Skin Lesions Associated With E. Coli Sepsis In A Patient With Acute Leukemia, Robert A. Chapman, Ellis J. Van Slyck, Tom Madhavan

The clinical course of a patient with acute leukemia and metastatic intradermal abscesses secondary to Escherichia coli sepsis is described. E. coli organisms, cultured first from the blood and later from the lesions, had an identical antibiotic susceptibility and biotyping profile. This complication has not been previously described. Granulocytopenia and varicose veins may have been critical predisposing factors. With more widespread use of progressive immunosuppressive regimens, unusual manifestations of common infections will be recognized more frequently.

A case of life-threatening cardiac arrhythmia associated with magnesium deficiency is described. The patient had no history of any cardiac disease, was not taking any medication and had no other accompanying electrolyte disorder. His arrhythmia was refractory to conventional treatment but responded to parenteral magnesium therapy. The role of hypomagnesemia in the predisposition to cardiac arrhythmia is discussed. To our knowledge, there are only two well-documented cases of ventricular irritability which responded to magnesium as the sole treatment. As awareness of such a relationshp increases and magnesium determinations become more available, this entity may be more commonly recognized, and the …

Dissecting Aneurysm Of Aorta Complicating Coarctation Of The Aorta: Case Report And Brief Literature Review, Remigio Garcia, Julius M. Ohorodnik, Donald J. Magilligan Jr.

The incidence of dissecting thoracic aorta aneurysm complicating coarctation of the aorta ranges from 1.7 to 23% in large series of coarctation of the aorta reported. In a review of 95 cases of dissection of the aorta, we found only one case with associated coarctation of the aorta. The presence of this anomaly, although rare, should be suspected and searched for in younger patients with dissection, in view of the surgical implications at the time of correction.

Th;s paper reviews some reasons why many ophthalmic surgeons do not implant intraocular lenses. It presents some parameters of a theoretically "ideal" lens, some of the disadvantages and advantages of the currently available anterior chamber intraocular lenses, and lastly, it offers designs for a new anterior chamber intraocular lens. While preserving the advantages of currently available anterior chamber intraocular lenses, the new designs may have certain advantages. Currently under investigation, the new lenses may be available for use in man within the next year.

This report describes a series of geophysical surveys performed at the Idaho National Engineering Laboratory (INEL). The main purpose of the surveys was to evaluate techniques, principally ground-penetrating radar, for detecting and mapping radioactive wastes buried in shallow trenches and pits.

In 1972, the Nicaraguan Foundation for Development (Fundación Nicaraguense de Desarrollo—FUNDE), one of two programs sponsored by the Nicaraguan Institute of Development, became aware of market women's need for credit. This issue of SEEDS describes FUNDE's experience in developing savings and loan cooperatives to meet this need. This summary stresses the human aspects of the process as much as the financial and technical ones. The project has been successful because the cooperatives have built upon the existing market women's culture, utilizing all the subtle and complex interpersonal relationships established over the years. The Hanover Street Project, formally known as the United Women’s Woodworking and Welding Project, is an experiment in training women for jobs usually held only by men. Begun in 1976, this was the first such program of the Jamaica Women's Bureau, established by the government during International Women's Year to ensure that women participate fully in Jamaica’s development. The project demonstrated that low-income women can learn non-traditional skills and can work together to improve their lives. Through trial and error, the project is providing the Women's Bureau with a wealth of information about teaching technical skills, working with other government …

The Mraru Women's Group in rural Kenya, like many community women's organizations around the world, is an example of a deeply rooted tradition of association and self-help among women. In 1971 the group began to gather its resources to solve a common problem—transportation. They raised money, bought a bus, and began a public transport service that made money; they now face other difficult questions such as reinvesting profits, serving members' broader needs, and maintaining a strong economic base. The Mraru Women's Group has shown unusual creativity and persistence in identifying common needs and organizing to meet them. They have also …

The purpose of this study was to compare the proportion of women in the experimental group who had a successful breastfeeding experience following post-discharge nursing intervention with the proportion of women in the control group who had a successful breastfeeding experience without nursing intervention. Criteria defining success in breastfeeding were established before the study. Chi-square analysis was used to test for significance with the preset level of significance being α = 0.05. The sample comprised 25 mother-infant dyads with 12 in the experimental group and 13 in the control group. Both groups were similar in type of delivery (spontaneous or …

Feral pigeons (Columbia livia) are not harmless birds. Many potential infections of humans silently exist in pigeons which are not apparent. They have the potential for transmission of over 30 diseases to humans plus another ten to domestic animals. Environmental pollution from pigeon droppings is quite evident. Air pollution involves more than noxious gases from automobile exhausts and belching smokestacks. One serious air pollutant is air-borne fungi, which are agents for infectious diseases. There is much information written about the problem, but it remains primarily in the professional journals and technical references, neatly stacked away on library shelves. …

Rapid progress has been made in understanding the complexity of the immune system. The current status of lymphocyte subpopulations with regard to ontogenic development, their surface and intracellular markers, and functions in various Immunologic phenomena has been reviewed. Heterogeneity of T lymphocytes as defined by receptors for immunoglobulin isotypes is discussed in detail. Various cell types and their products involved in immunoregulatlon are also presented.

A Method For Evaluating The Platelet Surface Response By Using Electron Microscopy, Jeanne M. Riddle

Our method for evaluating the surface response of any given platelet population, which has evolved over the past two decades, provides a measure of both the adhesive and cohesive qualities of platelets. General morphologic features of the adherent, whole platelets can be assessed by transmission electron microscopy, and the results are reproducible when serial samples are collected on normal subjects and patient groups. With this technique, different metabolites and drugs can be combined with platelet populations that are either normoresponsive or hyperactive to determine if these substances trigger or inhibit the platelet response. Functions And Interrelationships Of Leukocytes In Inflammation As Elucidated By The Rebuck Skin Window Technique, Sheikh M. Saeed

This article provides an overview of the functions of human leukocytes in inflammation as elucidated by the Rebuck skin window technique. The migration sequence of various leukocytes into the field of inflammation is described, as well as cytologic, cytochemical, and transformational changes, and the interrelationships of responding leukocytes. Since it was introduced in 1955, the Rebuck skin window technique has provided an excellent means of studying in vivo inflammatory response to different phlogistic agents in normal individuals and in various disease states. This simple technique continues to prove fruitful to further study and to monitoring of many disease states.

We describe a rare case of a localized primary small intestinal lymphoma with features of both the Mediterranean and the Western type. This case demonstrates the need to regard intestinal lymphoma as a spectrum of diseases with the Mediterranean and the Western types at opposite ends and many variants in between. The role of the plasma cell infiltrate has been interpreted as reactive as well as neoplastic. In our case, both lymphocytic and plasmacytic proliferations spread widely in the intestinal submucosa, extended into muscularis propria and subserosal fat, and involved mesenteric lymph nodes. Bovine high molecular weight kininogen (bHMWK) partially corrects the aPTT of Fitzgerald-trait plasma, which is congenitally deficient in HMWK. The relationship between the structure and activity of HMWK was clarified by studying the effects of different fragments of bHMWK on the aPTT of Fitzgerald-trait plasma. The peptides studied, all in equimolar concentrations, were lys-bradykinin-free HMWK, bradyk In In-fragment 1-2-tree HMWK, heavy chain, fragment 1 -2-Hght chain, and light chain. Bradykinin- fragment 1-2-free HMWK, heavy chain, and light chain have little or no correcting activity upon Eitzgeraldtrait plasma aPTT. Two different stimuli, tetanus toxoid (TT) and autologous fibrin (AF), were used to investigate the inflammatory response in ten patients with ankylosing spondylitis (AS); in seven persons who had a family history of AS and were HLA-627 positive but had no clinical signs of AS; and in eight nonarthritic controls. The purpose was to determine whether the leukocytic response in an experimental Rebuck skin window lesion was the same or different from that seen in the synovial fluid of AS patients when in vivo-formed fibrin flakes were present. Functional And Biochemical Alterations Of Platelets In Atherosclerosis And Thromboembolism, Orhan N. Ulutin

In atherosclerosis, platelets appear to circulate in an activated state. Our findings are based on results obtained from 240 patients. These platelets demonstrated an increased degree of surface activation and an increased amount of aggregation. Platelet aggregation was also demonstrated by a heightened sensitivity to adrenalin and ADP. Greater than normal secretion of ADP, PF-4, platelet fibrinogen, and platelet antiplasmin was noted. Platelet antiplasmin was increased, whereas platelet antithrombin III was decreased. Membrane transport of glucose was found to be defective in atherosclerosis. There appears to be no alteration in the phospholipid distribution, although the amount of phospholipids increases. The …
