Stature At Time Of Diagnosis Of Type 1 Diabetes Mellitus, John H. Diliberti, Koryn Carver, Elaine Parton, Joan P. Totka, Gail Mick, Kenneth Mccormick

Objective. To assess the stature of children with type 1 diabetes mellitus at diagnosis.

Methods. We collected data from 451 records of children who were examined in a pediatric diabetes clinic and used data from the Third National Health and Nutrition Examination Survey for 10 522 children as control group. Analytical techniques included linear and logistic regression modeling. A semiquantitative meta-analysis evaluated 38 earlier publications that contain information on height at the onset of diabetes.

Results. Children < 1 year of age were shorter than their peers by 1 standard deviation, whereas those from 3 years to near puberty were taller by approximately 0.3 standard deviation. Adjusting for parental height caused this difference to disappear for the older children but not for the infants. The meta-analysis results paralleled these observations.

Conclusions. Taller children generally seem to experience increased risk for development of diabetes mellitus type 1, except perhaps during infancy or early adolescence. …

This project was an assessment of elderly health care needs and access. Three urban San Bernardino communities in zip codes 92405, 92410, and 92411 were targeted. The assessment was structured according to King's theoretical construction of community as a multilevel interaction between personal, interpersonal and social systems. The components of the assessment included digital photographs, web-based internet assessments, key informant interviews, and community business visits.

The purpose of this study was to describe home health care nurses' knowledge of advance directives. Dorothea Orem's Theory of Self-Care guided this descriptive correlational study which investigated the relationship between knowledge of advance directives and the variables of nursing educational preparation and years of nursing experience. A convenience sample of 40 subjects employed by a Michigan home care agency completed a 39 item questionnaire adapted from Crego and Lipp (1998). The mean knowledge score was 84.68%. Analysis of the demographic profile showed no significant difference in knowledge levels by educational preparation. Years of nursing experience was found to have …

Complementary and alternative therapies (CAT) are being used by increasing numbers of individuals as both self-care and dependent-care strategies. Nursing leadership is needed to provide guidance and support to individuals using CAT Nurse-client relationships that center on therapeutic alliance may be a vehicle for supporting appropriate use of CAT as self-care and dependent care modalities. Eighty-seven mothers whose asthmatic children were involved in a 6-month glyconutritional supplementation trial were surveyed to evaluate their perceptions of therapeutic alliance using the Kim Alliance Scale (KAS). The conceptual framework for this study was Self-efficacy (Bandura, 1977), based on Social Cognitive Theory (Bandura, 1986). The purpose of this study was to determine if there is a relationship between exercise knowledge and exercise self-efficacy for the prevention of osteoporosis in young adults. Population selection was based on clinical research findings that bone health is affected by habits early in life, such as calcium intake and establishing regular exercise, which affect bone health in later years. Bottle-Feeding Mothers' Experiences Of Coping In A Pro-Breastfeeding Context : The Case For A Caring-Options-Responsive Model Of Midwifery Services, Elizabeth P. Duffy

The original contribution of this qualitative study is that it sketches the front-line of the contested domain of infant feeding choices by exploring the under-researched experiences and perceptions of mothers who actively choose to bottle-feed their babies. Twelve bottle-feeding mothers in Western Australia participated in open-ended, in-depth interviews. The interview findings were further explored using participant observations of relevant hospital practices and critical, hermeneutic re-readings of midwifery's professional and policy documents. The thesis argues that bottle-feeding is marginalised by the midwifery profession that currently defines its practices in accordance with policies such as the Baby Friendly Hospital Initiative. Objective: To describe expectant women's experiences with the onset of preterm labor.

Design: Qualitative, using grounded theory methods.

Setting: Southwestern tertiary women's hospital.

Participants: Thirty pregnant women who were less than 35 weeks gestation, had experienced preterm labor within the past 7 days, and had no previous experience with preterm labor.

Data Source: Taped and transcribed interviews.

Results: Themes that emerged from the interview data included the following: recognition and naming of sensations, a consistent pattern of attribution of symptoms, the threat or risk inferred by the attributed cause of the symptom pattern, the associated certainty or uncertainty about these …

ISSUES AND PURPOSE. Most children who are dependent on technology for survival live with their families at home. This study explores the perceptions of parents and home care nurses regarding rearing the technology-dependent child.

DESIGN AND METHODS. In this qualitative study, interviews were conducted with 16 parents whose child is technology dependent and 15 registered nurses who provided home care.

RESULTS. Rearing the child who is technology dependent is similar to but different from raising other children. Parental communication and negotiation of child-rearing expectations with home care nurses is essential.

Purpose: To present exemplars of physical activity research in nursing, illustrate the importance of physical activity research across the lifespan, and recommend directions for theory development and research.

Methods: Studies of physical activity and exercise currently being conducted by nurse investigators were reported and critiqued by attendees of the Midwest Nursing Research Society 2000 Preconference session entitled “Promoting Physical Activity Among Diverse Groups Across the Health Continuum.” Physical activity and exercise literature during the past decade was reviewed. Databases searched included Medline, CINAHL, Wilson, and ERIC.

The purpose of this descriptive study was to identify the known modifiable risk factors for osteoporosis in the college population. Pender's Theory of Health Promotion served as the theoretical framework for this study. The research question was as follows : What preventive behaviors are college students participating in that will increase their risk of osteoporosis? Risk factors were measured using the Osteoporosis Lifestyle Survey, a tool with face validity. Nationwide, approximately 4.1 million Americans suffer from chronic heart failure accounting for estimated direct and indirect costs of over $21 billion spent annually for health care. The incidence of heart failure has doubled each decade since the 1940's and slightly more men were inflicted with the illness. This trend changed three years ago, with a decline in the overall incidence of heart failure and a slight predominance of the illness in women rather than men. Cardiovascular disease, osteoporosis, and dementia are three disease processes that have been directly linked to estrogen deficiency. Cardiovascular disease (CVD) is the number one cause of death for all postmenopausal women, including those with end-stage renal disease (ESRD). Osteoporosis and dementia are disease processes that profoundly affect the quality of life for postmenopausal women. Neural tube defects (NTDs) are among the most serious birth defects affecting approximately 4,000 pregnancies in the United States each year. More than one third of these pregnancies are spontaneously aborted. Thus, about 2,500 infants per year are born with an NTD. Research spanning two decades has shown that women who consume folic acid- containing supplements have 50% to 75% fewer infants with NTDs than women who do not consume folic acid supplements. The United States Public Health Service recommends that age appropriate preconception care be provided to all women of reproductive age. Preconception health care provides a woman the opportunity to assess her health status before pregnancy, make appropriate health promoting behavior changes and increase the potential for a healthy pregnancy outcome. Neural tube defects, fetal alcohol syndrome, congenital malformations and low birth weight are examples of adverse infant conditions that can be reduced through preconception health care interventions.
