We report four siblings who had polysomnographically documented sleep apnea. Two presented with the typical clinical picture of sleep apnea syndrome including daytime somnolence and snoring, had repetitive obstructive apneic episodes during sleep, and were effectively treated with upper airway surgery. The other two were asymptomatic and showed infrequent apneic episodes during sleep. This family illustrates the distinction between the sleep apnea syndrome and infrequent apneic episodes during sleep. The sleep apnea syndrome is associated with daytime symptomatology and requires treatment. The presence of apneic episodes during sleep in all four siblings has implications regarding the predisposing factors (eg, upper …

Clinical Manifestations Of Acute Pulmonary Embolism: Henry Ford Hospital Experience, A Five-Year Review, Kenneth V. Leeper Jr., John Popovich Jr., Deborah Adams, Paul D. Stein

Clinical findings of 112 patients with angiographically proven pulmonary embolism over a five-year period were analyzed. Recent immobilization, chronic heart disease, deep venous thrombosis, malignancy, and recent surgery were the most frequent predisposing factors. Only 5% had no identifiable risk factors. Presenting syndromes were circulatory collapse (25%), pulmonary infarction (59%), and uncomplicated pulmonary embolism (18%). Dyspnea and pleuritic chest pain were the predominant symptoms. The combination of dyspnea, pleuritic chest pain, and hemoptysis occurred in 23% of the patients. The most frequent signs were tachypnea, tachycardia, and rales. Disseminated Strongyloides Stercoralis And Aspergillus Fumigatus Presenting As Diffuse Interstitial Pneumonitis In A Steroid-Dependent Chronic Obstructive Pulmonary Disease Patient, Larry Tankanow, Michael S. Eichenhorn

We describe a patient with long-standing steroid-dependent chronic obstructive pulmonary disease who was admitted with new diffuse interstitial pulmonary infiltrates. Strongyloides stercoralis was present in the sputum and stool and responded to treatment. However, complicating disseminating Aspergillus fumigatus infection eventuated in the patient's death. Strongyloides stercoralis infection should be considered in patients with relevant exposure history, and complicating fungal and gram-negative bacterial infections which such patients may develop should be suspected.

Cancer Mortality Among Residents Of Huron, Sanilac, And Tbscola Counties As Compared To Other Michigan Residents, 1971-1985, Jennifer A. Booms, William Benish

Age-adjusted cancer mortality rates in the "Thumb" area of Michigan (Huron, Sanilac, and Tuscola counties) were compared with those for the rest of the state from 1971 to 1985. Comparisons were made after partitioning the mortality data on the basis of the patient's sex, year of death (1971 to 1975, 1976 to 1980, or 1981 to 1985), and site of primary tumor. International Classification of Diseases categories were combined to define 29 cancer site groups. The analysis was limited to the white population. Enteric-Coated Fenoprofen In Large-Joint Osteoarthritis, Gilbert B. Bluhm, Harris H. Macilwain, Walter W. Offen

Enteric-coated fenoprofen was compared with standard fenoprofen (Nalfon, Dista, Indianapolis, IN) in a randomized, double-blind, parallel, three-month trial of 113 outpatients (mean age, 64 years) with large-joint osteoarthritis. A previous study showed that enteric coating of fenoprofen reduces gastrointestinal microbleeding and may offer a safety advantage. Both treatments provided statistically significant improvement in pain measures (tenderness on pressure, pain at rest, pain with weight-bearing activity, and pain on passive motion). Adverse experiences were similar with both treatments and of an expected type, severity, and incidence for nonsteroidal anti-inflammatory drugs. Potency Of Current Levothyroxine Preparations Evaluated By High-Performance Liquid Chromatography, Sheldon S. Stoffer, Walter E. Szpunar

Ten different levothyroxine products manufactured by six companies were analyzed by high-performance liquid chromatography. Although nine products met the United States Pharmacopeia requirements, one product was found to have only 47% of expected potency. Until these products become more uniform, we would not recommend interchangeability of levothyroxine preparations.

Sexual behavior is both a health issue and a moral issue. Hence public education on sexuality has traditionally come from two main sources, those individuals or groups concerned with health and those concerned with morals. In his book No Magic Bullet (Oxford University Press, 1985), Allan Brandt describes the tensions that developed between these groups over the control of sexually transmitted diseases in the United States early in this century. Both groups wanted to control sexual behavior, but for different reasons. Public health personnel wanted to change sexual behavior in order to control infection and disease, while moralists believed that …

Breast cancer incidence continues to increase in the United States, with more than 130,000 new cases and 42,000 deaths expected this year. Today, one in every ten women in the United States will develop breast cancer at sometime during their lives. Breast cancer can be prevented by early prophylactic bilateral mastectomy, ie, removal of the breasts before any signs or symptoms develop. This practice was advocated by a male gynecologist colleague of mine. In vitro studies suggest that neonates and adults may have different cardiac chronotropic responses to a-adrenergic stimulation. To investigate these differences in vivo, three groups of dogs were studied. Group I = 12 puppies, ages 3-7 days; group II = 12 puppies ages 8-15 days, and group III = seven adult dogs. Heart rate and blood pressure determinations were made in the control setting and then after combined β-adrenergic and parasympathetic blockade (propranolol 0.6 mg/kg and bilateral vagotomies). Alpha-stimulation was then achieved with phenylephrine given in doses of 0.5, 1.0, and 10.0 μg/kg/ min. A second, high dose of propranolol …

Why is it that so little is known about PMS? First of all, there is no agreement on what PMS is. If investigators are referring to different sets of symptoms, how can studies be comparable? In fact, they are not. Not only are the symptoms different, but the timing of symptoms is different, Some studies describe symptoms that occur 7 to 10 days before the onset of menses, while others include symptoms that begin 14 days before the onset of menses and continue for 7 days after onset. Previous studies have suggested that the atrioventricular nodal functional refractory period in the neonate is equal to or shorter than that of the ventricle, providing little or no protection to the ventricle against rapid atrial rates and allowing closely coupled atrial beats to fall within the ventricular vulnerable period. We evalu­ated atrioventricular node function in 21 mongrel neonatal puppies, 3-15 days old, and 15 adult dogs utilizing intracardiac His bundle recording and stimulation techniques. The mean atrioventricular nodal functional refractory period (173.1 ± 20.0 ms) exceeded both the ventricular effective refractory period (139.5 ± 14.3) and ventricular functional refractory period …

The Oral Assessment Guide (OAG) can be used by the clinician to evaluate changes in the oral cavity. The OAG is comprised of eight categories of mucositis assessment of the mouth on a scale of 1 to 3, with “1” indicating normal and “3” indicating breakdown or loss of function. The OAG has been validated and is reliable in the current published format. Permission is granted for use in the current format. Any modifications will require new testing for validity and reliability. The OAG has been widely used in cancer and other populations and published in professional journals and books.

Thirteen nutritional parameters viz body mass index (BMI), mid arm circumference (MAC), mid calf circumference (MCC), triceps skin fold thickness (TST), abdominal skin fold thickness (ASFT), abdominal girth at umbilicus (AG U), haemoglobin (Hb), total leucocytes count (WBC), absolute lymphocyte count (ALC), serum proteins (Prot.), serum albumin (AIb), serum total iron binding capacity (TIBC) and serum iron (Fe) were estimated in 52 (36 male and 16 female) consecutive patients admitted to a surgical service at the Abbasi Shaheed Hospital, Karachi. Sulfiting agents, including sodium and potassium bisulfite, sodium and potassium metabisulfite, sodium sulfite, and sulfur dioxide, have enjoyed widespread use as food and drug ingredients. The oral ingestion of these sulfiting agents is now known to trigger asthma in a small subset of the asthmatic population. The best evidence suggests that perhaps 150,000 to 200,000 individuals in the United States may be sulfite sensitive. Although the mechanism of sulfite-induced asthma remains unknown, several possibilities have been considered, including inhalation of sulfur dioxide (SO2) while swallowing, an IgE-mediated reaction, and a deficiency of sulfite oxidase leading to impaired sulfite …

Clinical Use Of A Standard Kit-Preparation Of Radiolabeled Monoclonal Antibody 96.5 In The Diagnostic Imaging Of Metastatic Melanoma., Mansoor Saleh, D M. Miller, L Peterson, C D. Russell, M W. Unger

We studied the efficiency of a standard-kit preparation using 1 mg 111In-labeled 96.5 monoclonal antibody in combination with 19 mg of unlabeled antibody in the diagnostic imaging of 27 patients with documented metastatic melanoma. Twenty-three of 26 patients (88%) demonstrated immunoscintigraphic localization of tumor. Of 104 metastatic sites previously documented by conventional studies, 62 (60%) were identified by immunoscintigraphy. A total of 77 sites demonstrated localization of radiolabeled antibody. Fifty-four (70%) corresponded to known sites of disease; eight sites (10%) were "discovered" by immunoscintigraphy and subsequently confirmed by conventional studies; 15 imaged sites (20%) could not be confirmed by conventional …
