We describe how the federal government, the City of Chicago, and the State of Illinois worked together to increase the availability and accessibility of health care services on Chicago's underserved west side by reopening a bankrupt, federally-funded community health center. The federal government made the building available to the City which then contracted with a state university medical center to be the provider of services. This partnering has allowed the Chicago Department of Health to offer services in a previously underserved area. The University has gained an opportunity for community-based primary care teaching, as well as community relations. Patients have …

Data on the epidemiology of prostate cancer from the 1930s to the present document a dramatic racial difference in incidence, survival, and mortality rates in American men. American black men have the highest incidence and mortality rates of prostate cancer in the world. Survival data have been related to access to medical care, genetic and environmental factors, and cultural differences, including diet and social habits. Most reports present conflicting data with no clear positive correlations, and conclusions are often speculative. Better controlled, prospective studies of epidemiologic variables and a comprehensive genetic evaluation of black families with prostate cancer are needed …

Epidermoid cancer of the head and neck occurs most frequently in individuals with predisposing factors for neoplastic development. Primarily these include exposure to smoking and alcohol, although other predisposing factors may he present. Site-speciflc symptoms along with a history of predisposing environmental exposure can direct physicians to suspect head and neck cancer. Endoscopy and ancillary testing are performed for tumor staging and subsequent treatment planning.

Over a 4 year period, nine of 180 (5%) infants weighing less than 2000 G, admitted to the Aga Khan University Hospital (AKUH) developed necrotizing enterocolitis (NEC). An outbreak of NEC occurred in 1989, during which six infants developed the clinical illness. Overall incidence was 1.1%. Thirty-one birth weight and gestation matched controls were selected for comparison. Risk factors usually considered as predisposing factors, i.e., low 5 min Apgar score, rate of maternal complications, respiratory distress syndrome, mechanical ventilation, umbilical catheterisation, patient ductus arteriosus, use of antibiotics and feeding practices were found with equal frequency in both cases and controls. …

By the year 2000, Latinos will become the largest minority group in the United States. Acquired Immunodeficiency Syndrome (AIDS) is occurring at a disproportionately high rate among Latinos. As care-giver and health decision-maker in the family, the Latina needs to know about AIDS and AIDS prevention. This research identified and offers an explanation of how the Latina comes to know about AIDS and AIDS prevention within the context of the family and the Latino culture. Grounded theory methodology was used to explore the complex socialization patterns within the Latino community that affect how the Latina perceives HIV/AIDS. Data were collected …

Age-adjusted female hip fracture incidence has been noted to be higher in industrialized countries than non-industrialized countries. A possible explanation which has received little attention is that elevated metabolic acid production associated with a high animal protein diet might lead to chronic bone buffering and bone dissolution. In an attempt to examine this hypothesis, cross-cultural variations in animal protein consumption and hip fracture incidence were studied. When female fracture rates derived from 34 published studies in 16 countries were regressed against estimates of dietary animal protein, a strong, positive association was found. This association could not plausibly be explained by …

Murder, She Wrote Or Was It Merely Selective Nontreatment?, George P. Smith Ii
Scholarly Articles
This article will both explore and thereby establish the medical, ethical, and legal validity of selective nontreatment of severely handicapped newborns. A construct for principled decision-making, tied to a basic recognition of the right of self-determination, as shaped by compassion and validated principles of triage and cost-benefit analysis, will be seen as the most effective means for the states-and not the federal government-to evaluate the intensely complex issues associated with allocating scarce medical resources to defective infants. Governmental intrusions into the familial decision- making forum in these circumstances must be kept to a minimum and allowed only in grave cases.

Intrauterine devices are the most widely used of all reversible contraceptive methods worldwide. Modern IUDs are among the most effective and long-acting methods of family planning and are acceptable options for many women. This monograph presents highlights of the clinical performance of the Copper T 380 over eight years, including the latest data on effectiveness, expulsions, and continuation rates. It incorporates data from Population Council and World Health Organization studies, and work by scientists at Family Health International and in several countries. The purpose of this independent study is a literature review of the principles of training and a critique of the shoulder strength training programs for increasing the throwing velocity of the baseball pitcher. In addition, a sport specific movement analysis, based on biomechanics, physiology and kinesiology of the baseball overhand pitch will be examined. The critique will be based on the principles of training reviewed earlier and a sport specific movement analysis. Finally, a proposal of a potential training program and possible topics for research will be discussed.

The studies in this issue of Quality/Calidad/Qualité describe three efforts to involve women in the development of reproductive health care materials in different settings in the developing world. In Cairo, a women's health collective produced a comprehensive reference book for women; in Peru, a woman's group, with the extensive involvement of their nonliterate audience, developed a series of illustrated teaching materials; and in the South Pacific, an all-woman production crew produced three motivation and teaching videos developed in response to the expressed needs of Pacific Island women. Parasitological studies of archaeological sites can be used to interpret past behavior and living conditions. During the 1980s problem-oriented research into prehistoric- and historical-period parasitism developed and resulted in thefield of archaeoparasitology. Archaeoparasitology attempts to integrate parasite data into archaeological theory and interpretation. Within the last decade, four major archaeoparasitological laboratories emerged. They developed interpretive frameworks that apply parasitological data to a remarkable variety of prehistoric behaviors. Parasite remains can be used to reconstruct aspects of diet, health, and other behaviors such as transhumance and trade. Finally. analysis of the distribution of parasite remains can be used to interpret aspects …

Trypanosome Infection Rates In Tsetse Flies (Diptera: Glossinidae) And Cattle During Tsetse Control Operations In The Kagera River Region Of Rwanda, Steve Mihok, Leonard H. Otieno, Christopher S. Tarimo

Trypanosome infections were monitored in three species of tsetse fly (Glossina pallidipes Austen, G. morsitans centralis Machado, and G. brevipalpis News-tead) at four locations in the Kagera River region of Rwanda from May 1989 to September 1990. Two of the four areas (Mpanga Ranch and Bukora Ranch) were subjected to tsetse fly suppression operations with odour-baited traps. Proboscis infections of the Trypanosoma congolense and T. vivax types accounted for roughly equal numbers of the 207 mature infections detected (3.8%). Variation in infection rates was area-specific rather than tsetse species-specific. Order of magnitude differences in tsetse fly densities among areas were …
