Urothelial neoplasia is a unique cancer in that it consists of a spectrum of tumors with different biologic behaviors. The most common urothelial neoplasm is the low grade superficial papillary carcinoma or papilloma which may recur numerous times but does not result in significant morbidity or mortality. A variant of the superficial papillary carcinoma, which represents approximately 10% of the tumors, is the noninvasive papillary neoplasm which progresses to a less differentiated invasive transitional cell carcinoma (TCC). Effect Of Cyclosporine On The Rate Of Renal Function Recovery After Renal Transplantation, Francis Dumler

To assess the effect of cyclosporine therapy on the rate of renal function recovery after renal transplantation, patients with no clinical evidence of rejection and who were treated with either cyclosporine or azathioprine in addition to steroid therapy were studied (n = 74). Of the patients with immediate renal function (n = 57), those receiving organs from living, related donors had a faster recovery rate of glomerular filtration than patients with cadaveric grafts (azathioprine, 15 ± 2 versus 7 ± 1 mL/min/day, P = 0.0001; cyclosporine, 14 ± 3 versus 6 ± 1 mL/min/day, P = 0.001). Quinidine-Induced Immune Thrombocytopenia, Mansoor N. Saleh, Nadhav Dhodaphar, Karen Allen, Albert F. Lobuglio

We have identified six cases of quinidine-induced immune thrombocytopenia based on clinical evidence and in association with elevated amounts of platelet surface IgG. The degree of thrombocytopenia did not correlate with severity of clinical symptoms, nor did it predict the amount of IgG on the platelet surface. Three of the patients recovered promptly after drug discontinuation alone whereas the other patients received additional corticosteroid therapy. The clinical presentation, mode of diagnosis, and therapeutic considerations in the treatment of drug-induced thrombocytopenia are discussed.

Surgical Management Of Spinal Epidural Disease: An Update, Jack P. Rock, Dale V. Hoekstra, Henry H. Schmidek

Management of spinal cord compression from metastatic malignant disease remains unsatisfactory. Results of surgical decompression are at best less than those of radiation therapy alone. However, new surgical approaches now focus on removing the anterior-situated tumor tissue which produces neural compression in about 85% of the cases. The results of these procedures that allow removal of the ventrally compressing tumor show significant improvement in the management of patients with spinal epidural disease. We review the surgical strategy of these new approaches and the attendant results.

Echo-Doppler Features Of Pulmonary Valve Endocarditis, Jihad Bitar, Mohsin Alam

Three patients with fever and pneumonitis were found to have pulmonary valve vegetations by echocardiography. Pulsed and color flow Doppler studies demonstrated mild to moderate pulmonary valve insufficiency in these patients. All patients responded to the appropriate antibiotics without requiring valve surgery. Echocardiography is of value in diagnosing pulmonary valve endocarditis and should be considered in every patient with recurrent pneumonitis.

Cerebral Infarction Associated With Cocaine Use, Iliana Meza, Carlos A. Estrada, Justo A. Montalvo, Walter N. Hidalgo, Jennie Andresen

We report the case of a young man with an acute infarction of the left putamen and caudate nucleus, whose symptoms appeared six hours after intranasal use of approximately 0.5 g of cocaine hydrochloride. It seems probable that in this patient cocaine consumption played a role in the development of stroke.

The Journal of Sociology & Social Welfare
Using data from clinic records of patients examined and treated at a public health facility, the reporting of unconfirmed cases is examined and the correspondence between public health profiles of patients at risk to sexually transmitted disease and the data are discussed. Implications relating to the findings and to public health policy are also discussed.

From The Gulf To The Rio Grande: Human Adaptation In Central, South, And Lower Pecos Texas, Thomas Hester, Stephen L. Black, D. Gentry Steele, Ben W. Olive, Anne A. Fox, Karl Reinhard, Leland C. Bement

The South Texas area, Region 3 of the Southwestern Division, U.S. Army Corps of Engineers, is synthesized from archeological and bioarcheological perspectives. Three distinct geographic units within Region 3 are treated in detail: Central Texas Plateau Prairie, South Texas Plains, and Lower Pecos Canyonlands. More than 11,000 years of human adaptation are chronicled for this area, stretching from the Gulf of Mexico to the Rio Grande along the border with northeastern Mexico. Particular attention is devoted to a consideration of the region's prehistoric record; significant problems and data gaps are outlined. For the first time, a compilation has been done …

Toxin Genotypes And Plasmid Profiles As Determinants Of Systemic Sequelae In Escherichia Coli 0157:H7 Infections, Stephen Ostroff, Phillip Tarr, Marguerite A. Neill, Jay H. Lewi, Nancy Hargrett-Bean, John M. Kobayashi

In 1987, 93 Escherichia coli 0157:H7 isolates were collected during routine surveillance for this pathogen in the state of Washington. Toxin genotypes and plasmid profiles were correlated with the clinical sequelae of illness in 88 of the 93 patients from whom these strains were isolated. Thirteen plasmid patterns were observed among the 88 tested isolates; four patterns accounted for 82(7o of the isolates. Genetic probing for Shiga-like toxins (SLT) I and II demonstrated the presence of both genes in 67 (76%), SLT I alone in three (3%), and SLT II alone in 18 (20%). Thousands of businesses in the United States have implemented programs to promote physical fitness among their employees (Falkenberg, 1987). The programs range from company-paid memberships at private health and fitness clubs to complete work site fitness facilities. These programs have been developed not only to improve and maintain employee health but also to promote psychological well-being and productivity and to reduce absenteeism, insurance claims, and stress. Recent research focusing on the use of employee fitness programs to improve work-related behaviors suggests that these programs may improve absenteeism, job satisfaction, tenure. and health care costs, especially among female employees (Baun, Bernacki, …

En esta edición de Quality/Calidad/Qualité se da información sobre un programa de planificación familiar postnatal que se lleva a cabo en el Centro Sfax del Hospital de Tunisia para Maternidad y Recién Nacidos. Este programa, desarrollado conjuntamente por el programa nacional de planificación familiar y por el personal de Maternidad, ha tenido extraordinario éxito en lograr que un mayor número de madres regresen después de seis semanas a su visita postnatal.
