In a list of 17 high-income countries, the United States ranks last in terms of life expectancy for males and second-to-last for females. The U.S. population also experiences worse outcomes compared with its peers in nine key areas: infant mortality and low birth weight; injuries and homicides; adolescent pregnancy and sexually transmitted infections; HIV and AIDS; drug- related deaths; obesity and diabetes; heart disease; chronic lung disease; and disability. In addition, the United States sees persistent racial, ethnic, socioeconomic, and geographic disparities in health.

Law enforcement and legal services are 2 systems that respond to reports of abuse against women through programs such as adult protective services (APS) and domestic violence (DV). APS and DV systems operate independently and define the cause of the abuse differently. The designation of a woman as having suffered domestic violence or having been abused often depends on which system she enters. This designation can lead to different options for women to obtain services. Aging may further complicate access and usability of these systems to respond to the needs of abused elderly. Moral hazard and adverse selection create inefficiencies in private health insurance markets and understanding the relative importance of each factor is critical for policy.   We use claims data from a large firm which changed health insurance plan options to isolate moral hazard from plan selection.  Using an instrumental variables quantile regression approach, we estimate the differential causal impact of each health insurance plan on the entire distribution of medical expenditures.  We account for systematic sample attrition during the sample period by conditioning on a nonseparable sample selection adjustment.   Our estimates imply that 54% of the additional medical spending …
