Objective: This dissertation is made of two academic papers followed by a chapter discussing policy implications of the findings. The objective of this dissertation is to better understand if high-serious mental illness (SMI) nursing homes (NHs) provide poorer quality of care (e.g., operationalized as staffing levels, NH Compare quality star ratings, and health deficiencies) and whether quality in high-SMI NHs is confounded by payer mix (e.g., the proportion of residents who are Medicaid-reimbursed). A modified Donabedian Structure, Process, Outcome (SPO) Model was used as the main theoretical framework.

There is a growing literature of clinical recommendations for transgender and gender diverse (TGD) affirming behavioral health care, yet it is unknown to what extent these recommendations are rooted in evidence-based practice (EBP). This systematic review included 65 articles published between 2009 and 2018 with recommendations for behavioral health services with TGD adults, emphasizing general clinical care. Coded variables included type of article, participant demographics, aspects of EBP, and whether care was informed by objective assessment. Most articles did not equally draw from all components of EBP. Recommendations for specific clinical problems are increasingly available and address diversity within TGD …

Community Psychiatric Services for the Elderly (CPSE) at a large teaching hospital in Toronto has offered a monthly support group for caregivers of persons with dementia for over 30 years. Participants were surveyed in January 2018 about group attendance, benefits of attending the group, and areas for improvement. Results are shared in this case study. A total of 8 participants responded to the survey with a response rate of 47%. The feedback was predominantly positive. The most prevailing theme that emerged from the survey responses was group members’ appreciation for the opportunity to meet with one another to discuss relevant …

We present a literature review which summarizes the data supporting one of the alternative perspectives on the effect of alcohol consumption on cognitive aging – the possible positive effect of low to moderate drinking. Some of the main theories about aging, the mechanisms of brain aging, and the pathogenesis of cognitive decline and dementia are briefly described. In this context, the putative mechanisms of the protective action of non-alcoholic components in alcoholic beverages or low doses of ethanol against oxidative stress, inflammation, mitochondrial dysfunction, brain insulin resistance, and production of amyloid-β peptides are presented. Loneliness in older adults has increased during the COVID-19 pandemic, even though loneliness has been a prior public health concern for this population. Loneliness is a perceived lack of social interaction when an individual desires more (Wong, 2017). Abstract
The current study evaluated the relationship between social participation and quality of life (QoL) for older adults during the COVID-19 pandemic. Community-dwelling adults (N = 230) age 65 or older completed an online survey providing demographic data to identify changes in lifestyle routine during the pandemic using the COVID-19 Quality of Life (COV19-QoL) scores. Analyses revealed significant differences in reported lifestyle routines in relation to the onset of the COVID-19 pandemic and significant differences in COV19-QoL scores across various demographic groups. Amidst the COVID-19 pandemic, older adults are advised to remain in their homes and personal spaces since they are at greater risk for COVID-19 related illnesses, hospitalizations, and deaths. Health professionals and caregivers are concerned that preventative measures, especially social isolation, may have long-term psychological and age-related effects among older adults. In Kentucky, low levels of physical activity and social engagement negatively impact health. The concepts of ambiguous loss and perceived control will be used to explore ways to embrace the unknown during COVID-19. By defining COVID-19 as an ambiguous loss, effective therapeutic interventions emerge that can guide clinicians in creating lasting change amidst widespread uncertainty. Four ambiguous loss interventions will be proposed to alleviate the distress of living in fearful semi-isolation for an unknown period of time. While opioid prescribing rates have drawn researchers’ attention, little is known about the mechanisms through which income inequality affects opioid prescribing rates and even less focuses on whether there is a rural/urban difference in mediating pathways. Applying mediation analysis techniques to a unique ZIP code–level dataset from several sources maintained by the Centers for Medicare and Medicaid Services, we explicitly examine two mechanisms through residential stability and social isolation by rural/urban status and find that (1) income inequality is not directly related to opioid prescribing rates, but it exerts its influence on opioid prescribing via poor residential stability and elevated …

Purpose and Rationale
The aim of the evidence-based practice (EBP) project is to improve psychological outcomes for socially isolated older adults enrolled in a Homebound Program (HP) through the implementation of an EBP intervention. Social isolation has a multitude of negative consequences, including impacting the psychological health (e.g. depression, loneliness, anxiety) of the older adult population.
Synthesis of Evidence
Meta-analyses, systematic reviews, and randomized controlled trials addressing the clinical problem of social isolation were identified. Through critical appraisal of the evidence, physical activity, psychotherapy, and socialization interventions have shown to decrease depression and loneliness in older adults. There were mixed …

Hurricane María devastated Puerto Rico in 2017 and resulted in adverse long-term outcomes. Psychological sense of community (PSOC) may serve as a protective factor against the effects of Hurricane María for older adults in Puerto Rico. Using a three-paper format, this dissertation draws on a resilience framework and theories of PSOC and the Ecological Theory of Aging to examine the role of PSOC among older adults in Puerto Rico two years after Hurricane María.

Our systematic review asks the question: In ethnic minority elders with mental illness, how does culturally sensitive care, compared to current practices, affect patient outcomes? Our project goal is identifying evidence based practice with the goal of improving patient outcomes for ethnic minority elders with mental illnesses.
Methodology:
We will compile evidence from 20 current peer-reviewed studies. These studies will be selected from the CINAHL Plus med database, and key search terms will include: culturally sensitive care, culture, ethnic, mental health, intervention. Older adults with dementia experience difficulties throughout the progression of the disease, including decreased quality of life and mental health (Weise et al., 2018). This study seeks to demonstrate the intersecting structural and compounding challenges African American custodial grandparents experience collectively, rather than as separate non-competing factors, which has been modeled in prior studies. Using a mixed-method research design, the study explored the challenges faced by African American and white custodial grandparents. These challenges included difficulties attaining different types of support, respite care, and programs for teens and special needs grandchildren. Results showed that caregiving challenges among African Americans were more pervasive than their White counterparts. AbstractIneffective communication and behavioral management skills are quality and safety issues that were identified at a local adult day care facility in the Southwest United States. Administrators at the facility identified a knowledge gap in communication and behavior management techniques for patients with cognitive impairment. An educational program, with a PowerPoint presentation and verbal feedback as key components, was developed and used to address the identified gap in practice related to individuals with cognitive impairments at the facility. The Alzheimer's Association reported Alzheimer's as the sixth-leading cause of death in the United States, and a cure does not exist. Donepezil, a cholinesterase inhibitor drug, is frequently prescribed to treat Alzheimer's disease. Still, patients must continuously take the medication for years to receive any measurable improvement in quality of life after developing Alzheimer's Dementia. Village owned enterprises (BUMDes) are one of the places for entrepreneurial learning for rural communities. Likewise, what happened at BUMDes Serang Makmur Sejahtera in Serang Village, Karangreja District, Purbalingga Regency. Background and Objectives: Among the multiple factors posited to drive the health inequities that black men experience, the fundamental role of stress in the production of poor health is a key component. Allostatic load (AL) is considered to be a byproduct of stressors related to cumulative disadvantage. Exposure to chronic stress is associated with poorer mental health including depressive symptoms. Few studies have investigated how AL contributes to depressive symptoms among black men. The purpose of the cross-sectional study was to examine the association between AL and depressive symptoms among middle- to old age black men.

Few studies have explored self-efficacy training with persons with hearing loss (PHLs), yet alone with their communication partners (CPs). The purpose of this mixed-method study was to examine the impact of self-efficacy training as a framework for an Interprofessional Psychosocial Group Aural Rehabilitation (IPGAR) workshop with PHLs and their CPs. Four PHLs and their four CPs consented to participate in the IPGAR workshop that employed interventions including short lectures, psychosocial exercises, communication strategies training, speech perception training, adaptive/stress reduction exercises, and group discussions relevant to mutually established shared goals for each couple.
