Background and significance: It is estimated that 20% of nurses leave the profession within the first year due to an onset of burnout (Henson, 2020). Prior to the pandemic of 2020, workforce projected models estimated large-scale shortages in full time employee (FTE) registered nurses as demand continued to outpace supply (Lauer, 2022). Researchers estimate projections from 2024 into the year 2032 are now skewed, as pandemic-related contributors and their effects on burnout in the nursing profession have not been thoroughly assessed (Saravanan et al., 2022). A clinical trial of a multi-strain vaginal synbiotic (NCT05659745, registered 12/19/2022 at clinicaltrials.gov) led to an optimal vaginal microbiome dominated by L. crispatus (CST I). The synbiotic led to a significant increase in L. crispatus compared to placebo (p < 0.05), and conversion to CST I was significantly higher with the vaginal synbiotic than with placebo (90 vs 11%; p < 0.002). Mechanistically, the synbiotic reduced Gardnerella vaginalis and Candida, clinically important microbes.

Catheter-associated urinary tract infections (CAUTIs) contribute greatly to the burden of healthcare-associated infections. Acinetobacter baumannii is a Gram-negative bacterium with high levels of antibiotic resistance that is of increasing concern as a CAUTI pathogen. A. baumannii expresses fibrinogen-binding adhesins (Abp1D and Abp2D) that mediate biofilm formation on catheters, which become coated with fibrinogen upon insertion. Here we develop a protein subunit vaccine against the Abp1D and Abp2D receptor binding domains (RBD) and show that vaccination significantly reduces bacterial titers in a female mouse model of CAUTI. Cervical spinal chordomas and chondrosarcomas present significant therapeutic challenges due to their proximity to the spinal cord and resistance to conventional radiotherapy. This retrospective study investigated the capability of proton arc therapy (PAT) to maintain or improve adequate target coverage while reducing dose to the spinal cord for cervical spinal chordomas and chondrosarcomas over standard fixed beam intensity modulated proton therapy (IMPT). While both IMPT and PAT use pencil beam scanning (PBS) to with energy modulation to deliver dose in discrete layers, IMPT relies on a limited number of fixed beam angles, whereas PAT delivers dose over an arc trajectory. …

Purpose: Given the identified importance of assessing the spiritual domain of health as a component of cultural competence and holistic care in physical therapy and athletic training and the importance that their governing bodies have indicated that spiritual care should play in treating patients, this current study examined how faculty, at entry-level Doctor of Physical Therapy (DPT) and Master of Athletic Training (MAT) programs in Texas, perceive the importance and inclusion of spiritual dimensions of care in their programs. Background: Vaginismus is a condition characterized by involuntary pelvic floor muscle contractions, which can make vaginal penetration painful or impossible. This disorder has both physical and psychological components, often leading to significant emotional distress, anxiety, and relationship difficulties. Effective management requires a holistic, multidisciplinary approach, combining physical therapy with psychological support. Purpose
Non-small cell lung cancer (NSCLC) patients benefit by starting radiation therapy (RT) promptly upon diagnosis. Diagnostic scan-based radiation therapy planning (DBRP) eliminates the need for computed tomography (CT) simulation. This is a retrospective study comparing dosimetric differences between volumetric arc therapy (VMAT) diagnostic scan-based plans recalculated onto simulation CT scans.
Methods
Radiotherapy (60 or 66 Gy in 20-30 fractions) for eight NSCLC patients was planned on diagnostic CTs. The plans were recalculated onto the fused simulation CTs, mimicking first treatment setups. This retrospective planning study compared dosimetric outcomes and treatment efficiency between flattening filter free (FFF) and standard volumetric modulated arc therapy (VMAT) plans for left breast accelerated partial breast irradiation (APBI) under deep inspiration breath hold (DIBH) and free-breathing (FB) conditions. Ten patients with early stage left breast cancer were replanned to create four plans each: FFF VMAT and standard VMAT in DIBH and FB. All plans prescribed 30 Gy in 5 fractions. Knowledge-based planning (KBP) using RapidPlan (RP) has demonstrated the ability to produce high quality radiation therapy treatment plans while reducing time. This study evaluated whether the inclusion of department specific standardized planning structures, including rings and other optimization tools, improves plan quality when using a prostate and lymph node RP model, or if the model alone is sufficient to provide a quality plan without the use of these additional planning structures.

The blood-brain barrier (BBB), formed by specialized endothelial cells (ECs), regulates the extracellular composition of the central nervous system (CNS). Little is known about whether there are regional specializations of the BBB that may control the function of specific neural circuits. We use single cell RNA-seq to characterize ECs from nine CNS regions in male mice: cortex, hippocampus, cerebellum, spinal cord, striatum, thalamus, hypothalamus, midbrain, and medulla/pons. Although there is a core BBB transcriptional profile, there are significant regional specializations. S100β is a significant signaling molecule and biomarker that is primarily expressed in the brain. At low physiological concentrations, S100β induces astrocyte maturation, microglial migration, and neural proliferation. However, high concentrations activate inflammatory and pro-apoptotic pathways. Due to this dual role, increased research is being invested into the role of S100β in neuronal homeostasis and inflammation. In fact, increased S100β expression is seen in many neuropathologies including Alzheimer’s disease, Parkinson’s disease, cerebral ischemia, and traumatic brain injury. MHC class I polypeptide-related sequence B (MICB) is a ligand for NKG2D. We have shown NK cells are central to lung transplant acute lung injury (ALI) via NKG2D activation, and increased MICB in bronchoalveolar lavage predicts ALI severity. Separately, we found a MICB polymorphism (MICBG406A) is associated with decreased ALI risk. We hypothesized this polymorphism would protect against severe SARS-CoV-2 respiratory disease. We analyzed 1,036 patients hospitalized with SARS-CoV-2 infection from IMPACC. Associations between MICBG406A and outcomes were determined by linear regression or Cox proportional hazards models. Fiber’s effect on our gut microbes has been studied extensively, while knowledge of the effect of protein is sparse. Colonic putrefaction of dietary protein is known to produce several potentially detrimental compounds. This randomized controlled trial (RCT) uses a pre- and post-study design to investigate the effect of consuming higher levels of dietary protein on the gut microbiota and the metabolites it produces. Pseudolysin (PLN) protein is a major virulent factor in Pseudomonas aeruginosa infection. PLN belongs to the family of zinc-dependent metalloproteases. It is naturally inhibited by a smaller protein called streptomyces metalloprotease inhibitor (SMPI), secreted by Streptomyces nigrescens. Previous and current studies demonstrate that the interaction between PLN and SMPI offers unique mechanistic insights for drug development against infections caused by P. aeruginosa. This study investigates a dual-acting drug delivery system using naringenin (NAR) as a folate receptor ligand to enhance intestinal uptake and encapsulated NAR for combating inflammation. The dual-acting systems were tested in vitro on cisplatin-induced human kidney-2 cells and in vivo in a mouse model of cisplatin-induced acute kidney injury (AKI). NAR-loaded passive nanoparticles [P2Ns(NAR)] and dual-acting systems [P2Ns-NAR(NAR)] showed notable advantages over unformulated NAR, reducing the required dose by up to 57 and 79%, respectively. These nanoparticles modulated immune responses, restored T cell function, and shifted macrophage polarization from proinflammatory M1 to tissue-repairing M2. The mechanosensitive channel of small conductance (MscS) from E. coli (EcMscS) has served as the prevailing model system for understanding mechanotransduction in ion channels. Trypanosoma cruzi, the protozoan parasite causing Chagas disease, encodes a miniature MscS ortholog (TcMscS) critical for parasite development and infectivity. TcMscS contains a minimal portion of the canonical EcMscS fold yet maintains mechanosensitive channel activity, thus presenting a unique model system to assess the essential molecular determinants underlying mechanotransduction. Using cryo-electron microscopy and molecular dynamics simulations, we show that TcMscS contains two short membrane-embedded helices that would not fully cross an intact lipid bilayer. IL-33 is a key driver of type 2 inflammation and implicated in pathology of chronic obstructive pulmonary disease (COPD) and asthma. However, the mechanism for IL-33 secretion and regulation in the context of chronic airway disease is poorly understood. We previously reported an airway disease-associated isoform IL-33Δ34 that escapes nuclear sequestration and is tonically secreted from epithelial cells. Here, we describe how this IL-33Δ34 isoform interacts with HSP70 within cells and is targeted to secretory organelles through coordinated binding to phosphatidylserine (PS) and delivered to compartments for unconventional protein secretion (CUPS). temporomandibular joint (TMJ). Signs of OA were then correlated to gross alterations of the aorta and common carotid arteries seen with atherosclerotic cardiovascular disease. This study aimed to investigate the prevalence of porcine reproductive and respiratory syndrome virus (PRRSV), porcine parvovirus (PPV), and porcine circovirus type 2 (PCV2) infections associated with fetal losses in 10 commercial swine herds in Thailand. Samples were collected from 100 sows and included 25 aborted fetuses, 35 mummified fetuses, and 40 stillborn piglets. The average parity number of the sows was 2.8 ± 2.4, ranging from 0 to 11. Background: Firearm violence is a leading cause of death among U.S. children under age 18. Urban vegetation, a vital component of the living environment, has been found to influence firearm violence, yet the relationship between different types of street vegetation and firearm violence with children as the victim at the microgeographic scale remains unclear.
Methods: We analyzed 2316 child firearm assault incidents from Los Angeles, New York City, and Philadelphia from 2015 to 2021. Using a spatial case-control approach, we matched assault locations with control locations within 1-km. For each location, we applied deep learning algorithms to Google Street View …
