The Janus Kinase/Signal Transducers and Activators of Transcription (JAK/STAT) pathway plays a critical role in cytokine-mediated responses in both innate and adaptive immunity, and dysregulation of the JAK/STAT pathway is linked to many inflammatory disorders. Multiple Sclerosis (MS) is an inflammatory demyelinating disease that affects the central nervous system, and both innate and adaptive immunity are involved in disease progression. STAT3 signaling is critically involved in MS pathology and is negatively regulated by Suppressor Of Cytokine Signaling 3 (SOCS3). In 2017, there was an estimated 1.8 million new HIV-1 infections worldwide. Development of an effective HIV-1 vaccine would begin to quell this global pandemic. HIV-1 envelope (Env) glycoprotein is the main vaccine candidate target due to the immune systems ability to generate broadly neutralizing antibodies (bnAbs) against Env. Approximately 90 N-glycans form a glycan shield that is the primary interface between the virus and host immune system. Key glycan motifs within the glycan shield are targets for bnAbs and are necessary for HIV-1 infectivity. Current estimates project that there are approximately 5.4 million Americans affected by Alzheimer’s disease (AD). While this number is expected to dramatically increase in the years to come, there is currently no disease modifying therapeutic to prevent or slow AD progression. Cognitive decline is a clinical hallmark of AD, while accumulation of amyloid-β (Aβ) is a pathological hallmark. Work in the field has demonstrated that mitigating Aβ levels may serve as a rational therapeutic avenue to slow progression of the disease. Although there are several known causes of epilepsy, little is understood about the development of epilepsy or epileptogenesis. What has been observed through multiple studies of epilepsy is an alteration of the proteomic profile, along with a distinct change in post-translational modifications (PTM). Francisella tularensis is the causative agent of the human disease tularemia. It is highly infectious with as few as 10 microorganisms via inhalation causing a lethal infection. F. tularensis infects a variety of cell types, including macrophages and neutrophils, since it needs to enter, survive and proliferate in order to cause pathogenicity. Endometriosis is a chronic gynaecological disease characterised by the presence of endometrial cells in extra-uterine regions. One of the main factors impacting on the fertility of women affected by endometriosis is the poor oocyte quality. Acute myeloid leukemia (AML) with an internal tandem duplication in Fms-related tyrosine kinase 3 (FLT3-ITD) is identified as a subgroup with poor outcome and intrinsic resistance to chemotherapy and therefore urgent need for development of novel therapeutic strategies.

Methods: The antitumor effects of melatonin alone or combined with sorafenib were evaluated via flow cytometry and immunoblotting assays in FLT-ITD AML cells. Also, the ex vivo and in vivo models were used to test the synergistic effects of melatonin and sorafenib against leukemia with FLT3/ITD mutation.

LIN28 is an RNA-binding protein, a known as regulator of microRNAs biogenesis and play a role in multiple cellular functions such as stem cell maintenance, cell metabolism and implicated in oncogenesis. In cancer study, LIN28 protein is frequently expressed and associated with tumor aggressiveness and poor disease outcome. Previous studies have shown that paralog-LIN28B was upregulated in during CCA development. However, the role of LIN28B in cholangiocarcinogenesis remains largely unknown. การวิจัยนี้เป็นการศึกษาเชิงวิเคราะห์แบบไปข้างหน้า (Prospective study) มีวัตถุประสงค์เพื่อประเมินความถูกต้องเชิงพยากรณ์ของแบบประเมินอันตรายจากบ้านชนิดที่ผู้วิจัยสร้างขึ้น คือ แบบประเมิน Thai Home Fall Hazard Assessment Tool (Thai-HFHAT) และชนิดที่ผู้วิจัยแปลผลงานวิจัยต่างประเทศ คือ แบบประเมิน Modified Home Falls and Accidents Screening Tool (Modified HOME FAST) และ แบบประเมิน Modified Home Falls and Accidents Screening Tool-Self Report (Modified HOME FAST-SR) ทำการศึกษาในกลุ่มตัวอย่างผู้สูงอายุที่มีอายุ ≥ 60 ปี จำนวน 450 คน ทำการติดตามกลุ่มตัวอย่างเป็นระยะเวลา 1 ปี ใช้สถิติ Cox's proportional hazard model ในการประมาณค่าอัตราเสี่ยงต่อการเกิดโรค (Hazard Ratios: HR) วิเคราะห์พื้นที่ใต้โค้ง receiver operating characteristic (ROC) และ วิเคราะห์ความตรงเชิงพยากรณ์ (predictive validity) ประกอบด้วย sensitivity, specificity, positive predictive value, negative predictive value, Likelihood Ratio ผลการศึกษาพบว่า กลุ่มตัวอย่างผู้สูงอายุจำนวนทั้งหมด 450 คน มีอุบัติการณ์ของการเกิดการล้มรายใหม่ 123 ราย คิดเป็นอัตราอุบัติการณ์เท่ากับร้อยละ 27.33 (95% CI: 22.72, 32.61) ต่อปี นับเป็นจำนวนครั้งทั้งสิ้น 334 ครั้ง จากจำนวนที่ศึกษาทั้งหมด 784 ครั้ง ในระยะติดตาม 163,550 …

ยาทีโนโฟเวียร์เป็นยาต้านไวรัสเอชไอวีที่ใช้เป็นส่วนประกอบของสูตรยาหลักในการรักษาผู้ใหญ่ที่ติดเชื้อเอชไอวีในประเทศไทย ยาทีโนโฟเวียร์ทำให้การทำงานของไตลดลง แต่ไม่มีข้อมูลเกี่ยวกับอุบัติการณ์ รวมทั้งระยะเวลาที่ทำให้เกิดการทำงานของไตผิดปกติในผู้ใหญ่ไทยที่ใช้ยาทีโนโฟเวียร์ วัตถุประสงค์การวิจัย เพื่อศึกษาความสัมพันธ์ระหว่างการใช้ยาทีโนโฟเวียร์กับการเกิดภาวะการทำงานของไตบกพร่องและโรคไตเรื้อรัง การศึกษานี้เป็นการศึกษาเชิงวิเคราะห์แบบ Retrospective cohort ทำการศึกษากับผู้ใหญ่ไทยที่ติดเชื้อเอชไอวีที่ไม่เคยได้รับยาต้านไวรัสเอชไอวีและเริ่มมารับการรักษาที่คลินิกเอชไอวี รพ.ตำรวจ ตั้งแต่ 1 มกราคม พ.ศ.2552-31 ธันวาคม พ.ศ.2558 เปรียบเทียบอัตราอุบัติการณ์ รวมทั้งระยะเวลาของการเกิดการทำงานของไตผิดปกติในกลุ่มที่ใช้ยาต้านไวรัสเอชไอวีที่มีและไม่มียาทีโนโฟเวียร์เป็นส่วนประกอบในสูตรยากลุ่มละ 700 คน ผลการศึกษาพบว่า อัตราอุบัติการณ์ของการเกิดภาวะการทำงานของไตบกพร่องในกลุ่มที่ได้รับและไม่ได้รับยาทีโนโฟเวียร์ คือ 27.66/1,000 คน-ปี และ 5.54/1,000 คน-ปี อัตราอุบัติการณ์ของการเกิดโรคไตเรื้อรังในกลุ่มที่ได้รับและไม่ได้รับยาทีโนโฟเวียร์ คือ 4.52/1,000 คน-ปี และ 2.29/1,000 คน-ปี จากการวิเคราะห์ด้วย Mixed model method พบว่า ค่าเฉลี่ยอัตราการกรองของไตของผู้ป่วยลดลงทั้ง 2 กลุ่ม โดยกลุ่มที่ได้รับยาทีโนโฟเวียร์มีค่าเฉลี่ยของอัตราการกรองของไตน้อยกว่า 1.92 มิลลิลิตร/นาที/1.73 ตารางเมตร (p-value = 0.022) Adjusted hazard ratio ของการเกิดภาวะการทำงานของไตบกพร่องในผู้ป่วยที่ได้รับยาทีโนโฟเวียร์ คือ 5.57 (95% CI 2.87-10.79, p-value <0.001) Adjusted hazard ratio ของการเกิดโรคไตเรื้อรังในกลุ่มที่ได้รับยาทีโนโฟเวียร์ คือ 2.36 (95% CI 0.76-7.33, p-value = 0.138) อัตราการกรองของไตลดลงอย่างรวดเร็วในช่วง 2-3 ปีแรกหลังเริ่มได้รับยาทีโนโฟเวียร์ การตรวจติดตามการทำงานของไตอย่างใกล้ชิดจะช่วยป้องกันไม่ให้ผู้ป่วยเกิดความผิดปกติของไตในอนาคต

ภาวะซึมเศร้าและคุณภาพการนอนของพยาบาลในโรงพยาบาลตติยภูมิแห่งหนึ่งในเขตกรุงเทพมหานคร, ปริยนุช จันทร์สถิตานนท์

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาหาความสัมพันธ์ระหว่างคุณภาพการนอนและภาวะซึมเศร้า รวมไปถึงความชุกและปัจจัยอื่น ๆ ที่เกี่ยวข้องกับภาวะซึมเศร้า เป็นการศึกษาเชิงพรรณนา ณ จุดใดจุดหนึ่ง โดยกลุ่มตัวอย่างที่ทำการศึกษาคือพยาบาลที่ปฏิบัติงานในโรงพยาบาลนพรัตนราชธานีและอยู่ในช่วงปีงบประมาณ พ.ศ. 2562 จำนวนทั้งสิ้น 638 คน เครื่องมือที่ใช้คือ แบบสอบถามประเมินภาวะซึมเศร้า Patient Health Questionnaire-9 (PHQ-9) ฉบับภาษาไทยและแบบประเมินคุณภาพการนอนหลับฉบับภาษาไทย ซึ่งดัดแปลงมาจาก the Pittsburgh sleep quality index (PSQI) ข้อมูลความชุกของภาวะซึมเศร้าและผลรวมประเมินคุณภาพการนอนจะถูกนำเสนอในรูปแบบร้อยละ ส่วนข้อมูลด้านปัจจัยและการหาความสัมพันธ์นำเสนอในรูปแบบ crude และ adjusted odds ratios (ORs) ผลการศึกษาพบว่า คุณภาพการนอนและภาวะซึมเศร้ามีความสัมพันธ์กันอย่างมีนัยสำคัญทางสถิติ โดยกลุ่มที่คุณภาพการนอนไม่ดีเกิดภาวะซึมเศร้าเป็น 3.38 เท่าของกลุ่มที่มีคุณภาพการนอนที่ดี โดยความชุกของภาวะซึมเศร้าในพยาบาลคิดเป็นร้อยละ 17.3 โดยพยาบาลส่วนใหญ่มีคุณภาพการนอนที่ไม่ดีคิดเป็นร้อยละ 61 ปัจจัยอื่น ๆ ที่พบว่ามีความสัมพันธ์กับการเกิดภาวะซึมเศร้าคือ ได้แก่ การออกกำลังกาย ดัชนีมวลกาย ความพึงพอใจในงาน สวัสดิการและการได้รับการสนับสนุนและคุณภาพการนอน สรุปผลการศึกษา คุณภาพการนอนมีผลต่อการเกิดภาวะซึมเศร้า รวมถึงปัจจัยอื่น ๆ ที่อาจส่งเสริมให้มีความรุนแรงมากขึ้น ดังนั้นการศึกษานี้จึงเป็นข้อมูลเบื้องต้น เพื่อเป็นข้อมูลนำไปสู่การวางแผนและกำหนดแนวทางการแก้ไข้ปัญหาการเกิดภาวะซึมเศร้าในพยาบาล

การศึกษาเชิงทดลองแบบสุ่มชนิดมีกลุ่มควบคุมนี้มีวัตถุประสงค์เพื่อศึกษาประสิทธิผลของการกระตุ้นพฤติกรรมที่มีต่อภาวะซึมเศร้าในผู้สูงอายุ ดำเนินการวิจัยในโรงพยาบาลส่งเสริมสุขภาพประจำตำบล (รพ.สต.) จำนวน 2 แห่ง จังหวัดสมุทรสงคราม ประเทศไทย โดยสุ่มให้ได้รับการกระตุ้นพฤติกรรม (กลุ่มทดลอง) และได้รับการดูแลแบบปกติ (กลุ่มควบคุม) รพ.สต.แต่ละแห่งสุ่มเลือกผู้สูงอายุที่มีคุณสมบัติตามเกณฑ์แห่งละ 41 คน เข้าร่วมการวิจัย การกระตุ้นพฤติกรรมประกอบด้วย (1) การลงตารางกิจกรรม (2) การติดตามกิจกรรม (3) การปรับกิจกรรมรวมถึงการติดตามจำนวนก้าวเดินในแต่ละวัน วิเคราะห์ผลลัพธ์เดือนที่ 0, 3, 6 และ 9 ด้วยสถิติ Generalized Estimating Equations (GEE), และ Generalized Mixed Model ผลการศึกษาตลอดระยะเวลา 9 เดือน พบว่า กลุ่มทดลองมีค่าเฉลี่ยภาวะซึมเศร้า [-2.42 คะแนน (95%CI: -3.84, -1.00)] (ขนาดผลการทดลอง Cohen’s d = 1.12, 1.00, และ 0.74 ตามลำดับ), ค่าเฉลี่ยภาวะสุขภาพจิตด้านซึมเศร้า และด้านความเครียด [-1.47 และ -1.87 คะแนน (95%CI: -2.43, -0.50 และ -2.94, -0.79 ตามลำดับ)] ลดลงอย่างมีนัยสำคัญทางสถิติเมื่อเปรียบเทียบกับกลุ่มควบคุม ส่วนด้านวิตกกังวลลดลงเพียงแค่ 6 เดือน [-0.87 คะแนน (95%CI: -1.52, -0.23)] จำนวนก้าวเดินเฉลี่ยต่อวันของกลุ่มทดลอง (2,595 ก้าว) เพิ่มขึ้นเมื่อเปรียบเทียบกับกลุ่มควบคุม (1,816 ก้าว) ไม่แตกต่างกัน ค่าเฉลี่ยความแปรปรวนของอัตราการเต้นของหัวใจ (HRV) ค่าส่วนเบี่ยงเบนมาตรฐาน (SDNN) [8.00 ms (95%CI: 1.51, 14.49)] และค่าช่วงคลื่นความถี่สูง (HF) [0.59 ms2 (95%CI: …

โรคเบาหวาน (Diabetes mellitus, DM) เป็นภาวะผิดปกติที่ร่างกายมีระดับน้ำตาลในเลือดสูงกว่าปกติ จนนำไปสู่อาการแทรกซ้อนต่าง ๆ อาทิ โรคไตวายเรื้อรังระยะสุดท้าย และโรคไขมันพอกตับที่ไม่ได้เกิดจากแอลกอฮอล์ (Non-alcoholic fatty liver disease, NAFLD) เพื่อควบคุมระดับน้ำตาลในเลือด ผู้ป่วยโรคเบาหวานจะได้รับการแนะนำให้บริโภคอาหารในแต่ละมื้อลดลง และเพิ่มมื้ออาหารว่างเป็นอาหารที่มีค่าดัชนีอาหารต่ำ เป็นที่ทราบกันดีว่าพืชในตระกูลถั่วและงามีประโยชน์ต่อสุขภาพหลายประการ การศึกษาในพืชตระกูลถั่วพบว่าสามารถป้องกันภาวะไขมันในเลือดสูงรวมถึงบรรเทาภาวะโรคเบาหวานได้ และยังมีการศึกษาที่พบว่างา (Sesame) ช่วยลดคอเลสเตอรอล (Cholesterol) ในเลือด นอกจากนี้ถั่วเขียวทอดและงาคั่วเป็นหนึ่งในอาหารกินเล่นที่นิยมของคนไทย สมควรที่จะส่งเสริมผู้ป่วยโรคเบาหวานบริโภคถั่วเขียวทอดหรืองาคั่วเป็นอาหารว่าง หากแต่ในปัจจุบันยังไม่มีหลักฐานเพียงพอที่จะทำให้เชื่อมั่นได้ว่า ถั่วเขียวทอดและงาคั่ว สามารถลดความเสี่ยงในการเกิดภาวะแทรกซ้อนของโรคเบาหวานได้จริงหรือไม่ ในงานวิจัยครั้งนี้จึงมีจุดประสงค์ที่จะศึกษาผลของการบริโภคถั่วเขียวทอดและงาดำคั่วต่อการดำเนินโรคเบาหวานชนิดที่ 2 ในหนูทดลอง โดยการแบ่งหนูทดลองออกเป็น 4 กลุ่ม คือ หนูปกติกลุ่มควบคุม (Normal control group; NC) หนูเบาหวานกลุ่มควบคุม (Diabetes mellitus control group; DMC) หนูเบาหวานที่บริโภคถั่วเขียวทอด (Fried mung bean group; FMB) และหนูเบาหวานที่บริโภคงาดำคั่ว (Roasted black sesame group; RBS) หนูทดลองนอกเหนือจากกลุ่มปกติควบคุมจะถูกกระตุ้นให้เป็นเบาหวาน และบริโภคอาหารไขมันสูง พบว่าในสัปดาห์ที่ 12 ของการทดลอง ระดับน้ำตาล HbA1C ระดับไขมันและคริอะตินินในเลือดในหนูทดลองแต่ละกลุ่มไม่มีความแตกต่างกัน แต่เมื่อทำการศึกษาพยาธิสภาพของไตและตับด้วยกล้องจุลทรรศน์แบบแสง และกล้องจุลทรรศน์อิเล็กตรอนแบบส่องผ่าน พบว่าหนูเบาหวานกลุ่มควบคุมมีปริมาณไขมันสะสมในตับสูง และมีความหนาของเยื่อฐานโกลเมอรูลัส (Glomerular basement membrane) และความกว้างของขาเซลล์โพโดไซต์ (podocyte foot process) มากกว่าหนูปกติกลุ่มควบคุมอย่างมีนัยสำคัญ ซึ่งเป็นภาวะแทรกซ้อนระยะแรกของตับและไตในโรคเบาหวาน และพบว่าหนูที่ได้รับถั่วเขียวทอดและงาดำคั่ว ไม่พบความผิดปกติทางพยาธิสภาพของตับและไตดังกล่าว จากผลการทดลองทั้งหมดในการวิจัยครั้งนี้อาจกล่าวได้ว่าการบริโภคถั่วเขียวทอดหรืองาดำคั่วสามารถบรรเทาพยาธิสภาพของไตและตับที่เกิดจากโรคเบาหวานในหนูทดลองที่เป็นเบาหวานชนิดที่ 2 ได้

จิ้งจกเป็นสัตว์เลื้อยคลานขนาดเล็กที่จัดอยู่ในสกุล Hemidactylus สามารถพบได้ในที่อยู่อาศัยของมนุษย์ ได้มีการรายงานว่าจิ้งจกสามารถเป็นแหล่งกักเก็บโรคได้ เช่น แบคทีเรีย และโปรโตซัว สำหรับในประเทศไทยเชื้อ Leishmania spp. และ Trypanosoma spp. มีการติดต่อสู่คนและสัตว์โดยการกัดของริ้นฝอยทรายเพศเมีย อย่างไรก็ตามข้อมูลในสัตว์ที่เป็นแหล่งกักเก็บโรคของเชื้อโปรโตซัวดังกล่าวในประเทศไทยยังมีอยู่อย่างจำกัด ในการศึกษาครั้งนี้มีวัตถุประสงค์เพื่อตรวจหาเชื้อ Leishmania และ Trypanosoma DNA ในจิ้งจกที่เก็บจากบ้านของผู้ป่วย leishmaniasis ในภาคใต้ของประเทศไทย จากตัวอย่างจำนวน 57 ตัวอย่างซึ่งได้จากจิ้งจกทั้งหมด 19 ตัว ประกอบด้วยตับ ม้าม และหัวใจ ตัวอย่างทั้งหมดถูกนำมาใช้ในการตรวจหา DNA ของเชื้อ Leishmania spp. และ Trypanosoma spp. โดยเทคนิค Polymerase Chain Reaction (PCR) จากตำแหน่งยีน ITS1 และ SSU rRNA ตามลำดับ โดยผลการทดลองพบว่า มี 15 ตัวอย่างที่ตรวจพบเชื้อ Trypanosoma spp. ประกอบไปด้วย ตับ 4 ตัวอย่าง ม้าม 5 ตัวอย่าง และหัวใจ 6 ตัวอย่างจากตัวอย่างจิ้งจกทั้งหมด 9 ตัว แต่ไม่สามารถตรวจพบเชื้อ Leishmania spp. ในการศึกษาครั้งนี้ จากผลแผนภูมิวิวัฒนาการที่ตำแหน่งยีน ITS1 แสดงให้เห็นว่า เชื้อ Trypanosoma spp. จัดอยู่ใน An04/Frog1 ของกลุ่ม Anura clade และผลแผนภูมิวิวัฒนาการของจิ้งจกที่ตำแหน่งยีน cytb แสดงให้เห็นว่าจิ้งจกทั้งหมดในการศึกษาครั้งนี้มีความเหมือนกับจิ้งจกสายพันธุ์ Hemidactylus platyurus ที่จัดอยู่ในกลุ่ม tropical asian clade โดยการศึกษานี้เป็นการตรวจพบเชื้อ Trypanosoma spp.ในจิ้งจกเป็นครั้งแรก ซึ่งข้อมูลที่ได้จากการศึกษาครั้งนี้แสดงให้เห็นว่าจิ้งจกอาจมีศักยภาพเป็นแหล่งเก็บโรคของเชื้อ Trypanosoma spp. นอกจากนี้การศึกษาอาจเป็นประโยขน์ในการป้องกันและควบคุมโรค Trypanosomiasis …

การประเมินการตกสู่เบื้องล่างของเม็ดเลือดแดงหลังการตายเพื่อประมาณระยะเวลาหลังการตายโดยใช้เครื่องวัดสี, นิชาภา พุ่มจิตร

การประมาณระยะเวลาหลังการตาย (Postmortem interval, PMI) มีความสำคัญมากในงานด้านนิติเวชศาสตร์ แต่ในปัจจุบันยังไม่มีเครื่องมือหรือวิธีที่จะสามารถประเมินระยะเวลาหลังการตายได้อย่างแน่นอน ต้องอาศัยความเชี่ยวชาญจากแพทย์ผู้ทำการชันสูตรเป็นผู้ประมาณจากการเปลี่ยนแปลงหลังการตาย (Postmortem Change) ซึ่งเป็นการเปลี่ยนแปลงของปฏิกิริยาทางเคมีหลายอย่างในร่างกายของมนุษย์เมื่อเสียชีวิต หนึ่งในการเปลี่ยนแปลงหลังการตายที่สามารถนำมาใช้ในการประมาณหาระยะเวลาหลังการตายคือ การตกสู่เบื้องล่างของเม็ดเลือดแดงหลังการตาย (Livor mortis) การตรวจโดยใช้เครื่องวัดสี (Colorimeter) ซึ่งเป็นเครื่องมือทางวิทยาศาสตร์ที่ใช้ในวัดค่าความดูดกลืนแสงที่ความยาวคลื่นใดคลื่นหนึ่ง ทำให้สามารถประเมินสีได้มากกว่าที่เห็นด้วยตาเปล่าจึงถูกนำมาใช้ในการประเมินการตกสู่เบื้องล่างของเม็ดเลือดแดงหลังการตายของผู้เสียชีวิตในงานวิจัยนี้ วัตถุประสงค์ของงานวิจัยนี้ คือ เพื่อประเมินระยะเวลาหลังการตายด้วยความแตกต่างของสีผิวของผู้เสียชีวิตที่เปลี่ยนแปลงไปตามการตกสู่เบื้องล่างของเม็ดเลือดแดงหลังการตายโดยการใช้เครื่องวัดสีรุ่น NR20XE โดยทำการวัดค่าสีผิว (L, a, b, c, h และ ∆E) บริเวณหน้าท้องส่วนบนที่ต่อจาก xiphoid process เป็นจุดอ้างอิงและวัดสีผิวบริเวณกระดูกสันหลังส่วนเอวชิ้นที่ 1 และ 2 (Lumbar lordosis) โดยวัดทุกๆ 2 ชั่วโมง (2, 4, 6 และ 8 ชั่วโมง) และนำค่าที่ได้มาวิเคราะห์ทางสถิติค่าสัมประสิทธิ์สหสัมพันธ์ของสเปียร์แมน ผลการศึกษาจากตัวอย่างทั้งหมด 27 ศพ โดยเป็นศพเพศชาย 10 คนและศพเพศหญิง 17 คน ศพตัวอย่างเพศชายมีอายุเฉลี่ย 62.30 ± 13.38 ปีและศพตัวอย่างเพศหญิงมีอายุเฉลี่ย 71.53 ± 13.22 ปี ตามลำดับและสาเหตุของการเสียชีวิตของศพตัวอย่าง แบ่งเป็นการเสียชีวิตจากโรคมะเร็งชนิดต่างๆ โรคเรื้อรังและโรคติดเชื้อ และพบว่า L, b, c, h และ ∆E กับระยะเวลาหลังการตายมีความสัมพันธ์กันอย่างมีนัยสำคัญทางสถิติที่ระดับนัยสำคัญ 0.05 เครื่องวัดสีรุ่น NR20XE สามารถประเมินระยะเวลาหลังตายโดยมีความสัมพันธ์ระหว่างการเปลี่ยนแปลงของสีกับระยะเวลาหลังการตายอยู่ในระดับปานกลาง ซึ่งข้อมูลที่ได้อาจจะนำไปปรับปรุงพัฒนาเป็นเครื่องมือที่สามารถประเมินหาระยะเวลาหลังการตายได้อย่างแม่นยำมากยิ่งขึ้นในงานทางด้านนิติวิทยาศาสตร์ในอนาคต

Pericytes เป็นเซลล์ที่วางตัวอยู่ติดกับผนังด้านนอกของหลอดเลือดขนาดเล็ก ได้แก่ capillary และ post-capillary venule เซลล์นี้ทำหน้าที่หลักในการค้ำจุนหลอดเลือดดังกล่าว และมีความสำคัญต่อทั้งกระบวนการทางสรีรวิทยาและพยาธิวิทยาของหลอดเลือด โดยเฉพาะอย่างยิ่งในการสร้างหลอดเลือดใหม่ ปัจจุบันอนุภาคทองคำระดับนาโนเมตร (AuNPs) ถูกนำมาใช้เพื่อยับยั้งการสร้างหลอดเลือดใหม่ โดยยับยั้งการแบ่งตัว การเคลื่อนย้าย และกระบวนการ tube formation โดยมุ่งเน้นผลไปยัง endothelial cells เพียงอย่างเดียว แต่อย่างไรก็ตามยังไม่มีการศึกษาถึงผลของ AuNPs ต่อลักษณะสัณฐานและการทำงานของ pericytes การศึกษานี้จึงนำ 20 nm AuNPs ที่ความเข้มข้น 30 ppm มาทดสอบกับ pericytes โดยการเคลื่อนย้ายของเซลล์ศึกษาโดยใช้เทคนิค transwell migration, และระดับการแสดงออกของยีน Ki-67 ซึ่งเป็นตัวบ่งชี้การแบ่งตัวของเซลล์ และ PDGFR-β ซึ่งเป็นตัวบ่งชี้ pericytes โดยใช้เทคนิค real-time RT-qPCR และลักษณะสัณฐานศึกษาด้วยกล้องจุลทรรศน์อิเล็คตรอนแบบส่องผ่าน (TEM) นอกจากนี้ศึกษาการสร้างหลอดเลือดฝอยจำลองแบบสามมิติบน Matrigel โดยนำ pericytes ที่ได้รับ AuNPs มาเพาะเลี้ยงร่วมกับเซลล์ human umbilical cord endothelial cells ผลการศึกษาพบว่า AuNPs มีผลทำให้การเคลื่อนย้าย การแบ่งตัว และการแสดงออกระดับ mRNA ของยีน PDGFR-β ของ pericytes ลดลงอย่างมีนัยสำคัญ และผลการศึกษาภายใต้กล้อง TEM เปิดเผยให้เห็นอนุภาคทองคำที่ถูกบรรจุอยู่ใน organelles ต่างๆ ได้แก่ late endosomes, lysosomes และ mitochondria และที่น่าสนใจคือ mitochondria เหล่านี้มีลักษณะบวมพองหรือเสียสภาพ ในส่วนของการศึกษาจำนวนของการสร้างหลอดเลือดจำลอง พบว่ามีจำนวนลดลงอย่างมีนัยสำคัญในกลุ่มของ pericytes ที่ถูกใส่ AuNPs และพบว่า pericytes ในกลุ่มนี้มีลักษณะเป็นทรงกลม ไม่ยืดแขนงเพื่อสร้าง tube ส่งผลให้ tube มีลักษณะไม่สมบูรณ์เมื่อเปรียบเทียบกับกลุ่มควบคุม …

วัตถุประสงค์: เพื่อศึกษาเปรียบเทียบระดับพรีจีโนมิกอาร์เอ็นเอของเชื้อไวรัสตับอักเสบบีในตัวอย่าง พลาสมา ที่เก็บไว้ในอุณหภูมิห้อง (25 องศาเซลเซียส) และ 4 องศาเซลเซียสภายในเวลา 48 ชั่วโมง วิธีการศึกษา: นำตัวอย่างพลาสมาจากผู้ป่วยไวรัสตับอักเสบบี จำนวน 40 คน โดยเจาะเลือด และปั่นแยกภายในเวลา 2 ชั่วโมง นำพลาสมาเก็บไว้ที่อุณหภูมิ 4 และ 25 องศาเซลเซียสที่เวลา 2, 6, 12, 24 และ 48 ชั่วโมง แล้วนำมาวัดระดับพรีจีโนมิกอาร์เอ็นเอพร้อมกัน โดยวิธีดิจิทัลพีซีอาร์ และเปรียบเทียบความแตกต่างของระดับพรีจีโนมิกอาร์เอ็นเอ ตั้งต้นและที่อุณหภูมิและจุดเวลาต่าง ๆ ผลการศึกษา: การเปลี่ยนแปลงของระดับพรีจีโนมิกอาร์เอ็นเอ เทียบกับเวลาตั้งต้น มีความแตกต่างอย่างมีนัยสำคัญทางสถิติ ที่เวลาการจัดเก็บตั้งแต่ 12 ชั่วโมงขึ้นไป ในอุณหภูมิจัดเก็บ 4 องศาเซลเซียส และ ที่อุณหภูมิจัดเก็บ 25 องศาเซลเซียส มีความแตกต่างอย่างมีนัยสำคัญทางสถิติ ที่เวลาการจัดเก็บตั้งแต่ 6 ชั่วโมงขึ้นไป แต่การเปลี่ยนแปลงของระดับพรีจีโนมิกอาร์เอ็นเอไม่มากกว่า 0.5 log จำนวนตัวต่อมิลลิลิตร จึงไม่มีนัยสำคัญทางคลินิก สรุปผล: ระดับพรีจีโนมิกอาร์เอ็นเอในพลาสมามีความคงตัวที่อุณหภูมิห้อง (25 องศาเซลเซียส) และ 4 องศาเซลเซียสในทุกช่วงเวลาระหว่าง 48 ชั่วโมง ก่อนกระบวนการวัดระดับ

วัตถุประสงค์: เพื่อศึกษาการทำงานของ Convolutional Neural Network (CNN) ในการตรวจหาและวินิจฉัยโรคจากภาพอัลตร้าซาวด์ของก้อนในตับในเวชปฏิบัติ โดยผ่านตัวชี้วัดได้แก่ ความไว (sensitivity), ความจำเพาะ (specificity) และ ความถูกต้อง (accuracy) วิธีการศึกษา: เก็บภาพอัลตร้าซาวด์ตับในผู้ป่วยที่มีความเสี่ยงต่อการเกิดมะเร็งตับปฐมภูมิที่มารับการตรวจรักษาคลินิกโรคตับ โรงพยาบาลจุฬาลงกรณ์ ในช่วง 8 เดือน โดยใช้เครื่องอัลตร้าซาวด์ 4 เครื่อง บันทึกข้อมูลก้อนในตับและพื้นหลังที่เป็นเนื้อตับ จากนั้นทำการลงผลการวินิจฉัยสุดท้ายของก้อนที่ตับ อ้างอิงจากผลเอกซเรย์คอมพิวเตอร์ เอกซเรย์คลื่นแม่เหล็กไฟฟ้า หรือ การเจาะ/ตัดก้อนที่ตับมาตรวจทางพยาธิวิทยา หลังจากนั้นนำภาพก้อนในตับที่รวบรวมได้ไปทดสอบการทำงานของ CNN ผลการศึกษา: ผู้ป่วยที่ตรวจพบก้อนในตับทั้งหมด 156 คน เป็นผู้ป่วยตับแข็ง 79 คน และไม่เป็นตับแข็ง 77 คน ภาพก้อนในตับทั้งหมด 569 lesions แบ่งเป็น HCC 138 lesions, cyst 177 lesions, hemangioma 113 lesions, focal fat infiltration (FFI) 47 lesions และ focal fat sparing (FFS) 94 lesions อัตราการตรวจพบก้อนในตับของ CNN เท่ากับร้อยละ 52 โดยอัตราการตรวจพบก้อนในตับในกลุ่มที่เป็นตับแข็งสูงกว่าในกลุ่มที่ไม่เป็นตับแข็งอย่างมีนัยสำคัญ คือร้อยละ 61 เทียบกับร้อยละ 45 (p= 0.0002) การวินิจฉัยชนิดของก้อนในตับพบว่า CNN มีความไวร้อยละ 87, ความจำเพาะร้อยละ 97 และ ความถูกต้องร้อยละ 95 สรุปผล: CNN สามารถวินิจฉัยชนิดของก้อนในตับที่พบบ่อยได้เป็นอย่างดี โดยพื้นหลังของเนื้อตับไม่ส่งผลต่อการวินิจฉัยชนิดของก้อนในตับ

Atopic dermatitis (AD) is an inflammatory skin disease characterized by chronic and recurrent eczematous rash. AD occurs in both children and adults, but children are more affected. Aspergillus flavus is a fungus found in the environment causing keratitis, cutaneous infections, sinusitis, invasive pulmonary aspergillosis in humans. Although this fungus can be treated with antifungal agents, these main antifungal agents have many side effects. Trehalase is an enzyme for digesting trehalose into two glucose subunits and is essential for virulence in many fungi. A trehalase inhibitor, called validamycin A, has been used effectively against a rice fungal pathogen, Rhizoctonia solani. Cytolethal distending toxin subunit B (CdtB) is a DNase I-like bacterial genotoxin that causes DNA damage leading to cell cycle arrest and cell death. Previous reports demonstrated that CdtB has low level of DNase activity in vitro. Besides, a mutation at histone H2B phosphorylation site required for chromatin condensation during apoptosis (S10A) conferred partial resistance to CdtB in Saccharomyces cerevisiae model. Despite strengthened antimicrobial therapy, biofilm infections of Pseudomonas aeruginosa and Acinetobacter baumannii are associated with poor prognosis and limited therapeutic options. Assessing antibiotics on planktonic bacteria can result in failure against biofilm infections. Currently, antibiotics to treat biofilm infections are administered empirically, usually without considering the susceptibility of the biofilm objectively before beginning treatment. Parkinson’s disease (PD) is the second most common neurodegenerative disorder. This study consisted of two parts. The purpose of part I was to investigate the difference in concentrations of DA, NE, 5-HT and their metabolites (3-methoxytyramine (3-MET), homovanillic acid (HVA), normetanephrine (NM), vanillylmandelic acid (VMA) and 5-hydroxyindoleacetic acid (5-HIAA)) in peripheral body fluids between control and PD groups.
