The Influence Of Support Systems And Internal System Resources On Family Well-Being Of Caregivers Of A Depressed Family Member, Diane K. Richardson

Health professionals are interested in families who have a high level of well-being in spite of coping with the difficulties related to caring for a depressed elderly family member at home. The Resiliency Model of Family Stress, Adjustment, and Adaptation provided the conceptual framework to explore the hypothesis of a positive relationship between family social support and family well-being, and family internal system resources and family well-being. Thirty caregivers volunteered and were interviewed in their homes using the Family Crisis Oriented Personal Scale (F-COPES), the Family Hardiness Index (FHI), and the Family APGAR. Personal values are increasingly recognized as important factors in work environments, including health care systems (Cameron & Whetten, 1995; Covey, Merrill, & Merrill, 1995; Fagermoen, 1997). The personal values of nursing students have been explored (Eddy, Elfrink, Weis, & Schank, 1994; Sivberg & Petersson, 1997) as have those of nurses, in the context of work design (Koerner, 1993 ). Factors of nurses' work satisfaction have also been studied extensively (Avallone & Gibbon, 1998; Brown, Naughton, & Nolan, 1998; Johnston,1997; Kangas, Kee, & McKee-Waddle,1999; Seymour & Buscherhof,1990). Objective: To compare outcomes and charges of health care delivery to extremely low-birth-weight infants by neonatal nurse practitioners (NNP) and pediatric residents.
Design: Retrospective cohort study.
Setting: A 56-bed neonatal intensive care unit (NICU) in a university teaching hospital.
Methods: Study population included all infants with birth weights less than 1000 g who were admitted to the NICU during the 2-year period between September 1, 1994, and August 31, 1996. Infants who died earlier than 12 hours of age, or who were admitted after 1 week of age or with major malformations, chromosomal abnormalities, or congenital infections were excluded. There …

Almost twenty years ago, Pope John Paul II in his apostolic exhortation Familiaris Consortio called on scholars to study the anthropological and moral differences between the recourse to the natural rhythms of a woman’s menstrual cycle (i.e., natural family planning) and contraception (1). Although natural family planning (NFP) and contraception can both be used to prevent pregnancy, there are conspicuous differences between use of natural methods and contraception. Through The Eye Of The Beholder: Multiple Perspectives On Quality In Women's Health Care, Jaynelle F. Stichler, Marianne E. Weiss

Quality is an illusive concept with different meanings to different people. Providers often define quality in terms of patient outcomes, professional standards of practice, predetermined criteria used to measure quality, and even subjective opinion. Patients describe quality in terms of the interpersonal aspects of care, how well they were treated, and the responsiveness of the provider to their needs. This qualitative study using a semi-structured interview defined quality from the perspectives of patients, physicians, nurses, and payers associated with a hospital-based women's service line, and how the attributes of quality varied among the multiple groups. ทรัพยากรมนุษย์มีความสำคัญต่อการพัฒนาองค์การเป็นอย่างยิ่ง เพราะทรัพยากรมนุษย์ มีส่วนในการพัฒนาองค์การให้มีศักยภาพและประสิทธิภาพ การดูแลทรัพยากรมนุษย์ใน องค์การจึงเป็นสิ่งที่ผู้บริหารองค์การต้องคำนึงถึง โดยเฉพาะขวัญกำลังใจ ความรู้สึกต่อองค์การที่ตน ทำงานอยู่ รวมทั้งบรรยากาศในการทำงานของบุคลากรในองค์การด้วย ทั้งนี้เพื่อนำไปสู่การบริหาร จัดการให้เหมาะสม ก่อให้เกิดการพัฒนาองค์การ \nการศึกษานี้เป็นการศึกษาแบบตัดขวาง มีวัตถุประสงค์เพื่อศึกษา 1) ความคิดเห็นต่อ บรรยากาศองค์การของฝ่ายบริการพยาบาลโรงพยาบาลสงขลานครินทร์ 2) ปัจจัยที่มีผลต่อบรรยากาศ ด้านการทํางานเป็นทีม การให้บริการ การเปลี่ยนแปลงงาน การปรับปรุงระบบงาน เป้าหมายใน การทำงานร่วมกัน และความพึงพอใจที่มีต่อการทำงานในองค์การ กลุ่มตัวอย่างที่ศึกษา ได้แก่ พยาบาล ผู้ช่วยพยาบาล พนักงานผู้ช่วยการพยาบาล เสมียน คนงาน และเจ้าหน้าที่อื่น ๆ จำนวนทั้งสิ้น 600 ราย วิธีการเก็บข้อมูล โดยแบบสอบถาม เครื่องมือที่ใช้ในการเก็บข้อมูลเป็นแบบสอบถาม จำนวนทั้งสิ้น 55 ข้อ ผลการศึกษา พบว่า (1) กลุ่มตัวอย่างมีอายุเฉลี่ยประมาณ 32 ปี มีระยะ เวลาในการปฏิบัติงานประมาณ 11 ปี มีโอกาสได้รับการอบรมเฉลี่ย 1 ครั้ง ตลอดช่วงระยะเวลาที่ปฏิบัติงานในฝ่ายบริการพยาบาล (2) ความคิดเห็นต่อบรรยากาศองค์การด้านความสามารถใน การเปลี่ยนแปลงวิธีการทำงาน การทำงานเป็นทีมการใช้ความคิดสร้างสรรค์ การให้ความหมายของ คุณภาพ การตอบสนองความต้องการของผู้ป่วย/ลูกค้า เป้าหมายและอุดมการณ์ร่วมกัน และ ความพึงพอใจในงานอยู่ในระดับปานกลางค่อนไปทางมาก ส่วนความสัมพันธ์ของผู้ป่วยใน และ การปรับปรุงระบบงานค่อนไปทางน้อย ดังนี้ 6.16 6.24 6.10 7.05 6.75 6.09 6.22 5.84 และ 4.80 (3) เมื่อศึกษาถึงปัจจัยที่มีผลต่อความคิดเห็นต่อบรรยากาศในการทำงาน โดยการวิเคราะห์สมการ ถดถอย พบว่า ระดับการศึกษาและโอกาสที่ได้รับการอบรมเกี่ยวกับ Hospital accreditation มีผลต่อ ความรู้สึกต่อบรรยากาศในการทำงาน (beta.132 และ beta.126,p<.05) (4) ผลของการเปรียบเทียบ ความคิดเห็นต่อบรรยากาศองค์การของผู้ที่ทำงานในระดับพยาบาลประจำการ ผู้ช่วยพยาบาล พนักงาน ช่วยเหลือการพยาบาล เสมียน และคนงาน โดยการวิเคราะห์ความแปรปรวนของตัวแปร พบว่า มีความแตกต่างกันในด้านการปรับปรุงระบบงาน เป้าหมายและอุดมการณ์ร่วมกันของบุคลากร อย่างมี นัยสําคัญทางสถิติที่ระดับ .05 ส่วนด้านการทํางานเป็นทีม การใช้ความคิดสร้างสรรค์ การให้ความหมาย ของคุณภาพ การตอบสนองความต้องการของผู้ป่วย ความสัมพันธ์ระหว่างผู้ป่วย และความพึงพอใจ ต่อการทํางาน ไม่มีความแตกต่างกันอย่างมีนัยสําคัญทางสถิติที่ระดับ .05 นอกจากนี้ในส่วนของข้อคำถาม ปลายเปิด คณะผู้วิจัยได้นำมาวิเคราะห์โดยใช้โปรแกรม Ethnograph 4.0 พบว่า ก) ส่วนใหญ่ต้องการ เห็นภาพของโรงพยาบาลสงขลานครินทร์ในสายตาของผู้รับบริการว่าเป็นโรงพยาบาลที่มีคุณภาพ การบริการที่ดี บุคลากรมีความสามารถ และมีเครื่องมือทันสมัย รวมทั้งค่าบริการไม่แพงนัก ข) ในด้าน อุปสรรค พบว่า มีอุปสรรคที่นโยบายการบริหาร บรรยากาศในการทํางานไม่เอื้ออํานวย ซึ่งมีทั้งจาก บุคลากร การได้มีส่วนร่วมในการบริหารจัดการ และนโยบายขององค์การ ค) สิ่งที่น่าจะได้พัฒนาและ นําไปสู่ความสําเร็จของการทํางานที่มีประสิทธิภาพ คือ การส่งเสริมด้านวิชาการ การส่งเสริมความสามัคคี การมีส่วนร่วมในการพัฒนาหน่วยงาน ปลูกจิตสํานึกในการทํางานและรักงานของบุคลากรทุกระดับ การได้รับการยอมรับจากผู้ร่วมงาน ไม่มีการแบ่งระดับ และผู้บริหารควรยอมรับฟังปัญหา

การวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาสมรรถนะของหัวหน้าฝ่ายการพยาบาล โรงพยาบาลชุมชนโดยรวมและรายด้านตามการรับรู้ของตนเอง และศึกษาความสัมพันธ์ระหว่างภูมิหลัง ของบุคคล คุณลักษณะส่วนบุคคล และสภาพแวดล้อมในหน่วยงาน กับสมรรถนะของหัวหน้า ฝ่ายการพยาบาลตามการรับรู้ของตนเอง โรงพยาบาลชุมชน สังกัดกระทรวงสาธารณสุข กลุ่มตัวอย่าง ที่ศึกษา คือ หัวหน้าฝ่ายการพยาบาล โรงพยาบาลชุมชน สังกัดกระทรวงสาธารณสุข จำนวน 262 คน \nเครื่องมือที่ใช้รวบรวมข้อมูล คือ แบบสอบถามที่สอบถามเกี่ยวกับภูมิหลังของบุคคล คุณลักษณะส่วนบุคคล สภาพแวดล้อมในหน่วยงาน และแบบประเมินสมรรถนะหัวหน้า ฝ่ายการพยาบาล โรงพยาบาลชุมชน ค่าความเที่ยงทั้งฉบับ มีค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ .98 สถิติ ที่ใช้ในการวิเคราะห์ข้อมูล คือ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์การจรณ์ ค่าไค-สแควร์ และค่าสัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน \nผลการวิจัยพบว่า \n1. หัวหน้าฝ่ายการพยาบาลมีสมรรถนะโดยรวมอยู่ในระดับปานกลาง โดยมีสมรรถนะ ด้านภาวะผู้นำสมรรถนะด้านการปฏิบัติการพยาบาล และสมรรถนะด้านการฝึกอบรม อยู่ในระดับสูง ส่วนสมรรถนะด้านการบริหารงาน สมรรถนะด้านการตลาด สมรรถนะด้านการสนับสนุนงาน การสาธารณสุขมูลฐาน สมรรถนะด้านการใช้เทคโนโลยีทันสมัย อยู่ในระดับปานกลาง และสมรรถนะ ด้านวิชาการและการวิจัย อยู่ในระดับต่ำ\n2. ภูมิหลังของบุคคลด้านอายุ ประสบการณ์ในการทำงาน ประสบการณ์ในตำแหน่ง และประสบการณ์ในการศึกษาอบรมเฉพาะตำแหน่ง ไม่มีความสัมพันธ์กับสมรรถนะโดยรวมของหัวหน้าฝ่ายการพยาบาล \n3. ภูมิหลังของบุคคลด้านระดับการศึกษา มีความสัมพันธ์กับสมรรถนะโดยรวมของ \nหัวหน้าฝ่ายการพยาบาล อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 \n4. คุณลักษณะส่วนบุคคล มีความสัมพันธ์ทางบวกในระดับปานกลางกับสมรรถนะ \nโดยรวม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 \n5. สภาพแวดล้อมในหน่วยงาน ด้านลักษณะและสภาพการทํางานในหน่วยงาน โอกาส ในการแสดงความสามารถ และการมีพี่เลี้ยง มีความสัมพันธ์ทางบวกในระดับปานกลาง กับสมรรถนะ โดยรวมของหัวหน้าฝ่ายการพยาบาล อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 \n6. สภาพแวดล้อมในหน่วยงาน ด้านขนาดของโรงพยาบาล และการก้าวหน้าในตำแหน่ง ตามบันไดอาชีพไม่มีความสัมพันธ์กับสมรรถนะโดยรวมของหัวหน้าฝ่ายการพยาบาล

การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อศึกษาการจัดกิจกรรมและสภาพแวดล้อมการเรียนการ สอนภาคปฏิบัติวิชาการพยาบาลผู้สูงอายุ ของอาจารย์พยาบาล ในวิทยาลัยพยาบาล สังกัดสถาบันพระบรมราชชนก จำแนกตาม อายุ วุฒิการศึกษา ประสบการณ์ในการปฏิบัติงานด้านการสอน และการได้รับการฝึกหรืออบรมเกี่ยวกับการพยาบาลผู้สูงอายุ ประชากรและกลุ่มตัวอย่าง คืออาจารย์ พยาบาลซึ่งเป็นผู้จัดการเรียนการสอนภาคปฏิบัติวิชาการพยาบาลผู้สูงอายุ ในวิทยาลัยพยาบาล สังกัดสถาบันพระบรมราชชนก จำนวน 68 คน เครื่องมือที่ใช้ในการวิจัยเป็นแบบสอบถามและ แบบสัมภาษณ์เชิงลึกที่ผ่านการทดสอบความตรงตามเนื้อหาและมีค่าความเที่ยงเท่ากับ .97 \nผลการวิจัยพบว่าอาจารย์พยาบาลจัดกิจกรรมการเรียนการสอนภาคปฏิบัติวิชาการ พยาบาลผู้สูงอายุในขั้นวางแผน ขั้นดำเนินการ และขั้นประเมินผล อยู่ในระดับสูง ( X = 4.25, 4.05, 4.10 ตามลำดับ) สภาพแวดล้อมการเรียนการสอนทางกายภาพอยู่ในระดับปานกลาง (X = 3.31) และ สภาพแวดล้อมทางจิตใจและสังคมอยู่ในระดับสูง (X = 3.90) การจัดกิจกรรมและสภาพแวดล้อม การเรียนการสอนภาคปฏิบัติวิชาการพยาบาลผู้สูงอายุของอาจารย์พยาบาล จำแนกตามอายุ วุฒิการศึกษา ประสบการณ์ในการปฏิบัติงานด้านการสอน และการได้รับการศึกษาหรืออบรมเกี่ยวกับการพยาบาลผู้สูงอายุไม่แตกต่างกัน การจัดกิจกรรมและสภาพแวดล้อม การเรียนการสอนภาคปฏิบัติ สามารถอธิบายได้ดังนี้ ในขั้นวางแผน อาจารย์พยาบาลมีการประชุมปรึกษาและศึกษาถึงลักษณะวิชา วัตถุประสงค์ของวิชา พร้อมทั้งวางแผนกิจกรรมการเรียนการสอนภาคปฏิบัติ และประสานงานกับแหล่งฝึกภาคปฏิบัติ ทั้งแบบเป็นทางการและไม่เป็นทางการ ในขั้นดำเนินการ พบว่าอาจารย์พยาบาลใช้วิธีการสอนหลาย วิธีผสมผสานกัน และมีการกำหนดให้นักศึกษาจัดกิจกรรมในชมรมผู้สูงอายุหรือสถานสงเคราะห์ เพื่อ ให้นักศึกษามีส่วนร่วมในการเรียนการสอน ในขั้นการประเมินผล พบว่าเกณฑ์การประเมินผลและวิธีการประเมินผล ขึ้นอยู่กับการจัดกิจกรรมการเรียนการสอนภาคปฏิบัติของอาจารย์พยาบาล สําหรับ สภาพแวดล้อมทางกายภาพ พบว่าสภาพแวดล้อมทางกายภาพภายในโรงพยาบาล ขาดอุปกรณ์ที่ เอื้อต่อการพยาบาลผู้สูงอายุ แต่ภายในสถานสงเคราะห์มีความเหมาะสมมากกว่า และสภาพแวดล้อม ทางจิตใจและสังคม พบว่าอาจารย์พยาบาล พยาบาลประจําการและนักศึกษาพยาบาลมีสัมพันธภาพที่ดี ต่อกัน

Little is known of the way that nurses' roles have evolved within a hospital setting in which efficient and affective interventions include caring for sicker patients. The emergence of the Patient Care Coordinator (PCC) is one role that nursing developed to participate in an interdisciplinary collaborative team approach to caring for sicker patients.
This qualitative study examined the exemplary role of eleven PCCs who practice in a regional nonprofit medical center. Research methodologies included interview, non-participant observation, and analysis of historical data. Medical advances in disease management has improved quality of life for women with cystic fibrosis (CF) and now enables them to consider pregnancy. The purpose of the study was to examine women's perceptions of how the demands of having CF influenced their ability to care for their children and continue to meet their own health needs. When elderly people are cared for In Aged Care Facilities (ACFs) it is known that their family members frequently suffer negative effects. These effects may be alleviated by social support and, because they feel better, family members may then offer more support to residents. In this study, the researcher tested a model with a sample of 213 family members of ACF residents. The purpose of this study was to identify ADN faculty and ADN student perceptions of the most effective clinical teaching characteristics (ECTC). An exploratory, descriptive design was implemented. ADN faculty and ADN students were surveyed using the Clinical Teacher Characteristics Instrument (CTCI). The CTCI consists of 20 characteristics of clinical teachers that are ranked on a five point Likert-type scale according to importance. The collected data were analyzed using descriptive statistics. This paper describes some theoretical perspectives of spirituality, the concepts of spiritual care and spiritual needs, and barriers to providing spiritual care as described in the literature. The two most frequently described barriers are lack of comfort with the topic and lack of education. Because there is so little written about the discussion of spirituality in schools of nursing, a descriptive study using nursing students on the central coast of California was conducted. The Spiritual Involvement and Beliefs Scale (SIBS) was used to measure intrinsic spirituality. This study looked for relationships between the scores on this scale, and the students' …
