Struggling With Disconnection: The Lived Experience Of Being Homeless And In Need Of Healthcare, Nonceba Mbangata
Theses and Graduate Projects
The intent of this field project was to study the disconnection that is experienced by homeless persons within the healthcare system. Homelessness is a growing concern that affects all age groups throughout the United States. It is not a secret that the increase in homelessness is stressing the health care system and limiting choices of becoming a healthy individual. The state of the health care system today and the nomadic habits of homeless persons make it impossible for those struggling with homelessness to stay connected to health care services. This project will illuminate the lived experiences of homeless persons as …

Sleep And Affect In Older Adults: Using Multilevel Modeling To Examine Daily Associations, Christina S. Mccrae, Joseph P. H. Mcnamara, Meredeth Rowe, Joseph M. Dzierzewski, Judith Dirk, Michael Marsiske, Jason G. Craggs
Nursing Faculty Publications
The main objective of the present study was to examine daily associations (intraindividual variability or IIV) between sleep and affect in older adults. Greater understanding of these associations is important, because both sleep and affect represent modifiable behaviors that can have a major influence on older adults’ health and well-being. We collected sleep diaries, actigraphy, and affect data concurrently for 14 days in 103 community-dwelling older adults. Multilevel modeling was used to assess the sleep–affect relationship at both the group (between-persons) and individual (within-person or IIV) levels. We hypothesized that nights characterized by better sleep would be associated with days …

Parenting Foster Children With Chronic Illness And Complex Medical Needs, Lori S. Lauver Phd, Rn
College of Nursing Faculty Papers & Presentations
The experience of parenting foster children with chronic illness and complex medical needs was explored in a phenomenological inquiry with 10 foster families. Thirteen participants currently fostering chronically ill children with complex medical needs were interviewed. Recorded interviews were transcribed and analyzed using van Manen's method. Data analysis yielded five essential themes: Foster parents described being committed to the child in their care, coming to know the needs of a medically complex foster child, and identifying effective and ineffective interventions encountered through day-to-day living with a medically complex child. Furthermore, they shared what it was like to experience loss of …

Expectation Modulates Human Brain Responses To Acute Cocaine: A Functional Magnetic Resonance Imaging Study, Peter Kufahl, Zhu Li, Robert C. Risinger, Charles Rainey, Linda B. Piacentine, Gaohong Wu, Alan S. Bloom, Zheng Yang, Shi-Jiang Li
College of Nursing Faculty Research and Publications
Background
Human expectation of psychoactive drugs significantly alters drug effects and behavioral responses. However, their neurophysiological mechanisms are not clear. This study investigates how cocaine expectation modulates human brain responses to acute cocaine administration.
Methods
Twenty-six right-handed non–treatment-seeking regular cocaine abusers participated in this study. Changes in blood oxygenation level-dependent (BOLD) signals were measured, and online behavioral ratings during cocaine expectation and acute cocaine administration were recorded.
Results
Distinct regional characteristics in BOLD responses to expected and unexpected cocaine infusions were observed in the medial orbitofrontal gyrus (Brodmann area [BA] 11), frontal pole (BA 10), and anterior cingulate gyrus regions. …
A Comparison Of Perinatal Care Providers' Use Of The National Institute Of Child Health And Human Development Standardized Terminology In Documentation Of Intrapartal Fetal Heart Rate Patterns, Faye M. Sigman
Dissertations
The purpose of this study was to determine if perinatal team members; nurses (RN) and primary care providers (PCP), were using the NICHD standardized terminology to document Fetal Heart Rate patterns during labor. Agreement in documentation of FHR and agreement in concept between the RN and PCP was also studied. A descriptive, comparative research design was used. Cohen?s Kappa statistics measured agreement in documentation of FHR patterns and Chi square measured agreement in concept, p< 0.05 for each. A retrospective medical records chart review was performed on 400 charts, meeting inclusion criteria, from three community hospitals. There were three data collection points and four criteria reviewed. This study found the use of NICHD terminology to document FHR alarmingly low (RN=51%; PCP=13%). It was used most often for decelerations (81%) RN, (22%) PCP, and least often for variability (19%) RN, (3%) PCP. Incomplete documentation was extremely high for the PCP (69%) and 81 charts (20%) had no FHR documentation. Agreement in documentation varied between the RN and PCP. They agreed most often on accelerations (81.4%) and least often on baseline rate (41.5%). When looking at all there points in time the RN and PCP agreed in documentation 59% but agreed in concept 78%. There were four areas where the RN and PCP agreed in their use of NICHD terminology: Accelerations on admission n=151, Kappa=0.091, p=0.007; variability during labor n=68, Kappa=0.27, p=0.015; variability prior to delivery n=33 Kappa=0.33, p=0.010, and decelerations during labor n=103, Kappa=0.16, p=0.018. Data from this study supports expanding this research, to identifying barriers to documentation. It also appears that education in use of NICHD terminology is needed.

The Effect Of A Nurse-Respiratory Therapist Weaning Protocol On The Duration Of Mechanical Ventilation And The Incidence Of Ventilator-Associated Pneumonia, Andrea Lucas
Theses, Dissertations and Capstones
The purpose of this research study was to show the effectiveness of implementing a standardized nurse-respiratory driven weaning protocol in decreasing the time spent on mechanical ventilation and the incidence of ventilator-associated pneumonia in mechanically ventilated patients compared to using individual physician methods. Chart reviews were performed to compare the incidence of ventilator-associated pneumonia (VAP) prior to the initiation of the nurse-respiratory driven weaning protocol. Sister Callista Roy’s Adaptation theory was used as a framework for this study to show that nurses could promote adaption by using a standardized ventilator weaning protocol. The study did not show statistical significance that …

Bridge Over The Theory-Practice Divide, Judith Wilson
Nursing Conference Papers
This qualitative study was conducted to explore how the student midwife in the final stages of her Postgraduate Diploma of Midwifery Course perceived the theory practice gap. Much research has been published about the theory practice gap from the perspective of educators or employers but little is published on the view of the student who sits in the centre of this dilemma. This study was conducted by interviewing 5 student midwives with a set of open ended questions. The first of these questions sought to ascertain if indeed the student perceived such a gap existed. Further questions explored factors, if …
