Background: Fall rates continue to be a significant problem in health care facilities despite all the research and tool kits created to prevent falls. Hospitals that have successfully reduced fall rates find it challenging to sustain a reduction in the fall rates. During hospitalizations, 3%-20% of patients fall, with 30%-50% resulting in injury (Joint Commission, [JC], 2015).
Internal Evidence: Despite Medical City Alliance (MCA) having an evidence-based fall program, the fall rate has not decreased. Falls are one of the quality measures identified at the division level that needs improvement.

At a regional Veterans Administration (VA) hospital, nurses performing case management were unable to perform face-to-face visits with patients due to coronavirus. Care coordination and discharge planning efforts occurred through telephonic case management to preserve personal protective equipment and reduce transmission rates through social distancing. Trust and rapport between patients and nurses were reduced without personal contact. Peplau’s theory of interpersonal relations described six nurse roles and three phases of relationship development required to create therapeutic nurse-patient relationships. Connectedness was a key concept considered to be the path to a successful meaningful therapeutic nurse-patient relationship (Hrabe, 2005).

Surveys within years and particularly the COVID-19 pandemic have revealed increasing rates of loneliness. More research is also emerging linking loneliness with a myriad of negative health outcomes as well as reduced quality of life. There is little research about effective interventions addressing loneliness. This paper reviews the available literature on the topic and proposes a pen pal program for college students and older adults developed based on available evidence.

The purpose of this study was to describe the relationship between aging female informal caregivers of persons with dementia (PWD) and care recipients’ PWD demographic factors, aging female informal caregivers of PWD social support factors, positive and negative feelings, a sense of flourishing, stress, and life satisfaction.

Older adults with dementia are living longer and the majority are receiving care by aging female informal caregivers. Purpose and Rationale
The aim of the evidence-based practice (EBP) project is to improve psychological outcomes for socially isolated older adults enrolled in a Homebound Program (HP) through the implementation of an EBP intervention. Social isolation has a multitude of negative consequences, including impacting the psychological health (e.g. depression, loneliness, anxiety) of the older adult population.
Synthesis of Evidence
Meta-analyses, systematic reviews, and randomized controlled trials addressing the clinical problem of social isolation were identified. การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง เพื่อเปรียบเทียบผลของโปรแกรมการให้ความรู้สุขวิทยาการนอนร่วมกับการออกกำลังกายแบบชี่กงวิถีไทยต่ออาการนอนไม่หลับของผู้ป่วยสูงอายุหลังเปลี่ยนข้อเข่าเทียมก่อนและหลังการทดลอง และเปรียบเทียบระหว่างกลุ่มที่ได้รับโปรแกรม ฯ กับกลุ่มที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่าง คือผู้ป่วยสูงอายุ 60 ปีขึ้นไปที่ได้รับการเปลี่ยนข้อเข่าเทียม ตั้งแต่ 1 เดือนและไม่เกิน 3 เดือนหลังผ่าตัดเปลี่ยนข้อเข่าเทียม ที่มารับการรักษา ณ ห้องตรวจกระดูกและข้อ หอผู้ป่วยนอก โรงพยาบาลเลิดสิน คัดเลือกกลุ่มตัวอย่างตามเกณฑ์ที่กำหนด จำนวน 40 คน แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลองกลุ่มละ 20 คน โดยจับคู่ให้ทั้งสองกลุ่มมีความคล้ายคลึงกันในเรื่อง อายุ เพศและระดับอาการนอนไม่หลับ โดยกลุ่มควบคุมได้รับการพยาบาลตามปกติ ขณะที่กลุ่มทดลองเข้าร่วมโปรแกรมการให้ความรู้สุขวิทยาการนอนร่วมกับการออกกำลังกายแบบชี่กงวิถีไทยซึ่งผู้วิจัยสร้างขึ้นจากแนวคิดการให้ความรู้สุขวิทยาการนอนของ Irish และคณะ (2015) และแนวคิดการออกกำลังกายแบบชี่กงวิถีไทยของ สุรีพร ธนศิลป์ (2557) เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือ แบบสอบถามอาการนอนไม่หลับของ Morin (1993) ฉบับที่แปลเป็นภาษาไทยโดย พัทรีญา แก้วแพง (2547) วิเคราะห์ข้อมูลโดยใช้สถิติ ร้อยละ ค่าเฉลี่ย และ t-test ผลการวิจัยสรุปได้ดังนี้ 1. อาการนอนไม่หลับของผู้ป่วยสูงอายุหลังเปลี่ยนข้อเข่าเทียมหลังเข้าร่วมโปรแกรมการให้ความรู้สุขวิทยาการนอนร่วมกับการออกกำลังกายแบบชี่กงวิถีไทยน้อยกว่าก่อนได้รับโปรแกรมการให้ความรู้สุขวิทยาการนอนร่วมกับการออกกำลังกายแบบชี่กงวิถีไทย อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. อาการนอนไม่หลับของผู้ป่วยสูงอายุหลังเปลี่ยนข้อเข่าเทียมในกลุ่มที่เข้าร่วมโปรแกรมการให้ความรู้สุขวิทยาการนอนร่วมกับการออกกำลังกายแบบชี่กงวิถีไทยน้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

วิจัยนี้เป็นวิจัยแบบกึ่งทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบคุณภาพชีวิตด้านสุขภาพในผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลาม ก่อนได้รับโปรแกรมและภายหลังได้รับโปรแกรมในสัปดาห์ที่ 1 สัปดาห์ที่ 2 และสัปดาห์ที่ 6 กลุ่มตัวอย่างคือ ผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลาม ซึ่งมีคุณสมบัติตามเกณฑ์ที่กำหนดจำนวน 24 คน ได้รับโปรแกรมเป็นเวลา 2 สัปดาห์ เครื่องมือในการวิจัย คือ โปรแกรมการดูแลวิถีพุทธ และ แบบประเมิน Functional Assessment of Chronic Illness Therapy–Palliative Care (FACIT-Pal) ฉบับภาษาไทย วิเคราะห์ค่าดัชนีความตรงตามเนื้อหาของแบบประเมิน FACIT-Pal ได้ 0.89 และค่าสัมประสิทธิ์แอลฟาครอนบาคได้ 0.94 วิเคราะห์ผลด้วยสถิติการวิเคราะห์ความแปรปรวนทางเดียวแบบวัดซ้ำ (One-way repeated ANOVA) ผลวิจัยสรุปได้ดังนี้ คุณภาพชีวิตด้านสุขภาพโดยรวมของผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลามหลังได้รับโปรแกรมสัปดาห์ที่ 1 และสัปดาห์ที่ 2 มากกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ .05 เมื่อจำแนกรายมิติพบว่ามิติความผาสุกด้านจิตอารมณ์หลังได้รับโปรแกรม สัปดาห์ที่1 สัปดาห์ที่ 2 และ สัปดาห์ที่ 6 มากกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่.05 สรุปได้ว่า โปรแกรมการดูแลวิถีพุทธสามารถส่งเสริมคุณภาพชีวิตด้านสุขภาพของผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลามได้ แม้ว่าในสัปดาห์ที่ 6 คุณภาพชีวิตด้านสุขภาพมีแนวโน้มลดลง แต่ผลลัพธ์ของโปรแกรมยังคงสามารถส่งเสริมความผาสุกด้านจิตอารมณ์ของผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลามได้ต่อเนื่อง

การศึกษาครั้งนี้เป็นการวิจัยเชิงหาความสัมพันธ์ เพื่อศึกษาคุณภาพการนอนหลับในผู้ป่วยโรคหัวใจขาดเลือดเฉียบพลัน และศึกษาความสัมพันธ์ระหว่างปัจจัยคัดสรร ได้แก่ ความรุนแรงของอาการเจ็บหน้าอก ภาวะซึมเศร้า ความวิตกกังวล สถานะการสูบบุหรี่ อาการเหนื่อยล้า กับคุณภาพการนอนหลับ กลุ่มตัวอย่าง คือ ผู้ป่วยโรคหัวใจขาดเลือดเฉียบพลันอายุ 18 ปีขึ้นไป จำนวน 152 คน ซึ่งคัดเลือกแบบเฉพาะเจาะจงจากโรงพยาบาลสองแห่งในกรุงเทพมหานคร ตามเกณฑ์การคัดเข้า เครื่องมือที่ใช้ ได้แก่ แบบประเมินความรุนแรงของอาการเจ็บหน้าอก แบบสอบถามภาวะซึมเศร้า แบบประเมินความวิตกกังวล แบบประเมินอาการเหนื่อยล้า และแบบวัดคุณภาพการนอนหลับ ซึ่งแบบสอบถามมีค่าสัมประสิทธิ์สหสัมพันธ์แอลฟาครอนบาค มากกว่าเท่ากับ 0.70 วิเคราะห์ข้อมูลด้วยสถิติเชิงบรรยายและทดสอบความสัมพันธ์ด้วยสถิติสหสัมพันธ์เพียร์สัน ผลการวิจัยพบว่า กลุ่มตัวอย่างส่วนใหญ่ (ร้อยละ 84.9) มีคุณภาพการนอนหลับดี ความวิตกกังวล สถานะการสูบบุหรี่ อาการเหนื่อยล้า และ ภาวะซึมเศร้ามีความสัมพันธ์ทางลบกับคุณภาพการนอนหลับ (r= -.197, -.175, -.440, -.528; p< .05) ส่วนความรุนแรงของอาการเจ็บหน้าอกไม่มีความสัมพันธ์กับคุณภาพการนอนหลับ

การศึกษาครั้งนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการจัดการตนเองที่เน้นการออกกำลังกายแบบแกว่งแขนต่อระดับฮีโมโกลบินเอวันซี ของผู้ป่วยโรคเบาหวานเข้าจอประสาทตา กลุ่มตัวอย่างได้แก่ผู้ป่วยโรคเบาหวานเข้าจอประสาทตาที่มารับการรักษาที่แผนกผู้ป่วยนอกจักษุโรงพยาบาลเมตตาประชารักษ์ (วัดไร่ขิง) จังหวัดนครปฐม เป็นผู้ป่วยโรคเบาหวานเข้าจอประสาทตามีอายุมากกว่าหรือเท่ากับ 30 ปีขึ้นไป ได้รับการวินิจฉัยว่าเป็นโรคเบาหวานเข้าจอประสาทตา เลือกตัวอย่างแบบเฉพาะเจาะจง ตามเกณฑ์ที่กำหนด จำนวน 44 คน แบ่งเป็นกลุ่มทดลอง 22 คนและกลุ่มควบคุม 22 คน จับคู่โดยคำนึงถึง ระยะเวลาการเป็นโรคเบาหวาน ชนิดของยาที่ใช้ และค่าดัชนีมวลกาย กลุ่มทดลองได้รับโปรแกรมการจัดการตนเองที่เน้นการออกกำลังกายแบบแกว่งแขน และกลุ่มควบคุมได้รับการพยาบาลตามปกติ ระยะเวลา 12 สัปดาห์ เครื่องมือที่ใช้ในการวิจัย ได้แก่ โปรแกรมการจัดการตนเองที่เน้นการออกกำลังกายแบบแกว่งแขน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือแบบบันทึกข้อมูลส่วนบุคคล และเครื่องตรวจระดับฮีโมโกลบินเอวันซี วิเคราะห์ข้อมูลโดยใช้ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติการทดสอบที ผลการวิจัยพบว่า 1..ค่าฮีโมโกลบินเอวันซีของผู้ป่วยโรคเบาหวานเข้าจอประสาทตาในกลุ่มทดลองหลังได้รับโปรแกรม ( x = 8.30 ± 1.55 ) มีค่าต่ำกว่าก่อนการทดลอง ( x = 9.14 ± 1.46) อย่างมีนัยสำคัญทางสถิติ ที่ระดับ .05 (t= 3.23, p <05) 2. ผลต่างของฮีโมโกลบินเอวันซีก่อนและหลังเข้าร่วมวิจัยของกลุ่มทดลอง ( d = 0.84 ± 1.21) มากกว่ากลุ่มควบคุม ( d = 0.13 ± 0.85) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (t= 2.24 , p < .05 )

การวิจัยนี้เป็นการวิจัยกึ่งทดลอง เพื่อศึกษาผลของโปรแกรมสนับสนุนข้อมูลผ่านไลน์แอพพลิเคชั่นร่วมกับการบริหารกาย-จิตแบบชี่กงวิถีไทยต่อความวิตกกังวลก่อนการผ่าตัดในผู้ป่วยมะเร็งเต้านม กลุ่มตัวอย่างคือผู้ป่วยมะเร็งเต้านมที่ได้รับการนัดหมายเพื่อผ่าตัด Mastectomy และ Breast Conserving Surgery ในโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย คัดเลือกแบบเฉพาะเจาะจงคุณสมบัติตามที่กำหนด แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 22 คน ได้รับการจับคู่ด้วย อายุ วิธีการผ่าตัดและการได้รับยาเคมีบำบัดก่อนผ่าตัด กลุ่มควบคุมได้รับการพยาบาลตามปกติและกลุ่มทดลองได้รับโปรแกรมสนับสนุนข้อมูลผ่านไลน์แอพพลิเคชั่นร่วมกับการบริหารกาย-จิตแบบชี่กงวิถีไทยเป็นเวลา 2 สัปดาห์ก่อนผ่าตัด เครื่องมือที่ใช้ในการวิจัยมี 3 ชุด คือ 1) เครื่องมือเก็บรวบรวมข้อมูล ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล, แบบประเมินความวิตกกังวลขณะเผชิญ STAI Form Y-I 2) เครื่องมือทดลอง ได้แก่ โปรแกรมสนับสนุนข้อมูลผ่านไลน์แอพพลิเคชั่นร่วมกับการบริหารกาย-จิตแบบชี่กงวิถีไทย 3) เครื่องมือกำกับการทดลอง ได้แก่ แบบสอบถามความรู้เรื่องโรคมะเร็งเต้านมและการผ่าตัด, แบบสอบถามความรู้เรื่องการเตรียมสุขภาพก่อนการผ่าตัดและประโยชน์ของชี่กง และแบบบันทึกการทำชี่กง ตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 5 ท่าน ทดสอบค่าความเที่ยงของแบบประเมินความวิตกกังวลขณะเผชิญโดยใช้สูตรสัมประสิทธิ์แอลฟาคอนบาค ได้ค่าความเที่ยงเท่ากับ 0.88 วิเคราะห์ข้อมูลโดยใช้สถิติร้อยละ ค่าเฉลี่ย และสถิติทดสอบค่าที (t-test) ผลการวิจัยสรุปได้ดังนี้ 1. ความวิตกกังวลก่อนผ่าตัดของผู้ป่วยมะเร็งเต้านมในกลุ่มทดลองภายหลังได้รับโปรแกรมต่ำกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ความวิตกกังวลก่อนผ่าตัดภายหลังการทดลองของผู้ป่วยมะเร็งเต้านมในกลุ่มทดลองต่ำกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

Background: Chronic lower respiratory disease, including asthma, is the fourth leading cause of death in the U.S. Financially, asthma accounts for approximately 54 billion dollars in healthcare spending annually. The National Heart Lung and Blood Institute [NHLBI] established guidelines to reduce impairment and to reduce future asthma attacks.
Purpose: This Quality Improvement (QI) pilot implemented the Asthma Control Test (ACT) and the Asthma Action Plan (AAP) per NHLBI guidelines. This project aimed to increase provider use of the ACT and AAP to 50% in patients with a diagnosis of asthma over one month.

Objective: To explore the feasibility of Auricular Point Acupressure (APA) to self-manage pain among patients with Alzheimer’s disease and related disorders (ADRD) and their caregivers (patient-caregiver dyads). Methods: A two-phase study design was used. Phase 1 was designed to explore the experiences of the dyads in using APA to manage chronic pain. People’s expectations of aging are shaped by various sources and experiences. These aging expectations subsequently influence a person’s overall health and well-being. I intended to gain a deeper understanding of the various sources by which Filipino older adults attribute and reshape negative and positive expectations regarding aging. Introduction: Traumatic hip fracture remains a major cause of disability, decline, and mortality for adults over 65. Gaps remain in the consistency of care and services hip fracture patients receive during hospitalization and post-discharge, leading to adverse outcomes, including MI, surgical site infection, poorly managed pain, and increased mortality. To help address these inconsistencies, the AAOS released updated clinical practice guidelines for management of hip fracture care in the elderly in 2014. These guidelines examine the entire hospitalization process for this population and set forth evidence-based standards to assist with improving outcomes for hip fracture patients.

This experiential component or doctoral capstone project involved a partnership with Habitat for Humanity of Greater Memphis. Through this agency, projects were completed based on identified needs of the Aging in Place and CAPABLE programs. The Aging in Place (AIP) program provides critical home repairs to low-income adults aged 60 years and older. The CAPABLE program provides the same critical home repairs along with a select number of visits from a registered nurse and an occupational therapist to help the older adults age in place safely.

Introduction. Falls are the leading cause of injury and injury death in adults over the age of 65. A retrospective examination at Level 1 trauma center in the Midwest found that 19.4% (118/605) of older adults admitted with a ground level fall were readmitted with a subsequent fall and injury. The aim of this quality improvement initiative is to determine if the implementation of an evidenced based patient toolkit and a discharge algorithm for providers will reduce outpatient falls.
