Dental morbidity is a chronic problem that affects a child's ability to attend school, to learn, and to enjoy good health. Of all childhood diseases, oral disease ranks as one of the highest and is considered one of the most common and the least treated. The prevalence of dental caries is high among children and adolescents in school populations. This study explored the prevalence of dental caries in a California Central Coast middle school during the 2002-03 school year (N=526). Findings indicated that severe dental caries existed in both boys and girls and in students of various ethnic groups. Approximately …

Treatment Of Hypertriglyceridemia With Omega-3 Fatty Acids: A Systematic Review, Amanda Gloria Lewis

Purpose: To 1) critically appraise available randomized controlled trials (RTCs) addressing the efficacy of long-chain ω-3 fatty acids as a secondary prevention agent of hypertriglyceridemia, and 2) make recommendations for clinical practice.
Data Sources: All RCTs identified from several databases from 1993-2003 were reviewed by two independent reviewers who extracted data from each study and used the previously tested Boyack and Lookinland Methodological Quality Index (MQI) to determine study quality.
Results: Ten studies reported long-chain ω-3 fatty acids to be effective in the treatment of hypertriglyceridemia. The average decrease in triglycerides (TG) was 29%, total cholesterol (TC) 11.6%, very low …

ln recent years, the number of Somali persons being treated in health care settings in the community of Rochester is increasing. The understanding of the concept of pain within this population is important to consider for nursing practice in this region and across the U.S. A focus group was used to explore the expression of pain with 4 Somali women in Rochester Minnesota. This was a non-experimental quantitative study using survey method to assess diabetic knowledge of 102 adult Vietnamese immigrants. The Health Belief Model provided the theoretical framework, and the 24-item diabetes knowledge questionnaire was adopted from the Starr County Texas study to use for this study. Data were categorized and analyzed using descriptive methods such as frequency and percentage. The findings indicated that lack of knowledge about diabetes was a significant issue among the Vietnamese immigrant population. Health care providers should develop an appropriate cultural educational program about diabetes to help the Vietnamese population to detect diabetes early and to control …

Background: Cardiac transplantation has been a treatment option for patients with end stage heart failure for the past 35 years. As evaluation and treatment protocols emerge and evolve it is the responsibility of the healthcare provider to assist the recipient in attaining the best quality of life (QOL) possible. Method: A demographic survey, along with the SF-36v2 generic survey, which measures quality of life in physical and functioning domains, was mailed to 100 heart transplant recipients in a large health plan in Northern California. Since 1961 newborn screening for errors of metabolism (EM) has improved the diagnosis, treatment and outcome of newborns with an EM. Recently, advances in laboratory technology with tandem mass spectrometry (MS/MS) has increased the identification of newborns with an EM. With a single dried filter paper blood spot (Guthrie R, Susi A. A simple phenylalanine method for detecting PKU in large populations of newborn infants. Pediatrics. 1963;32:338-343). Technological changes, interpersonal relationships, and scarce resources have created a complex environment for health professionals. Nurses, as the largest group of health care providers, are confronted with moral issues when dealing with managed care policies, end-of-life care and workplace or institutional issues. Moral issues are expressed as moral distress, which has been linked to decreased satisfaction, termination of employment and leaving the profession of nursing altogether. Moral distress, therefore, decreases the number of nurses and contributes to the critical nursing shortage. Bulimia nervosa is a common eating disorder that predominantly affects young women. There are three main models of purging in bulimia. Resulting medical complications are related to the particular mode and frequency of purging. Commonly, there are oral and gastrointestinal complications along with serious electrolyte and endocrine complications. The majority of the medical complications of bulimia nervosa are treatable if diagnosed in a timely fashion. Some of these patients require inpatient hospitalization, and others can be managed along a continuum of outpatient care. The American Psychiatric Association has comprehensive treatment guidelines for the management of bulimia. Primary care physicians and …

The development of pressure ulcers is a common occurrence among older persons who may be temporarily immobilized or confined to a bed. An estimated 60,000 deaths annually can be attributed to complications from pressure ulcers. Guidelines recommended by the Agency for Health Care Policy and Research have been disseminated nationally and continue to be recommended as the standard of care for pressure ulcer prevention. In spite of the use of the recommended guidelines, pressure ulcer rates persist, suggesting there are other factors contributing to pressure ulcer development in long-term care facilities.

Childbirth education provided to women is an Integral aspect of the childbirth experience. In Malawi, midwives face a major challenge because one of their major roles is to provide childbirth information to women. However, there are no existing Childbirth Education Programs to facilitate this process. The purpose of this study was, therefore, threefold. Change In Diet, Physical Activity, And Body Weight In Female College Freshman, Scott M. Butler, David R. Black, Carolyn L. Blue, Randall J. Gretebeck

Objective: To examine diet, physical activity, and body-weight changes associated with relocation from home to university.
Methods: Diet, fitness/physical activity, body-weight parameters and self-efficacy were assessed among 54 freshman women upon college entry and 5 months later.
Results: Although caloric intake significantly decreased, a significant increase occurred in body-weight parameters that may be attributed to significant decreases in total physical activity.
Conclusions: Interventions are needed aimed at increasing physical activity; improving diet quality related to consumption of vegetables, fruits, breads and pasta, and meats; and decreasing alcohol consumption.

Problem: Fetal Alcohol Syndrome (FAS) is preventable, under-diagnosed, and under-reported. Wisconsin rates for alcohol use and binge drinking in childbearing-age women exceed the national average. FAS prevalence in Wisconsin has not previously been systematically evaluated. Methods: The Wisconsin Fetal Alcohol Syndrome Screening Project (WFASSP) used a multi-stage, multi-source prospective population-based screening methodology to identify children born in 1998-1999 in Southeast Wisconsin who met a surveillance case definition for FAS. The 4-stage methodology used screening of electronic birth files, abstraction of neonatal medical records, and direct assessment of facial features, growth, and development at age 2 to 3 years. Nurses have identified a need for improving their knowledge and skills in providing end-of-life care. Critical care nursing textbooks can serve as an important source of information on end-of-life care for critical care nurses. Hence, an analysis of end-of-life content in 14 critical care nursing textbooks was conducted. Critical care nursing textbooks used for review were published in 1995 or later and identified from the libraries at the University of Wisconsin-Madison and Brigham Young University. It is important to understand constructs essential to low-fat-diet and physical-activity behaviors of low-income culturally diverse middle-school-age students, because ethnicity and socioeconomic status influence body weight status, and these groups are most at risk for obesity later in life. In this descriptive study, constructs important in low-fat diet and physical activity in low- to middle-income, culturally diverse middle school students were examined. Revised for cultural/developmental appropriateness and reading level, instruments incorporating constructs from the Health Promotion and Transtheoretical Models were tested with 221 youth. The purpose of this study was to determine the feasibility of a flexible multiple daily insulin (FMDI) regimen in routine pediatric diabetes care by comparing HbA1c, body mass index (BMI), and episodes of severe hypoglycemia (SH) before and after initiation of FMDI therapy. Data from 44 patients (2-16 years old), on a conventional insulin (CI) regimen, were collected during quarterly diabetes clinic visits. These patients were transitioned from CI to FMDI regimen: pre-meal lispro (bolus) and once or twice daily Humulin Ultralente with or without bedtime Humulin NPH as the basal insulin. Heart failure affects more than 4.6 million people in the United States (Dahl & Penque, 2000). While mortality from myocardial infarction is on the decline, congestive heart failure numbers continue to rise. Statistics indicate that 550,000 cases occur each year, at an estimated cost of $21.4 billion a year (National Institutes of Health, National Heart, Lung and Blood Institute, 2002). More women than men are dying of cardiovascular disease. The misconception persists that heart disease is a "man's disease" and not an urgent health care issue for women. In the United States, heart disease kills more women than all cancers combined. In 1997, 228,679 women died due to cardiovascular disease, and 41,443 women died from breast cancer. Yet, women are still not assessed effectively for their risk factors for cardiovascular disease; therefore, women are under-diagnosed and under-treated for this disease.
