Background: There are currently no guidelines for screening for thyroid peroxidase antibodies (TPOAb) during pregnancy. Currently, there is some debate as to whether pregnant women should be screened for TPOAb, universally, or using a risk-based approach. In addition, there is conflicting evidence suggesting the implications of positive TPOAb testing during pregnancy and their effect on not only the pregnancy, but also the postpartum period and the fetus. Background: Neonatal outcomes have not improved over the past 30 years in spite of increases in funding and utilization. New models of prenatal care, such as Centering Pregnancy, need to be evaluated for effectiveness. This critical review of the literature focuses on comparing birth outcomes, maternal weight gain, and adequacy of prenatal care between group and traditional care models. Results: Newborns with mothers in group care were more likely to be born at later gestational ages and with higher birth weights. Mothers in group care were more likely to use contraception postpartum and have better prenatal attendance. Women in at-risk …

Nurse-midwives and other obstetric providers should explore the most current literature for possible ways that perineal trauma and injury during vaginal birth can be avoided in patients. Purpose: To determine if hands-on perineum care, given by healthcare providers during labor and delivery, decreases perineal trauma and injury in women giving birth vaginally, compared with as opposed to women delivering vaginally who do not receive any hands-on perineum care. Purpose: The purpose of the critical review of literature was to identify and analyze benefits of delayed umbilical cord clamping in full term and preterm infants with a focus on iron stores, hemoglobin, hematocrit, and hyperbilirubinemia. Methods: Twenty-two articles were included in the critical literature review with the purpose of determining whether delayed umbilical cord clamping caused increases in iron stores, hemoglobin and hematocrit, or had no clinical significant increases in hyperbilirubinemia causing neonatal jaundice and the need for phototherapy treatment. Primary care providers are optimally placed to address this high-priority health issue. The purpose of the project was to implement a practice change in the delivery of nutrition and physical activity information during well child visits. Recent literature indicates that turnover is a major problem for health care organizations, since it is costly and negatively affects patient care. Several strategies have been highlighted to help improve retention rates, however hospice nurses have special needs. Methods. The purpose of the study was to examine the effects of interdisciplinary rounds (IDR) on a neurology medical-surgical inpatient unit. Existing research shows that IDR improves nurse-physician relationships and satisfaction and improves patient outcomes. Using the Collaboration and Satisfaction about Care Decisions (CSACD) tool developed by Baggs, nurse-physician collaboration and satisfaction was measured pre and post implementation of interdisciplinary rounds. Twenty-one nurses and five physicians participated in the study which included education sessions, a two week trial of IDR, and a pre and post survey. Common trends observed impacting HF patients and concerning inpatient nurses on a medical cardiology unit were identified and discussed. Understanding accessing and adhering to a low salt diet, fluid restriction, and daily weights were all commonly identified challenges by direct care nurses of HF patients. Problem and Background: Childhood obesity has doubled in children and quadrupled in adolescents in the past 3 decades, increasing from 5% to 17% in one generation. The highest rates of childhood obesity are among Mexican Americans. Obesity can increase health morbidities, and as obese children continue to grow, these health concerns progress to adulthood. In spite of enhanced education and communication between mothers of obese children and their primary care providers, minimal improvement has taken place in terms of outcomes and lifestyle changes.

Background: Positive deviant individuals practice beneficial behaviors in spite of having qualities characterizing them as high risk for unhealthy behaviors.

Objective: This study aimed to identify and understand factors distinguishing low-income African American women who breastfeed the longest (positive deviants) from those who breastfeed for a shorter duration or do not breastfeed.

Methods: Seven mini-focus groups on infant-feeding attitudes and experiences were conducted with 25 low-income African American women, grouped by infant-feeding practice. It can also be defined as the limited or uncertain ability to acquire acceptable foods in socially acceptable ways.
