One person, [the victim]; aged twenty-seven was injured in the accident whilst conducting an intrusive visual surface search within allocated and marked boundaries as part of a Battle Area Clearance operation. After the accident he was transported to the Jabel Amel Hospital in Tyre where he was admitted and treated before being discharged after twenty – eight hours.

The M77 involved in this accident did not detonate properly and may have been damaged. If this was visible, it would imply that the Victim made an error by touching it. But if the damage was not visible, the Victim was just extremely lucky.

On Thursday 5th of July 2007 at approx.1130 hrs, land caretaker ([Name removed]) informed one member of [International demining NGO] EOD team (via phone) that his workers ([Name removed] and his father [Name removed]) found one sub-munitions (1xM46), while working inside the citrus orchard, at Task CBU-193B. The M46 was located hanging on the branch knob of lemon tree approx. 1.6 m from the ground level, in an area which had been cleared by [International demining NGO] using sub-surface Battle Area Clearance (BAC) procedures (Schonstedt GA-72Cd Magnetic Locator). On the 5th July 2007 at 1228hrs, two [International commercial demining agency] BAC searchers were involved in an accident, which was caused by an uncontrolled detonation of an M77 sub-munitions. The detonation took place during a period when all searchers were not conducting search/clearance operations but were preparing to start work after a 10 minutes rest period.

On 2nd Jul 2007, two sub-munitions (1x M77 & 1xM42) were reported as found by the [International commercial company] completions team during a final inspection of CBU 771, before handover to the MACC SL. These items were located by the local land owner (woman) while harvesting her wheat crop. They were located in an area immediately adjacent to her house (within the task site) which had been reported as cleared by [International commercial company] sub-surface. The items were located on the surface. The owner reported the find to [International commercial company] completions team during and internal completion QA.

On the morning of 21st June 07 at approximately 0616hrs [Demining group] BAC Teams commenced operations at CBU 071. At approximately 1104hrs, an uncontrolled explosion occurred in the plantation at UTM 728309/3676516. An [Demining group] searcher, Mr. [the Victim] who was conducting search operations in the area of the detonation was not directly involved in causing the detonation but he was in the immediate area of the detonation where he was conducting sub surface clearance operations.

On the morning of 21st May 2007 at approximately 0630hrs [Demining group] BAC Team 2 re-deployed back to CBU 040 to start operations. The terrain was already cleared with the use of mine detectors and was clearly limited with red painted rocks and stones. Around 09.30 Hr 1MC [Victim No.1] stepped on a cluster M77, which caused the explosion, following the investigation of the crater the cluster, was lying subsurface. On the day of the accident the team consisted of 10 members: Site Supervisor, Team Leader, 6 x searchers, medic and driver. [The Victim]. acting Team Leader, was supervising three searchers in the area of Box C3, [Names removed] who were conducting visual search of the area. [Name removed] was standing in the visually cleared area to the rear of the searchers supervising the surface search operations. At approximately 1008hrs, an uncontrolled explosion occurred in box C3, involving [the Victim]. At the time of the accident all team members were wearing PPE (jacket and visor).
