The Dmpa Service Provider: Profile, Problems And Prospects—Philippines, Maria Carmela Patron, Marilou P. Costello

This report presents the results of interviews conducted with 60 trained DMPA service providers from seven of the ten local government units (LGUs) covered by Phase I of the Philippine Department of Health's DMPA Reintroduction Program. DMPA, or Depot-medroxyprogesterone acetate, is an injectable contraceptive commonly known as Depo-Provera. The interviews were undertaken as part of the DMPA Monitoring and Follow-up Studies sponsored by the Population Council under the Asia and Near East Operations Research and Technical Assistance (ANE OR/TA) Project. While the monitoring study and the follow-up survey focused on DMPA users and dropouts, this study centered on the service …

More than ever, one has to design and implement creative options to achieve satisfactory health service at low costs. The political turnover in Congress shifts more responsibility to local governments. This means less funding and less willingness by the national government to be held accountable for health and social services. Abundant literature exist on interventions for weight loss documenting the existence of a weight loss maintenance problem (Wadden & Bell, 1990; Kay man et al., 1990; Wadden et al., 1989). Very few individuals are successful at maintaining their weight loss for at least two years for reasons that are not clearly understood. This retrospective study examined factors, identified in a review of the scientific literature, shown to be associated with weight loss maintenance. The purpose of this study was to determine whether differences in health beliefs exist between participants attending worksite prenatal education classes and participants receiving only prenatal education self-help materials. A multiple treatment study was performed on a sample of convenience of 169 women enrolled in Fruit of the Loom's worksite prenatal education program. Group 1 subjects received class and materials while Group 2 subjects received materials only. Study participants were asked to complete an 18-item Likert scale questionnaire prior to enrolling in the program and again upon completion. The present study examines the effect of nutritional intervention on two outcomes of pregnancy: birth weight of the baby and gestational age. The study further examines the difference in nutritional intervention in rural and urban areas. The research method used is ex-post facto design. Data for the study is extracted from the health records and tracking sheets of women participating in the Nutrition Intervention Project in Virginia Department of Health. Background. Vaccination rates are frequently considered a surrogate measure of protection. To provide more accurate estimates, serum levels of antibody against tetanus were measured as part of the third National Health and Nutrition Examination Survey (NHANES III), which studied a representative sample of the civilian, noninstitutionalized population of the United States.

Methods. We measured tetanus antitoxin using a solid- phase enzyme immunoassay in serum samples from 10,618 persons six years of age and older who were examined during phase 1 of NHANES III in 1988 to 1991.

Isolation And Characterization Of A B-D-Glucuronidase-Producing Strain Of Escherichia Coli Serotype O157:H7 In The United States, Peggy S. Hayes, Kristina Blom, Peter Feng, Jay Lewis, Nancy A. Strockbine, Bala Swaminathan

A phenotypic variant of Escherichia coli serotype O157:H7 (G5101) was isolated from a patient with bloody diarrhea. Strain G5101 does not ferment sorbitol but is β-D-glucuronidase and urease positive. Serotyping and colony hybridization using a serotype-specific DNA probe confirmed that the isolate was O157:H7. G5101 produces Shiga-like toxins I and II and contains an eae gene that is highly conserved in the O157:H7 serotype. This strain would have been missed by laboratories that screen for the sorbitol-negative, β-D-glucuronidase- negative phenotype in isolating E. coli O157:H7 from clinical and food specimens.

Fulminant hepatic failure (FHF) is defined by the appearance of severe liver injury with hepatic encephalopathy in a previously healthy person. There are an estimated 2,000 cases of FHF in the United States yearly, representing 0.1% of all deaths and, perhaps, 6% of liver related deaths. The causes of FHF are many, the chief ones in the United States being hepatitis A; B; non-A, non-B and drug induced liver disease. There are no specific therapies for FHF, however, liver transplantation is recommended for situations in which spontaneous recovery appears unlikely. Objective: To identify the impact of dietary intake and activity level on postpartum weight change.

Methods: White (n = 121) and black (n = 224) women, 7-12 months postpartum, participating in the Special Supplemental Feeding Program for Women, Infants, and Children were assessed for dietary intake, activity level, body weight, and other maternal characteristics.

Results: For both black and white women, the most important variables in predicting postpartum weight loss were pre-pregnancy weight, gestational weight gain, parity, and prenatal exercise. After these factors were controlled, race predicted that black women retained 6.4 lb more than white women. These results may …

India's national family welfare program has two objectives—to address the needs of families, notably women and children, and to reduce population growth rates. In reality, the program has been disproportionately focused on achieving demographic targets by increasing contraceptive prevalence. In this process, women's needs have been overlooked, in terms of poor reproductive health (RH). Perceived Stress And Health-Promoting Behaviors In Nursing Students Using Pender's Health Promotion Model, Gail E. Dunham

The purpose of this descriptive, correlational study was to add support to the literature that there is a relationship between the perceived stress (Student Stress Inventory) that nursing students report and their practice of health-promoting behaviors (Health Promoting Lifestyle Inventory). A convenience sample of 36 first-year associate degree nursing students was obtained. The conceptual framework used was Pender's Health Promotion Model. No relationships were found between demographic variables and perceived stress. Results indicated that subjects reported they engaged in health-promoting behaviors more than sometimes and they perceived themselves overall as slightly stressed. This issue of Quality/Calidad/Qualité tells the story of a group of Brazilian women who came together in 1981 with the shared belief—not as widely understood then as it is now—that conventional approaches to women's health and family planning services were overly medicalized, incapable of dealing with the relationship between sexuality and contraception, and disempowering to clients. They started the Coletivo which runs a clinic, a training institute, and a public education program that has “given voice” to thousands of women, provided leadership to the Brazilian government and women’s health movement, and has been active in research and international policy affairs. …

This article uses tuberculosis as the paradigm for exploring rational standards for the exercise of compulsory public health powers. Extant doctrine in disability and constitutional law provides a lens for examining judicial review of state interventions. The author first sets out the central epidemiological and biological aspects of tuberculosis to demonstrate the strength of the governmental interest in curtailing the epidemic. Second, he examines the interventions of testing, screening, and confinement of persons with tuberculosis, where he focuses on two congregate settings--correctional and health care facilities--that present substantial health risks and are principal foci for the exercise of state intervention. …

This report presents the results of a survey of 899 DMPA acceptors who availed of injectable contraceptive services from public health facilities under the Philippine Department of Health's (DOH) DMPA Reintroduction Program. The survey is part of the DMPA Monitoring and Follow-up Studies sponsored by the Population Council’s Manila office in response to a request by the DOH to provide operations research support to the program. Depot-medroxyprogesterone acetate (DMPA), commonly known as Depo-Provera, is a three-month injectable contraceptive. This report presents the results of the DMPA Monitoring Study undertaken by Population Council, Manila, as a technical assistance project for the DMPA Reintroduction Program of the Department of Health. The primary objective of the study is to provide data on DMPA (injectable contraceptive) utilization and continuation rates that may be used to project the logistical needs of the program for the next four years (1995–1998). This report contains descriptions of 13 family planning (FP) projects recently implemented in Bangladesh. The results in terms of the contraceptive prevalence rate (CPR) are analyzed to investigate the degree to which improved FP service delivery in Bangladesh can increase contraceptive use in the present status of demand, investigate the programmatic factors most associated with increased prevalence, and make these projects more widely known. Criteria for inclusion included coverage of a geographic area where household distribution of contraceptive supplies was available, and where at least one measurement of CPR has been made through credible survey efforts since July 1988. This study attempted to examine user perspectives regarding the desirability of the diaphragm as a contraceptive method when included among other methods distributed freely through a family planning (FP) clinic. It sought to introduce on an experimental basis, the diaphragm into an ongoing and well-established FP clinic to increase contraceptive choice. This was primarily a qualitative study aimed at understanding women's perceptions about the risks and benefits, the reasons for use of the diaphragm, and the practical difficulties of using it effectively. This report is based on data from 12 focus group discussions conducted throughout the Philippines as part of the DMPA Monitoring and Follow-up Studies. In two separate complementary exploratory descriptive studies the cover and timing of vaccine administration to children in the 0-18 month age group in the remote Kimberley Region of Western Australia were quantified, and reasons for delay in vaccine administration were investigated. The adapted PRECEDE model was used to guide the research and to integrate and interpret the findings. Study One: Three separate computer immunisation databases were combined to provide the first validated age-appropriate infant immunisation data in Western Australia.
