This research was designed to investigate whether there would be a difference in mean exhaled carbon monoxide levels and mean number of cigarettes smoked for two randomized groups of smokers.Smoking cessation therapy was manipulated in smokers with a chronic, productive cough (COPD) and those who were asymptomatic (normal). Individual therapy was compared to self-help therapy.The main purpose of this study was an attempt to define the smoking cessation therapy that worked best for each of these two groups.With the information gained from this research, it was hoped that educating different types of smokers (those with and without lung disease) about …

Inverted papilloma of the nose and paranasal sinuses is an unusual benign neoplasm that has the propensity for local tissue destruction and an association with malignant degeneration. The tumor represents an inverted growth of epithelium rather than outward proliferation. It most commonly arises from the lateral nasal wall with growth into the nose and paranasal sinuses and only rarely originates from the septum. Presenting symptoms are usually unilateral nasal obstruction and clear rhinorrhea. Surgical excision is the recommended treatment though the aggressiveness ofthe approach has varied. Transnasal local excision has been associated with a high recurrence rate. Lateral rhinotomy with …

Hyperbaric oxygen (HBO) is an effective adjunct in the management of selected otolaryngologic problems including radiation-induced necrosis of the temporal hone, malignant external otitis, mandibular osteoradionecrosis and refractory osteomyelitis, soft tissue head and neck necrotizing fasciitis, compromised skin flaps and grafts, acute air or gas embolism, and otologic barotrauma. We describe the management of a patient with insidious Staphylococcus aureus osteomyelitis of the temporal bone by the use of HBO preoperatively and postoperatively in conjunction with surgical debridement. The possible application of angiogenic agents and tetracycline hone-labeling in combination with HBO therapy in the management of refractory neurotologic disease is …

The current system of referral of patients complaining of hearing loss usually requires a dual appointment with otolaryngology and audiology. Most of these patients have sensorineural hearing loss for which there is rarely surgical or medical treatment. This study tests the hypothesis that these patients would receive appropriate treatment and that health care dollars would be saved if an audiological assessment could identify those patients who require medical or surgical care. One hundred consecutive patients whose sole complaint was hearing loss were evaluated using audiograms and "hearing abilities questionnaires" by five audiologists using subjective and objective criteria to determine the …

Systemic Dissemination Of Pneumocystis Carinii In A Patient With Acquired Immunodeficiency Syndrome, Mahul B. Amin, Michael P. Abrash, Eugene Mezger, Gary F. Sekerak

Pneumocystis carinii is usually considered a respiratory tract pathogen; however, there are reported cases of limited and generalized dissemination of the organism from the lungs of immunocompromised patients. We present the autopsy findings of a 29-year-old male with acquired immunodeficiency syndrome (AIDS) and recurrent Pneumocystis carinii pneumonia who developed abnormal liver function tests. The patient had received aerosolized pentamidine because of toxic reactions to other modes of therapy. The postmortem examination revealed Pneumocystis in the lungs, liver, spleen, kidney, myocardium, thymus, pancreas, thyroid gland, bilateral parathyroid and adrenal glands, gastrointestinal mucosa, perihilar and mesenteric lymph nodes, and bone marrow. Infant Hearing Screening 1984 To 1989: The Henry Ford Hospital Experience, Gary P. Jacobson, Mary Jo Burtka, Jeanne A. Wharton, Craig W. Newman, Neil Shepherd, Robert G. Turner

From 1984 to 1989 the Infant Hearing Screening (IHS) program at Henry Ford Hospital identified 1,300 infants as being "at risk" for hearing loss. The prevalence of significant sensorineural hearing loss in this sample was 1.4%. Additionally, 80 infants who passed the IHS program and reached 3 years of age were found to have normal hearing sensitivity by conventional audiometric techniques (ie, no false-negative predictions). There were three false-positive predictions. It was discovered that infants of low birthweight (ie, < 1,500 g) were three times more likely to fail IHS than those whose weight exceeded 1,500 g. A higher return rate was found for infants failing an initial hearing screening conducted in the neonatal intensive care unit in comparison to those screened as outpatients one week postdischarge. The sensitivity and specificity of behavioral observation audiometry were 43% and 92%, respectively, when brainstem auditory-evoked potentials was used as the criterion validity measure.

A rapid diagnostic method employing a polymerase chain reaction procedure (PCR) was used to identify Shigella and enteroinvasive Escherichia Coli. This procedure amplified a region of the invasive-associated locus (ial) from a crude DNA extract of feces. A synthetic 21-base oligonucleotide corresponding to the ial gene sequence was shown to specifically hybridize only with enteroinvasive E. Coli (EIEC) strains and Shigella species. Upon PCR amplification, a 320-base pair fragment was generated in DNA extracted from feces reconstituted with EIEC or Shigella flexneri but not in DNA from 70 normal stools lacking these organisms and could be readily …

The proposals in the article are derived from the experience of PRO-PATER, in São Paulo, Brazil, and briefly, from the activities of the Profamilia Men’s Clinic in Colombia. The experiences of both PRO-PATER and Profamilia suggest that, although the reproductive health needs of men may be different from those of women, they are still very interested. A Decade Of A Maturing Epidemic: An Assessment And Directions For Future Public Policy, Lawrence O. Gostin

The author's goal in this article, is not merely to propose public health strategies for the future, but also to examine why government has been so slow, so equivocal, in its public health response to the acquired immune deficiency syndrome (AIDS) epidemic. He argues that there has been a fundamental ambivalence in perceptions of the epidemic. For some, AIDS is perceived as a disease, with sympathy for sufferers. Once AIDS is viewed as a disease, like other catastrophic diseases, it follows that public policy will be based upon science and epidemiology--health education, research and treatment.

The purpose of this study was to investigate the nutrition knowledge of wait staff in 24-hour, full menu/table service restaurants. Also explored was the relationship of sex, age, education level, average number of hours worked per week, total length of employment as a wait staff member, and personal dietary interest to knowledge of nutrition in wait staff. In the fall of 1990, 160 questionnaires were delivered to wait staff at eight 24-hour, full menu/table service restaurants in the Waterloo/Cedar Falls area. Vision To reduce suffering and death from cancer using information collected by the Louisiana Tumor Registry. History Cancer registration in Louisiana began in 1947 at the Charity Hospital Tumor Registry in New Orleans. In 1974, as part of its Surveillance, Epidemiology and End Results (SEER) Program, the National Cancer Institute provided funds for a population-based cancer incidence and survival registry covering Jefferson, Orleans, and St. Bernard parishes. Transcriptional Regulation Of The Human Calcitonin Gene: A Progress Report, Sara Peleg, Gilbert J. Cote, Ronald V. Abruzzese, Robert F. Gagel

We have applied DNA transfer techniques lo study the transcriptional regulation of the calcitonin (CT) gene in a C-cell line (TT) derived from a human medullary thyroid carcinoma. TT cells were transfected with a fusion gene containing the CT gene promoter and 5' -flanking DNA attached to the promoter-less growth hormone gene (reporter). We quantitated the reporter gene product to monitor transcriptional activation by the CT promoter and deletion mutants of the 5' -flanking DNA. We found that the proximal CT promoter which includes the DNA sequence from +1 to -129 bp upstream from the CT transcription start site did …

This report summarizes the results of a partial evaluation of the 1988-89 Drug Abuse Resistance Education (DARE) effort implemented in all sixth grades in the Nashville, Tennessee, Metropolitan Public School system by trained instructors from the Nashville Metropolitan Police Department. As documented in the following pages, the evaluation effort had two major components.

Screening For Pheochromocytoma In The Men 2 Syndrome, J. Kotzerke, C. Stibane, H. Dralle, H. Wiese, W. Burchert

Twenty-two patients with the multiple endocrine neoplasia type 2 (MEN 2) syndrome were screened for pheochromocytoma since it is a major cause of morbidity in MEN 2 families. Clinical symptoms, biochemical parameters, ultrasound, computed tomography or magnetic resonance imaging, and meta-iodo-benzylguanidine (MIBG) scintigraphy were evaluated for detection of adrenal tumors. Clinical symptoms and plasma or urine catecholamines appeared to be nonspecific, whereas MIBG scintigraphy was highly specific and the most sensitive parameter. Patients older than age 30 should be scintigraphically screened at least once despite the radiation exposure. Demonstration of only slight uptake is not an indication for surgery but …

AIDS prevention research has established the effectiveness of multiple component group interventions. It has not identified whether the effectiveness is due to the format of the intervention (i.e., rehearsal, feedback, etc.) or its instructional content. This study investigates the impact of instructional content delivered in the inexpensive and readily obtained printed medium. Thirteen gay or bisexual college-aged men received an information pamphlet and an AIDS-related communication skills training booklet. Subjects were evaluated on measures of knowledge, safe and risky behaviors, and communication behaviors in each of three assessment sessions. The purpose of this study was to examine the content of family life education curricula in Virginia's public schools and determine its appropriateness for the mentally handicapped students. It was the intent of this study to determine if existing curricula were designed to address the needs of mentally handicapped students according to a standard curricula designed by the American Association for Health, Physical Education and Recreation and the Sex Information and Education Council of the United States. The purpose of this study was to determine the general Job satisfaction level of direct care mental health employees and to determine any significant differences in Job satisfaction levels between fulltime and part-time employees. The sample consisted of 60 direct care mental health employees In a residential treatment center in Virginia. There were 28 respondents for a 47% response rate.

The 20 item Minnesota Satisfaction Questionnaire (Short Form> and a demographic questionnaire were the survey instruments used. Hysterectomy is the second (after cesarean section) most commonly performed major surgical procedure in the United States today.1 About [650,000 American women will have hysterectomies in 1989. Thirty-seven percent of all US women will have had a hysterectomy by the time they reach age 60. The number of hysterectomies increased dramatically Born 1965 to 1975, but then leveled off and decreased to the present level of about 7 per 1000 women. The ~ghest rates occur in women aged 35 to 44 years, the age group the so called baby boomers are now entering in large numbers. Therefore, if the age-specific …
