Gestational diabetes mellitus (GDM) is the most common metabolic disorder of pregnancy, and rates of this disease have been steadily increasing over the past ten years. GDM disproportionately impacts racial and ethnic minorities at higher rates than their Caucasian counterparts. In this study, the PRECEDE model framework was employed during a literature review to gain a greater understanding of the complex risk factors contributing to differences in the development, diagnosis, and treatment of GDM among these population subgroups. Background/Objectives: Nutrients such as vitamin B12, folate (B9), and polyunsaturated fatty acids (PUFAs) may independently influence cognitive health, but their combined effects and interactions remain unclear. This study aims to investigate the effects of B12, folate, and PUFAs, including their interactions, on cognitive function in cognitively healthy older adults, considering effect modification by sex and amyloid-beta status.
Methods: A cross-sectional analysis was conducted using data from 321 participants aged 60–85 y enrolled in the UK CHARIOT–PRO SubStudy. 17β-oestradiol (E2) inhibits overeating and promotes brown adipose tissue (BAT) thermogenesis, whereas prolactin (PRL) does the opposite. During lactation, the simultaneous decline in E2 and surge in PRL contribute to maternal metabolic adaptations, including hyperphagia and suppressed BAT thermogenesis. However, the underlying neuroendocrine mechanisms remain unclear. Here, we find that oestrogen receptor alpha (ERα)-expressing neurons in the medial basal hypothalamus (MBH), specifically the arcuate nucleus of the hypothalamus and the ventrolateral subdivision of the ventromedial hypothalamus (vlVMH), are suppressed during lactation. Deletion of ERα from MBH neurons in virgin female mice induces metabolic phenotypes characteristic of lactation, including hyperprolactinemia, hyperphagia …

Pregnant and lactating women in sub-Saharan Africa are vulnerable to micronutrient inadequacies, with risk of adverse pregnancy outcomes. Adequate intakes of diverse foods are associated with better micronutrient status and recommended by the World Health Organization as part of healthy eating counselling during antenatal care. However, our understanding of community knowledge of dietary diversity within the context of maternal diets is limited. We used a descriptive qualitative approach to explore community perceptions of dietary diversity during pregnancy and lactation, as well as influencing factors in sub-Saharan Africa. According to recent studies, disparities in the United States indicated a prevalence of adverse outcomes associated with maternal nutrition and perinatal mental health among women facing socioeconomic challenges. The purpose of this quantitative cross-sectional study was to investigate the association between maternal nutrition, perinatal mental health, and birth outcomes with a focus on women facing socioeconomic challenges. The theoretical framework that guided this study was the socioecological model. Cardiovascular disease (CVD), including coronary artery disease, heart failure, myocardial infarction, and stroke, remains a leading cause of global mortality, with approximately 20.5 million deaths in 2021. Despite advancements in medical science, CVD prevalence continues to rise, underscoring the need for preventive approaches and increased awareness. CVD is closely associated with oral diseases, and poor oral health is linked to worse cardiovascular outcomes. From Pregnancy To Postpartum: A Longitudinal Analysis Of Maternal Depressive Symptoms And Hemoglobin Concentration In Uttar Pradesh, India, Abdullah Nurus Salam Khan

Background: Perinatal depression and maternal anemia impact maternal health, pregnancy outcomes, and infant growth in low- and middle-income countries. This study examines the prevalence and risk factors of these conditions during pregnancy and postpartum in rural Uttar Pradesh, India. Methods: A prospective cohort study was conducted among 475 pregnant women recruited during early pregnancy (≤20 weeks gestation) and followed through late pregnancy (≥27 weeks) and postpartum (< 42 days of childbirth). The outcome variables were, i) depressive symptoms, assessed using the Edinburgh Postnatal Depression Scale (EPDS), and ii) hemoglobin concentrations, measured using HemoCue Hb 301 analyzer from capillary blood. Multivariable linear regression examined associations of the outcome variables with sociodemographic, obstetric, nutritional, interpersonal, and perinatal factors. Path analysis evaluated direct and indirect paths influencing postpartum hemoglobin. Results: Prevalence of probable depression (EPDS score≥13) peaked in early pregnancy (24%) and declined in late pregnancy (17%) and postpartum (4%). Mean hemoglobin declined from early (10.5 g/dL) to late pregnancy (9.7 g/dL), and partially recovering in postpartum (10.4 g/dL). Household food insecurity was associated with higher depressive symptoms during pregnancy and postpartum (early pregnancy:β=2.61, p=0.011; late pregnancy:β=2.84, p< 0.001; postpartum:β=1.40, p=0.023). Psychological intimate partner violence was associated with greater depressive symptoms in early pregnancy (β=3.19, p=0.001). Receiving more antenatal visits was associated with decrease in depressive symptoms in late pregnancy (β=-0.46, p=0.001) and hemoglobin at postpartum (β=-0.11, p=0.070). Consumption of more food groups was associated with increase in depressive symptoms (β=0.63, p=0.004) and hemoglobin in late pregnancy (β=0.10, p=0.026). Iron folic acid (IFA) consumption was associated with increase in hemoglobin in late pregnancy (β=0.01, p< 0.001) and postpartum (β=0.004, p=0.031). Experiencing postpartum health complications was associated with increase in depressive symptoms (β=0.41, p=0.013) and decrease in hemoglobin concentrations (β=-0.18, p=0.036). Antenatal hemoglobin concentrations mediated the associations of IFA supplementation and dietary diversity with postpartum concentrations. Conclusion: Perinatal depression was associated with socioeconomic vulnerabilities, food insecurity, and intimate partner violence, while hemoglobin concentrations were associated with maternal nutrition and perinatal care. Integrating mental health services and comprehensive nutrition interventions into antenatal and postpartum care can improve psychological and physical well-being of mothers during and after pregnancy.

Supine sagittal abdominal diameter (SAD), also known as abdominal height, has been proposed as a simple measure for assessing abdominal adiposity. We aimed to determine whether SAD from DXA performed for osteoporosis assessment predicts major adverse cardiovascular events (MACEs) using the population-based DXA registry for the Province of Manitoba, Canada. The study population comprised 72 974 individuals aged 40 yr and older with baseline DXA assessment between February 1999 and March 2018. The absence of clear baseline prevalence data for MCAs hinders the development of targeted interventions to mitigate their burden. Background/aims: Multi-society experts proposed the adoption of new terminology, metabolic dysfunctionassociated steatotic liver disease (MASLD) and steatotic liver disease (SLD). We studied the current prevalence of SLD and its subcategories in the US.
Methods: Using the recent National Health and Nutrition Examination Survey from 2017 to 2023, we analyzed data from 12,199 participants (≥18 years) who completed transient elastography. SLD and its subcategories, including MASLD, metabolic and alcohol-related liver disease (MetALD), and alcohol-related liver disease (ALD), were categorized according to consensus nomenclature.

Immune correlates of protection against infection with Mycobacterium tuberculosis (Mtb) remain elusive. In this issue of the JCI, Dallmann-Sauer and authors demonstrate that lack of tuberculin skin test (TST) and interferon γ release assay (IGRA) conversion among people with HIV despite years-long Mtb exposure is associated with alveolar lymphocytosis, including specific poly-cytotoxic T cells, and M1-type alveolar macrophages with a stronger ex vivo response to the pathogen. The complexity of diabetes makes early diagnosis and effective management challenging for healthcare settings. This study developed and evaluated models using machine learning algorithms to predict the risk of diabetes. The data were primarily sourced from the Indonesia Family Life Survey (IFLS). Diabetes status was identified using A1c whole blood sample values (A1c WBS ³5.7%). Sociodemographic and anthropometric factors were set as predictors, most of which were significantly correlated with diabetes status. This study examined the intensity of different physical activity (PA) breaks for elementary school children in a simulated classroom setting. Researchers compared the energy expenditure (Kcals/min, METs) of PA breaks developed for the Physical Activity Across the Curriculum (PAAC-3) program, delivered via video or in-person, to commercially available GoNoodle© videos. 71 children (age 9.0 ± 1.1 yrs., 47 males, 24 females, 18% minority) participated in 10-minute PA breaks. Ten years have passed since the last regional assessment of the state of Fogarty International Center-funded research ethics education and challenges facing research ethics in Latin America and the Caribbean basin. Program directors and faculty from four international research ethics education programs met in Spring 2024 to discuss their progress, challenges, and future priorities. The themes that emerged from these discussions were the structural difficulties faced by regional ethicists, the use of pedagogical innovations to mitigate different barriers faced by fellows, and the need to anticipate future challenges for the region including climate change.

Mothers’ characteristics influence children’s health and development. We conducted a cross-sectional study to assess the influence of mothers’ education level, employment status, and exposure to domestic violence on diarrhea among children under 5 years old using secondary data from 2010, 2015, and 2020 Rwanda Demographic Health Surveys. By the year 2050, it is projected that the population of individuals aged 60 years and older will increase by 47 percent, reaching a total of 82 million in the United States. Many older adults experience depressive symptoms, and some never recover from it. Depression is one of the most common psychiatric disorders among older adults. Approximately 3.2% of community-dwelling older adults experience major depressive disorder in a 12-month period, although many more experience clinically significant depressive symptoms, with a recent estimate of 35% based on a global meta-analysis. The COVID-19 pandemic presents several challenges during pregnancy including thromboembolic complications, direct placental infection, transplacental transmission, and systemic hyperinflammatory state. The liver is the second most commonly affected organ in SARS-CoV-2 infection after the lungs. Mechanisms of liver injury in COVID-19 patients can include: direct viral cytopathic effect, worsening of underlying liver disease, cytokine storm, hypoxic ischemic injury, and cholangiopathy leading to persistent marked cholestasis. Improving children's oral health is a national priority. Parental knowledge and self-efficacy influence children's health behaviors; however, the relationship between parental oral health-related knowledge, self-efficacy, child oral health-related quality of life (OHQoL), and dental caries remains unclear, especially among low-income, ethnically diverse U.S. families. BACKGROUND: Sepsis is one of the leading causes of maternal mortality globally. In 2023, the Azithromycin Prevention in Labor Use (A-PLUS) trial showed intrapartum azithromycin for women planning a vaginal birth reduced the risk of maternal sepsis or death and infection. We aimed to evaluate the cost-effectiveness of intrapartum azithromycin for pregnant people planning a vaginal birth in low-income and middle-income countries (LMICs) using A-PLUS trial data.

The rising prevalence of obesity among pregnant women presents significant health risks for mothers and children, beginning as early as intrauterine life throughout adulthood. The purpose of this quantitative cross-sectional study was to investigate whether demographic factors influenced obesity awareness as a personal issue in pregnant Hispanic women. The precaution adoption process model guided the study, a theoretical framework focusing on stages of health behavior change, including awareness, decision making, and action. Objectives: To explore the feasibility and surgical outcomes of robot-assisted vaginal natural orifice transluminal endoscopic surgery (RA-vNOTES) for women suffering from gynecologic disease.
Methods: We performed an observational study reporting and analyzing the perioperative outcomes of 298 patients with gynecologic disease who underwent RA-vNOTES in a single institution from June 2019 to August 2024.
Results: A total of 298 patients with a median age of 41 years and median body mass index of 29 kg/m2 underwent RA-vNOTES. Clostridioides difficile is a gram-positive spore-forming pathogen that commonly causes diarrheal infections in the developed world. Although C. difficile is a genetically diverse species, certain ribotypes are overrepresented in human infections, and it is unclear if metabolic adaptations are essential for the emergence of these epidemic ribotypes. To identify ribotype-specific metabolic differences, we therefore tested carbon substrate utilization by 88 C. difficile isolates and looked for differences in growth between 22 ribotypes. As expected, C. difficile was capable of growing on a variety of carbon substrates.
