracture. The incidence of wrist fractures is the same as in some Northwestern European countries, but the incidence of hip fractures is much lower. The mortality after hip fractures is high …

Persistent Obstructive Jaundice Associated With Chronic Pancreatitis, C. Grodsinsky, M. A. Block

Persistent obstructive jaundice due to stenosis of the distal common bile duct by chronic pancreatitis has been documented in 13 patients of 253 for whom the diagnosis of chronic pancreatitis has been established. All 13 cases required operation; obstructive jaundice was present at surgery in nine; four had recurring jaundice for prolonged periods. None had associated biliary lithiasis. All cases showed evidence of excess alcohol intake. The jaundice, often associated with mild to moderate pain, was disabling and life-endangering and was relieved by operation. The Effect Of Post-Hospital Nursing Intervention On Success In Breastfeeding, Melinda Hoskins

The purpose of this study was to compare the proportion of women in the experimental group who had a successful breastfeeding experience following post-discharge nursing intervention with the proportion of women in the control group who had a successful breastfeeding experience without nursing intervention. Criteria defining success in breastfeeding were established before the study. Chi-square analysis was used to test for significance with the preset level of significance being α = 0.05. The sample comprised 25 mother-infant dyads with 12 in the experimental group and 13 in the control group. Both groups were similar in type of delivery (spontaneous or …

The Management Of Iatrogenic Chylous Ascites, Joseph W. Lewis, Edward H. Storer

Iatrogenic chylous ascites, although extremely rare, appears to follow direct lymphatic injury during intra-abdominal surgery or to be due to specific postoperative complications such as pancreatitis. Statistically, truncal vagotomy has produced the largest number of cases. Any noticeable lymphatic injuries at the time of surgery should be controlled with appropriate sutures or clips. If iatrogenic chylous ascites develops postoperatively, a course of conservative treatment including multiple paracenteses should be tried for several weeks if reasonable nutritional balance can be maintained. If fluid volume does not diminish or the patient's nutritional status is jeopardized, early exploration should be undertaken. A lipophilic …

Diagnostic And Therapeutic Immunology Of Renal Cell Cancer, Richard C. Klugo

There is evidence that renal cell carcinoma can alter the host immunologic system in several modalities. Diagnostic immunologic monitoring techniques reviewed in this report include delayed cutaneous hypersensitivity reaction (DCHR), absolute peripheral lymphocyte count (APLC), lymphocyte mitogen response (LMR), T-lymphocyte population (erythrocyte rosette), microcytotoxicity assay, in vitro monocyte chemotaxis, and serum blocking factors. Therapeutic immunologic modalities include xenogeneic immune ribonucleic acid (RNA), intradermal BCG, preoperative transcatheter renal artery embolization, immune plasma transfusion, transfer factor, and polymerized autologous tumor.

This Charity Hospital Tumor Registry monograph covers the period 1948 through 1975, and includes all patients with a diagnosis of cancer who were seen at Charity Hospital of Louisiana at New Orleans. A total of 45,493 cases were abstracted of which 41,049 are described in the monograph. This Registry includes skin cancer and all in situ cancers as well as 189 benign mixed tumors of the salivary gland, 262 meningiomas of the brain, 191 carcinoid tumors of the intestine, and 58 granulosa cell tumors of the ovary. Excluded from analysis were 4,444 cases because of lack of histologic confirmation or …

The objective was to identify the leisure preferences of elderly in Bowling Green Nutrition Programs. This objective was to be accomplished through the utilization of a leisure preference test. Since no leisure preference test was found, the study was significant not only for the results obtained but also to evaluate this new test.

The test was designed after the format utilized in the Edwards Personal Preference Test. Major categories were selected and each was composed of five leisure opportunity statements. Each category was placed in opposition to every other category four times to comprise the test. A test was then …

Proper nutrition is essential in the care of the infant with cleft lip or palate, as these infants are often plagued with feeding difficulties from birth. This paper reviews recent literature on the many feeding practices in use today and discusses the current trend toward a multidisciplinary approach to treatment for the cleft lip/palate child. It also presents a brief review of experimental animal studies which have implicated nutritional deficiences in the causation of cleft lip/palate.

Progress in the management of Crohn's disease is limited by our inability to compare different patients. Two methods of evaluating the severity of disease have recently been published. Of these, the National Cooperative Crohn's Disease Study (NCCDS) method has been utilized in a study reported on at several national meetings but still in progress. Antibiotics in intensive prolonged dosage, e.g. ampicillin, 500 mg qid for several months, have recently been reported to benefit greater than 90% of patients not responding to conventional therapy. Metronidazole and dapsone continue to be mentioned in letters to journals. Sulfasalazine has been found useful in …

A number of patients with and without uveitis are used to show that this study of aqueous microbiology suggests the following hypotheses: 1. Routine techniques currently used in most microbiology laboratories overlook classical and variant bacteria which might be isolated from clinical specimens with improved microbiological techniques. 2. Bacterial forms may be present in other ocular tissue when not present in the aqueous. 3. Polymicrobial infections probably occur rather frequently, especially in chronic or subacute inflammatory states. 4. Aqueous microbiological and cytological studies were performed on 134 anterior chamber taps done on 118 patients both with and without uveitis. Cell wall defective bacterial forms (CWDF) were recovered from about 40% of these aqueous specimens and classical bacteria were recovered from 12%. A statistically significant difference could not be found between the incidence of bacterial cell wall defective forms in the various anatomical types of uveitis. Ten patients who developed anastomotic edema following large or small intestinal anastomosis had a rapid recovery after treatment with dexamethasone. The clinical and radiologic picture of adynamic ileus disappeared rapidly. There were no Immediate or long-term (more than two years) complications or side effects associated with the use of dexamethasone. This preliminary report indicates that dexamethasone may be a beneficial agent In the control of anastomotic edema.

The National Consumer Health Information and Promotion Act was enacted in 1976. In response to this legislation, the nation's health education efforts were greatly expanded through the creation of a Bureau of Health Education within the Center for Disease Control and establishment of the Office of Health Information and Promotion in the Office of the Assistant Secretary for Health, Department of Health, Education and Welfare. Changes in the lens are difficult to visualize and the family history may alert the examiner to the possibility of cataractous changes.
