Hepatitis C Education, Testing, And Treatment In Homeless Shelters: Models For Linkage To Care In High-Risk Populations, Erin H. Higley
All Electronic Theses and Dissertations
Background: With the increase of IV drug use, Hepatitis C viral (HCV) infections have increased considerably among the homeless population. The introduction of direct-acting antiviral (DAA) medications has made treating marginalized populations much easier. However, getting homeless patients and people who inject drugs (PWID) linked to care remains a challenge worldwide. More research is needed to ensure that all persons with HCV are able to access treatment regardless of social or economic status. Purpose: The purpose of this critical review of research is to identify models of care for treating HCV among the homeless. This review was done to support …

Pre-exposure prophylaxis (PrEP) is an effective tool for preventing human immunodeficiency virus (HIV) acquisition in sexually active at-risk individuals such as men who have sex with men (MSM). The purpose of this study is to examine factors associated with intent to engage in risky sexual behavior among HIV-negative college aged (18-24) MSM who are currently adherent to PrEP or who have expressed interest in the future adoption of PrEP. A multiracial/ethnic sample of 31 men expressing interest in the adoption of PrEP and 6 men currently taking PrEP completed a quantitative survey identifying key themes regarding attitudes towards PrEP and …
Improving Nursing Knowledge Of Heart Failure Discharge Self-Care Instructions, Maria Patricia Picar Deza
Improving Nursing Knowledge Of Heart Failure Discharge Self-Care Instructions, Maria Patricia Picar Deza
Walden Dissertations and Doctoral Studies
Patients with heart failure (HF) have recurrent symptoms that lead to frequent hospital readmissions. Moreover, HF is one of the leading causes of 30-day hospital readmissions. Evidence shows that patients with a high level of HF self-care have decreased readmissions. HF self-care education is commonly delivered by the nurse at hospital discharge, and the patient’s transition to self-care is a critical point in the continuum of care. However, nurses must be knowledgeable of patient self-care in HF to effectively convey this information to the patient upon discharge. Therefore, this staff education project question asked whether nursing knowledge of HF patient …

Using Unfolding Case Studies In A Traditional Classroom Setting To Enhance Critical Thinking Skills In Pre-Licensure Bachelor Of Science Nursing Students, Elaine M. Lloyd
Master's Theses and Doctoral Dissertations
Nursing education reform is needed for today’s generational mix of pre-licensure nursing students to prepare them to effectively care for clients’ ever-evolving healthcare needs. This mixed-methods, quasi-experimental study was designed to measure if the use of unfolding case studies (UCS) in a traditional classroom setting (TCS) would (a) enhance critical thinking skills of the experimental group more than the control group as measured by the Health Science Reasoning Test (HSRT), (b) explore if course content examinations were higher in the experimental group versus the control group, (c) explore the perceptions of a subset of Bachelor of Science Nursing (BSN) pre-licensure …
Antimicrobial Stewardship In An Urgent-Primary Care Clinic, Jian Connell
Antimicrobial Stewardship In An Urgent-Primary Care Clinic, Jian Connell
West Chester University Doctoral Projects
Antimicrobial resistance is a significant risk that threatens life, increases health complications, decreases antibiotic effectiveness, and increases healthcare cost. Overprescribing of antibiotics in outpatient settings is a key factor that has led to increased antibiotic resistance. There are more than 150 million antibiotics prescribed in outpatients annually; 30 percent of these antibiotic prescriptions are either unnecessary or inappropriately prescribed, with acute respiratory infections holding the position of the highest unnecessary use of antibiotics at 50 percent. Antibiotic stewardship is the effort to improve and measure antibiotic prescribing so that antibiotics are only prescribed when it is appropriate and necessary. This …

Methods To Increase The Rate Of Childhood Vaccinating, Mary Beth Tillman
Walden Dissertations and Doctoral Studies
Vaccinations such as polio, diphtheria, varicella, and meningitis help to protect a child from life-threatening illnesses which have previously been contained by routine vaccinations. However, misinformation, negative press, and other contributing factors, have influenced caregivers to refuse to vaccinate their children placing the children at risk for contracting the disease. The Centers for Disease Control and Prevention recommends regular vaccinations for children beginning as newborns, yet the vaccination rate remains below acceptable levels. This project was developed to provide an educational program guided by Orem’s self-care theory and intended to increase nurses’ knowledge of vaccination and ultimately vaccination rates. A …

In 2016, over 42,000 Americans died from opiate overdose with more than two million Americans having OUD. The need to care for patients with OUD has crossed all realms of nursing, including the perinatal specialty. Framed within the Iowa model of evidence-based practice, the purpose of this project was to develop a perinatal staff education program on OUD focusing on addiction as a brain disorder with care of the perinatal woman and identification and treatment of the newborn with …
Parents’ Perceptions Of Healthcare Influences On Their Decisions To Vaccinate Their Children, Patricia Frances Badiola Harris
Parents’ Perceptions Of Healthcare Influences On Their Decisions To Vaccinate Their Children, Patricia Frances Badiola Harris
Walden Dissertations and Doctoral Studies
Declining immunization rates are associated with higher incidents of vaccine-preventable diseases. The purpose of this qualitative phenomenological inquiry was to explore the perceptions of vaccine-hesitant parents regarding their healthcare experiences. Ajzen and Fishbein‘s theory of reasoned actions and its key concepts (the intention to perform behaviors, attitudes, subjective norms, and external variables) was used as a framework to understand influences on parents' decisions to vaccinate their children. The research questions for the study examined the healthcare experiences of vaccine-hesitant parents, how these experiences influenced their decisions to vaccinate their children, and how this group perceived the current strategies promoting vaccinations …
Improving Sedation Management In Mechanically Ventilated Patients, Jannette Lord
Improving Sedation Management In Mechanically Ventilated Patients, Jannette Lord
Walden Dissertations and Doctoral Studies
Abstract
In the intensive care unit (ICU), sedation management of the critically ill, mechanically
ventilated patient is a source of concern. Optimal sedation management is integral to
critical care practice, yet optimal levels of sedation are not consistently applied.
Suboptimal sedation carries significant risks for patients, as inadequate sedation or
oversedation may lead to prolonged ventilator days, ventilator-associated pneumonia
(VAP), extended length of ICU stay, and costs. The purpose of this quality improvement
(QI) project was to improve sedation management of mechanically ventilated patients by
improving staff nurses’ knowledge of and attitudes toward sedation management,
ultimately achieving a zero-ventilated associated …

Malaria has been studied for years, and yet remains one of the most significant vector-borne diseases that travelers encounter in malaria-endemic regions (CDC, 2019). Education is noted to be critical in disease prevention, management, and eradication. Student missionaries are at high risk for infection and can have a global influence that is improved through education. A pilot study was conducted on eleven students with a pre-test and post-test design after a malaria education webinar. The purpose of this pilot study was to assess the baseline level of education in student missionaries, as well as to assess the effect of malaria …
Multifaceted Interventions To Improve Hand Hygiene Compliance Among Nurses, Christopher Hoffman
Multifaceted Interventions To Improve Hand Hygiene Compliance Among Nurses, Christopher Hoffman
DNP Projects
The WHO has identified hand hygiene as the single “most important measure to avoid the transmission of harmful germs and prevent healthcare-associated infections” (World Health Organization [WHO], 2009) and subsequently created guidelines specifically focused on hand hygiene and prevention of hospital-acquired infections (HAIs). Nurses know and understand the importance of performing hand hygiene before and after patient contact, but often compliance is subpar. An extensive literature review was conducted for methods and interventions to improve hand hygiene compliance. The review of literature found seven systematic reviews which helped shape this study. The purpose of this study is to examine the …
Shift Work: Gut Health And Metabolic Disease, Hailey Lane
Shift Work: Gut Health And Metabolic Disease, Hailey Lane
Senior Honors Theses
The number of people engaged in shift work continues to rise, and with it the research linking shift work to various diseases and conditions. Shift work changes the body’s sleeping patterns and circadian rhythms which has an effect on all parts of the body including the heart, immunity, cancer development, and inflammatory processes. Altered sleeping patterns and circadian rhythms change the composition and anatomy of the gut and microbiome, which in turn affects the development of metabolic diseases. Shift work also changes one’s eating patterns and habits. Altered eating habits similarly changes the composition and anatomy of the gut and …

The Hispanic community is among the ethnic minorities that are adversely affected by HIV/AIDS in the United States. HIV transmission rates and rates of death are high among Hispanic communities because of delays in HIV testing and late participation in HIV treatment, especially in the state where this study was conducted. However, advanced practices for screening and early treatment of HIV can improve patient outcomes and inform practice. Thus, this systematic review was conducted to identify the best available evidence for HIV screening and care in the Hispanic population in the Western United States that shares a border with Mexico. …
Educational Program For Decreasing Catheter-Associated Urinary Tract Infections, Josephine Crentsil
Educational Program For Decreasing Catheter-Associated Urinary Tract Infections, Josephine Crentsil
Walden Dissertations and Doctoral Studies
Catheter-associated urinary tract infection (CAUTI) is the most common hospital-acquired illness. Insertion and maintenance of urinary catheters are critical factors that predispose patients to CAUTI. Studies have shown that many nurses have a knowledge deficit regarding evidence-based practices in the insertion and management of urinary catheters, which can help prevent CAUTI. This is a concern because nurses are typically the first point of contact with patients and are responsible for the promotion of evidence-based practice and preventative measures. Therefore, this project aimed to develop an educational intervention for nurses with a focus on proper insertion and maintenance of indwelling urinary …

Retail Pharmacy Clinic Antibiotic Stewardship Program And Guidelines, Abdulrahman Sudi Amiri
Walden Dissertations and Doctoral Studies
Overuse of antibiotics poses serious consequences for patient's well-being, including the risk for antibiotic resistance and adverse effects from inadequate and timely treatment. The emergence and frequency of multi-drug resistant bacteria are alarming global public health care experts, as these strains are resisting effective management of life-threatening infections. The gap in practice was that a retail pharmacy clinic did not have a practice guideline for the implementation of a wait-and-see prescription program as part of an antimicrobial stewardship program. The purpose of this project was to coordinate an interprofessional team to develop guidelines for this primary care setting. The practice-focused …
Increasing Compliance Of Chlorhexidine Wipes To Prevent Hospital-Acquired Infections, Kelly Jean Dunn
Increasing Compliance Of Chlorhexidine Wipes To Prevent Hospital-Acquired Infections, Kelly Jean Dunn
Walden Dissertations and Doctoral Studies
AbstractHealthcare-acquired infections (HAIs) are complications of healthcare linked to increased mortality, morbidity, and length of stay. Assiduous surveillance and constant reeducation can decrease HAI incidence and reduce the healthcare burden caused by these events. Evidence has demonstrated that using 2% chlorhexidine wipes instead of bathing with soap and water greatly reduces the bacteria on the skin and prevents infections such as catheter-associated urinary tract infections and central line-associated bloodstream infections. The practice-focused question for this quality assurance doctoral project was to evaluate if an educational intervention improved compliance of chlorhexidine wipe use among ICU nurses with the hope of improving …

A review of the literature revealed best practices for suicide screening including universal education and screening and targeted education and screening for at-risk populations. The rate for suicides in the county is also higher than the rates for the region, state, and nation. The purpose of this project was …
Examining Body Mass Index And Sepsis Mortality At One Year After Sepsis, Jamie D. Robinson
Examining Body Mass Index And Sepsis Mortality At One Year After Sepsis, Jamie D. Robinson
Theses and Dissertations
Introduction: Sepsis is a serious and life-threatening syndrome affecting 1.7 million Americans annually and resulting in approximately 270,000 deaths. An “obesity paradox” where obese individuals have lower sepsis mortality than their non-obese counterparts has been described. The problem is that the longevity of the effect is unknown, and few studies have examined the obesity paradox after 1-2 months post-diagnosis.
Methods: This retrospective cohort study examined clinical, demographic, and biomarker variables thought to affect sepsis mortality at three-time points: 30 days, 180 days, and at one year post-sepsis diagnosis in order to shed light on specific factors that might define …
