Radical Surgery In The Treatment Of Localized Carcinoma Of The Prostate, Dinesh J. Telang, Brian J. Miles, Riad N. Farah, Ray H. Littleton, Aaron K. Kirkemo, James O. Peabody, David A. Burks, Caleb Fleming, Joseph C. Cerny

Henry Ford Hospital Medical Journal

New methods of early detection combined with recent advances in surgical techniques have resulted in more patients undergoing radical surgery for treatment of localized carcinoma of the prostate. Over 350 radical prostatectomies have been performed by our group since January 1987. We review the role of radical prostatectomy in the treatment of prostate cancer and our experience with 100 patients undergoing radical retropubic prostatectomy since the advent of nerve-sparing techniques to preserve potency.

Maximal Androgen Ablation: A Review, Brian J. Miles, Joseph Babiarz Mar 1992

Maximal Androgen Ablation: A Review, Brian J. Miles, Joseph Babiarz

Henry Ford Hospital Medical Journal

Primary management of advanced (stage D) adenocarcinoma of the prostate is androgen ablation. Since this principle was discovered in the early 1940s, therapeutic alternatives and "progress" have centered around different ways to obtain castrate levels of androgens. The role of adrenal androgens in supporting prostate or prostatic cancer growth has been debated for decades and until recently was believed to be minimal. In the 1980s the concept of maximum androgen suppression, involving both the testes and adrenal glands, was reintroduced with some investigators claiming exceptional results. We review studies that have examined this concept, with emphasis on the largest trial …

Megakaryoblastic Termination Of Myeloproliferative Disorders, Mahul B. Amin, Koichi Maeda, John L. Carey, Ramesh V. Babu, B. K. S. Raman

Henry Ford Hospital Medical Journal

Megakaryoblastic termination of myeloproliferative disorders is rare. The morphology of megakaryoblastic transformation can be subtle and is often mistaken for myeloid or lymphoid proliferations. Previously reported observations suggest a relatively poor prognosis for this category of patients, making precise diagnosis imperative. A multifaceted approach using morphology, ultrastructure, cytochemistry, and immunological membrane analysis may be helpful. We present two cases of myeloproliferative disorder with aggressive megakaryoblastic phases (myelofibrosis with agnogenic myeloid metaplasia and chronic myeloid leukemia with blast crisis). The clinical course is described and the results of the morphological, cytochemical, ultrastructural, and cytogenetic studies of both cases are presented. In …

Cerebral Oxygen Extraction During Severe Viral Encephalitis, Mark G. Goetting, Marwan L. Haddad Mar 1992

Cerebral Oxygen Extraction During Severe Viral Encephalitis, Mark G. Goetting, Marwan L. Haddad

Henry Ford Hospital Medical Journal

Viral encephalitis can cause devastating neurologic injury. Little is known about cerebral hemodynamics and metabolism in this condition. We report two patients with severe encephalitis, one proven and the other suspected to be due to herpes simplex, in whom the global cerebral oxygen extraction ratio (OER) and carbon dioxide (CO2) responsiveness was assessed. OER was low in both patients throughout the acute period. CO2 responsiveness was present initially in both and disappeared later in the more severely affected child. These cases demonstrate that cerebral hyperemia occurs in severe viral encephalitis and that hyperventilation can effectively reduce the intracranial pressure.

The Emerging Role Of Htlv-I/Ii And Hiv-1 Among Intravenous Drug Users In Detroit, Anthony F. Ognjan, Chris A. Lewandowski, Bradley T. Belian, John Burczak, Norman Markowitz, Helen Lee, Louis D. Saravolatz

Henry Ford Hospital Medical Journal

During 1987-1988, a seroprevalence study of the human immunodeficiency virus (HIV-1) and the human T-cell lymphoma/leukemia virus (HTLV-l/II) was performed among Detroit intravenous drug users unaffiliated with substance abuse programs. Seroprevalence data along with patient demographic information were compared to a similar study performed in 1985-1986. In the earlier study, 12 (12.5%) of 96 individuals tested positive for HIV-1. Of the 74 available negative samples retested in 1987-1988 for retroviruses, 7 (9.5%) tested positive for HTLV-I/II. Thus, the overall retroviral (HIV-1, HTLV-I/II) seropositive rate for 1985-1986 was 22%. In 1987-1988, 11 (15.7%) of 70 individuals tested positive for HIV-1 and …

Interpretation Of The Pattern In Rate Ratios Across Strata, Paul F. Visintainer, Suzanne Havstad Mar 1992

Interpretation Of The Pattern In Rate Ratios Across Strata, Paul F. Visintainer, Suzanne Havstad

Henry Ford Hospital Medical Journal

The pattern in the ratio of disease rates over strata is a summary statistic used to describe the changing risk of disease in one group relative to another. While patterns of the ratios of disease rates over strata appear to correspond to specific changes in disease rates, plots of the disease rates over strata seem to contradict the information yielded by the ratios. For example, if disease rates from populations A and B have identical rates of decline (parallel lines), the difference in the rates (A - B) at each strata remains constant, while the ratio of the rates (AIB) …

Presentation And Evaluation Of Patients With Epidermoid Head And Neck Cancers, Michael S. Benninger Mar 1992

Presentation And Evaluation Of Patients With Epidermoid Head And Neck Cancers, Michael S. Benninger

Henry Ford Hospital Medical Journal

Epidermoid cancer of the head and neck occurs most frequently in individuals with predisposing factors for neoplastic development. Primarily these include exposure to smoking and alcohol, although other predisposing factors may he present. Site-speciflc symptoms along with a history of predisposing environmental exposure can direct physicians to suspect head and neck cancer. Endoscopy and ancillary testing are performed for tumor staging and subsequent treatment planning.

A Comparison Of Standard-Dose And High-Dose Epinephrine In Cardiac Arrest Outside The Hospital, Charles G. Brown, Daniel R. Martin, Paul E. Pepe, Harlan Stueven, Richard O. Commins, Edgar Gonzalez, Michael Jastremski, The Multicenter High-Dose Epinephrine Study Group

Emergency Medicine Publications

BACKGROUND.

Experimental and uncontrolled clinical evidence suggests that intravenous epinephrine in doses higher than currently recommended may improve outcome after cardiac arrest. We conducted a prospective, multicenter study comparing standard-dose epinephrine with high-dose epinephrine in the management of cardiac arrest outside the hospital.

METHODS.

Adult patients were enrolled in the study if they remained in ventricular fibrillation, or if they had asystole or electromechanical dissociation, at the time the first drug was to be administered to treat the cardiac arrest. Patients were randomly assigned to receive either 0.02 mg of epinephrine per kilogram of body weight (standard-dose group, 632 patients) …

Elder-Caregiving Among University Employees: Responsibilities And Needs, Constance L. Coogle, Edward F. Ansello Jan 1992

Elder-Caregiving Among University Employees: Responsibilities And Needs, Constance L. Coogle, Edward F. Ansello

Virginia Center on Aging Publications

Although many institutions of higher education often house the researchers who study eldercare, few have documented the need for information and services among their estimated 460,000 employees who face the difficult task of effectively balancing work and eldercare responsibilities. Even fewer have established programs to address this need.

Drs. Constance Coogle and Edward Ansello have recently completed an exhaustive survey of VCU employees (N = 11,430) to determine the extent to which they provide care to disabled parents or spouses.[1] A longer, more detailed survey was sent to those who indicated that they were eldercaregivers to measure their perceived …

Finding Hiv-Infected Women - The Clinician's Role, Mary Guinan Jan 1992

Finding Hiv-Infected Women - The Clinician's Role, Mary Guinan

School of Public Health Faculty Research

An estimated 100,000 women are currently infected with the human immunodeficiency virus (HIV) in the United States, and a great majority of them are unaware of their condition. Approximately 20,000 HIV-infected women were identified through publicly funded HIV screening programs in 1989 and 1990, and an unknown number through private screening. Because most HIV-infected women are believed to be in the lower socioeconomic strata, it is unlikely that a significant number were identified in the private sector. Therefore, up to 80% of HIV-positive women may not know they are infected.

Insulin-Like Growth Factor Ii Acts Through An Endogenous Growth Pathway Regulated By Imprinting In Early Mouse Embryos, Daniel A. Rappolee, Karin S. Sturm, Ole Behrendtsen, Gilbert A. Schultz, Roger A. Pedersen, Zena Werb Jan 1992

Pedersen, Zena Werb

Department of Obstetrics and Gynecology Research Publications

We present evidence that insulin-like growth factor II (IGF-II) mediates growth in early mouse embryos and forms a pathway in which imprinted genes influence development during preimplantation stages, mRNA and protein for IGF-II were expressed in preimplantation mouse embryos, but the related factors IGF-I and insulin were not. IGF-I and insulin receptors and the IGF-II/mannose-6-phosphate receptor were expressed. Exogenous IGF-II or IGF-I increased the cell number in cultured blastocysts, but a mutant form of IGF-II that strongly binds only the IGF-II receptor did not. Reduction of IGF-II expression by antisense IGF-II oligonucleotides decreased the rate of progression to the blastocyst …

Prevalence Of Iron Deficiency Anaemia In Children Of The Urban Slums Of Karachi, Ayesha Molla, Mohammad Khurshid, A. Majid Molla

Department of Pathology and Laboratory Medicine

A preliminary baseline survey was conducted to estimate the prevalence of anaemia in a group of 391 children aged 6-60 months, randomly selected from three urban slums of Karachi. Haemoglobin and the red cell indices including haematocrit, MCV, MCH, MCHC, ABC and red cell distribution width (RDW) were estimated for each of the selected children. Ferritin estimation was done on 354 (9 1%) children to assess the iron storage status. According to WHO criteria, the accepted cut-off point for anaemia screening in children is set at 11 gm/dl, 70 fI and 20 pg for haemoglobin, MCV and MCH respectively. Following …

The purpose of this study was to determine women's: current knowledge of the menopause and of the disease osteoporosis; attitudes to estrogen replacement therapy (ERT); frequency of menopausal symptoms; knowledge-seeking behaviours In relation to the menopause; and, open-ended responses to menopausal issues. This study surveyed 70 women who responded to an advertisement in Community Newspapers. Data were collected using a "Knowledge and attitudes to the menopause and health related issues" (KAMHRI) questionnaire. The KAMHRI questionnaire was developed by the researcher and …

The Use Of Self Blood Glucose Monitoring And Its Relationship To Effective Management For People With Insulin Dependent Diabetes Mellitus, Maureen Unsworth Jan 1992

The Use Of Self Blood Glucose Monitoring And Its Relationship To Effective Management For People With Insulin Dependent Diabetes Mellitus, Maureen Unsworth

Theses : Honours

It is believed that people who effectively self manage their own diabetes will achieve higher levels of health and well being (Pender, 1987). Although diabetes education has increased rapidly in the last 10 years, the emphasis has been placed on increasing people's knowledge of diabetes as a medical condition, which does not necessarily ensure effective self management of diabetes. Previous research suggests that self blood glucose monitoring (SBOM) is an effective tool for promoting the use of blood glucose levels as a basis for initiating behaviour changes related to eating, exercise and insulin dosage. Yet, while most people with diabetes …

Yet Another Cause Of Chronic Viral Hepatitis?, H A. Shah, S Abid, S Hamid, S W. Jafri, Z Abbas, T Qaseem, H Shaikh, A H. Khan

Section of Gastroenterology

The clinical features, course and histology of liver in 20 patients; mostly middle aged to elderly females, closely resembling chronic Non A Non B hepatitis is presented. They presented quite late in their disease and therefore, complications such as variceal bleeds, ascites and encephalopathy were frequent. Our patients were negative for hepatitis B and C virus serology. Metabolic and immune causes of chronic liver disease were also ruled out. To the best of our knowledge, this is the first study of its kind elaborating the clinical features, course and histology of liver in chronic Non B Non C hepatitis and …

Dartmouth Scholarship

Fc gamma Rs (Fc gamma RI, Fc gamma RII, and Fc gamma RIII) are highly expressed on human mononuclear phagocytes and function in the clearance of immune complexes and opsonized pathogens. We have examined the role of Fc gamma R in mediating antibody-dependent clearance of HIV-1 by human monocytes and monocyte-derived macrophages by using bispecific antibodies (BsAbs) to independently target the virus to Fc gamma RI, Fc gamma RII, or Fc gamma RIII. Virus production was markedly reduced in monocytes cultured with strain HIV-1IIIB opsonized with BsAbs that target the virus to either Fc gamma RI or Fc gamma RII …
