A deminer from the NGO [International demining organisation] was killed in Pailin province on Friday after detonating a mine while attempting to clear an area in the former Khmer Rouge stronghold.

One employee of the British demining organisation [name removed] died and another was injured after an antipersonnel mine exploded while they were working in Pailin province.

According to the investigation report and as seems from the pictures of scene, the deminer was working with a big shovel. On 19th of April 09 one of the MACCA operations assistant found the team using big shovel and raised a non-conformity report, but the team leader and then the operations officer of [Demining group] did not accept the non-conformity report. However the problem was showed to be solved during the re-audit visits, but in reality it had not been solved and consequently the accident occurred.

Chisel is used for excavating of safe margin of the reading points, meaning 15 cm behind the start point of signal.

It is the conclusion of investigation team that the involved deminer was excavation a signal in an area where the ground surface was hard with dense vegetation, and considering the crater created as a result of the explosion it seems that the deminer was excavating the signal about 30 cm on right side of the working lane where he was not in a stable position for excavating the signal by scraper; the excavation of this lane required to be done in next clearance lane.

On 9th Oct 2007 at 08:10 Hrs, the accident happened on [Victim No.1] the Asst. Team Leader of [Demining group] EOD-4 while he was shifting UXO to pit. There were three antipersonnel PMN-2 mines near to the UXO and the Asst. On 29 Oct 06 an accident involving a staff member of KEC (The Indian contractor currently engaged to refurbish the power-line) detonated a PMN2 AP whilst he was excavating at internal base of Tower No 60. Towers 60 and 62 are located in what was addressed as MF Code 01-0103-11-041 during the 1996 clearance. The [name removed] survey data of the time corroborates the information supplied by Mr [name removed] that some towers were considered clear and others were considered to be mined.

The deminer stated that the two mines that he had found earlier that day had only been 5cm deep, and that he did not expect to find another mine so close to the last one [even though the previous three had been with 2 metres of each other], or so deep; he therefore assumed that the signal was not a mine.
