Integrase strand transfer inhibitors have transformed human immunodeficiency virus (HIV) therapy, yet the widely prescribed drug dolutegravir (DTG) has been linked to developmental toxicity, and its teratogenic mechanism remains unclear. Here, we used zebrafish to dissect DTG toxicity during early vertebrate development. DTG exposure from 2-4 h post-fertilization (hpf) to 24 hpf produced high mortality and abnormal morphology. Co-treatment with folates partially restored normal morphology, whereas calcium had no effect. Strikingly, supplementation with magnesium (Mg) partially rescued DTG-exposed embryos, implicating Mg availability in protection. In competitive binding assays, Mg increased binding of folate to purified folate receptor (FOLR1) by 30% …

Congenital aromatase deficiency is an autosomal recessive CYP19A1 loss-of-function disorder that blocks conversion of androgens to estrogens. A man with homozygous CYP19A1 c.628G > A (exon 5) presented with longstanding fatigue, low libido, and desire to maintain fertility. Physiologic estrogen replacement was started with transdermal estradiol 0.0375 mg. After three months of estrogen therapy, the patient reported markedly improved energy and libido. Follow-up evaluation showed a rise in serum estradiol with normalization of gonadotropins, while maintaining spermatogenesis. This case illustrates the essential role of estrogen in male physiology, sexual function, and fertility. Estradiol replacement can alleviate hypoestrogenic symptoms with preservation of …

Background: Pediatric acute lymphoblastic leukemia (ALL) is the most common cancer in children, and its incidence and outcomes vary by race and ethnicity.
Methods: The Reducing Ethnic Disparities in Acute Leukemia (REDIAL) Consortium has assembled a cohort of patients < 25 years at diagnosis to examine genetic and neighborhood-level factors influencing ALL. As the cohort is ongoing, for this report, we examined sociodemographic and cytogenetic factors that might be associated with ALL outcome disparities.
Results: Among 2,512 patients diagnosed between 2005 and 2020, we observed distinct patterns of genetic ancestry and residential characteristics across self-reported race and ethnicity. Latino children, who comprised 55.3% of the cohort, had a mean Amerindigenous ancestry proportion of 50.9%, whereas non-Latino White children exhibited a predominantly European ancestry (mean = 78%). Neighborhood-level analysis revealed significant socioeconomic and geographic differences by …

Bergsonian Blend: Subjective Temporality Within Modernist Aesthetics Of Mental Illness, Lydia Victoria Gervais
Electronic Theses and Dissertations
Henri Bergson was a modernist French philosopher who wrote extensively on the topic of the lived experience of time, which he called durée, or duration. His contemporary, Sigmund Freud, was developing psychoanalytic theory in a growing field of psychological study, which serves as a prominent ancestor to modern psychiatry and current psychoanalytical literary studies. Fiction writers of the 1900s absorbed the zeitgeist of melded chronology and consciousness, interweaving themes of time and trauma in their works. Utilizing Henri Bergson’s durée as a philosophical framework, I examine the moralization of time-consciousness in connection with mental illness and investigate representations of …
Addressing Substance Use Disorder In Rural Appalachia: Improving Access And Treatment Outcomes For Justice-Involved Individuals, Jessica Muldowney
Addressing Substance Use Disorder In Rural Appalachia: Improving Access And Treatment Outcomes For Justice-Involved Individuals, Jessica Muldowney
Electronic Theses and Dissertations
Background: Opioid use disorder (OUD) represents a critical public health crisis disproportionately affecting justice-involved (JI) populations, with approximately 65% of incarcerated individuals having substance use disorders (James & Beatty, 2006). Rural Appalachian communities face compounded challenges due to limited healthcare access, provider shortages, and cultural stigma surrounding addiction treatment. Despite evidence demonstrating that medication-assisted treatment (MAT) reduces recidivism and overdose deaths, only 20% of correctional facilities offer these services (Klein & Seppala, 2019).
Objective: This mixed-methods study evaluated organizational readiness and infrastructure capacity for implementing evidence-based OUD interventions, specifically MAT programs across three rural counties in northwestern North Carolina.
Methods: …

Short-term mission (STM) trips are limited duration service experiences, commonly organized by faith-based organizations (FBO), in which volunteers travel domestically or international to provide ministry, community outreach, and/or health-related services. In recent decades, the number of these trips organized by organizations in the United States has increased exponentially, but the phenomenon is difficult to accurately measure due to its decentralized nature and the lack of standardized terminology. Despite their prominence, limited research has been conducted to examine planning, implementation, and evaluation processes of international STMs organized by FBO, particularly through a public health lens. The purpose of this study was …

Ucva Ontology: Standardizing Local Context Factors To Support The Analysis Of Unwarranted Clinical Variation, Cui Tao, Susan Fenton
Faculty, Staff and Student Publications
Objectives: Unwarranted clinical variation (UCV), defined as care provided to a patient that is not proportional to the patient's needs, clinical characteristics, or preferences, results in negative patient outcomes. UCV detection is performed to compare healthcare outcomes across regions or organizations and reduce UCV incidences. However, the lack of standardized UCV concepts and the local context factors hinder the interoperability of UCV analyses between organizations. Currently, UCV analysis relies on centralized analysis, posing challenges due to the proprietary nature of an organization's healthcare and operational data. Standardizing analysis factors enables interoperable analyses by establishing a common context between organizations. This …

Purpose: Growth modulation is an established technique for limb deformity correction and is increasingly applied to spinal deformities. While distraction-based posterior and anterior compressive methods have been explored, spinal growth modulation through fixation across vertebral growth centers remains unstudied. We hypothesized that unilateral trans-endplate screws-spinal epiphysiodesis trajectory (SET) screws-could induce partial anterior growth arrest and promote scoliotic deformity in a porcine model.
Methods: Four male piglets (two experimental, two control) underwent unilateral posterior spinal instrumentation at four lower thoracic levels at eight weeks of age. Experimental animals received trans-endplate SET screws; controls received pedicle screws. Radiographs obtained three months postoperatively …
Personality Characteristics Of Emergency Nurses And Their Subjective Fit In The Emergency Department, Jose Michael Maria
Personality Characteristics Of Emergency Nurses And Their Subjective Fit In The Emergency Department, Jose Michael Maria
Theses & Dissertations
Nursing workforce challenges in the United States persist despite widespread recruitment and retention efforts, and the COVID-19 pandemic has further exacerbated shortages, turnover, and workforce instability. Despite interventions such as residency programs, financial incentives, and organizational initiatives, long-term retention remains inconsistent, particularly in high-demand settings like emergency departments. Ongoing issues such as overcrowding, rising patient acuity, and workforce attrition contribute to burnout, compassion fatigue, and compromised patient outcomes, perpetuating a cycle of staffing strain. Grounded in Person–Environment Fit theory, this research proposes that aligning emergency nurses’ personality traits with the specialty practice environment, the emergency department, may offer a more …
Caring For Transgender People In Healthcare: A Qualitative Descriptive Study With Nurses Working In An Acute Healthcare Setting, Gabriela Roxana Matson
Caring For Transgender People In Healthcare: A Qualitative Descriptive Study With Nurses Working In An Acute Healthcare Setting, Gabriela Roxana Matson
Theses & Dissertations
Transgender individuals frequently encounter numerous obstacles when accessing healthcare services, including stigma, discrimination, and inadequate understanding from healthcare providers. Structural, interpersonal, and anticipation barriers significantly hinder transgender patients' access to care. Structural barriers include a lack of healthcare providers knowledgeable in transgender health and financial constraints. Interpersonal barriers involve discrimination, such as refusal to use correct pronouns or provide gender-affirming care. Anticipation barriers stem from fear of mistreatment, causing many to avoid seeking medical care altogether. Nurses play a vital role in upholding inclusive and affirming healthcare This qualitative descriptive study explored the subjective experiences of registered nurses who provided …

Research outputs 2022 to 2026
Aim: To synthesize qualitative evidence on the experiences of undergraduate nursing students and new graduate nurses engaged in formal paid employment models or health-related work during their undergraduate studies, to examine how these employment-based clinical experiences influence their transition to nursing practice, and to identify elements that facilitate or hinder this transition. Design: Qualitative systematic review. Methods: Searches were conducted in MEDLINE, Embase, Emcare (Ovid), CINAHL Complete (EBSCOhost), ERIC (ProQuest), and gray literature sources, including ProQuest Dissertations and Theses and Google Scholar. No date or country limits were applied, although only English-language studies were included. The review followed JBI methodology …

Ascaris infection-ascariasis-is the most common human parasitic worm infection worldwide and induces damage and cellular changes within the stomach. Helicobacter pylori is a bacterium of the stomach that also causes gastric pathologies globally. Importantly, Ascaris, H. pylori, and gastric diseases all disproportionately affect individuals in resource-limited settings. However, the associations between ascariasis, H. pylori infection, and gastric diseases remain relatively unexplored. We synthesized the existing research associating Ascaris infection with H. pylori infection and/or gastric pathologies. Records were identified in OVID Medline, Embase and Web of Science using search terms for ascariasis, H. pylori infection, and gastric abnormalities and additional …
Factors Influencing Patient Recall For Follow-Up In The Multi-Stage Deployment Of Medical Intelligent Screening Systems, Wenjie Chen
Dissertations and Theses Collection (Open Access)
With the deep integration of artificial intelligence (AI) technology in the healthcare sector, intelligent screening systems have achieved significant breakthroughs in predictive accuracy; however, they commonly face a management challenge in clinical implementation: accurate early warnings but limited patient recall. To address this ineffective technological empowerment issue in practice, this study proposes a theoretical framework featuringbidirectional interactions between "technological empowerment" and"relational empowerment" based on empowerment theory to systematicallyinvestigate the dynamic evolution mechanisms and boundary conditions of patient recall willingness under multi-stage, cross-departmental deployment of medical intelligent screening systems. Using a leading municipal hospital as the primary experimental setting, the study …
