Postmenopausal Osteoporosis: The Evolution Of Our Concepts Of Its Cause, Harold M. Frost
Henry Ford Hospital Medical Journal
Research findings in the last 20 years concerning senile and postmenopausal osteoporosis indicate that the skeletal condition associated with these diseases arises because some systemic factor acts directly upon the bone marrow tissues. There is a secondary tendency to accelerate loss of bone greater than in the normal aging process. The loss occurs only on those surfaces in physical contact with the bone marrow tissue.

Comparative Gastroscopic Study Of Choline Salicylate And Aspirin, S. C. Danao, B. M. Schuman

Choline salicylate is the anhydrous choline salt of salicylic acid. It possesses the therapeutic effects of aspirin. Clinical trials suggest that it is less irritating to the gastric mucosa. A double-blind gastroscopic study of the acute effects on the gastric mucosa of choline salicylate, aspirin and matching placebo was carried out. Using the fiberoptic gastroscope. the stomach was examined and photographed before, and ten minutes after, the patient had swallowed the tablets. Results were evaluated on the basis of combined endoscopic and photographic observations. This study suggests that choline salicylate in its tablet form causes less gastric mucosal injury than …

Immunologic studies show that man develops specific antibodies to a variety of serum proteins of bovine, sheep, goat, chicken, and rabbit origins as well as antibodies against cow's milk protein. The data in this report emphasize that these hetero-antibodies are more frequently found in individuals with immunological problems and/or chromosomal aberrations. The clinical significance of human hetero-antibodies against these animal proteins deserves closer clinical observation for an understanding of these immunological problems.

The model of renal autotransplantation, with immediate contralateral nephrectomy in the dog, is used to demonstrate that continuous cooling by means of a cooling jacket during vascular anastomoses is effective in preserving renal function acutely and over a two-month follow-up period.

This review of pre- and post-operative management of six cases of small bowel perforation secondary to blunt abdominal trauma provides the opportunity to analyze the problems involved in prompt diagnosis and adequate care. Some of the possible mechanisms producing the perforation are discussed. In order to prevent undue delays in diagnosis, changes in clinical picture should warrant early surgical exploration. This avoids increased rates of mortality and prolonged morbidity.

Effects Of An Eeg "Seizure Discharge" On Operant Responding, Richard M. Lee, Julia A. Lee, Hendrik Van Den Ende, Lorne D. Proctor

A post-stimulus "seizure discharge" recorded in the amygdaloid area of monkey brains was investigated primarily by studying its effects upon behavior. Three different operant conditioning schedules were used with animals implanted with multiple depth electrodes. The schedules involved the factors of number discrimination, attention, sequential responding, and self-stimulation. Although the electrical brain stimulation caused a disruption in responding, no behavioral effects could be attributed to the recorded discharge itself. This result, and previous findings, lead to the tentative conclusion that the discharge results from a physico-chemical, brain-electrode interface phenomena and is not neurophysiological in nature.

An important nursing objective is to assist patients through stressful experiences. It has been assumed that cardiac surgery constitutes such a stress situation. For this reason, twenty-two patients undergoing cardiac surgery at Loma Linda University Hospital studied for the purpose of exploring the degree and time-course of were psychological stress.
The methodology consisted of administration of psychological tests before and after surgery; determination of urine catecholamines (UCA) before, during and after surgery; administration of a coping assessment tool before surgery, and a recall interview on the fifth day after surgery. The urinary output of uric acid from the purebred Dalmatian is similar in amount to that in man. Also, this breed of dog has a higher plasma uric acid level than other dogs and, like man, this hyperuricemia is accompanied more frequently with renal and bladder lithiasis. Allopurinol is effective therapy in both man and dog. Study of the fate of uric acid in the Dalmatian shows that the liver does not oxidize the available uric acid completely although it is capable of doing so when liver homogenates are studied. In an experimental study of gastrocolic and duodenocolic fistulas, it was clearly seen in 10 animals that duodenocolic fistulas result in the most serious loss of nutrition. Death from starvation occurred in all 42 animals in 30 days. Gastrocolic fistulas similarly resulted in weight loss and death of the animals in an average of 42 days. Lengthy fistulas between the stomach and colon caused no obvious disturbances in the physiology of the gastrointestinal tract. The significant microscopic changes included destruction of the mucous membrane and a decrease in size of the villi. The other noteworthy observation was that fistulous aggregations …

Infiltration to an injury site or area of bacterial contamination by polymorphonuclear leucocytes (or neutrophils) is an early and essential phase of the early inflammatory reaction. Their action on foreign matter is the most critical step taken by body defense mechanisms in the first 24 hours. Anything which interferes with that action, phagocytosis, greatly hampers early inflammation. This study was designed to see the effect of elevated glucose levels on neutrophil phagocytosis in a unique strain of genetically diabetic mice.

An investigation has been conducted to consider the effects of midpalatal suture expansion on the cranial-facial complex. Nine patients were treated that had constricted maxillae. Data was obtained from frontal and lateral headplate radiographs which were taken at four intervals during treatment. The Rocky Mountain Data Systems diagnosis was utilized to interpret the radiographic data.
The data from the nine cases were averaged to obtain a composit [sic] change at each of the four times.

In 98 patients with primary hyperparathyroidism, 15 had objective evidence of duodenal ulcer disease. Following operative correction of the hyperparathyroidism, improvement of duodenal ulcer disease occurred usually, but not uniformly, if unassociated with non-beta islet cell tumors. Correction of hyperparathyroidism in patients with the Zollinger-Ellison syndrome does not significantly influence the peptic ulcer which should be given primary aggressive surgical treatment.

Because of theoretical as well as practical difficulty in the total hip endoprostheses available to orthopaedic surgeons at the present time, an anlysis of the present and potential difficulties reveals that substitution of a roller-bearing for the present slider-bearing prosthesis and the establishment of a larger end-bearing surface on the acetabular side would be of benefit. Thus, the Ring prosthesis was re-designed to satisfy these hopes. A modification of the Ring's approach gives complete acetabular exposure and control of the prosthetic insertion. A comparison is made between incidence of fever in 132 patients with various types of disseminated cancer and 57 patients with diseases other than cancer during a 2- month observation period. Fifty-three febrile episodes were observed in cancer patients. Of these, 15 were of undetermined origin and 6 were noninfectious; none of the episodes occurred immediately following surgery. In the control group of 57 patients, there were 12 febrile episodes; of these, 7 were due to proven bacterial infection, 2 were due lo possible bacterial infection, and 3 were of undetermined etiology.
