Purpose: The purpose of this QI project was to create a standardized order set aiming to increase consistency in lab work completion for improved monitoring of pituitary dysfunction in patients diagnosed with sinonasal and nasopharyngeal malignancies. Background: Radiation therapy is important in the treatment plan for this population. The hypothalamic-pituitary axis is frequently included within the radiation field, contributing to radiation induced pituitary dysfunction. There is no established order set for outpatient pituitary lab monitoring at the organization. วัตถุประสงค์: เพื่อศึกษาอาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ และศึกษาความสัมพันธ์ระหว่าง อายุ เพศ ภาวะอ้วน การสูบบุหรี่ โรคประจำตัว ประวัติการรับวัคซีนป้องกันโควิค-19 ก่อนการติดเชื้อ ความรุนแรงของอาการติดเชื้อโควิด-19 การรักษาที่ได้รับระหว่างพักรักษาตัวในโรงพยาบาล ความเครียด และกิจกรรมทางกายกับอาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ
รูปแบบการวิจัย: เชิงพรรณนาและหาความสัมพันธ์
วิธีการดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ที่มีอายุเกิน 60 ปีขึ้นไปทั้งเพศชายและหญิงที่มีประวัติติดเชื้อไวรัสโคโรนา 2019 เป็นเวลาอย่างน้อย 4 สัปดาห์ขึ้นไป โดยนับตั้งแต่วันแรกของการติดเชื้อ และเคยเข้ารับการรักษาทั้งเป็นผู้ป่วยนอก ผู้ป่วยใน หรือหอผู้ป่วยเฉพาะกิจโควิด-19 (Hospitel) ในโรงพยาบาลราชวิถี โรงพยาบาลตำรวจ และโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย จำนวน 87 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบบันทึกข้อมูลส่วนบุคคล แบบวัดความรู้สึกเครียด แบบสอบถามกิจกรรมทางกายระดับสากล และแบบสอบถามอาการหลงเหลือภายหลังการติดเชื้อ ไวรัสโคโรน่า 19 ในผู้สูงอายุ โดยเครื่องมือมีคุณภาพทั้งความตรงตามเนื้อหาและความเที่ยง วิเคราะห์ข้อมูลโดยใช้สถิติสัมประสิทธิ์สหสัมพันธ์อีต้า สถิติสหสัมพันธ์สเปียร์แมน และสถิติสหสัมพันธ์เพียร์สัน
ผลการศึกษา: 1) อาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ พบมากที่สุด 10 อันดับแรก ได้แก่ อ่อนเพลีย/เหนื่อยล้า ไอ นอนไม่หลับ มีเสมหะ เจ็บคอ ปวดกล้ามเนื้อ มีน้ำมูก หายใจลำบาก ปวดตามข้อ และปวดหลัง 2) ปัจจัยที่มีความสัมพันธ์กับอาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ อย่างมีนัยสำคัญทางสถิติที่ระดับ p<.05 ประกอบด้วย ภาวะอ้วน (rp = .69) เพศ ( = .31) ความเครียด (rp = .33) โรคประจำตัว ( = .23) อายุ (rs = -.23) และประวัติการรับวัคซีนป้องกันโควิค-19 ก่อนการติดเชื้อ (rp = -.16) ส่วนการสูบบุหรี่ ความรุนแรงของอาการติดเชื้อโควิด-19 การรักษาที่ได้รับระหว่างพักรักษาตัวในโรงพยาบาล และกิจกรรมทางกาย ไม่มีความสัมพันธ์กับอาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ
สรุป: สามารถนำผลการวิจัยที่ได้รับไปพัฒนาโปรแกรมการดูแลผู้สูงอายุที่ติดเชื้อไวรัสโคโรน่า 19 เพื่อลดปัจจัยการเกิดอาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ได้ต่อไป

Aim: The aim of this study is to synthesise literature on specialised nursing career frameworks to inform the development of an aged care nursing career framework. Design: An integrative review was conducted. Method: The review followed Whittemore and Knafl's five-step integrative review method. To appraise the quality of the studies, the Quality Appraisal for Diverse Studies tool for peer-reviewed articles and the ACCODS (Authority, Accuracy, Coverage, Objectivity, Date, and Significance) checklist for grey literature were used. Immigrants to Canada face unique barriers to accessing health care including communication barriers, socioeconomic limitations, and a lack of health care system navigation knowledge. Arabs in particular face mainstream negative attitudes, propagated through political discourses as well as entertainment media. We sought to understand how older adult Arab immigrants and family caregivers accessed and experienced health care in Ontario. We conducted a critical constructivist grounded theory, using Charmaz’s approach to grounded theory. Background: Financial health literacy (FHL) is an emerging concept with unique application to older adults. Multiple complexities converge in older age that create increased financial vulnerability (e.g., retirement, chronic disease). However, low financial and health literacy are common in older adults, impacting their ability to self-manage. The AWV’s focus on prevention offers an opportunity to incorporate a proactive approach to financial-related assessment. This study will explore the perceived value of assessing FHL in primary care settings and the financial-related barriers and facilitators to implementing the personalized prevention plan.

Hispanic older adults represent a growing population in the United States and may be disproportionately affected by cognitive impairment and depression. We theorize that cognitive impairment would be more prevalent in the older Hispanic community and would affect the levels of depression in both Hispanic and non-Hispanic Whites older adults. This cross-sectional study examined the association between cognitive impairment and depression in 157 older adults, including Hispanic (n = 91, 57.96%, age: M = 76.4, SD = 6.8) and non-Hispanic White (n = 66, 42.04%, age: M = 76.2, SD = 6.5). บทความฉบับนี้มีวัตถุประสงค์เพื่อนำเสนอบทบาทพยาบาลผู้สูงอายุในการส่งเสริมสุขภาพและป้องกันภาวะเปราะบางของผู้สูงอายุในคลินิกผู้สูงอายุ ภาวะเปราะบางเป็นกลุ่มอาการสูงอายุชนิดหนึ่งซึ่งเป็นผลของร่างกายเปลี่ยนจากสภาพที่แข็งแรงสู่ภาวะอ่อนแอ เป็นปัญหาสุขภาพสำคัญที่พบได้บ่อยในผู้สูงอายุ ส่งผลให้ผู้สูงอายุมีความสามารถในการทำกิจวัตรประจำวันลดลง ต้องพึ่งพาผู้อื่น รวมทั้งมีความเสี่ยงต่อการหกล้ม กระดูกหัก และอาจนำไปสู่ภาวะทุพพลภาพและเพิ่มอัตราการเสียชีวิต อย่างไรก็ตามภาวะเปราะบางมีสาเหตุการเกิดที่เกี่ยวข้องหลายปัจจัยร่วมกัน ซึ่งการป้องกันที่มีประสิทธิภาพจะต้องอาศัยการป้องกันที่หลากหลายทั้งด้านร่างกาย จิต และสังคม พยาบาลผู้สูงอายุเป็นหนึ่งในทีมสหวิชาชีพที่ช่วยดูแลสุขภาพผู้สูงอายุ ดังนั้นพยาบาลจึงมีบทบาทสำคัญในการส่งเสริมสุขภาพผู้สูงอายุและครอบครัวป้องกันภาวะเปราะบาง โดยบทบาทหลัก ประกอบด้วย 1) บทบาทผู้ประเมินภาวะสุขภาพ 2) บทบาทผู้จัดการการดูแล 3) บทบาทผู้สอนและให้คำแนะนำ 4) บทบาทผู้ให้คำปรึกษา และ 5) บทบาทผู้ประสานงาน ทั้งนี้การส่งเสริมสุขภาพและป้องกันภาวะเปราะบางจะช่วยชะลอภาวะพึ่งพิง นำมาซึ่งสุขภาวะที่ดี

Background: Post-code debriefs are shown to improve teamwork and communication, increase performance, and reduce burnout. According to the University of Kentucky HealthCare (UKHC) policy, teams are to perform a post-code debrief immediately after an in-hospital cardiac arrest (IHCA). However, the current policy does not provide guidance on what to include during a debrief session. At UK post-code debrief sessions are documented as occurring 60-72% of the time; however, personal observation and discussions with other bedside and rapid response nurses suggest that post-code debriefs are underperformed when compared to the documentation. Objective: The emergency department (ED) triage process for adult patients presenting with suspected cardiac abnormalities should be reliable; however, delays in identifying significant cardiac rhythms can negatively impact patient outcomes. The American Heart Association (AHA) and the American College of Cardiology (ACC) recommend that patients presenting to the ED with cardiac-related concerns undergo an electrocardiogram (EKG) within 10 minutes of arrival. Hospital-acquired pressure injuries (HAPIs), particularly sacral and heel injuries, remain a persistent challenge in critical care settings, leading to increased morbidity, mortality, and healthcare costs.
Purpose
This quality improvement project aimed to improve compliance with a pressure injury prevention (PIP) workflow by integrating a dual RN check-off system within EPIC, thereby reducing sacral and heel pressure injuries. Half of residents in nursing facilities fall annually, and one in every ten falls will lead to a severe injury. Background: Psychotropic medications are often inappropriately prescribed for behavioural and psychological symptoms of dementia (BPSD), posing significant risks such as falls, stroke and death. Although non-pharmacological interventions (NPIs) are the first-line treatment for BPSD, their use in practice remains limited. Objectives: This study explored general practitioners’ (GPs’) and physicians’ perspectives on using psychotropic medications compared to NPIs for managing BPSD in Australian residential aged care homes (RACHs). Background: Providing patient falls prevention education can help reduce falls in hospitals, yet research exploring staff perceptions about providing falls education in hospitals is limited. Objective: We sought to determine enablers and barriers to implementing a hospital falls prevention education program (the Safe Recovery Program) from the clinical staff perspective. Methods: Purposive sampling was used to recruit health professionals (N = 40) from 12 acute medical and surgical wards at a 450-bed hospital in Perth, Western Australia. Background:
Sleep is a crucial healing and restorative component of older person care but is often negatively impacted through the effects of institutionalization in residential aged care (RAC). Currently, information about how sleep occurs is limited.
Methods:
Through the lens of person-centered care, this study examined submissions to the Australian Royal Commission into Aged Care Quality and Safety. Aims: To explore the experience of registered nurses and their mentors in the implementation of the Gerontological Nursing Competencies in long-term aged care and the perceived effectiveness and suitability of the programme to support nurse development. Background: The global population is ageing and needs a reliable aged-care nursing workforce. Lack of meaning and purpose in life can increase disability, stress, mortality, and the risk of cognitive impairment. Finding meaning and purpose helps people with dementia adjust to the diagnosis and can be accomplished via expressions of identity such as life review, a formal process of systematically reviewing and reflecting on one’s lived life. Introduction: Healthcare student acceptance into graduate and professional school is based on Grade Point Average (GPA), Graduate Record Examination (GRE) scores, interviews, and letters of recommendation, but not on cognition levels. However, educators need to understand students’ cognitive levels to facilitate constructivist learning theory and inquiry-based learning. Introduction: Many widowhood cultural practices are detrimental to the health and well-being of widows in Nigeria. Little is known about the experiences of widowhood for Nigerian American women, their cultural practices, and how it affects their health.
Purpose: Understand widowhood cultural practices among Nigerian Americans and how these experiences may affect health and wellbeing.
Methods: The qualitative ethnonursing method was used to collect data from 25 Nigerian American widows. Data were analyzed using NVivo qualitative data manager software guided by Leininger’s four phases of qualitative analysis.

Nursing home residents with dementia diagnoses should have the opportunity to participate fully in community activities and receive the proper assistance and accommodations to do so. This project developed a tailored activity program to promote socialization, occupational justice, and quality of life for residents in an assistive living facility with various diagnoses. This program provides both environmental and activity adaptations to support full engagement for all participants regardless of any preexisting deficits.

Objective: Patient immobility during hospitalization contributes to avoidable complications including increased length of stay, pressure injuries, thromboembolic events, reduced functional capacity, and increased rates of discharge to rehabilitation facilities. The Bedside Mobility Assessment Tool (BMAT) was developed to empower Registered Nurses to safely assess and mobilize patients without a physical therapy evaluation. Aim: This quality improvement projects aims to enhance nurse compliance with BMAT documentation by 10% in three medical-surgical units of a 244-bed Northern California Hospital. With more than 439,000 annual U.S. deaths associated with childhood adversity, it is important to understand and prevent exposure to Adverse Childhood Events and Early Life Adversity. The purpose of this literature review is to identify and examine the pathophysiology between adverse childhood experiences (ACE’s) and their negative effects on health across the lifespan. Fifteen peer-reviewed articles that were published in notable journals were analyzed to include in this review. Problem: Simulation is an effective teaching strategy for undergraduate nursing students, providing a safe environment for learning in a low-risk setting. The coronavirus 2 (SARS-CoV-2) and the subsequent pandemic transformed the healthcare delivery system in the U.S. Telemedicine became an essential mode of healthcare, and its usage increased significantly during the public health emergency. Telemedicine improved access to care, especially for vulnerable patient populations and those with chronic medical conditions.
