Small Area Variations In Health Care Delivery, John E. Wennberg Md, Mph

Health information about total populations is a prerequisite for sound decision-making and planning in the health care field. Experience with a population-based health data system in Vermont reveals that there are wide variations in resource input, utilization of services, and expenditures among neighboring communities. Results show prima facie inequalities in the input of resources that are associated with income transfer from areas of lower expenditure to areas of higher expenditure. Variations in utilization indicate that there is considerable uncertainty about the effectiveness of different levels of aggregate, as well as specific kinds of, health services. The notion of cerebral dominance has been explored since the time of Broca. Two non-invasive techniques designed to assess hemispheric dominance have subsequently been developed. These two methods are commonly referred to as dichotic listening (simultaneous auditory stimulation) and rapid visual field stimulation (tachistoscopic viewing).

It has been found in dichotic listening studies that when verbal material is presented simultaneously to both ears, the stimuli are correctly identified most often in the right ear. Similar results have been found in tachistoscopic studies, with verbal material eliciting a right visual field advantage. Collectively, these findings have been used to support a …

A Comparison Of Aphasic And Non-Brain Injured Adults On A Dichotic Cv-Syllable Listening Task, Janet E. Shanks

Normal listeners traditionally show a right ear preference for dichotically presented speech stimuli, and a left ear preference for dichotically presented non-speech stimuli. Although some inter-subject variability is observed within and between groups of non-brain damaged subjects, the performance of the group as a whole is rather predictable and homogeneous. However, results from experiments in which brain damaged subjects have been studied have been less straightforward. Although left brain damaged subjects have consistently shown a bilateral deficit in reporting dichotic speech stimuli, a great deal of variability in performance is observed within the groups. Of nine patients with pericardial effusion due to various causes, four developed cardiac tamponade. Electrical alternans was present In all four, being total in three, and ventricular in one. From the diagnostic point of view, the alternans corresponded with the clinical diagnosis of cardiac tamponade and the radiological signs of a large pericardial effusion. The pericardial fluid was hemorrhagic in three patients and transudate (hydropericardium) in the fourth. In two patients alternans was present with heart rates below 100 per minute. This two-year study consisted of a comparison of oxandrolone and growth hormone for the treatment of five children with documented growth hormone deficiency. Previously, androgens have been reported to be relatively ineffective in accelerating linear growth In growth hormone deficient children. Oxandrolone was administered for one year. Growth hormone was added in the second six months and then was given as a single agent in the third six months. Growth accelerated markedly in all patients. Only one child showed more rapid growth with the addition of growth hormone while two children actually grew more rapidly under the Influence of oxandrolone …

The major objective of the work was to clarify some of the adaptive mechanisms that a common widespread species would show as a possible requirement for survival in its natural habitats. Analysis of various populations of Xanthium (cocklebur) indicated differences in chlorophyll levels and leaf chloroplasts ultrastructure between populations. The study also revealed some insight into intrapopulational variation in dark requirements for flowering of selected populations. In addition, caloric values of dried leaf material of cocklebur were analyzed for interpopulational differences. Efficacy Of Betaprone With Ultraviolet Irradiation On Hepatitis B Antigen In Human Plasma Pools (A Retrospective Study), Gerald A. Logrippo, Hajime Hayashi

In a retrospective study, hepatitis B antigen (HB Ag) was found in 14 lots of human plasma pools (16-20 liters each) that had been treated by the combined Betaprone (BPL) with ultraviolet (UV) irradiation procedure. No evidence of clinical hepatitis was found during a six-month follow-up after 185.2 liters of HB Ag positive plasma were Infused into 257 patients. Since HB Ag positive blood products treated by BPL with UV produced no clinical hepatitis, it is evident that our treatment is efficacious against hepatitis virus agents.

Surgical Treatment Of Liver Trauma: Technical Considerations And Case Reports, C. Grodsinsky, T. A. Fox Jr., B. E. Brush

The incidence of abdominal trauma, and thus that of liver trauma, has been increasing in the past few years. Improvements in the understanding of the anatomy of the liver and improvements in diagnostic techniques have resulted in shorter and better operations with less delay between injury and surgery. Improvements in transportation to adequate hospital facilities and vigorous resuscitative measures have also contributed to lowering the mortality rate for this type of injury. Suggestions are made for the surgical management of liver injuries, together with illustrative case reports.

Near Basal Blood Pressure: A Method Applicable To Clinical Practice, John R. Caldwell

A method is described for determining near basal blood pressure (NBBP) measurements that is applicable to outpatient clinic or office use in daily practice. In our clinic, patients' mean casual pressure was significantly higher than the NBBP, the difference being 40 mm Hg systolic (p < .001) and 18 mm Hg diastolic (p < .001). In hospitalized patients the casual blood pressure was higher than the fourth to sixth day hospital average, and the hospital average was higher than NBBP. The difference between the last two was 15 mm Hg systolic (p <. 005) and 9 mm Hg diastolic (p < .005). In patients recording blood pressure readings at home the average weekly home-recorded blood pressure was not significantly different from NBBP during the same week, -1 mm Hg systolic (p > .80) and 4 mm Hg diastolic (p > .10). NBBP by this method offers a practical way to estimate the average daily blood pressure.

The research described in the present paper is based upon a parsimonious analysis of behavior which utilizes principles that have been empirically determined. These principles as discussed by Ullmann and Krasner (1966) and Ulrich, Stachnik, and Mabry (1966) have suggested the wide applicability of behavior modification techniques, not only with animals, but with human subjects in diverse settings. One setting that has previously been left unexplored by proponents of behavior modification, which promises to be an area of growing concern, is that of dental hygiene.

This study was concerned with the problem of depression among elderly patients following surgery. The purpose of the study was to find out if older men following transurethral prostatectomy experienced a higher level of depression than normal elderly people in the general population. An attempt was made in the study to suggest factors in the patients' illness or environment which may have influenced the amount of depression they experienced.

Thirty patients, sixty-five years or older, were included in the study sample. Depression was measured by a self-rating depression scale (Zung) three weeks following their operations. Other facts about the patients …

Propagation of Tembusu virus was studied in LLC-MK2 cell culture. This culture supported the multiplilication of the virus but showed no CPE after infection (20 days observation). The virus formed clear plagues in LLC-MK2 cells when concentration of glutamine and amino acids were reduced. Vitamins were required for increasing numbers of plaques. The LD50 in suckling mice by intracerebral inoculation was equal to 0.6 PFU on the basis of plaque assay in MK2 cells. Study of effect of various concentrations of serum in the first overlay media on plaque formation showed that plaque formation in MK2 cells was best in …

High dietary levels of rapeseed oil containing high and low erucic acid and soybean oil were fed to ducks from one day of age through the early growing period. Clinical signs and mortality, hematology, gross and histopathologic changes were studied. The fallowing results were obtained : 1. Fatty infiltration occurred in the heart, skeletal muscles, spleen and kidney in ducks fed the high erucic acid rapeseed oil ration.
