Background: Delirium, an acute change in cognition, occurs in 21% of hospitalized patients and is linked to increased length of stay, readmissions, and increased mortality (Shen et al., 2024). In older adults, delirium prevalence increases and is linked to poor outcomes that inhibit patients' health care goals (Shen et al., 2024). Despite the well-published harms of delirium and standardized prevention measures, delirium prevention is not prioritized. Methods: HELP, originally the Hospital Elder Life Program, now preferred to be known as HELP, is a standardized delirium prevention model specifically designed to address the unique needs of hospitalized older adult patients. Background: Advance care planning (ACP) supports patient autonomy and promotes care aligned with individual values and preferences. Despite these benefits, fewer than half of adults aged 50–80 have completed formal ACP documentation, and community-dwelling older adults rarely engage in ACP before serious illness, representing a persistent gap in early engagement.
Purpose: This quality improvement (QI) project evaluated the effectiveness of a community-based ACP educational intervention in improving ACP knowledge, confidence, and engagement among older adults.

Antibiotic overuse among older adults remains a significant patient safety and public health concern. Recent national data indicate that approximately 30%–50% of outpatient antibiotic prescriptions may be unnecessary, particularly for viral or self-limiting conditions (Chua et al., 2021). Antimicrobial resistance has been associated with more than 1.27 million deaths globally (Murray et al., 2022). This quality improvement project evaluated a multifaceted antimicrobial stewardship intervention in primary and long-term care settings. Baseline review of 167 antibiotic cases revealed 37.1% were inappropriate or questionable, and 28.7% lacked adequate diagnostic documentation. Background: Dementia is a growing global health concern, with behavioral and psychological symptoms (BPSD) contributing significantly to diminished quality of life and caregiver burden. Non-pharmacological interventions, such as gardening and nature-based therapies, are increasingly recognized for their therapeutic potential. Implementation of a Turn Team to Reduce Hospital-Acquired Pressure Injuries in a Medical Intensive Care Unit
PURPOSE
The project aimed to reduce hospital-acquired pressure injury (HAPI) rates in a Medical Intensive Care Unit (MICU) by improving nurses' knowledge of the Braden Scale and implementing a structured two-nurse turn team workflow to reposition high-risk patients every two hours.
RESULTS
Repositioning compliance improved substantially and consistently exceeded the project goal of 90% after implementing a pod-based workflow. The HAPI prevalence rate decreased from a baseline of 8% to below the project goal of ≤5% during implementation but was not sustained in the …

This integrative literature review examines the relationship between quantity of visitation and quality of life (QOL) of nursing home residents. The purpose of this review is to examine existing research to determine whether an optimal “dosage” of visitation exists that meaningfully enhances QOL for this vulnerable population. Establishing an evidence based- guideline may help reduce uncertainty for families and promote more consistent improvements in resident well-being. This quality improvement project focuses on reducing the average technology-induced anxiety score among senior community members through 1:1 mentorship sessions. Aim During an eight-week period from September 10, 2025, to November 5, 2025, this project will reduce the severity of technology-induced anxiety by 25% from the established baseline. Methods: A 5P Microsystem assessment was conducted alongside an initial questionnaire to gauge the residents’ interests and concerns regarding technology use. Problem: Medication technicians have minimal formal training and education in medication administration. Insufficient knowledge and a lack of confidence present safety concerns, such as medication errors, which decrease the quality of patient care.
Context: The setting is an assisted living facility in the San Francisco Peninsula that provides around-the-clock patient care, promoting independence in a safe and home-like environment for 53 patients.
Interventions: The proposed intervention involves implementing a quality improvement (QI) project to educate medication technicians on medication administration safety. Background: Older Spanish-speaking Hispanics are a rapidly growing population in the United States that face disproportionally high rates of diabetes and inadequate health literacy. Familism is valued among Hispanics and caregivers may play an important role in overcoming the social determinants of health facing this vulnerable population. Methods: This descriptive correlation study sought to explore correlations between the caregiver’s health literacy and the patient’s control of diabetes. Findings: Mixed effects model found a significant correlation between the caregiver’s print literacy and the patient’s hemoglobin A1c. Background: Despite the ease of receiving OCT, patients’ OCT adherence is low. With this low prevalence rate, patients are not likely obtaining the full life-promoting effects of OCT. Since medication adherence is often related to lived experiences and standardized medication adherence instruments may not capture these experiences completely, the purpose of this study was to explore how the lived experiences of older adults with cancer influence their adherence to OCT.

Introduction: The lack of education and use of the Cognitive Assessment and Care Planning Services (CACPS) visit creates a gap in care for Medicare patients over the age of 65 with cognitive impairment.
Methods: Recommendations were created from a robust collection of graded, evidence-based literature. The recommendations were presented to a set of stakeholders who rated each recommendation on a Likert scale from 1 (poor) to 4 (excellent).
Results: The twelve stakeholders agreed with the recommendations to support the education and use of the CACPS visit for Medicare patients 65 and older.

การศึกษาครั้งนี้เป็นการวิจัยแบบบรรยายเชิงทำนาย เพื่อศึกษาความสามารถของปัจจัยคัดสรรในการทำนายพฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้าน โดยปัจจัย ประกอบด้วย 1) ความรู้เกี่ยวกับวัณโรค 2) ทัศนคติต่อวัณโรค 3) แรงจูงใจในการป้องกันวัณโรค 4) การรับรู้ความสามารถของตนเองในการป้องกันวัณโรค และ 5) การรับรู้อุปสรรคเกี่ยวกับการป้องกันวัณโรค กลุ่มตัวอย่างเป็นผู้สัมผัสวัณโรคร่วมบ้าน จำนวน 111 คน ซึ่งเลือกอย่างเจาะจงจากโรงพยาบาล 3 แห่งในเขตกรุงเทพมหานคร เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามชนิดตอบด้วยตนเอง วิเคราะห์ด้วยสถิติเชิงพรรณนาและการถดถอยพหุคูณ ผลการวิจัย พบว่า พฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้านโดยรวม อยู่ในระดับดี (Mean = 44.2, SD = 6.5) ปัจจัยที่สามารถทำนายพฤติกรรมการป้องกันวัณโรคอย่างมีนัยสำคัญทางสถิติ ได้แก่ ทัศนคติต่อวัณโรค (b = .374, p<.05) การรับรู ้ความสามารถของตนเอง ในการป้องกันวัณโรค (b = .244, p <.05) และ การรับรู ้อุปสรรคเกี ่ยวกับการป้องกันวัณโรค (b = .166, p <.05) ปัจจัยดังกล่าวสามารถร่วมทำนายพฤติกรรมการป้องกันวัณโรคได้ร้อยละ 32.8 อย่างมีนัยสำคัญทางสถิติ (R2 = .328, p <.05) อย่างไรก็ตามในการศึกษานี้ ความรู ้เกี ่ยวกับวัณโรค และแรงจูงใจในการป้องกันวัณโรค ไม่สามารถทำนายพฤติกรรมการป้องกันวัณโรคในผู ้สัมผัสวัณ โรคร่วมบ้าน

การวิจัยแบบกึ่งทดลองนี้มีวัตถุประสงค์ เพื่อศึกษาผลของโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันต่อพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม เบาหวาน ความดันโลหิตสูง ไขมันในเลือด 1 โรคขึ้นไป ที่มีอายุตั้งแต่ 35 – 80 ปี คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 23 คน จับคู่ทั้งสองกลุ่มให้มีลักษณะคล้ายคลึงกันในเรื่องอายุ จำนวนโรคร่วม จำนวนกลุ่มยาที่ใช้รักษาโรค เปรียบเทียบข้อมูลพื้นฐานด้วยสถิติ Chi-square test และ Fisher’s Exact Test กลุ่มตัวอย่างทั้งสองกลุ่มมีคุณลักษณะพื้นฐานที่ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติในทุกตัวแปร (p > .05) กลุ่มควบคุมจะได้รับการพยาบาลปกติ ในขณะที่กลุ่มทดลองได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชัน เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารของผู้ป่วยที่มีภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม และแบบสอบถามประเมินความรอบรู้ด้านสุขภาพเกี่ยวกับพฤติกรรมการจัดการตนเองด้านการบริโภคอาหาร ทดสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.92 และ 1.00 และค่าความเที่ยงเท่ากับ 0.91 และ 0.73 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิตทดสอบค่าที ผลการวิจัยสรุปได้ดังนี้ 1. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม กลุ่มทดลองหลังได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่าก่อนได้รับโปรแกรม ฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม ที่ได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง เพื่อศึกษาผลของโปรแกรมสนับสนุนการให้ข้อมูลแบบรูปธรรม-ปรนัยต่อความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกในห้องฉุกเฉิน โดยกลุ่มตัวอย่างในการศึกษาครั้งนี้คือ สมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกที่นำส่งผู้ป่วยมารับบริการที่ห้องฉุกเฉินของโรงพยาบาลสรรพสิทธิประสงค์ แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 30 คน โดยคัดเลือกตามแบบเฉพาะเจาะจงตามเกณฑ์ที่กำหนด โปรแกรมรูปธรรม-ปรนัย และแบบสอบถาม ผ่านผู้ทรงคุณวุฒิ 5 ท่าน ได้ค่าความเที่ยง 0.84 โปรแกรมการสนับสนุนการให้ข้อมูลแบบรูปธรรม–ปรนัยต่อความวิตกกังวล มีเครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ประกอบด้วย ข้อมูลทั่วไปของผู้ป่วยโรคหลอดเลือดสมอง และแบบประเมินความวิตกกังวลขณะเผชิญ (The State Anxiety Inventory (STAI) Form Y) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติทดสอบ Independent t-test และสถิติ paired t-test ผลการวิจัย: ผลการวิจัยพบว่าสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองที่ห้องฉุกเฉิน ในกลุ่มทดลอง ภายหลังได้รับโปรแกรมคะแนนความวิตกกังวลตํ่ากว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (p < .05) และคะแนนความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยในกลุ่มทดลองภายหลังได้รับโปรแกรมตํ่ากว่าในสมาชิกครอบครัวของผู้ป่วยได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติ (p < .05)

การวิจัยครั้งนี้เป็นการศึกษาเชิงกึ่งทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบการฟื้นตัวด้านการทำหน้าที่ของลำไส้หลังผ่าตัดในผู้ป่วยมะเร็งลำไส้ใหญ่และทวารหนักที่ได้รับการผ่าตัดแบบเปิดช่องท้องระหว่างกลุ่มที่ได้รับโปรแกรมการนวดกดจุดร่วมกับการจัดการอาการ กับกลุ่มที่ได้รับการพยาบาลตามปกติ ที่เข้ารับการรักษาในหอผู้ป่วยวิกฤต ในวันที่ 1 ถึง 5 หลังผ่าตัด กลุ่มตัวอย่างคือผู้ป่วยมะเร็งลำไส้ใหญ่ และทวารหนักที่ได้รับการผ่าตัดแบบเปิดช่องท้อง เข้ารับการรักษาตัวที่หอผู้ป่วยวิกฤตศัลยกรรมในโรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติ ทั้งเพศชายและหญิงที่มีอายุมากกว่า 20 ปี จำนวน 44 ราย แบ่งเป็นกลุ่มควบคุม 22 ราย และกลุ่มทดลอง 22 ราย จับคู่ ให้กลุ่มตัวอย่างทั้งสองกลุ่มมีลักษณะใกล้เคียงกันในเรื่องของเพศ ดัชนีมวลกาย และระดับโพแทสเซียมในเลือดหลังผ่าตัด โดยกลุ่มทดลองได้รับโปรแกรมการจัดการอาการร่วมกับการนวดกดจุด โดยการเคลื่อนไหวร่างกายวันละ 1 ครั้ง ครั้งละ 10-15 นาที ร่วมกับการนวดกดจุดบริเวณจุด Zusanli (ST-36) วันละ 2 ครั้ง ครั้งละ 10 นาที ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือวิจัยประกอบด้วยโปรแกรมการนวดกดจุดร่วมกับการจัดการอาการที่ออกแบบเพื่อส่งเสริมการฟื้นตัวด้านการทำหน้าที่ของลำไส้หลังผ่าตัด และแบบประเมินการฟื้นตัวด้านการทำหน้าที่ของลำไส้ ซึ่งผ่านการทดสอบความเที่ยง โดยมีค่าสัมประสิทธิ์แอลฟาครอนบาคเท่ากับ 0.87 การวิเคราะห์ข้อมูลใช้สถิติเชิงพรรณนา Mann–Whitney U test และ Friedman test ผลการศึกษาพบว่า คะแนนเฉลี่ยการฟื้นตัวด้านการทำหน้าที่ของลำไส้ในช่วงวันที่ 1 ถึง 5 หลังผ่าตัดของผู้ป่วยที่ได้รับโปรแกรมการจัดการอาการร่วมกับการนวดกดจุดสูงกว่าผู้ป่วยที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 (p < 0.05) (x² = 82.13, p < 0.001 และ x² = 5.62, p < 0.001 ตามลำดับ)

การศึกษาความสัมพันธ์เชิงทำนายนี้ มีวัตถุประสงค์ เพื่อศึกษาความร่วมมือในการรับประทานยาต้านการแข็งตัวของเลือด และปัจจัยที่สามารถทำนายความร่วมมือในการรับประทานยา ได้แก่ อายุ ความรู้ ความเชื่อเกี่ยวกับความจำเป็นของยา การรับรู้อุปสรรคในการรับประทานยา ภาวะโรคร่วม และสัมพันธภาพระหว่างบุคลากรการแพทย์ ในผู้ป่วยที่มีภาวะหัวใจห้องบนสั่นพลิ้วที่รับประทานยาต้านการแข็งตัวของเลือดชนิด warfarin จำนวน 90 คน ที่มารับบริการ ณ แผนกผู้ป่วยนอกโรคหัวใจ โรงพยาบาลตำรวจ โรงพยาบาลรามาธิบดี และโรงพยาบาลราชวิถี เครื่องมือที่ใช้ในการวิจัย ได้แก่ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามความร่วมมือในการรับประทานยา 3) แบบสอบถามความรู้เกี่ยวกับยา 4) แบบสอบถามความเชื่อเกี่ยวกับความจำเป็นของยา 5) แบบสอบถามการรับรู้อุปสรรคในการรับประทานยา 6) แบบสอบถามภาวะโรคร่วม และ 7) แบบสอบถามสัมพันธภาพระหว่างบุคลากรการแพทย์ เครื่องมือมีค่าดัชนีความตรงตามเนื้อหาเท่ากับ .88, .94, 1.00, 1.00 และ 1.00 ตามลำดับ ส่วนแบบสอบถามภาวะโรคร่วมเป็นแบบประเมินมาตรฐาน และค่าสัมประสิทธิ์แอลฟาครอนบาคเท่ากับ .792, .758, .790, .811, .811 และ .934 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติบรรยายและการถดถอยพหุคูณแบบขั้นตอน ผลการศึกษา พบว่า กลุ่มตัวอย่างมีความร่วมมือในการรับประทานยาต้านการแข็งตัวของเลือดอยู่ในระดับปานกลางถึงสูง (Mean = 28.29, SD = 2.90) และปัจจัยที่สามารถทำนายความร่วมมือในการรับประทานยาได้อย่างมีนัยสำคัญทางสถิติ ได้แก่ ความรู้ (β = .297, p < .01) การรับรู้อุปสรรคในการรับประทานยา (β =–.342, p < .01) ความเชื่อเกี่ยวกับความจำเป็นของยา (β = .255, p < .01) และภาวะโรคร่วม (β =–.178, p < .05) โดยสามารถร่วมกันทำนายความร่วมมือในการรับประทานยาได้ร้อยละ 57.5 (Adjusted R² = .575, p < .05) ส่วนอายุและสัมพันธภาพระหว่างบุคลากรการแพทย์พบว่าไม่สามารถร่วมทำนายความร่วมมือในการรับประทานยาของผู้ป่วยได้
